Comparison of trunk muscle thickness according to the type of feedback during spinal stabilization exercise in standing posture

  • Objective: Patients with low back pain can possibly have impaired core muscle function, which is the common cause of low back pain. Spinal stabilization exercises are recommended for prevention and reinforcement. This study aimed to compare the effects of different types of feedback on abdominal and lumbar multifidus (LM) muscle recruitment during spinal stabilization exercises. Design: Cross-sectional study. Methods: Fifty-seven healthy subjects (sex=male 21/female 36, age=21.28±1.60 years) were divided into three different groups: the control group (n=19), the auditory feedback (AF) group (n=19), and the visual and auditory feedback (VAF) group (n=19). The control group received no feedback, whereas the AF group only received AF during exercises and the VAF group received the AF and visual feedback through the real-time ultrasound images. The main outcome measure was the assessment of the thickness of the abdominal muscles and LM measured by a dual ultrasound. Results: When VAF was applied, the thickness of the transverse abdominis significantly increased rather than when feedback was not applied or with AF only (p<0.05). The VAF group showed significant differences in both the control group and the AF group in the post-hoc test (p<0.05), and there was no significant difference between the control group and the AF group. Conclusions: With spinal stabilization exercises, VAF should be applied in standing posture for healthy adults to further promote the production of effective contractions.

Changes in muscle activity of the abdominal muscles according to exercise method and speed during dead bug exercise

  • Objective: There are many types of exercises with upper and lower-limb action for activation of abdominal muscles for trunk stabilization. A comparison of the different exercise methods been very useful to enhance the result from the exercise for treatment. The purpose of this study was to investigate through surface electromyography (EMG) the changes in abdominal muscle activity during the performance of three different dead-bug exercise methods performed at three different speeds. Design: Cross-sectional study. Methods: The subjects were 30 healthy adults (13 males and 17 females). We instructed the subjects to perform three different dead-bug exercises. We also applied three different speeds to the dead-bug exercises; 60 bpm, 90 bpm, and 120 bpm with use of a metronome. The assessment of EMG was percentage of maximal voluntary isometric contraction on the rectus abdominis (RA), external oblique (EO), and internal oblique (IO). Results: EMG activation of the RA, EO and IO muscles was significantly greater at the higher speed (p<0.05). There was a significant increase in abdominal muscle activity during the dead-bug exercise performed with both the upper and lower extremities compared to that with only the upper extremity or the lower extremities (p<0.05). Conclusions: These findings demonstrate that the performance of the dead-bug exercise with both the upper and lower extremities combined at a high speed is more effective compared to other exercises. Therefore, it is suggested that more favourable and effective outcomes may occur when the type and speed of the exercise is chosen appropriately.

  • Objectives : The purpose of this study is to find correlation about 7-Zone-Diagnostic System and Clinical Parameters using Oriental Health Examination. Methods : Subjects of our study were 325 cases who took Oriental Health Examination. We collected the data of body composition analysis, 7-Zone-Diagnostic System, blood analysis and devided by 7-Zone-Diagnostic system to acceleration, normalcy and depression. Results and Conclusions : The values of muscle mass, body fat mass, abdomen obesity rate, right arm water index, left arm water index, trunk water index, right leg water index, left leg water index, BMI, BMR, GOT, GPT, ${\gamma}$-GTP, BUN, TC, triglyceride and hemoglobin were continuous with 7-Zone-Diagnostic system.

  • Identification and protection of the facial nerve is very important in the proper operation of the parotid tumor. Posterior approach which finds main trunk of the facial nerve by surgical landmark such as tragal pointer, tympanomastoid suture, and posterior belly of digastric muscle is most commonly used. In case of posterior located tumor, inferior approach may be used, in which the retromandibular vein is followed from the neck and inferior branch of the facial nerve is located. In general, the facial nerve lies superficial to the retromandibular vein. But we experienced the anomalous relationship of the facial nerve and the retromandibular vien. We report this case with a literature review.

  • The upper trunk of the brachial plexus is the most common area affected by neuralgic amyotrophy (NA), and paresis of the shoulder girdle muscle is the most prevalent manifestation. Posterior interosseous nerve palsy is a rare presentation in patients with NA. It results in dropped finger on the affected side and may be misdiagnosed as entrapment syndrome or compressive neuropathy. We report an unusual case of NA manifested as PIN palsy and suggest that knowledge of clinical NA phenotypes is crucial for early diagnosis of peripheral nerve palsies.

