가와사키병 환아에서 면역글로불린 투여 전후의 혈중 Nitric Oxide와 Tumor Necrosis Factor-α에 대한 연구 (Serum Nitric Oxide and Tumor Necrosis Factor-α Levels in Patients with Kawasaki Disease)

  • 목 적 : 가와사키병은 면역 조절 인자의 이상을 동반하는 전신적 혈관염으로 관상동맥질환을 초래한다. NO는 혈관내 평활근의 granulocyte cyclase의 기전에 영향을 미쳐 혈관의 이완을 유발하는 역할을 하며 과다하게 분비될 경우 혈관의 변성을 초래하는 것으로 알려져 있다. 본 저자들은 가와사키병에서 NO와 $TNF-{\alpha}$의 혈중 농도를 측정하여 관상동맥질환 발생과 연관이 있는지 알아보기 위해 본 연구를 실시하였다. 방 법 : 가와사키병 환아 24명을 관상동맥 확장이 없는 군(1군)과 관상동맥 확장이 있는 군(2군)으로 분류하여 각 군의 임상 양상과 면역글로불린 투여 전과 후, 회복기에서의 NO, $TNF-{\alpha}$의 혈중 농도를 면역효소법(ELISA)으로 측정하여 비교하였다. 대조군으로는 같은 시기에 내원한 열이 없는 정상 대조군(3군) 13명과 열이 있는 대조군(4군) 10명으로 하였다. 결 과 : 면역글로불린 투여 전의 혈중 NO는 1군($13.2{\pm}5.7{\mu}mol/L$), 2군($20.4{\pm}10.7{\mu}mol/L$)과 4군($12.4{\pm}8.9{\mu}mol/L$)이 3군($3.1{\pm}1.4{\mu}mol/L$)보다 높았고 2군이 1군과 4군에 비해 유의하게 높았다(P<0.05). $TNF-{\alpha}$는 2군($858.4{\pm}934.0pg/mL$)에서 3군($8.7{\pm}2.3pg/mL$)과 4군($226.7{\pm}647.2pg/mL$)에 비해 유의하게 높았으며 1군($522.4{\pm}859.6pg/mL$)도 3군에 비해 높았다(P<0.05). 면역글로불린 투여 후 NO는 1군, 2군과 4군이 3군에 비해 유의하게 높았으며 $TNF-{\alpha}$는 각 군별로 유의한 차이가 없었다. 가와사키병 관상동맥 확장군과 비확장군 모두에 있어 NO와 $TNF-{\alpha}$의 혈중 농도가 면역글로불린 투여 전에 가장 높았고 면역글로불린 투여 후와 회복기로 갈수록 감소하였다. 또한 관상동맥 확장군에서 백혈구 수치와 혈청 NO는 유의한 양의 상관관계가 있었다(r=0.430). 결 론 : 가와사키병 환자에 있어 NO, $TNF-{\alpha}$의 혈중 농도가 유의하게 높았으며 NO의 농도가 관상동맥이 확장된 환자에서 비 확장군보다 의미있게 높은 것으로 보아 NO가 관상동맥질환에 관여할 것으로 생각한다.

Effects of Ethyl Pyruvate on Allodynia, TNF-${\alpha}$ Expression, and Apoptosis in the Dorsal Root Ganglion after Spinal Nerve Ligation Injury

  • Background: It has been demonstrated that the expression of tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) and apoptotic cell death in the dorsal root ganglion (DRG) following spinal nerve constriction injury play a role in the initiation and continuation of hyperalgesia and allodynia. The present study was designed to investigate the effects of ethyl pyruvate (EP) on mechanical and cold allodynia, TNF-${\alpha}$ expression, and apoptosis in DRG after spinal nerve ligation injury. Methods: Rats were divided into 3 groups: control, pre-EP, and post-EP. EP (50 mg/kg) was intraperitoneally injected 30 minutes before (pre-EP) or after (post-EP) surgery. Behavioral tests to determine mechanical and cold allodynia were conducted before surgery and 4 and 7 days after surgery. Seven days after surgery, TNF-${\alpha}$ protein levels in DRG were evaluated by enzyme-linked immunosorbent assay, and DRG apoptosis was determined by immunohistochemical detection of activated caspase-3. Results: Treatment with EP significantly reduced mechanical and cold allodynia following spinal nerve ligation injury. TNF-${\alpha}$ protein levels in the pre-EP ($4.7{\pm}1.2$ pg/200 ${\mu}g$; P < 0.001) and post-EP ($6.4{\pm}1.8$ pg/200 ${\mu}g$; P < 0.001) groups were 2-3 times lower than the control group ($14.4{\pm}1.2$ pg/200 ${\mu}g$). The percentages of neurons and satellite cells that co-localized with caspase-3 were also significantly lower in the pre-EP and post-EP groups than the control group. Conclusions: These results demonstrate that EP has a strong anti-allodynic effect that acts through the inhibition of TNF-${\alpha}$ expression and apoptosis in DRG after spinal nerve ligation injury.

