Localized Pretibial Varicose Vein Caused by an Intraosseous Venous Anomaly

  • A 36-year-old man presented to the hospital with protruding blood vessels in his left lower leg accompanied by cramping. An ultrasonographic examination of the leg revealed focal reflux without truncal vein reflux. During phlebectomy, the varix was found to be connected to the intraosseous vein through a tibial opening. Postoperative computed tomography and magnetic resonance imaging showed an osteolytic lesion in the tibial shaft and an intraosseous vascular anomaly. The patient was discharged without complications and scheduled for periodic follow-ups. This young man's varicose vein seemed to be from a tibial intraosseous vascular anomaly, which is extremely rare.

  • The gallbladder is a pear-shaped, thin walled sac located on the inferior surface of the liver between the right and quadrate lobe, in a recess called fossa vesicae felleae. It is 7.5cm-12.5 cm in length, 3.5 cm in the largest width, and has a volume of about 45 ml with a remarkable capacity for expansion. There are many kinds of diagnostic methods to evaluate diseases of biliary tract including gallbladder-Plain abdomen, Oral cholecystography. Intravenous cholangiography, Percutaneous transhepatic cholangiography(PTC), Endoscopic retrograde cholangiography(ERCP), Operative or T-tube cholangiography, Ultrasonography, Radioisotope study, Computed tomography, and Angiography. Especially, ultrasonography is the most effective and noninvasive study in these days. Plain abdomen, oral cholecystography, intravenous cholangiography, and ultrasonography have been performed in our clinic. Methods and findings of above mentioned study are discussed with consideration of references.

  • Ultrasonography is a powerful and useful method for the examination of the various shoulder diseases. The use of high-resolution transducer and technical evolution allowed the improvement of the accuracy of detection of various shoulder pathology including the rotator cuff disease. However, its limitation is that there is marked disparity in the interpretation according to the operators' experience. This article describes the appropriate scan technique of ultrasonography around shoulder.

Hepatic Encephalopathy in a Dog (개의 간성 뇌증)

  • Hepatic encephalophthy was diagnosed with serum chemistry, abdominal radiography and ultrasonography in a 2.6kg, 4 year-old maltese dog showing signs of hypersalivation, involutary spasm of facial muscles, ataxia, behavior abnormalities like dullness, sleep disorder, restlessness. In serum chemistry, the level of alanine transferase and aspartate trasferase was mildly elevated, ammonia was severely increased. On abdominal radio-graphs, the size of liver was mildly decreased. In ultrasonographic examination, diffuse lesion with hyperechoic change and decreased vasculature were seen in the hepatic region. But vascular abnormalities of liver were not observed. Drug and dietary therapy were undertaken and severities of clinical sign were alleviated.

  • Eleven years old cocker spaniel referred to Veterianry Medical Teaching Hospital, Seoul National University was diagnosed as pheochromocytoma. The patient was transferred to the hospital after hit by car and the main clinical signs were tachypnea, tachycardia, edema of hindlimb and lethargy. On ultrasonographic examination, hyperechoic foci was detected and it was invaded into the caudal vena cava. Venography showed the filling defect and local dilation of the caudal vena. The diagnosis of pheochromocytoma is challenging due to the nonspecific clinical signs and severe concurrent disease in dogs. This report describes one dog with invasive pheochromocytoma in which radiography, ultrasonography and venacavography were key diagnositc tools. Ultrasonography was a usuful method for assessment of size, shape and margination of the tumor and invasiveness to the major vessels.

