파킨슨병의 한약 치료에 대한 최신 임상연구 동향 고찰 - 2010년부터 2014년까지 중국 논문을 중심으로 - (Review of Current Clinical Studies for Herbal Medicine of Parkinson’s Disease in Traditional Chinese Medicine)

  • The objective of this study was to review the current clinical studies about the effect of herbal medicine for Parkinson's disease in China over the last 5 years and then to suggest the foundation for treatment and further studies. We had searched for studies in China National Knowledge Infrastructure(CNKI, http://www.cnki.net) and PubMed from January 2010 to December 2014. Key words were the various combinations of '帕金森', '湯', '丸', '散', '中醫藥', 'Parkinson's disease', and 'herbal medicine'. Total 53 clinical studies were selected and analyzed. The most frequently used diagnostic criteria of Parkinson's disease was the Unified Parkinson's Disease Rating Scale(UPDRS). The most frequently used medical herb was Paeoniae Radix alba(白芍藥) more 30 times and the highest amount was Astragali radix(黃芪) 100g per day. The most frequent syndrome differentiation was liver kidney yin deficiency(肝腎陰 虛). We found out there are many clinical studies of herbal medicine for Parkinson's disease in China. These studies would be able to provide the basis of clinical research on Parkinson's disease, and also applied to the treatment of Parkinson's disease in Korea.

오정요법(五正療法)에 에 의한 파킨슨병 치료종결 1례에 대한 고찰 (A study on a case of treatment termination of Parkinson's disease treated by 5 upright life cure regulations therapy)

  • Parkinson's disease is a degenerative disease of a cranial nerve and has a main symptoms of irregular movement of muscle, stiffening, trembling which occurred by about 1% of population in the age of over 65. Moreover, the and prevalence rate and attack rate are soaring according to increase of elderly population. However, allopathy and surgery were done through dopamine and anticholinergic medicine for treatment but it developed a lot of complications due to medicine and progress since it makes slow progress or can't stop the treatment. Hereupon, I report that there is a case on one of the patients Young Jin Oriental Medical Clinic. The patient who is in state of treatment termination who doesn't need any further remedy and no worsening of symptoms after conduction of therapies of dialectic and 5 upright life cure regulations. 5 upright life cure regulations means five practive way for improvement of nature healing power. Upon undertaking the 5 upright life cure regulations, there were found significant results in such tests as Unified Parkinson Disease Rating Scale (UPDRS), Hoehn & Yahr Staging Scale, and Activity of Daily Living (ADL), and the ingestion of Benztropine 1mg and Requip 0.25mg was decreased from 3 times to 0 times. The study offers objective clinical data on Oriental Medicine treatment for Parkinson's disease which is one of representative neuro-degenerative diseases and thus broadens the application range of Oriental Medical treatment and presents the fundamental data on the clinical research on Parkinson's disease by adopting evidence-based medicine (EBM).

파킨슨병으로 유발된 진전이 한약 치료를 통하여 호전된 치험 1례 (A case of tremor in Parkinson's disease treated with Korean medicine)

  • As society ages, the seriousness of Parkinson's disease in the elderly increases. This study was performed in order to evaluate the effect of Korean medical treatments on Parkinson's symptoms. An elderly patient was diagnosed with Parkinson's disease by clinical symptoms, which were resting tremor, pill-rolling tremor and mild bradykinesia. We treated her disease with Korean medical treatment, including herbal medicine and acupuncture therapy and excluding western medication. For the herbal medicine, Insamyangyeong-tang (人參養榮湯), Bobi-tang (補脾湯), Dokhwaljihwang-tang (獨活地黃湯) and Sipimijihwang-tang (十二味地黃湯) were primarily used. In result, the clinical symptoms, measured by unified Parkinson's disease rating scale (UPDRS) and modified Hoehn and Yahr stage (H-Y stage), of Parkinson's disease in this patient had improved after the Korean medical treatment. This study suggests that Korean medical treatments can be applicable to improve symptoms in patients with Parkinson's disease.