미세변화신증후군 환아에서 Tumor Necrosis Factor-${\alpha}$의 혈중 및 요중 변화와 알부민 투과성에 미치는 영향 (Changes of Plasma and Urinary $TNF-{\alpha}$ in Children with Minimal Change Nephrotic Syndrome and Its Role in Albumin Permeability)

  • 목적 : 미세변화신증후군은 소아 원발성 신증후군의 가장 흔한 원인이다. 최근 $TNF-{\alpha}$가 미세변화신증후군의 병인기전과 밀접한 관계가 있다고 보고되고 있다. 이에 저자들은 미세변화신증후군 환아에서의 $TNF-{\alpha}$의 변화를 살펴보고 $TNF-{\alpha}$가 사구체 기저막의 투과성에 미치는 직접적인 영향을 알아보기 위하여 본 연구를 시행하였다. 방법 : 대상 환아는 신생검으로 미세변화신증후군이 확진된 만 2-15세 사이의 소아로서 이들에게서 혈액과 요를 채취하여 ELISA 방법으로 $TNF-{\alpha}$를 측정하였고, Millicell system을 사용하여 알부민에 대한 투과성을 측정하였다. 결과 : 재발시에 요중 $TNF-{\alpha}$는 대조군과 관해시에 비하여 유의하게 증가되어 있었으나(P<0.01), 혈중 $TNF-{\alpha}$는 유의한 변화를 보여주지 않았다. Millicell system을 이용한 알부민 투과성에 대한 실험결과 생리적인 농도 이상의 $TNF-{\alpha}$에서 알부민에 대한 투과성에 특이한 변화가 관찰되지 않았다. 결론 : 이상의 결과를 종합할 때 $TNF-{\alpha}$는 미세변화신증후군의 발병기전에서 일차적인 역할을 한다기 보다는 아마도 질환자체에 의한 이차적인 현상에 의하여 요중에서 증가된 것으로 생각된다.

  • 급성전골수구성 백혈병(Acute promyelocytic leukemia, APL)은 혈액암의 일종으로 치료의 성적이 좋지 않을 뿐 아니라 항암요법과 병행 하였을 경우 큰 효과를 보이는 것으로 알려져 있는 방사선 치료를 병행함에도 불구하고 정상세포에도 작용하여 부작용을 초래한다. 본 연구에서는 이러한 부작용을 감소시키기 위하여 감마선을 $TNF-{\alpha}$와 같이 처리하였을 경우 정상세포와 암세포의 세포 죽음에 어떠한 영향을 미치는지 확인하였다. HL-60 세포는 APL 세포주로서 사용하였고 DMSO를 처리하여 분화시킨 HL-60 세포는 정상과립구의 성질을 나타내어 정상대조군으로 이용하였다. 그 결과 $TNF-{\alpha}$와 함께 감마선을 처리한 HL-60 세포에서만 세포독성효과를 나타내었고 세포자멸사를 유도하여 세포가 죽음에 이르게 하였다. 결론적으로 $TNF-{\alpha}$는 항암치료의 부작용을 없애기 위해 저농도 감마선 치료 시 함께 사용하여 암 세포의 제거를 증가시켜 암의 치료효율을 높일 수 있는 유효물질로 사료된다.

Ethanol extract of Callophyllis japonica enhances nitric oxide and tumor necrosis factor-alpha production in mouse macrophage cell line, RAW 264.7 cells

  • Red seaweed (Callophyllis japonica) has long formed part of the diet of Asians, but the pharmacological properties of this plant have not been evaluated. In this study, we examined the effect of an ethanol extract of C. japonica on the generation of nitric oxide (NO) in RAW 264.7 cells. The C. japonica extract increased the generation of NO and tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$), which were detected by the Griess method and an enzyme-linked immunosorbent assay, respectively. The increased production of NO by C. japonica extract was inhibited by $N^G$-monomethyl-L-arginine ($100{\mu}M$), a specific inhibitor of NO production in the L-arginine-dependent pathway, and by the nuclear $factor-{\kappa}B$ ($NF-{\kappa}B$) inhibitor, pyrrolidine dithiocarbamate ($10-100{\mu}M$) in a dose-dependent manner. These findings demonstrate that C. japonica extract stimulates the production of NO and $TNF-{\alpha}$ in RAW 264.7 cells through the activation of $NF-{\kappa}B$ and that this extract might also inhibit the growth of the human leukemic cells.

