Predicting Surgical Complications in Adult Patients Undergoing Anterior Cervical Discectomy and Fusion Using Machine Learning

  • Objective: Machine learning algorithms excel at leveraging big data to identify complex patterns that can be used to aid in clinical decision-making. The objective of this study is to demonstrate the performance of machine learning models in predicting postoperative complications following anterior cervical discectomy and fusion (ACDF). Methods: Artificial neural network (ANN), logistic regression (LR), support vector machine (SVM), and random forest decision tree (RF) models were trained on a multicenter data set of patients undergoing ACDF to predict surgical complications based on readily available patient data. Following training, these models were compared to the predictive capability of American Society of Anesthesiologists (ASA) physical status classification. Results: A total of 20,879 patients were identified as having undergone ACDF. Following exclusion criteria, patients were divided into 14,615 patients for training and 6,264 for testing data sets. ANN and LR consistently outperformed ASA physical status classification in predicting every complication (p < 0.05). The ANN outperformed LR in predicting venous thromboembolism, wound complication, and mortality (p < 0.05). The SVM and RF models were no better than random chance at predicting any of the postoperative complications (p < 0.05). Conclusion: ANN and LR algorithms outperform ASA physical status classification for predicting individual postoperative complications. Additionally, neural networks have greater sensitivity than LR when predicting mortality and wound complications. With the growing size of medical data, the training of machine learning on these large datasets promises to improve risk prognostication, with the ability of continuously learning making them excellent tools in complex clinical scenarios.

원격탐사와 지리정보시스템의 산림분야 활용 (Application of Remote Sensing and Geographic Information System in Forest Sector)

  • 산림은 우리나라 토지피복 면적의 64%에 해당하는 넓은 면적을 차지한다. 이와 같이 넓은 면적의 산림을 조사, 모니터링, 관리하기 위해서는 원격탐사 및 지리정보시스템 기술이 필수적이다. 위성영상의 분광반사 특성을 이용하여 임상 및 수종분류가 가능하며, 이를 통해 임상도를 제작할 수 있다. 3차원 자료인 LiDAR를 이용하여 개체목의 위치와 수고 측정, 이를 통해 바이오매스와 탄소량 추정이 가능하다. 그 외에도 대상물의 반사특성을 이용해서 각종 지수들이 추출될 수 있는데, 예를 들어 식생지수와 표면토양지수 등을 통해 식생의 활력도와 산림 황폐화 정도를 파악 할 수 있다. 이러한 식생지수들의 변이를 파악하여 소나무 재선충병, 참나무 시들음병 등의 조기탐지 및 관리도 가능하다. 또한 A/R CDM, REDD+ 등 최근 기후변화 대응 사업에 있어서 원격탐사는 사업성 판단과 이산화탄소 흡수 및 저장량을 산정하는데 중요한 역할을 하고 있다. 기후변화 취약성 평가에서는 지리정보시스템의 시공간자료를 이용하여 국가 및 지자체 단위의 취약성이 시공간적으로 평가되고 있다. 또한, 시공간자료를 영향변수로 추가시킨 각종 모델을 통해 산림생장, 입목고사, 산사태 및 산불 등의 예측이 시공간적으로 이루어 질 수 있다.

온도조건이 뽕나무깍지벌레의 발육 및 수내분포에 미치는 영향 (Effects of Temperatures on Development and Distribution of Mulberry scale, Pseudaulacaspis pentagona, within Tree)

  • 핵과류를 가해하는 깍지벌레중 우점종인 뽕나무깍지벌레에 대한 용도조건이 충 발육 및 수관내분포에 미치는 영향과 월풍과의 관계를 구명하기 위하여 연구한 결과는 다용과 같다. 온도에 따른 각 stage의 성장비는 생육온도 범위의 항온에서는 크게 영향을 미치지 않았으며 각 stage의 발육영점과 유효적산온도는 난이 각각 $12.3^{\circ}C$, 46.9일도, 1령약총 $10.8^{\circ}C$, 183.8 일도, 2령약충 $9.8^{\circ}C$, 188.2일도였으며 총 약충 기간에서는 $10.3^{\circ}C$, 383.0일도이었다. 자성충의 수명은 $18^{\circ}C$, $22^{\circ}C$, $26^{\circ}C$, $30^{\circ}C$에서 각각 39.6일, 28.4일, 18.1일, 15.7일이었고 포란수 및 산란수는 각각 83.0개, 75.0개이었다. 또한 각 stage의 생육적온은 22~$26^{\circ}C$ 범위이었으며 $35^{\circ}C$에서는 부화되지 않았다. 여름철에 수관내 충의 밀도는 69.6%로 가지의 하향면에서 가장 높았으며 월동 자성충의 분포비율은 NE-SE변에서 가장 높았고 월동기간중 성충의 사충율은 36.2%이었다.

