The Effect of 3-Dimensional Posture Correction Exercise Program Incorporating PNF Respiratory Muscle Strengthening Exercise on the Spinal Alignment and the Trunk Expansion in Patients with Idiopathic Scoliosis

  • Objective: The purpose of this study was to investigate the effect of 3-dimensional posture correction(3DPC) exercise program incorporating PNF respiratory muscle strengthening(RMS) exercise on spinal alignment and trunk expansion in patients with idiopathic scoliosis and the difference in intervention effect by type of idiopathic scoliosis. Design: Comparative study using paired t-test and analysis of variance measures. Methods: The subjects of this study were 5 men and 24 women diagnosed with idiopathic scoliosis with a Cobb angle greater than 10 degrees and less than 45 degrees. A 3DPC exercise program incorporating PNF RMS exercise was conducted twice a week for 1 hour per session for 6 weeks, and Cobb angle, Angle of trunk rotation, and trunk expansion were measured before and after the intervention. Results: After the intervention, Cobb angle and trunk rotation angle showed a statistically significant decrease, and trunk expansion showed a statistically significant increase. In the effect difference by type, there was a statistically significant difference in the angle of trunk rotation of the thoracic in 3C and Single Lumbar. Conclusions: In conclusion, it is thought that a 3DPC exercise program incorporating PNF RMS exercise can be effectively used in clinical practice to improve spinal alignment and trunk expansion in patients with idiopathic scoliosis.

시각적 피드백을 이용한 전신 기울기 운동이 급성기 뇌졸중 환자의 체간 조절, 근력 및 균형에 미치는 효과: 무작위 대조군 예비연구 (Effects of Whole Body Tilt Exercise with Visual Feedback on Trunk Control, Strength, and Balance in Patients with Acute Stroke: a Randomized Controlled Pilot Study)

  • PURPOSE: This study was conducted to determine the effects of whole-body tilt exercise with visual feedback on trunk control, strength, and balance in patients with acute stroke. METHODS: The study included 18 patients with acute stroke who were randomly allocated to a Whole Body Tilt Exercise Group (WBTEG, N=9) and a General Trunk Exercise Group (GTEG, N=9). The WBTEG conducted whole-body tilt exercises with visual feedback, and the GTEG performed general trunk exercises. Both groups performed the exercises five times a week for 4 weeks. Outcomes were assessed using the trunk impairment scale (TIS), the trunk flexor and extensor strength test, the static balance test, and the Brunel Balance Assessment. RESULTS: After 4 weeks of intervention, both groups showed significantly improved TIS scores, muscle strength, and balance components (p<.05 in both groups). However, the improvement in TIS, muscle strength, and static balance in the WBTEG was significantly better than that in the GTEG (p<.05). CONCLUSION: Although both groups in this study showed post-intervention improvement, patients from the WBTEG who received visual feedback demonstrated more improvement. These findings indicate that whole-body tilt exercise with visual feedback may be effective at improving trunk control, trunk muscle strength, and balance in patients with acute stroke. Further studies are needed to gain a better understanding of the effectiveness of whole-body tilt exercise in patients with acute stroke.

복부 드로잉-인 기법이 평지 보행 시 몸통과 다리의 근 활성도에 미치는 효과 (Effects of Abdominal Drawing-in Maneuver on Muscle Activity of the Trunk and Legs during Flat Walking)

  • PURPOSE: This study examined the difference in muscle activity of the trunk and legs during flat walking with or without an abdominal drawing-in maneuver. METHODS: This study was conducted on 15 healthy males and eight females who were attending D University in Busan. This experiment was conducted after 15 minutes of abdominal drawing-in training using a pressure biofeedback unit before the experiment, and the difference in the muscle activity of the trunk and legs during flat walking with or without an abdominal drawing-in technique was investigated. Surface electromyography was used, and the electrode attachment site was the right sternocleidomastoid muscle, splenius capitis muscle, rectus abdominis muscle, external abdominal oblique muscle, transverse abdominis muscle, erector spinae muscle, vastus medialis muscle, and vastus lateralis muscle (TM DTS, Noraxon, USA). The data were analyzed statistically using a paired t-test on SPSS version 18.0 (IBM). RESULTS: The muscle activity of the rectus abdominis muscle, external abdominal oblique muscle, transverse abdominis muscle, vastus medialis muscle were increased significantly and maintained more than walking without maintaining an abdominal drawing-in maneuver (p < .05). Moreover, muscle activity of the erector spinae muscle was decreased significantly and maintained more than walking without maintaining an abdominal drawing-in maneuver (p < .05). CONCLUSION: Maintaining an abdominal drawing-in maneuver during flat walking is more effective during walking training.

