나이와 뇌실질부피 변화 및 혈관이상에 따른 총뇌혈류량 변화: 이차원 위상대조 자기공명영상을 이용한 연구 (Changes in Total Cerebral Blood Flow with Aging, Parenchymal Volume Changes, and Vascular Abnormalities: a Two-dimensional Phase-Contrast MRI Study)

  • 목적 : 이차원 위상대조 자기공명영상을 이용하여 나이변화와 뇌실질 부피변화 및 혈관이상의 정도에 따른 총뇌혈류량의 변화를 알고자 하였다. 대상 및 방법 : 12명의 지원자를 포함한 73명을 대상으로 T2강조 영상과 Time-of-flight 방법의 자기공명혈관촬영과 이차원 위상대조 자기공명영상을 얻었다. 정상군은 지원자 12명과 자기공명영상 및 자기공명혈관촬영에서 정상소견을 보인 21명으로서 이들은 18-29세, 30-49세, 50-66세 군으로 나누었다. 비정상군은 T2강조영상의 뇌실질부피 변화정도와 자기공명 혈관촬영의 동맥경화 정도에 따라 mild reduction군(17명) , marked reduction군(12명)으로 나누었고, 뇌실질이 증가한 increased volume군(6명)과 Moya-moya군(5명)으로 분류하였다 뇌혈류는 위상대조 자기공명영상의 속도-혈류 곡선으로부터 양쪽 내경동맥과 추골동맥에서 측정하고 합하여 뇌의 총뇌혈류량으로 하였으며, 각군 사이의 혈류량을 비교 관찰하였다. 결과 정상군의 총뇌혈류량은 18-29세군은$12.0{\pm}2.1ml/sec$, 30-49세군은 $11.8{\pm}1.9m1/sec$, 50-66세군은 $10.9{\pm}2.2ml/sec$였다. 비정상군 중에서 mild reduction 군은 $9.5{\pm}2.5ml/sec$, marked reduction 군은 $7.6{\pm}2.0ml/sec$, increased volume군은 $7.3{\pm}1.2ml/sec$, Moya-moya군은 $7.0{\pm}1.1ml/sec$였다. 결론 : 총뇌혈류량은 나이 증가에 따라 감소하였고, 뇌실질부피 감소와 동맥경화 정도에 따라 감소하였으며 increased volume군과 Moya-moya군에서도 감소하였다. 이차원 위상대조 자기공명영상은 나이변화나 뇌실질의 부피변화와 혈관이상을 초래하는 다양한 뇌질환에서 총뇌혈류량을 관찰할 수 있는 유용한 방법이라고 생각된다.

