  • We present a graphical algorithm and theorems for calculating the ranks of reaction networks. The ranks are needed to study behaviors of the networks from their structures. This approach can graphically simplify complex networks for the calculation. We show an example of a large network for the practical advantage.

  • Tuberous sclerosis complex (TSC, MIM#191100) is an autosomal dominant neurocutaneous syndrome caused by mutation or deletion of TSC1 encoding hamartin or TSC2 encoding tuberin and characterized by seizure, mental retardation, and multiple hamartomas or benign tumors in the skin, brain, retina, heart, kidney, and lungs. The TSC2 gene on chromosome 16p13.3 lies adjacent to the PKD1 gene which is responsible for autosomal dominant polycystic kidney disease (MIM#173900). The TSC2/PKD1 contiguous gene syndrome (TSC2/PKD1 CGDS, MIM#600273) is caused by deletion of both TSC2 and PKD1 gene. We recently experienced a 15 month-old boy and a 26 month-old girl with TSC2/PKD1 CGDS confirmed by multiplex ligation-dependent probe amplification (MLPA) analysis. They showed not only typical neurologic manifestations of TSC such as epilepsy, subependymal nodules, and subcortical tubers, but also polycystic kidney disease. The contiguous gene syndrome involving PKD1 and TSC2 should be suspected in children with enlarged polycystic kidneys and TSC. MLPA analysis is a useful method for the genetic confirmation of TSC2/PKD1 CGDS.

Prolonged blepharoptosis following repeated stellate ganglion block in a patient with brachial plexopathy after thoracoscopic surgery (흉강경 수술 후 상완신경병증 환자에서 반복된 성상신경절블록 후 발생한 지속적인 안검하수)

  • A 34-year-old female was suffered from pain and numbness of right arm for 2 months after undergoing a thoracoscopic procedure for a posterior mediastinal mass that was diagnosed as neurilemmoma. The patient was diagnosed as a complex regional pain syndrome type 2 with brachial plexopathy developed during thoracoscopic excision of posterior mediastinal mass, and stellate ganglion block (SGB) with 0.2% ropivacaine 10 mL was performed every 3-4 days. The patient revealed slightly prolonged blepharoptosis as Horner syndrome accompanied after every SGB and recovered. However, following the 23rd SGB, the blepharoptosis persisted and patient was recovered spontaneously from blepharoptosis after about 12 months. The possibility that the persistent blepharoptosis might be caused by brachial plexopathy related to patient's pathology or surgical manipulation and/or repeated SGB. If Horner syndrome occurs, its etiology should be assessed, and it would be necessary to explain and to assure the patient the possibility of recovery spontaneously from the complication within a year, without any sequelae.

A Study on the Hydraulic Pump/Motor Control in the Flywheel Hybrid Vehicle

  • In this study, a novel hybrid vehicle is proposed. The vehicle has a flywheel-engine hybrid system. Flywheels are more effective as energy charge systems than electric batteries in a respect of output power density. However, transmissions to effectively drive flywheels are very complex systems such as CVTs (Continuously Variable Transmissions). In the proposed hybrid vehicle, Constant Pressure System is employed, which is hydraulic power transmission. Using Constant Pressure Systems, hydraulic CVTs are easily realized with variable displacement pumps/motors. In this paper, firstly, the proposed flywheel hybrid vehicle making use of Constant Pressure System is described. Secondly, fuel consumption characteristics of the flywheel hybrid vehicle are experimentally examined with the stationary test facility, which employs a flywheel as a load emulating vehicle inertia. Finally, the experimental results and discussions are described. Fuel consumption of 26km/L is expected for 10 mode driving schedule with vehicle mass of 1500kg.

  • Bone marrow stroma is a complex tissue encompassing a number of cell types and supports hematopiesis, differentiation of erythreid, nyel and lymphoid lineages, and also maintains undifferentiated hematopoietic stem cells. Marrow-derived stem cells were composed of two populations, namely, hematopoietic stem cells that can differentiate into blood elements and mesenchymal stem cells that can give rise to connective tissues such as bone, cartilage, muscle, tendon, adipose and stroma. Differentiation requires environmental factors and unique intracellular signaling. For example, $TGF-{\beta}$ or BMP2 induces osteoblastic differentiation of mesenchymal stem are very exciting. However, the intrinsic controls involved in differentiation of stem cells are yet to be understood properly in order to exploit the same. This review presents an overview of the recent developments made in mesenchymal stem cell research with respect to osteogenesis.