  • Consideration for therapy method and oral motor function character of children with cerebral palsy. Therapists who treat for feeding disorder children owing the regression of oral motor function are necessary to gain knowledge about dysfunction of sensing, perception and cognition with baffling to eat and inhibition of primitive reflex, oral anatomy and function, and motor control (trunk, head, positioning of the upper limbs and the lower limbs and muscle tone). Oral motor function program is a comprehensive rehabilitation program which requires systematized enforcement and collaborated attempts to physiotherapists, occupational therapists, speech therapists, psychotherapists. Especially, physical therapists are not accustomed to oral motor program, hoping to provide diffusely and apply new therapy approach method for many areas (bell's palsy, respiratory failure, speech articulation). It will comprise to study owing to holistic approach with clinic.

  • Hydrotherapy is aimed at evoking short-term and long-term adaptation-mechanisms of patients. This means that a patient should be able to adapt to the changed environment. both mentally and physically. Hydrotherapy is widely used to treat patients with generalised rheumatic discases. Hydrotherapy may be applied as a single therapy. in combination with other therapeutic interventions or as one pan of a complex series of stimuli as e.g. during spa therapy. A treatment regimen in water is constructed according to the same rules as dry land exercise. However, methods specifically designed or adjusted for hydrotherapy can be used. The Halliwick is suitale in particular to facilitate arthrokinetic reactions as a part of training postural stability. The Bad Ragaz Ring is a hydortherapy modification of proprioceptive neuromuscular facilitation. Patterns of arms. logs and trunk are used to increase muscle function and flexibility. Other techniques are applicable in water too: mobilization and manipulation techniques of the joints can be integrated in hydrotherapeutic methods. The continuous research is needed for many interesting issues of hydrotherapy in new future.

  • 본 연구는 교각운동시 불안정한 지지면과 안정적인 지지면을 제공하여 체간 및 하지의 근 활성도의 변화를 알아보고자 하였다. 이 연구의 대상자는 20대 남, 여 14명이었다. 실험방법은 일반적 교각자세와 다이나믹 에어쿠션을 이용한 교각자세를 실시한 후에 무선 8채널 표면근전도 검사기를 이용하여 체간부의 척추기립근, 복직근, 대퇴의 대퇴직근, 외측 슬괵근, 하퇴부의 전경골근, 비복근 외측두에서 근활성 정도를 측정하였다. 체간부의 척추 기립근, 대퇴이두근, 가자미근은 불안정한 지지면에서 근활성도가 높게 나타났다(p<.05). 따라서, 교각운동에서 불안정한 지지면의 제공은 신체의 배측에 위치한 근육의 활성화를 나타내어, 교각운동시 지지면의 안정성의 변화만으로도근육활성도의 변화가 가능함을 알 수 있었다.

Effectiveness of orthoses for treatment in patients with spinal pain

  • Spinal pain is a common patient complaint in clinical practice. Conservative treatment methods include oral medication, physical therapy, injections, and spinal orthoses. The clinical application of orthoses is debated because of potential complications associated with long-term use, such as muscle weakness and joint contracture. We reviewed the orthoses most frequently used to manage spinal pain. We review the use of soft cervical and Philadelphia collars, lumbosacral corsets, and thoracolumbosacral orthosis to manage spinal pain. Spinal orthoses can help reduce pain by protecting the muscles and joints of the injured spinal region, preventing or correcting malformations, and limiting trunk flexion, extension, lateral flexion, and rotation. The short-term use of spinal orthoses is known to improve pain and disability during the treatment period without significant adverse effects. Spinal orthoses are expected to alleviate pain and improve patients' lifestyle.

An Ergonomics Evaluation on the Shovels of Korean Standard (삽에 대한 인간공학적 평가)

  • We Propose an improved shovel design to minimize physical discomfort and the risk of cumulative trauma disorder at shoveling tasks. The specifications of shovels in Korean standard data were scrutinized to check if they met the anthropometric specification of Korean population in 1997. We also applied "Body Map" pictograph and self-conscious checklist to field survey to analyze the discomfort of shoveling task. Thirty male subjects whose age were ranging from 18 to 65 voluntarily participated in the experiment. The measurement results show that shoveling workers were exposed to hazards of CTD's. Especially, when workers were involved in ditch digging and construction their trunk vend more than 120 degree. In order to compare the new designed shovel and the traditional K.S shovel, an analysis of variance was performed and we obtained very strong evidence that the new designed shovel was better in reducing physical discomfort. According to the results from electromyograrn experiment the new shovel contributed to improving subjective comfort level and reducing low back muscle fatigue. For futher study, the research may be extended to determine the relationship among shovel sharpness, type, digging motion and digging force.ing force.