운동 자세에 따른 몸통 들기 운동 시 가슴과 허리 폄 근육의 근활성도 비교 (Comparison of Muscle Activities of Thoracic and Lumbar Extensors during Trunk Lift Exercises According to Exercise Position)

  • The purpose of this study was to compare the muscle activities of the thoracic extensor(TE) and lumbar extensor(LE) during trunk lift (TL) exercise according to exercise position. Seventeen healthy subjects with no medial history of back pain were recruited for this study. Subjects performed the TL exercise in prone, quadruped and heel-sitting positions. The activities of the TE and LE were measured using surface electromyography during TL exercise in each exercise position. A one-way repeated-measures analysis of variance (ANOVA) was used to compare the normalized muscle activities of the TE and LE and the TE/LE ratio. The results showed that there was not significant effect of exercise position on the muscle activities of TE(p>.05). However, there was significant effect of exercise position on the muscle activities of LE and the TE/LE ratio(p>.05). Post hoc pair-wise comparisons with Bonferroni correction showed that both muscle activities of LE and TE/LE ratio in prone position were significantly different in those in heel-sitting and quadruped positions, during TL exercise, respectively. The TE/LE ratio was the greatest for TL exercise in heel-sitting position. Therefore, for selective activation of the TE muscle, we recommend performing the TL exercise in heel-sitting or quadruped position.

The Effect of Plank Exercises with Hip Abduction Using Sling on Trunk Muscle Activation in Healthy Adults

  • Purpose: The purpose of this study is to investigate the effects of healthy persons performing a plank exercise with hip abduction and use of sling on trunk muscle activity. Methods: Twenty-three subjects participated in this study. Muscle activities of the multifidus, rectus abdominis, external oblique and internal oblique were assessed with electromyography (EMG) while the subjects performed 6 various types of plank exercises in random order. Results: There was a significant difference in multifidus and internal oblique on the supported side and the multifidus, external oblique, and internal oblique on the elevated side (p<0.05), and differences in plank methods were significantly higher in the multifidus, rectus abdominis, external oblique, internal oblique of the supported side and the rectus abdominis, external oblique and internal oblique on the elevated side (p<0.05). In addition, post-hoc analysis of the various plank methods showed that muscle activity was significantly higher during the general plank exercise compared with the knee-supported-in-sling plank exercises (p<0.05), and was significantly higher with the ankle-supported-in-sling plank exercises compared with the general plank exercise. Conclusion: There was greater trunk muscle activity with plank exercises performed with the ankle-supported-in-sling and hip abduction plank exercises compared with the general or knee-supported-in-sling plank exercises. Through this study, various plank exercises have been discovered that can be selected at different intensities for core muscle training purposes based on trunk muscle strength.

경직성 양쪽다리 뇌성마비의 무릎관절 형태와 보행기 종류에 따른 근활성도 비교 (Comparison of Muscle Activations on Knee Joint Forms and Walker Types in Cerebral Palsy of Spastic Diplegia)

  • Purpose : The purpose of this study is to compare muscle activations of neck, trunk and leg in cerebral palsy of spastic diplegia with genu recurvatum and knee flexion contracture, when using anterior and posterior walkers. Methods : We selected 21 cerebral palsy and received the written consent to participate in this study. The inclusion criteria for participation required patients to have spastic diplegic CP; to be between 3~6 years of age, to have a GMFCS III grade, to have no botulinum toxin injection and orthopedics surgery within before six months starting the study. Measurements of muscle activities (sternocleidomastoid, splenius capitis, rectus abdominis, erector spinea, gluteus maximus, rectus femoris, medial hamstring and calf muscles) were evaluated anterior and posterior walker ambulations. Statistical evaluation of these data were accomplished by utilizing the paired t-test and independent t-test by SPSS 20.0 program. Significance level was set at p<.05. Results : The following results were obtained. There was significant difference on muscle activation of neck, trunk and legs(soleus except) in anterior and posterior walkers. There was no significant difference in muscle activation of neck but significant difference in muscle activation of trunk, legs between genu recurvatum and knee flexion contracture(rectus abdominis, medial hamstring when using anterior walker, rectus abdominis, erector spinea, gluteus maximus, medial hamstring when using posterior walker). Conclusion : The conclusion of this study is the different knee joint forms would have different effect on muscle activation of trunk and legs while cerebral palsy of spastic diplegic ambulated with anterior walker and posterior walker.