  • This article reviews the papers published in the Korean Journal of Air-Conditioning and Refrigeration Engineering during 2014. It is intended to understand the status of current research in the areas of heating, cooling, ventilation, sanitation, and indoor environments of buildings and plant facilities. Conclusions are as follows. (1) The research works on the thermal and fluid engineering have been reviewed as groups of heat and mass transfer, cooling and heating, and air-conditioning, the flow inside building rooms, and smoke control on fire. Research issues dealing with duct and pipe were reduced, but flows inside building rooms, and smoke controls were newly added in thermal and fluid engineering research area. (2) Research works on heat transfer area have been reviewed in the categories of heat transfer characteristics, pool boiling and condensing heat transfer and industrial heat exchangers. Researches on heat transfer characteristics included the results for thermal contact resistance measurement of metal interface, a fan coil with an oval-type heat exchanger, fouling characteristics of plate heat exchangers, effect of rib pitch in a two wall divergent channel, semi-empirical analysis in vertical mesoscale tubes, an integrated drying machine, microscale surface wrinkles, brazed plate heat exchangers, numerical analysis in printed circuit heat exchanger. In the area of pool boiling and condensing, non-uniform air flow, PCM applied thermal storage wall system, a new wavy cylindrical shape capsule, and HFC32/HFC152a mixtures on enhanced tubes, were actively studied. In the area of industrial heat exchangers, researches on solar water storage tank, effective design on the inserting part of refrigerator door gasket, impact of different boundary conditions in generating g-function, various construction of SCW type ground heat exchanger and a heat pump for closed cooling water heat recovery were performed. (3) In the field of refrigeration, various studies were carried out in the categories of refrigeration cycle, alternative refrigeration and modelling and controls including energy recoveries from industrial boilers and vehicles, improvement of dehumidification systems, novel defrost systems, fault diagnosis and optimum controls for heat pump systems. It is particularly notable that a substantial number of studies were dedicated for the development of air-conditioning and power recovery systems for electric vehicles in this year. (4) In building mechanical system research fields, seventeen studies were reported for achieving effective design of the mechanical systems, and also for maximizing the energy efficiency of buildings. The topics of the studies included energy performance, HVAC system, ventilation, and renewable energies, piping in the buildings. Proposed designs, performance performance tests using numerical methods and experiments provide useful information and key data which can improve the energy efficiency of the buildings. (5) The field of architectural environment was mostly focused on indoor environment and building energy. The main researches of indoor environment were related to the evaluation of work noise in tunnel construction and the simulation and development of a light-shelf system. The subjects of building energy were worked on the energy saving of office building applied with window blind and phase change material(PCM), a method of existing building energy simulation using energy audit data, the estimation of thermal consumption unit of apartment building and its case studies, dynamic window performance, a writing method of energy consumption report and energy estimation of apartment building using district heating system. The remained studies were related to the improvement of architectural engineering education system for plant engineering industry, estimating cooling and heating degree days for variable base temperature, a prediction method of underground temperature, the comfort control algorithm of car air conditioner, the smoke control performance evaluation of high-rise building, evaluation of thermal energy systems of bio safety laboratory and a development of measuring device of solar heat gain coefficient of fenestration system.

역할 창조를 위한 '몸틀(body schema)' 형성 연구 (A Study on Forming 'Body Schema' for Role Creating)

  • Formation of 'body schema' is the start for actor to create role and becomes the root and the foundation of existing as a role on the stage. For this, an actor needs to form 'scheme of role' with escaping from own 'body schema.' 'Schema of role' is formed by acquiring through synthesizing daily basic actions, namely, walking, standing, sitting, hand stretching, bending, and touching. The body schema, which was made with simple and usual actions, has fundamental significance in a sense of becoming the body in which the past traces in a role are habituated while energy as a role flows. As for the process of forming body schema, an actor first needs to obtain the visualized materials like photo, magazine, picture and image available for seeing a role specifically and clearly based on what analyzed a character. An actor needs to have three-dimensional image available for always recalling it in the head during acting. To do this, image data available for fundamentally capturing routine actions along with body structure are still more useful. Next, the body schema is formed by interaction with environment. Thus, there is a need of passing through the two-time process of forming body schema. Firstly, the body schema is made on routine actions in a role as physical condition of a role in actor's own everyday life. Secondly, the body schema is made on routine actions available for moving efficiently and economically in line with the environment of performance. A theatrical stage is the temporal space of rhythm and rule different from routine space. What forms body schema immediately in the second phase without body schema in the first phase ultimately becomes what exists as actor's own body, not the body of a role. The body schema, which was formed as the second process, is what truly has identity as a role in the ontological aspect, comes to experience the oppositional force in muscle, a qualitative change in energy, and emotional agitation in the physical aspect, and experiences perception, thinking, volition, and even consciousness with the entire body in the cognitive dimension. Thus, the formation of body schema can be known to be just a method of changing even spiritual and emotional layer. Body schema cannot be made if there is no process of embodiment and habit. Embodiment and habit are not simply the repeated, empty and mechanical action in the body. But, habit itself has very important meanings for forming body schema for role creating. First, habit allows the body itself to learn and understand a meaning. Second, habit relies upon environment, thereby allowing an actor of making the habituated body schema to recognize environment. Third, habit makes the mind. The habituated body schema is just the mind and the ego of a person who possesses the body schema. Fourth, habit comes to experience the expansion in energy and the expansion in existence. It may be experienced through interrelation among actor's body, tool, and environment. Fifth, habit makes identity of the body. Hence, this just becomes what secures identity of a role. These implications of habit are the formation of body schema, which is maintained with the body of being remembered firmly through being closely connected with the process of neural adaptation. Finally, it sought for possibility of practice as one method of forming body schema for role creating through Deleuze's '-becoming' theory. As 'actual animal-becoming' is real '-becoming' of forming structural transformation in the physical dimension, it meets with what the formation of body schema pursues actuality and reality. This was explained with a concept as saying of 'all '-becoming' molecular' by Deleuze/Guattari. 'Animal of having imitated animal's characteristic- becoming' is formed by which the body schema relies upon environment. In this way, relationship among the body, tool and environment has influence even upon a change in consciousness, thinking, and emotion, thereby being able to be useful for forming body schema in a sense of possibly experiencing ultimately expansion in role, namely, expansion in existence.