Tumor Necrosis Factor-α 238 G/A Polymorphism and Risk of Hepatocellular Carcinoma: Evidence from a Meta-analysis

  • Background: Tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) plays a very important role in the development and progression of cancer. Many epidemiological studies have evaluated associations between the TNF-${\alpha}$ 238 G/A polymorphism and hepatocellular carcinoma (HCC) risk, but the published data are inconclusive. Therefore, we performed the present meta-analysis. Methods: Electronic searches of several databases were conducted for all publications on the association between TNF-${\alpha}$ 238 G/A polymorphism and HCC through July 2012. Asummary odds ratio (OR) with its 95% confidence interval (CI) were calculated to evaluate the strength of this association. Results: Eleven case-control studies with a total of 1,572 HCC cases and 1,875 controls were finally included in this meta-analysis. Overall, the TNF-${\alpha}$ 238 G/A polymorphism was significantly associated with increased risk of hepatocellular carcinoma in three genetic comparison models (For A versus G: OR 1.32, 95%CI 1.04-1.69, P = 0.02, $I_2$ = 40%; for AG versus GG: OR 1.32, 95%CI 1.02-1.71, P = 0.03, $I_2$ = 40%; for AA/AG versus GG: OR 1.33, 95%CI 1.03-1.72, P = 0.03, $I_2$ = 41%) when all studies were pooled. Subgroup analysis by ethnicity further showed that there was a significant association between the TNF-${\alpha}$ 238 G/A polymorphism and risk of HCC in Asians under three genetic comparison models (For A versus G: OR 1.30, 95%CI 1.00-1.68, P = 0.05, $I_2$ = 45% for AA/AG versus GG: OR 1.31, 95%CI 1.00-1.71, P = 0.05, $I_2$ = 46%). Conclusions: This meta-analysis provided convincing evidence that the TNF-${\alpha}$ 238 G/A polymorphism is associated with increased susceptibility to HCC. However, more well-designed studies with large sample size are needed to validate this association in Caucasians.

  • 본 연구는 염증상태에서의 CLA의 효과와 작용기전을 알아보기 위해 LPS-자극 RAW 264.7 macrophages에 있어 ROS 생성과 TNF-${\alpha}$ 생산, NF-${\kappa}B$$PPAR{\gamma}$ 활성을 검토하였다. t10c12-CLA는 LPS로 자극하지 않은 비염증시의 RAW 세포에서는 ROS 생성을 증가시켜 TNF-${\alpha}$ 생산을 유도하였으며, 이 효과는 $PPAR{\gamma}$ 활성화에 의존해서 NF-${\kappa}B$ 활성 증가에 의해 매개되었다. 반면, LPS로 자극한 염증조건의 RAW 세포에서는 t10c12-CLA가 $PPAR{\gamma}$ 활성화에 의존하지 않는 경로로 ROS 생성 및 과도한 TNF-${\alpha}$ 생산을 억제하였다. 본 결과로부터 CLA는 ROS 생성을 통해 TNF-${\alpha}$ 생산 및 NF-${\kappa}B$ 활성을 염증 유무에 따라 조절하는 것으로 사료되었다.

  • One of attractive target for Rheumatoid Arthritis (RA) therapy is the cytokine, tumor necrosis factor-alpha (TNF-$\alpha$), which has been shown to be overproduced in the joint of RA patients. The clinical success of anti- TNFR biologics has validated TNF-$\alpha$ as a drug discovery target. Thus, inhibiting of formation of TNF-$\alpha$ has been emerged to an intriguing approach for RA therapy. Mometasone Furoate Suppresses PMA-Induced MUC-5AC and MUC-2 Production in Human Airway Epithelial Cells

  • Background: Mucus hypersecretion from airway epithelium is a characteristic feature of airway inflammatory diseases. Tumor necrosis factor ${\alpha}$ (TNF-${\alpha}$) regulates mucin synthesis. Glucocorticoids including mometasone fuorate (MF) have been used to attenuate airway inflammation. However, effects of MF on mucin production have not been reported. Methods: Effects of MF and budesonide (BUD) on the phorbol-12-myristate-13-acetate (PMA)-induction of mucin and TNF-${\alpha}$ in human airway epithelial cells (NCI-H292) were investigated in the present study. Confluent NCI-H292 cells were pretreated with PMA (200 nM) for 2 hours. Subsequently, the cells were stimulated with MF (1-500 ng/mL) or BUD (21.5 ng/mL) for 8 hours. Dexamethasone ($1{\mu}g/mL$) was used as the positive control. Real-time polymerase chain reaction was used to determine MUC2 and MUC5AC mRNA levels. The level of total mucin, MUC2, MUC5AC, and TNF-${\alpha}$ in culture supernatants were measured using enzyme-linked immunosorbent assay. Results: MF and BUD significantly suppressed MUC2 and MUC5AC gene expression in PMA-stimulated NCI-H292 cells. The inhibitory effects of the two steroid drugs were also observed in the production of total mucin, MUC2 and MUC5AC proteins, and TNF-${\alpha}$. Conclusion: Our findings demonstrated that MF and BUD attenuated mucin and TNF-${\alpha}$ production in PMA-induced human airway epithelial cells.