Finasteride therapy in a dog with benign prostatic hyperplasia

  • Benign prostatic hyperplasia (BPH) is frequently observed in intact middle-aged or older male dogs with symptoms ranging from asymptomatic to various symptoms such as urination, dyschezia, dysuria, tenesmus, and intermittent serosanguinous or clear serous exudate. A 4-year-old, intact male Welsh corgi with symptoms of intermittent hematuria and serosanguinous exudate at the urethral opening was diagnosed with BPH and intraprostatic hematocyst through ultrasonographic examination and cytology test. The dog was internationally adopted for reproductive purposes and needed to undergo BPH treatment while maintaining fertility. Thus, 2.5 mg finasteride was administered orally once a day for treatment option and therapeutic effects were observed, including loss of clinical symptoms and reduced prostate and intraparenchymal cyst size.

Infiltrative Lipoma Invading the Anal Sac in a Dog: A Rare Case Report

  • A 6-year-old female Chihuahua was presented to the Animal Medical Center for surgical resection of a perineal mass that had recently increased in size. Ultrasonography revealed a large, homogeneous fatty mass with irregular margins between the surrounding muscle layers due to infiltrating fatty tissues. Cytological findings from fine-needle aspirates revealed numerous sheets and clusters of adipocytes, which was consistent with the fatty mass. Based on ultrasonographic and cytological findings, an infiltrative lipoma was suspected. During the surgery, the perineal mass was found to be non-encapsulated, irregularly marginated, and extensively distributed into the surrounding muscles. The mass extended inside the pelvic cavity and left anal sac. The perineal mass was surgically removed and submitted for histopathologic examination. Histopathology confirmed that the mass was an infiltrative lipoma invading into the anal sac and surrounding muscles. The present report was an unusual presentation of infiltrative lipoma that invaded the anal sac in the perineum.

Ultrasonographic Appearance of the Gestational Structures Throughout Pregnancy in Shih-tzu bitches II. Estimation of Gestational Age by Measurement of the Fetal and Extra-fetal Structures (Shih-tzu견에서 임신일령에 따른 임신구조물의 초음파상 II. 태아 및 태아외구조물의 측정에 의한 임신일령의 추정)

  • Serial ultrasonographic examination was daily performed on 12 pregnant Shih-tzu bitches from day 15 until parturition to determine the size of gestational structures. Gestational age was timed from the day of ovulation (day 0), which was estimated to occur when plasma progesterone concentration was first increased above 4.0 ng/ml. Extra-fetal structures were measurable from days 17 to 46. Outer uterine diameter increased from $9.9{\pm}0.4$ mm (Mean${\pm}$SD) at day 20 to $44.3{\pm}0.8$ mm at day 46 and inner chorionic cavity diameter increased from $7.2{\pm}0.2$ mm at day 17 to $36.9{\pm}1.5$mm at day 41. Length of chorionic cavity or zonary placenta increased from $6.6{\pm}0.4$ mm at day 21 to $46.4{\pm}0.9$ mm at day 44. Of the extra-fetal structures, inner chorionic cavity diameter was the most accurate for estimation of gestational age until day 38. Fetal structures were measurable from days 23 to 60. Crown-rump length increased from $3.9{\pm}0.3$ mm at day 23 to $55.2{\pm}3.3$ mm at day 41, fetal body diameter increased from $7.9{\pm}0.6$ mm at day 32 to $47.8{\pm}2$ mm at day 60, and fetal head diameter increased from $6.3{\pm}1.1$ mm at day 29 to $25.6{\pm}0.2$ mm at day 60. Of the fetal structures, fetal head diameter was the most accurate for estimation of gestational age from day 38 until day 60.

  • Taking history and ultrasonography is important to detect foreign body which might be located in the soft tissue. We encountered a case of soft tissue foreign body (transobturator tape) in a patient who had been treated with the impression of adductor myositis resulting from vagina fistula. The foreign body was found through ultrasonographic examination and the patient's symptom was relieved after removal of the foreign body from the ultrasonography-guided operation. Clinicians should keep in mind the fact that foreign bodies may exist when the inflammation persists without appreciable cause and ultrasonography can be useful tool in case of small fibrous band not detected with magnetic resonance imaging due to inflammatory change of the surrounding tissues.