파킨슨 증후군 환자의 자세 불안정과 보행장애에 대한 한의치료의 효과: 후향적 연구 (Effects of Korean Medicine on Postural Instability and Gait Difficulty in Patient with Parkinsonism: Retrospective Study)

  • Objectives: The purpose of this study was to investigate the effects of Korean Medicine on postural instability and gait difficulty(PIGD) in patient with parkinsonism. Methods: We reviewed the charts of patients with parkinsonism who were admitted to the department of Korean internal medicine, stroke and neurological disorders center, Kyung Hee University Hospital at Gangdong from October 2009 to May 2017. We analyzed the data of UPDRS (Unified Parkinson's disease rating scale) and calculated PIGD related UPDRS before and after the admission. Results: The average of PIGD score for the 23 patients of Parkinsonism significantly decreased from $8.35{\pm}5.13$ to $4.52{\pm}3.68$ after treatment. The average of PIGD score for the 16 patients of idiopathic parkinson's disease and 7 patients of atypical Parkinsonism decreased from $9.13{\pm}4.46$ to $4.63{\pm}4.15$ and from $6.57{\pm}6.45$ to $4.29{\pm}2.56$ after treatment, respectively. Conclusions: These results provide that Korean Medicine has an effect on the motor function of patients who suffer from Parkinsonism with Postural Instability and Gait Difficulty. Furthermore it could be effective for improving activities of daily life.

  • 본 증례에서는 파킨슨병으로 인한 보행장애 및 자세불안정을 호소하는 환자 2명에 대하여 한의 치료를 시행한 결과 시공간적 보행지표, 운동형상학적 보행지표, UPDRS part III 점수에서 호전이 있었으며, 향후 보행장애 및 자세불안정을 호소하는 파킨슨병 환자에 한의 치료를 활용할 수 있음을 제시하였기에 증례를 보고하는 바이다.

  • Objectives : Postural instability and gait difficulty(PIGD) can develop in all Parkinson's disease patients, especially late in the disease course, but does not respond well to conventional dopamine treatment. This study aimed to report three cases of PIGD in Parkinson's disease patients treated with Korean and Western medicine treatment. Methods : We used acupuncture, bee venom acupuncture, herbal medicine and moxibustion to treat patients during hospitalization. They continued Western medication and received rehabilitation treatment. We observed the changes of PIGD using the Unified Parkinson's Disease Rating Scale(UPDRS). Results : After treatment, PIGD symptoms improved and UPDRS scores were decreased. Especially, scores of falling, walking, gait and postural stability subsections related with PIGD were decreased. In addition, general conditions of patients were improved. Conclusions : This study suggests that Korean medicine treatment could be effective in the treatment of PIGD in Parkinson's disease patients.

루이소체 치매로 추정되는 이차성 파킨슨증 환자의 Non-Motor Symptom Scale(NMSS)로 평가한 비운동성 증상을 한약과 침의 복합치료로 호전시킨 증례보고 1례 (A Case Report of Non-Motor Symptoms Evaluated Using the Non-Motor Symptom Scale in a Patient with Secondary Parkinsonism Presumed to be Probable Lewy Body Dementia and Improved with Combined Treatment with Herbal Medicine and Acupuncture)

  • Parkinson's syndrome is a degenerative brain disease that presents characteristic motor symptoms of tremor, rigidity, and gait disturbance. In addition to these motor symptoms, Parkinson's syndrome also presents non-motor symptoms (NMSs) such as sleep disturbance and cognitive decline. NMSs reduce patient's quality of life and psychosocial functioning and cause economic burden on the patient, so appropriate evaluation and treatment are required. Lewy body dementia is one of the several diseases belonging to Parkinson's syndrome. Its symptoms such as cognitive function, memory impairment, and hallucinations occur with Parkinsonism. Although drug therapy is being used with drug treatment to treat non-motor symptoms, it has limitations such as side effects, which stimulated interest in other complementary treatment methods such as oriental medicine treatment, dance, and yoga. The patient in this case complained of tremor in the right upper extremity, muscle hypertension and pain, and persistent vision, memory, and cognitive decline. The patient was diagnosed with probable Lewy body dementia. The patient was hospitalized for 4 months and received acupuncture and herbal medicines. After treatment, the patient's NMS scale scores decreased from 90 to 63, and the Unified Parkinson's Disease Rating Scale scores (summed I, II, and III) decreased from 17 points to 8 points. The Beck Depression Inventory score decreased from 22 points to 13 points. In addition, the patient's subjective evaluation revealed improvement. In this case, a patient diagnosed with probable Lewy body dementia who did not respond to the standard treatment and did not want to take medications showed improvement in not only motor symptoms but also NMSs after integrative Korean medicine treatment.