북서태평양에서 열대 저기압 발생빈도의 십년간 변동 특성 (Interdecadal Variation of Tropical Cyclone Genesis Frequency over the Western North Pacific)

  • 본 연구에서는 북서태평양상의 기후학적 평균 열대 저기압 발생 자료를 이용하여 이 해역의 북서구역과 남동구역에서의 열대 저기압 발생빈도의 시간변화에서 십년간 변동성을 보이고 반대 위상으로 존재함을 밝혔다. 열대 저기압 발생빈도는 북서구역에서 1980년대 중반 이후부터 최근까지(1986-2005) 증가하는 반면에, 남동구역에서 1951년부터 1970년대 초반(1951-1970)까지 높게 나타났다. 1986-2005년과 1951-1970년 사이의 차이점을 분석한 결과는 다음과 같다. i) 연직바람시어와 해수면 온도의 경우, 1986-2005년 동안에는 북서구역에서, 1951-1970년의 기간에는 남동구역에서 음의 연직시어와 양의 해수면 온도 아노말리를 나타내고 있었다. ii) 열대 저기압 이동빈도를 분석해 본 결과, 북서구역에서 발생한 열대 저기압들 중에서 1986~2005년 동안에는 주로 남중국해에 영향을 주는 빈도가 높았던 반면, 1951-1970년 동안에는 필리핀 동쪽 해상으로부터 동중국해를 지나 한국 및 일본을 통과하는 빈도가 높았다. 남동구역에서 발생한 열대 저기압들의 경우에 대략 $150^{\circ}E$를 기준으로 이동빈도의 공간분포가 동서로 나눠질 수 있다. 1951-1970년 동안의 열대 저기압들은 150oE의 서쪽에서, 1986-2005년 동안의 열대 저기압들은 $150^{\circ}E$의 동쪽에서 주로 이동하는 특성을 보였다. 특히, 1951-1970년 동안에 발생한 열대 저기압들은 필리핀의 동쪽 해상으로부터 남중국해 및 남중국 방향으로 이동하는 빈도가 높았다. iii) 두 기간 사이에 열대 저기압 이동빈도의 이러한 차이는 500 hPa 아노말리 기압패턴에 그 원인이 있다. 특히 1951-1970년 동안에는 동아시아 대륙에 저기압성 순환의 아노말리가 형성되어 필리핀 동쪽해상으로부터 동아시아 해안을 따라 남풍 아노말리가 강화되었다. 이 남풍 아노말리는 열대 저기압이 필리핀 동쪽해상으로부터 동아시아 해안을 따라 이동하기 쉽게 하는 지향류의 역할을 하였다.

