Repair of Coarctation (including tubular hypoplasia) in Infancy and Children (영아 및 소아 연령에서의 대동맥 교약증의 교정 수술)

  • We have experienced 44 cases of coarctation of aorta in the age of infancy and children from April 1986 to September 1989 at Seoul National University Children`s hospital. Patients were thirty males and fourteen females, and their age ranged from one month to ten years[mean 23.84 $\pm$33.06 months] with thirty-two infant cases. In the infantile age, congestive heart failure was the most common chief complaint[18/32], and above that age, frequent upper respiratory infection was most common[8/12]. We experienced thirteen cases of isolated COA, twenty-two cases of COA with VSD, eight cases of COA with VSD, eight cases of COA with intracardiac complex anomalies and one case of COA with atrial septal defect. The associated intracardiac complex anomalies were three Taussig-Bing type double outlet right ventricle, one single ventricle, one transposition of great arteries, one atrioventricular septal defect, one hypoplastic aortic arch with left heart hypoplasia, and one Tetralogy of Fallot. Operative techniques of COA were twenty-three subclavian flap arterioplasty, 12 resection and end to end anastomosis, eight onlay patch angioplasty, and I direct angioplasty after resection of web. Among the cases with other cardiac anomalies, staged operation was done in twenty-nine patients, and single stage total correction was performed only in three patients. There were seven operative mortality[15.9%], all being in infantile age group, and among fourteen cases associated with large VSD[Qp/Qs>2.0, mean pulmonary arterial pressure>50mmHg], four patients were died, but there was no mortality in patients with small VSD. With above results, we are intended to discuss about the interval between staged operation, the fate of VSD after coarctoplasty in case of COA with VSD, causes of death, complications etc.

RFJ: A Reliable and Fast Journaling Mechanism (RFJ: 신뢰적 고성능 데이터 버퍼 저널링 기법)

  • Modern file systems have journaling mechanism to maintain their stored state consistently even under unexpected system crashes or disasters. However, the journaling makes I/O throughput lower. This performance degradation comes from the ordering mechanism between the data buffer and metadata buffer and two-staged buffer writing. Especially, if the data buffer and metadata buffer are journalled at the same time, then it incurs significant performance degradation due to the two-staged writing. That shows the trade-off relation-ship between I/O performance and system reliability. In this paper, we propose RFJ: a reliable and fast jour-naling mechanism to deal with this trade-off relationship. We propose an ordering enforced writeback journaling mode and selective journaling mechanism. The Ordering enforced writeback journaling mode achieves low I/O latency and the selective journaling mechanism achieves high reliability. The experimental result shows that the performance of RFJ is almost 5x faster than the journal mode of Ext3 file system but it still supports the same reliability with the journal mode.

Sequential Assessment in Contests among Common Freshwater Goby, Rhinogobius brunneus(Pisces, Gobiidae)

  • The sequential assessment model describes a fight between two conspecific as an ongoing statistical sampling process, which makes it possible to predict fight length or repetition number of a behavioral element depending on relative RHP (resource holding potential: e.g. weight or fighting ability). We staged contests between males of common freshwater gobies to test some predictions of this model. Fights proceeded in a consistent sequence of phases. Most contests began with two contestants adopting lateral display, and then escalated to intense physical contacts. The length of contests was negatively correlated with weight difference between the contestants. The duration of complete phases was, however, independent of weight, and the prior information gained during complete phases did not appear to affect subsequent phases of the fight. Our results show that the contests of common freshwater gobies are well predicted by the sequential assessment model.

Surgical Management of Coronary Artery Disease Combined with Carotid Artery Stenosis -A Report of Two Cases- (경동맥 협착을 동반한 관상동맥 질환의 수술적 치료 -2례 보고-)

  • The optimal surgical approach to the patients with coronary artery disease combined with carotid artery stenosis is controversial. We report two cases of successful surgical management of the patients with combined obstructive coronary and carotid artery disease. The first case was a 69-year-old female who had unstable angina pectoris and a past medical history of left carotid endarterectomy. She was revealed to have triple vessel coronary disease and nearly total occlusion of right internal carotid artery. She was undergone staged right carotid endarterectomy 10 days before coronary bypass surgery. The second case, a 54-year-old male with a past medical history of left hemiparesis and dysarthria, was admitted due to unstable angina pectoris. He was revealed to have triple-vessel coronary disease and more than 90% stenosis of left internal carotid artery and 50% stenosis of right internal carotid artery. In the latter case, a combined coronary bypass surgery and left carotid endarterectomy was done. In both cases, postoperative neurologic complications were not observed.

