Two spaniel dogs were presented with a history of lethargy, weakness, refractory hypoglycemia, and episodic seizure. A pancreatic mass on the distal end of the left limb was identified through screening tests including dualphase CT angiography in each patient. Canine insulinoma was suspected with low blood glucose, low fructosamine, and normal to high serum insulin levels. Exploratory laparotomies were performed, and partial pancreatectomy using an ultrasonic-activated scalpel was achieved without enzyme leakage or fistula formation after surgery. The lesions were definitively diagnosed as insulinoma via histopathologic examination. One dog has been doing well until POD 870 with medication; in the other dog, diffuse hepatic nodules noted during surgery were confirmed as a liver metastasis. This dog died at POD 45. This case report addresses the utility of the ultrasonic scalpel in partial pancreatectomy for the treatment of canine insulinoma and differences in prognosis according to disease stage.

This report presents a rare case of feline exocrine pancreatic ductal carcinoma including treatment and prognosis. A 4.6-year-old castrated male American Shorthair cat, weighing 5 kg was presented with intermittent vomiting, diarrhea and loss of appetite. Through physical examination and radiography, a cranial abdominal mass was identified. Exploratory laparotomy was carried out. The largest mass was connected to the left lobe of the pancreas, and there were several small, nodular masses scattered along the abdominal lining. The pancreatic mass was removed using an ultrasonic scalpel, and the patient recovered favorably. Histopathologically, the resected lesion was diagnosed as an exocrine pancreatic ductal papillary carcinoma. The patient was maintained with conservative therapy and euthanized on post-operative day 262. Partial pancreatectomy using an ultrasonic scalpel was accomplished safely without evidence of pancreatic leakage.

Background Capsular contracture is a common complication of two-stage expander/implant breast reconstruction. To minimize the risk of this complication, capsulectomy is performed using monopolar cautery or ultrasonic surgical instrumentation, the latter of which can be conducted with a Harmonic scalpel. To date, there is disagreement regarding which of the two methods is superior. The purpose of this study was to compare postoperative outcomes between a group of patients who underwent surgery using a Harmonic scalpel and another group treated with monopolar cautery. Methods A retrospective chart review was conducted of patients who underwent capsulectomy as part of two-stage breast reconstruction between January 2018 and February 2019 and who received at least 1 month of follow-up after surgery. Operative time and postoperative outcomes, including drainage duration, were analyzed. Results In total, 36 female patients underwent capsulectomy. The monopolar group consisted of 18 patients and 22 breasts, while the Harmonic scalpel group consisted of 18 patients and 21 breasts. There was no statistically significant difference in demographics between the two groups. The Harmonic scalpel group had a significantly shorter mean drainage duration (6.65 days vs. 7.36 days) and a smaller mean total drainage volume (334.69 mL vs. 433.54 mL) than the monopolar cautery group (P<0.05). No statistically significant difference was observed with regard to seroma or hematoma formation. Conclusions The Harmonic scalpel approach for capsulectomy reduced the total drainage volume and drainage duration compared to the monopolar cautery approach. Therefore, this approach could serve as a good alternative to electrocautery.
The study aimed at determining the feasibility of three portal totally Laparoscopic cryptorchidectomy in small breed dogs. Four right sided and one left sided abdomen retained testicles in five dogs weighing 2.2~8.7 kg were retrieved successfully. Pampiniform plexus and vas deference were ligated and transected with Hem-o-lock/monopolar electrocauterization (1/5), or ultrasonic scalpel (4/5) and the latter provided more easy, quick and safe hemostasis and transection. The resected testis was easily removed through a insertion site. Mean surgical time was $19.9{\pm}15.2$ minutes and when the abdominal cavity was explored again, there were no major complications. The patient recovered favorably without unwilling events. This is the case report of laparoscopic cryptorchidectomies in small breed dogs weigh less than 10 kg. Laparoscopic cryptorchidectomy could be considered as an alternative in small animal surgery.
A 1-year-old, 2.35 kg spayed female American short hair cat was referred with episodic signs of heat at 3 months after ovariohysterectomy. Through the screening tests, bilateral cystic, ovary like masses were shown at the caudal to both kidneys with high serum estradiol concentration. It was considered that the patient was suffered from ovarian remnant syndrome. Laparoscopic exploration was performed, and each of mass lesions was resected by ultrasonic scalpel. Patient was recovered favorably and has been doing well without recurrence of estrus signs until 2 years after surgery.
A 12-year-old spayed, female Siberian Husky dog weighing 40 kg was referred for sudden collapse without a traumatic history. A splenic mass and hemoperitoneum were identified by ultrasonography and abdominocentesis at a local animal hospital. Emergency exploratory laparotomy was performed once the patient was stabilized. A splenic mass and engorged hepatic mass in the torsed left liver lobe were identified. Partial hepatic lobectomy of the left lateral lobe was performed using a laparoscopic linear self-cutting stapler and the spleen was also removed using an ultrasonic scalpel. Histopathologic examination revealed a diagnosis of splenic hemangiosarcoma with metastasis to the liver. Chemotherapy was not performed because the client did not give consent. The patient was checked regularly until death on POD 324. The metastatic nodules had spread to the lungs and other liver lobes and had gradually increased in size. Liver lobe torsion is a rare condition in dogs, especially accompanying neoplasia. This case supports a potential relationship between liver lobe torsion and neoplastic liver mass in dogs.
Aim: To examine lymph nodes obtained after lipolysis and liposuction of subcutaneous fat of the inguinal region of female vulvar cancer patients to explore the feasibility of clinical application. Methods: The field of operation was on the basis of the range of the conventional resection of inguinal lymph nodes. We injected lipolysis liquid fanwise, started liposuction after 15-20 minutes; then the subcutaneous fatty tissue was sucked out clearly by suction tube. We selected the first puncture holes located on 2-3 cm part below anterior superior spine, the others respectively being located 3cm and 6cm below the first for puncturing into the skin, imbedding a trocar to intorduce $CO_2$ gas and the specular body, and excise the lymph nodes by ultrasonic scalpel. The surgical field chamber was set with negative pressure drainage and was pressured with a soft saline bag after surgery. Results: A lacuna emerged from subcutaneous of the inguinal region after lipolysis and liposuction, with a wide fascia easily exposed at the bottom where lymph nodes could be readily excised. The number of lymph nodes of ten patients excised within the inguinal region on each side was 4-18. The excised average number of lymph nodes was 11 when we had mature technology. Conclusion: Most of adipose tissue was removed after lipolysis and liposuction of subcutaneous tissue of inguinal region, so that the included lymph nodes were exposed and easy to excise by endoscope. This surgery avoided the large incision of regular surgery of inguinal region, the results indicating that this approach is feasible and safe for used as an alternative technology.