새로운 간호윤리학 방법론;통합된 사례방법론 (An integrated Method of New Casuistry and Specified Principlism as Nursing Ethics Methodology)

  • The purpose of the study was to introduce an integrated approach of new Casuistry and specified principlism in resolving ethical problems and studying nursing ethics. In studying clinical ethics and nursing ethics, there is no systematic research method. While nurses often experience ethical dilemmas in practice, much of previous research on nursing ethics has focused merely on describing the existing problems. In addition, ethists presented theoretical analysis and critics rather than providing the specific problems solving strategies. There is a need in clinical situations for an integrated method which can provide the objective description for existing problem situations as well as specific problem solving methods. We inherit two distinct ways of discussing ethical issues. One of these frames these issues in terms of principles, rules, and other general ideas; the other focuses on the specific features of particular kinds of moral cases. In the first way general ethical rules relate to specific moral cases in a theoretical manner, with universal rules serving as "axioms" from which particular moral judgments are deduced as theorems. In the seconds, this relation is frankly practical. with general moral rules serving as "maxims", which can be fully understood only in terms of the paradigmatic cases that define their meaning and force. Theoretical arguments are structured in ways that free them from any dependence on the circumstances of their presentation and ensure them a validity of a kind that is not affected by the practical context of use. In formal arguments particular conclusions are deduced from("entailed by") the initial axioms or universal principles that are the apex of the argument. So the truth or certainty that attaches to those axioms flows downward to the specific instances to be "proved". In the language of formal logic, the axioms are major premises, the facts that specify the present instance are minor premises, and the conclusion to be "proved" is deduced (follows necessarily) from the initial presises. Practical arguments, by contrast, involve a wider range of factors than formal deductions and are read with an eye to their occasion of use. Instead of aiming at strict entailments, they draw on the outcomes of previous experience, carrying over the procedures used to resolve earlier problems and reapply them in new problmatic situations. Practical arguments depend for their power on how closely the present circumstances resemble those of the earlier precedent cases for which this particular type of argument was originally devised. So. in practical arguments, the truths and certitudes established in the precedent cases pass sideways, so as to provide "resolutions" of later problems. In the language of rational analysis, the facts of the present case define the gounds on which any resolution must be based; the general considerations that carried wight in similar situations provide warrants that help settle future cases. So the resolution of any problem holds good presumptively; its strengh depends on the similarities between the present case and the prededents; and its soundness can be challenged (or rebutted) in situations that are recognized ans exceptional. Jonsen & Toulmin (1988), and Jonsen (1991) introduce New Casuistry as a practical method. The oxford English Dictionary defines casuistry quite accurately as "that part of ethics which resolves cases of conscience, applying the general rules of religion and morality to particular instances in which circumstances alter cases or in which there appears to be a conflict of duties." They modified the casuistry of the medieval ages to use in clinical situations which is characterized by "the typology of cases and the analogy as an inference method". A case is the unit of analysis. The structure of case was made with interaction of situation and moral rules. The situation is what surrounds or stands around. The moral rule is the essence of case. The analogy can be objective because "the grounds, the warrants, the theoretical backing, the modal qualifiers" are identified in the cases. The specified principlism was the method that Degrazia (1992) integrated the principlism and the specification introduced by Richardson (1990). In this method, the principle is specified by adding information about limitations of the scope and restricting the range of the principle. This should be substantive qualifications. The integrated method is an combination of the New Casuistry and the specified principlism. For example, the study was "Ethical problems experienced by nurses in the care of terminally ill patients"(Um, 1994). A semi-structured in-depth interview was conducted for fifteen nurses who mainly took care of terminally ill patients. The first stage, twenty one cases were identified as relevant to the topic, and then were classified to four types of problems. For instance, one of these types was the patient's refusal of care. The second stage, the ethical problems in the case were defined, and then the case was analyzed. This was to analyze the reasons, the ethical values, and the related ethical principles in the cases. Then the interpretation was synthetically done by integration of the result of analysis and the situation. The third stage was the ordering phase of the cases, which was done according to the result of the interpretation and the common principles in the cases. The first two stages describe the methodology of new casuistry, and the final stage was for the methodology of the specified principlism. The common principles were the principle of autonomy and the principle of caring. The principle of autonomy was specified; when competent patients refused care, nurse should discontinue the care to respect for the patients' decision. The principle of caring was also specified; when the competent patients refused care, nurses should continue to provide the care in spite of the patients' refusal to preserve their life. These specification may lead the opposite behavior, which emphasizes the importance of nurse's will and intentions to make their decision in the clinical situations.