호흡운동을 병행한 몸통 근력운동과 심부 안정화 운동이 배근육 두께와 호흡에 미치는 영향 (Effect of Trunk Strength Exercise and Deep Stabilization Exercise Combined with Breathing Exercise on Abdominal Muscle Thickness and Respiration)

  • Purpose : The purpose of this study is to compare the effects on abdominal muscle thickness and breathing by applying trunk strength exercise and deep stabilization exercise along with breathing exercise, which is the main respiratory muscle during breathing, to present an efficient exercise method with diaphragm breathing. Methods : This study was performed on normal 6 females and 14 males subjects. They were divided into 2 groups which trunk strength exercise and deep stabilization exercise group. The trunk strength exercise group (TSE) attended prone press-up, crunch and pelvic tiling. The deep stabilization exercise group (DSE) attended abdominal drawing, horizontal side-support and bridging exercise. Breathing exercise was performed for each set break time for 1 minute. Results : First, in the comparison of the change in the thickness of the abdominal muscle between the trunk strength training group and the deep stabilization group before and after exercise, there was a statistically significant difference in the comparison of transverse abdominis (TrA), rectus femoris (RF), external oblique (EO), internal oblique (IO) (p<.05). However, there was no significant difference in any comparison between groups (p>.05). Second, in the comparison of changes in respiratory function between the trunk strength exercise group and the deep stabilization exercise group before and after exercise, there were statistically significant differences in the exerted forced vital capacity (FVC), forced expiratory volume at one second (FEV1), peak expiratory flow (PEF) in the comparison before and after the experiment (p<.05). However, there was no significant difference in any comparison between groups (p>.05). Conclusion : As a result of this study, it can be said that both trunk strength exercises and deep stabilization exercises along with diaphragm breathing are exercises that strengthen deep and superficial muscles, and have a positive effect on breathing function as well as muscle strength. However, it is not known which exercise was more effective, and because it was combined with breathing exercise, the interference effect appeared.

필라테스 호흡이 체어 동작에서 몸통 안정화 근육의 활성도에 미치는 영향 (Effect of Pilates Breathing on the Activity of Trunk Stabilizer Muscles during the Movements of Pilates Chair Exercise)

  • Purpose: This study aimed to investigate the changes in the muscle activity of the trunk stabilizer muscles before and after incorporating Pilates breathing during three types of Pilates chair exercises. Methods: This study included 33 healthy men and women in their 20's; they were recruited according to the inclusion and exclusion criteria. sEMG was used to measure the changes in the muscle activity in the internal oblique/transverse abdominis, rectus abdominis, and erector spinae during the three types of Pilates chair movements (footwork, twist footwork, and bridging) without and with the Pilates breathing integration. The muscle activities of the trunk stabilizers between without and with Pilates breathing were statistically analyzed and compared. Results: The internal oblique/transverse abdominis showed an increase and a significant difference in the muscle activity in all three movements of footwork, twist footwork, and bridging after the Pilates breathing integration (p<.001). The muscle activity of the rectus abdominis (p<.05) and the erector spinae (p<.05) also increased and showed a significant difference after the Pilates breathing incorporation, except in the bridging movement for the erector spinae. The increase in the rate after integrating Pilates breathing was relatively greater in internal oblique/transverse abdominis than in other muscles. Conclusion: When Pilates breathing was applied, the activities of the trunk stabilizer muscles increased significantly and immediately in all three movements of Pilates chair footwork, twist footwork, and bridging. This means that the use of breathing is expected to have a positive and immediate effect on the activation of trunk stabilizers, thus indicating that it can possibly be an effective re-enforcing tool to promote trunk stability when it is integrated to the Pilates chair exercise. Incorporating Pilates breathing also seemed to have a tendency to activate the deep trunk stabilizer muscles more than the superficial stabilizer muscles.

교각운동이 월경곤란증으로 인한 허리통증을 가진 여대생에게 미치는 영향 (The Effect of Bridging Exercise on Dysmenorrhea Back Pain of Women University Students)

  • Purpose : This study determines the effect and sustainability of the trunk stability exercise program, specifically using the bridge exercise, by measuring Visual Analogue Scale(VAS), Menstrual Distress Questionaire(MDQ) and muscle strength in women university students complaining of low back pain. Methods : The trunk stability exercise program was progressed by 22 women university students, for 4 weeks, (3 times a week, 1 hour per session). At this time, low back pain happened at the period of menstruation and was measured using VAS, Dysmenorrhea using the MDQ tool and abdominal and back muscle strength using HUR. This study was evaluated 4 times (before and after the exercise program, after 4 weeks and 8 weeks) to investigate the effect of time variation after the exercise program. Results : The trends showed a decrease in VAS and MDQ and an increase in abdominal and back muscle strength. Therefore, these results were evaluated positively from a statistical perspective(p<.05). Conclusions : The trunk stability exercise results in a decreased VAS and MDQ, and increases the muscle strength in women university students who complained of low back pain form dysmenorrhea. Therefore, the low back pain from dysmenorrhea could have been avoided and quality of everyday life improved.