Development of 'De novo' Aneurysm after Therapeutic Carotid Occlusion

  • Carotid occlusion is an inevitable therapeutic modality for the treatment of complex aneurysms such as giant, traumatic, and intracavernous aneurysms. Late complications of carotid occlusion include 'de novo' aneurysm formation at a distant site because of hemodynamic changes in the circle of Willis. We report a case of de novo aneurysm in a vessel that appeared to be normal on initial angiography. The patient developed an anterior communicating artery aneurysm and marked growth of a basilar bifurcation aneurysm 9 years after trapping of the left internal carotid artery for the treatment of a ruptured large saccular aneurysm involving ophthalmic and cavernous segments. We propose that patients who undergo therapeutic carotid occlusion should be periodically followed by magnetic resonance angiography or computed tomographic angiography to evaluate the possibility of de novo aneurysm formation; this advice is in line with previous reports.

  • Syncope is defined as a transient loss of consciousness and postural tone, characterized by rapid onset, short duration, and spontaneous recovery. Stellate ganglion block (SGB) is a nerve block method that is used for treatment of neuropathic pain in the head, neck and upper extremities, especially trigeminal neuralgia, postherpetic neuralgia and complex regional pain syndrome. SGB can modulate and stabilize the sympathetic nervous system, which prevents it from overexcitation and improves symptoms of syncope. The authors report a patient who was treated for pain and edema of both upper extremities with SGB, then showed improvement in recurrent syncope followed by chest pain and overall quality of life.

  • Major industrial accidents such as fires, explosions and toxic releases have occurred in Ulsan, Yeosu petrochemical complex area every year. In this study, we analyzed the causes of the previous major accidents in order to provide reasonable measures for safety regulations and process safety management. Consequently, It is necessary to make constant revisions of safety regulations and to improve process safety management in other to prevent major industrial accidents in the petrochemical complex areas.

Surgical Outcome of Intractable Seizure with Space-Occupying Lesion in Temporal Lobe (측두엽내 공간 점유 병소와 동반된 난치성 간질의 수술적 치료 성적)

  • Objective : The increasing use of sensitive neuroimaging techniques has demonstrated that significant percentage of patients with intractable complex partial seizures have brain masses, especially in temporal lobe. The optimal surgical solution for these patients is still open to debate. The purpose of our investigation is to evaluate the surgical outcome of patient with lesion-related temporal lobe epilepsy with respect to the types of surgery and the location of lesion. Patients and Methods : From DEC. 1993 to Dec. 1997, 35 patients with intractable epilepsy and space occupying temporal lobe lesion identified in preoperative MRI were included in this study. The types of surgery were lesionectomy, anterior temporal lobectomy with or without hippocampectomy. The location of lesion was divided as anteromedial group and lateral cortical group. The postoperative seizure outcomes according to the type of surgery and location of the lesion were compared. Results : Twenty-six of 34 patients(76.5%) were seizure-free after surgery. The Engel's class was favorable after anterior temporal lobectomy with or without hippocampectomy(p=.044) Conclusion : It is favorable to perform anterior temporal lobectomy for the treatment of intractable epilepsy with space-occipying lesion in temporal lobe. The resection of the hippocampus can be individualized.

Analysis of Lateral Meniscus Injury - Non-discoid versus Discoid - (외측 반월상 연골 손상의 분석 - 비원판형과 원판형의 비교 -)

  • Purpose: To know about the clinical characteristics such as causes, types, associated injuries and treatment of the lateral meniscus injuries. Materials and Methods: Retrospective analysis was performed for 129 lateral meniscus tears which were divided into 2 groups, Non-discoid and Discoid. Analysis included age and sex distribution, causes of injury, types and location of tear, associated injuries, and methods of treatment. Results: Both Non-discoid and Discoid were prevalent in man and Discoid group was equally distributed through 2nd to 5th decades and Non-discoid group was more prevalent in 4th to 5th decades. Sports injury was the most common cause in both groups and the Nondiscoid group tended to sustain minor injuries compared to Discoid group. Complex tear and body was most common type and site of tear in both groups. Associated injuries were common in Non-discoid group and isolated injuries were common in Discoid group. Partial meniscectomy was commonly done in Non-discoid group and subtotal meniscectomy in Discoid group based on the types of tear. Conclusion: Non-discoid and Discoid lateral meniscus injuries are similar in types and sites of injuries but different in causes of injury, associated injuries and methods of treatment. Therefore, proper diagnosis and treatment would be necessary according to the characteristics.
