과일 소비에 미치는 음식 관여도의 영향을 매개하는 요인 평가 (Measuring the Factors Mediating the Effect of Food Involvement on Fruit Consumption)

  • The purpose of this study was to measure the causal relationships among food involvement, health, mood, convenience, sensory appeal, weight control, and fruit consumption. A total of 290 questionnaires were completed. A structural equation model was used to measure the causal effects of the constructs, and the structural analysis results for the data indicated an excellent model fit. The effects of food involvement on health, mood, convenience, sensory appeal, weight control, and fruit consumption were statistically significant. As expected, health, mood, and weight control had significant effects on fruit consumption. Moreover, food involvement had a significant indirect effect on fruit consumption through health, mood, convenience, sensory appeal and weight control. For the future development and testing of conceptual models that integrate the relationships among personality traits, food choice motives, and fruit consumption, this study may approach a deeper understanding of the complex relationships among fruit consumption behavior-related variables. Greater understanding of these complex relationships can improve the managerial diagnosis of the problem and opportunities for different marketing strategies, including fruit production and fruit product development and marketing communications.

의료보험 민영화가 여성의 건강에 미치는 영향 : 칠레의 사례를 중심으로 (Lessons from Chile: The Impact of Privatization of Health Insurance on Women's Health)

  • Chile has been the first country in Latin America which has built a two-tiered health care system by partially privatizing the health insurance sector. Despite the intial decrease of health expenditure, more researches now show that health inequality within the Chilean health sector has been augmented with privatization of its insurance system. To explore such inequality, this article looks into the impact of privatization of health insurance on women's health. The author argues that privatization has intensified medicalization of women's body and, consequently, it worsened women's health in Chile. This article contributes to a more comprehensive understanding of market-oriented health care reform by linking it with medicalization process.

Selection Guide to Wearing Respirators According to Work Situations and On-site Applicability

  • Background: This study aims to introduce the formulation of the regulation for the selection of respirators for accident preparedness chemicals (APCs) according to chemical workplace situations and to determine on-site applicability. Methods: Workplaces were grouped into seven work categories, and APCs were classified into six groups to select adequate respirators. A survey was conducted to enhance the understanding of work situations and adequate respirators. The total number of subjects surveyed in 2018 was 201 managers and handlers, and that in 2019 was 91 handlers and 204 managers. Results: Adequate respirators were allocated to each cell using the matrix method. The study observed an overall lack of understanding of work situations, especially in the operation of open devices, which was the highest at 32.7%. Despite its implementation in 2015, 17.6% and 25.0% of the managers and APCs handlers, respectively, were unaware of the regulations for selecting respirators. Only 70.4% of the APCs handler wore respirators in compliance with regulations. Conclusion: The method for selecting respirators according to work situations using the matrix method is considered reasonable. Thus, this study suggests that the development of educational contents and reinforcing education should be essential steps to increasing awareness of regulations.

노인의 자가 보고에 따른 의료정보 이해 및 활용수준 (Older Adults' Self-reported Difficulty in Understanding and Utilizing Health Information)

  • 본 연구의 목적은 노인의 자가 보고에 따른 각종 의료정보의 주관적인 이해 및 활용수준에 대해 조사하고 신체적, 정신적 건강상태와의 연관성을 확인하며, 노인의 연령대 및 교육수준에 따라 구체적으로 어떠한 의료 정보의 이해와 활용에 어려움이 있었는지를 파악하기 위함이다. 본 연구의 대상자는 총 103명으로 2007년 7월부터 8월까지 대구, 경북, 부산지역의 노인복지회관과 노인정에서 선정되었다. 연구결과 노인의 자가 보고에 따른 의료정보 이해수준의 전체 평균점수는 15-75점 기준에 약 50점이었고, 가장 이해하기 어려워 한 부분은 의료인으로부터 받은 자료, 환자 교육자료, 병원서식 자료였다. 노인의 의료정보 활용수준은 노인의 신체적, 정신적 건강상태와 연관성이 있었으나, 의료정보 이해수준은 신체적, 정신적 건강상태와 연관성이 없었다. 또한 후기노인들은 전기노인들보다 약물명 이해수준과 병원용지 기입에 대한 자신감이 더 낮았고, 병원자료 이해부족으로 인한 건강상태 파악, 복약 설명서 이해부족으로 인한 약물 복용, 병원 설명서 이해부족으로 인한 예약 시간 도착에 있어서 더 많은 어려움을 경험하였다. 이 밖에도 초등학교 이하의 교육수준을 가진 노인일수록 의료인으로부터 받은 자료, 환자 교육자료, 병원서식, 진료 예약표, 약물명, 안내표지판 등에 대한 이해수준이 낮았고, 병원용지 기입이나 복약설명에 따른 약 복용에 대한 자신감이 낮았으며, 병원용지 기입과 건강상태 파악에 문제를 경험한 적이 더 많았다. 본 연구 결과를 바탕으로 의료인들은 의료기관의 시설 및 서식, 교육자료 등의 개발에 있어서 노인의 연령과 교육수준을 고려하여 정보의 이해와 처리를 강화할 수 있는 방법을 활용하여야 할 것이다.