Effects of Sagittal Spinopelvic Alignment on Motor Symptom and Respiratory Function in Mild to Moderate Parkinson's disease

  • Purpose: This study examined the effects of sagittal spinopelvic alignment on the clinical parameters, motor symptoms, and respiratory function in patients with mild to moderate Parkinson's disease (PD). Methods: This study was a prospective assessment of treated patients (n=28, Hoehn and Yahr (H&Y) stage 2-3) in a PD center. Twenty-eight subjects ($68.5{\pm}5.7yrs$) participated in this study. The clinical and demographic parameters, including age, sex, symptoms duration, treatment duration, and H&Y stage, were collected. Kinematic analysis was conducted in the upright standing posture with a motion capture system. A pulmonary function test (PFT) was performed in the sitting position using a spirometer. The motor symptoms were assessed on part III of the movement disorder society sponsored version of the unified Parkinson's disease rating scale (MDS-UPDRS). SPSS 18.0 was used to analyze the collected data. Results: The exceeding 12 degrees group of the lower trunk showed significantly higher on the clinical parameters than the below 12 degrees group. In addition, the exceeding 12 degrees group of the lower trunk showed a significantly lower forced expiratory volume at one second (FEV1) / forced vital capacity (FVC) (%) and 25-75% forced mid-expiratory flow (FEF) (L/s) than in the below group. On the other hand, there was no difference in the upper trunk and the cervical pelvis between the groups. Conclusion: These findings suggest that the sagittal balance in the lower trunk is related to the clinical parameters and respiratory function, but not the motor symptoms in patients with mild to moderate PD.

파킨슨병 환자의 경직에 대한 약물과 DBS 의 효과의 정량화 (Quantification of the Effect of Medication and Deep Brain Stimulation on Parkinsonian Rigidity)

  • This study aims to quantify the effects of medication (Med) and deep brain stimulation (DBS) on resting rigidity in patients with Parkinson's disease. We tested 10 limbs of five patients under each of four treatment conditions: 1) baseline, 2) DBS, 3) Med, 4) DBS + Med. Rigidity at the wrist joint was assessed using the Unified Parkinson's Disease Rating Scale (UPDRS). The examiner randomly imposed flexion and extension movement on patient's wrist joint. Resistance to passive movement was quantified by viscoelastic properties. Not only rigidity score but also damping constant showed improvements in rigidity by DBS and Med treatments (p<0.05). This indicates that the viscosity can represent the change in rigidity due to DBS as well as Med, which was manifested by UPDRS score.

  • The purpose of this study is to compare finger tapping (FT) movement of patients with Parkinson's disease (PD) with normal subjects. A gyrosensor system was used for the measurement of FT movement, because it provides angular velocity free from the gravitational artifact and it can be used during clinical FT test listed in unified PD rating scale (UPDRS). Forty PD patients (age: 65.7 ${\pm}$ 11.1 yrs, H&Y stage:2.3 ${\pm}$ 0.5), 14 age-matched elderly subjects (65${\pm}$3.9 yrs) and 17 healthy young subjects (24${\pm}$2.1yrs) participated in this study. Angular velocity of finger tapping movement was measured in both right and left index finger. As quantitative measures, root-mean-squared (RMS) angular velocity, RMS angle, peak power and total power were used. ANOVA showed that all measures were significantly different among three groups (p<0.001) in all quantitative measures. Post-hoc test revealed that all quantitative measures except peak power in patients with PD were significantly smaller than in both healthy elderly and young subjects (p<0.01). This suggests that the measures developed in this study can distinguish patients with PD from normal subjects.