  • Cerebral paragonimiasis is an important parasitic infestation found in Eastern Asia, West Africa, and Central/South America. It is frequently associated with abscess, granuloma or cystic lesions in chronic stage. Chronic symptoms include seizure, visual disturbance, movement and perception disorders, usually resulting from the spaceoccupying lesions. The authors report a case of chronic cerebral paragonimiasis combined with meningioma. A 69-year-old man presented with generalized seizure and impaired consciousness. CT scan and MR image of the brain revealed two intracranial lesions ; one was cerebral paragonimiasis in the left temporo-occipital lobe, and the other a huge falx meningioma. Two lesions were successfully removed in staged operations and the generalized seizure was controlled after removal of the cerebral paragonimiasis.

Median Sterontomy for Bilateral Resection of Emphysematous Bullae (정중 흉골 절개술을 통한 양측의 기종성 폐포의 절제)

  • The complicated pulmonary emphysema including “Giant bullae” and spontaneous pneumothorax often involve both lungs, and controversy exists concerning which is the more rational means of surgical treatment-bilateral simultaneous operation or two staged operation. We report three cases of the complicated bilateral bullous emphysema and two cases of bilateral spontaneous pneumothorax treated through median sternotomy. We performed the ligation of bullae, bullectomy, cystectomy, wedge resection, and left lower lobectomy through median sternotomy. No technical problems were encountered through this approach, which provided maximum benefit with one operation In conclusions, median sternotomy may be appropriate for resection of emphysematous bullae, specially in a severe COPD patient who may be poorly tolerated the superimposed loss of respiratory function due to incisional pain, because median sternotomy permit bilateral exploration, minimal impairment of pulmonary function, simultaneous restoration of pulmonary function, less incisional pain than routine lateral thoracotomy.

Creating Amnesia Synonyms in Traditional Korean Medicine : Database Utilization (한의학 고전 문헌 데이터베이스에서 활용할 건망 유의어 연구)

  • The aim of this study is to create a catalogue of amnesia synonyms in Traditional Korean Medicine for database utilization. A two-staged literature search was carried out. First, two databases(China National Knowledge Infrastructure:CNKI, Oriental Medicine Advanced Searching Integrated System:OASIS) were searched for eligible articles, and a set of candidate words was identified from the articles. Second, the candidate words were searched in 30 medical classics to check the frequency of use. As a result, 9 candidate words including 喜忘(huimang), 善忘(seonmang), 多忘(damang), 好忘(homang), 健忘(geonmang), 遂忘(sumang), 遺忘(yumang), 忘事(mangsa), and 易忘(imang) were identified from the 10 eligible articles. Among the 9 candidate words, 健忘(geonmang) was a descriptor and 7 other words including 喜忘(huimang), 善忘(seonmang), 多忘(damang), 好忘(homang), 遂忘(sumang), 遺忘(yumang), 忘事(mangsa) were non-descriptors. 易忘(imang) was not an adequate synonym for amnesia.

  • Knee revision arthroplasty following peri-prosthetic joint infections is a formidable challenge. Patients are at a high risk of recurrent infection, and the soft tissue over the revised implant is often of questionable quality. Flap reconstruction has improved the salvage rates of infected arthroplasties, and should be considered in all cases of revision arthroplasty. We present a challenging case requiring staged reconstruction with two free latissimus dorsi flaps after the initial use of a medial gastrocnemius flap.

  • A modified VIStA (Vortex Inertial Staged Air) burner has been developed and applied to a once-through type boiler. The secondary air is supplied through a swirler instead of nozzles, which stabilizes the flame and reduces carbon monoxide (CO) emissions. However, the modification increases the emission of nitrogen oxides (NOx). To balance emissions of the two pollutants, a divided flame was adopted. An air damper was installed to control the distribution of air to each combustion chamber, and three types of flame dividers were studied. The effects of the air-fuel ratio and combustion load on the NOx formation were investigated. The divided flame was found to reduce the NOx emission up to 25%, while keeping the CO to less than 10 ppm.

Process Optimization of Red Pepper Drying for the Improvement of Drying Efficiency (건조효율 향상을 위한 고추건조공정의 최적화)

  • Drying process in a fixed bed red pepper dryer was modeled and simulated. Drying efficiency describing the effectiveness of energy usage in red pepper drying was defined as a ratio of energy used for moisture evaporation to total energy consumption, and expressed in combination of measurable temperature variables. The efficiency was compared with real evaporative efficiency and tested in the simulated and experimental drying. An overall drying efficiency was derived, and analyzed for various control variables consisting of drying temperature, air recycle ratio and air flow rate. Optimal operation conditions of drying was then searched by Box's complex method by using it as an objective function. Carotenoids retention was simulated and put as a constraint of product quality in the optimization. The optimization results gave that two staged drying operation could improve the ding efficiency compared with single staged drying. As a technique for further energy saving automatic termination of drying appeared feasible by monitoring an exit air temperature from dryer.