정밀영양: 개인 간 대사 다양성을 이해하기 위한 접근 (Precision nutrition: approach for understanding intra-individual biological variation)

  • In the past few decades, great progress has been made on understanding the interaction between nutrition and health status. But despite this wealth of knowledge, health problems related to nutrition continue to increase. This leads us to postulate that the continuing trend may result from a lack of consideration for intra-individual biological variation on dietary responses. Precision nutrition utilizes personal information such as age, gender, lifestyle, diet intake, environmental exposure, genetic variants, microbiome, and epigenetics to provide better dietary advices and interventions. Recent technological advances in the artificial intelligence, big data analytics, cloud computing, and machine learning, have made it possible to process data on a scale and in ways that were previously impossible. A big data platform is built by collecting numerous parameters such as meal features, medical metadata, lifestyle variation, genome diversity and microbiome composition. Sophisticated techniques based on machine learning algorithm can be used to integrate and interpret multiple factors and provide dietary guidance at a personalized or stratified level. The development of a suitable machine learning algorithm would make it possible to suggest a personalized diet or functional food based on analysis of intra-individual metabolic variation. This novel precision nutrition might become one of the most exciting and promising approaches of improving health conditions, especially in the context of non-communicable disease prevention.

  • Antimicrobial resistance and emerging infectious diseases, including avian influenza, Ebola virus disease, and Zika virus disease have significantly affected humankind in recent years. In the premodern era, no distinction was made between animal and human medicine. However, as medical science developed, the gap between human and animal science grew deeper. Cooperation among human, animal, and environmental sciences to combat emerging public health threats has become an important issue under the One Health Initiative. Herein, we presented the history of One Health, reviewed current public health threats, and suggested opportunities for the field of public health through better understanding of the One Health paradigm.

만성질환 노인의 건강관리 프로그램 참여 경험 (Experiences of the Elderly with Chronic Diseases in a Health Management Program)

  • Objectives: This study was to explore how elderly with chronic disease in a health management program managed their health and to describe their health management strategies. Methods: The data were collected through in-depth interviews, with a purposeful sample of twelve in an urban community. The collected information was analyzed through grounded theory approaches by Strauss and Corbin. Results: Though open coding process, 35 concepts, 17 subcategories, 8 categories were identified. As for the participation experience process, four stages had surfaced, namely, 'Perception of chronic disease', 'Concerned about health', 'Developing life pattern', 'Restoring health'. These processes were within 'Restoring Health Equilibrium', core category. Consequently, the participants revealed in restoring relationship-oriented life and settling in family-focused life. Conclusions: These findings have implications for the understanding of health promotion for the elderly with chronic diseases and contributing to make a basic data for developing elderly health management program.

The Physical & Psychosocial Functioning of Korean Immigrant Elderly in Northwest America

  • The growing population of the elderly who have diverse health needs is receiving more attention from the health promotion field. This paper describes some of the findings of quantitative study into the health function of Korean immigrant elderly living in Washington State of Northwest America. One hundred-two community-dwelling Korean immigrant elderly, aged from 65 to 93 with a mean 77.97 years, were interviewed with a structured questionnaire to report their health function defined in three domains; perceived health status, physical functioning, and psychosocial functioning. The main results of the study were identified: (a) Overall, Korean immigrant elderly's perceived health status and physical functioning were good, but psychosocial functioning was poor; (b) Elderly with higher health functioning scores perceived better health status. Data indicates that health functioning measures are good indicators of the degree of perceived need for immigrant elderly. The paper argues that the understanding of physical and psychosocial functioning of elderly is vitally important in the provision of health care.

Integrating market chain assessments with zoonoses risk analysis in two cross-border pig value chains in Lao PDR

  • Objective: Lao PDR's recent accession to the World Trade Organization necessitates a greater understanding of the patterns and risk of livestock production in order to better align national policy with the Agreement on the Application of Sanitary and Phytosanitary Measures. This eco-health study was conducted to improve understanding of the interrelations between market chains and zoonotic infection risks at two strategic cross border points between Lao PDR, Thailand and Viet Nam. Methods: Information gained from smallholder farmer/trader interviews was integrated with serological surveys for pig-associated zoonoses-including hepatitis E virus (HEV), Taenia solium (T. solium) and trichinella-to identify potential linkages between disease risk and pig production and slaughter in low input systems common across the country. Results: Trichinella and HEV exposure was high in both humans and pigs in both study areas, significantly associated with pig slaughter and the subsequent consumption and handling of raw pork products. T. solium demonstrated a strong geographical and ethnic association with the northern study area bordering Vietnam. With the right knowledge and accessible, affordable inputs, the majority of smallholder farmers indicated a willingness to invest more in pig production, which could simultaneously improve livelihoods and decrease exposure to HEV, Trichinella, and T. solium through increased access to formal markets and an improved slaughter processes. Conclusion: The linkages identified when assessing disease risk in the context of potential economic and cultural drivers of transmission highlight the importance of a systems-based approach for the detection and control of zoonotic disease, and contributes to an improved understanding of the Lao PDR livestock sector.

A Review of the Types and Characteristics of Healthy Life Expectancy and Methodological Issues

  • An index that evaluates the health level of a population group considering both death and loss of function due to disease is called a summary measure of population health (SMPH). SMPHs are broadly divided into life year indices and life expectancy indices, the latter of which comprise healthy life expectancy (HLE). HLE is included as a policy target in various national and regional level healthcare plans, and the term "HLE" is commonly used in academia and by the public. However, the overall level of understanding of HLE-such as the precise definition of HLE and methods of calculating HLE-still seems to be low. As discussed in this study, the types of HLE are classified into disability-free life expectancy, disease-free life expectancy, quality-adjusted life expectancy, self-rated HLE, and disability-adjusted life expectancy. Their characteristics are examined to facilitate a correct understanding and appropriate utilization of HLE. In addition, the Sullivan method, as a representative method for calculating HLE, is presented in detail, and major issues in the process of calculating HLE, such as selection of the population group and age group, estimation of death probability, calculation of life years, and incorporation of health weights, are reviewed. This study will help researchers to select an appropriate HLE type and evaluate the validity of HLE research results, and it is expected to contribute to the vitalization of HLE research.