어리쌀바구미에 대한 식물정유의 훈증독성과 기피효과 (Fumigant Toxicity and the Repellent Effect of Essential Oils against Sitophilus zeamais Motschulsky)

  • 본 연구는 쌀을 비롯한 저장곡물을 가해하는 어리쌀바구미(Sitophilus zeamais) 성충에 대해서 8종의 식물정유의 훈증독성과 Y-tube를 이용한 후각반응으로 기피효과를 조사하였다. $10{\mu}{\ell}/L$ air의 농도에서 훈증독성시험 결과 pennyroyal과 tea tree 오일에서 각각 97%, 63%의 높은 살충효과를 보였다. 특히, pennyroyal oil($LT_{50}$, hour=3.87)은 tea tree oil($LT_{50}$, hour=4.09)보다 높고 빠른 살충력을 보였다. $10{\mu}{\ell}$/filter paper의 농도에서 후각계를 이용한 기피효과는 Cedarwood(80%), Cinnamon(83.3%), Cypress(81.1%), pennyroyal(97.1%) oil이 높은 기피활성을 보였으며, 이보다 낮은 $1{\mu}{\ell}$/filter paper의 농도에서도 Cinnamon(82.4%), Cypress(80.0%), pennyroyal(86.7%) 오일은 높은 기피활성을 나타냈다.

기도 이물의 임상적 고찰 (A Retrospective Review of Tracheobronchial Foreign Bodies)

  • Background : The development of bronchoscopic equipment along with the precision of radiographic techniques had reduced the mortality rate of patients with tracheobronchial foreign bodies but has been no change in the incidence of tracheobronchial foreign bodies since their introduction. The aim of this study was to assess the clinical characteristics of a tracheobronchial foreign body aspiration and to evaluate the efficacy of the treatment modality in children and adults. Methods : This is a retrospective review of 64 patients who underwent bronchoscopic procedures for the treatment of aspirated foreign bodies from December 1994 through March 2004 at the Chonnam national university hospital. Results : There were 47 males and 17 females, aged from 1 month to 78 years. Most of the patients had no underlying illness except for one patient with a cerebrovascular accident that contributed to the foreign body aspiration. The most common symptom was cough, which was noted in 54 patients (84.3%). The other presenting symptoms were dyspnea (48.8%), fever (20.3%), sputum (14%), vomiting (7.8%), and chest pain (4.6%). Those whose tracheobronchial foreign bodies were diagnosed more than 2 days after the aspiration (21 patients) were more likely to have pneumonia than those whose foreign bodies were diagnosed within 2 days (p = 0.009). Foreign bodies were visualized in the plain chest radiographs in 12 cases (18.8%), while others showed air trapping (21, 32.8%), pneumonia (15, 23.4%), atelectasis (7, 10.9%), and normal findings (9, 14.1%). The foreign bodies were more frequently found in the right bronchial tree (36) compared with the left bronchial tree (22, p = 0.04). In order to remove the foreign bodies, twenty (31.2%) cases were removed using flexible bronchoscopy, while 42 (65.6%) and 2 (3.2%) cases required rigid bronchoscopy and surgery, respectively. Conclusions : Tracheobronchial Foreign body aspiration had a bimodal age distribution in the infancy and old age around 60 years. They were found more frequently in the right bronchial tree. In addition, patients whose foreign bodies were diagnosed more than 2 days after the aspiration were more likely have a infection. Rigid bronchoscopy is the procedure of choice for uncooperative children and for those with foreign bodies lodged deeply in the small bronchial tree.

강원도 가리왕산 지역 천연 활엽수림에서 물푸레나무(Fraxinus rhynchophylla)의 천연하종과 치수 발생 및 고사 특성 (Seed Rain, Seedling Emergence and Mortality of Fraxinus rhynchophylla in Natural Broad-leaved Forests in the Mt. Gariwang Area, Gangwon-do)

  • 물푸레나무(Fraxinus rhynchophylla Hance)의 천연하종 갱신과 치수의 초기 생존 특성을 파악하기 위하여 강원도 가리왕산 지역 천연 활엽수림에서 물푸레나무를 대상으로 2011-2013년 3년 간 하종량, 2012-2014년의 치수 발생, 치수의 수명 분포(Lifespan distribution), 치수 생존과 고사, 초본 식생과의 관계를 분석하였다. 하종량과 치수 발생량은 연도별 유의한 차이를 보였고(p <.05). 하종량이 증가한 이듬해는 치수 발생량이 증가하였다. 2013년 하종량과 2014년 치수 발생량은 유의한 상관 관계를 보였고(p <.05), 이는 하종 이듬해 발아하는 물푸레나무의 치수 발생 특성과 일치한다. 치수는 늦봄에서 초여름 사이에 발생하였으며, 2014년은 발생 치수 중 78%가 초여름에 한꺼번에 발생하였다. 물푸레나무 치수는 발생 직후 대부분이 고사하였다. 2012년 8월 발생한 치수 집단은 초기 두 달 동안 56%가 고사하였고, 2014년 7월 발생한 치수 집단은 1개월 동안 38%가 고사하였다. 하층의 초본 식생으로 인한 피음과 지하부 경쟁은 치수 고사의 주요 원인이었다. 천연 활엽수림의 물푸레나무는 특정 해에 종자가 많이 열리는 해거리 경향을 보였고, 다량 결실 이듬해는 임상에 다량의 치수가 발생하였으나, 발생 당년 대부분이 고사하였다. 물푸레나무는 치수 고사율이 높으나, 주기적으로 다량의 치수가 발생하고 살아남은 소수의 치수가 활착에 성공하여 갱신하는 전략을 가진다. 우리나라에는 다양한 자생 교목 수종이 서식하나 이들 수종의 천연갱신에 대한 정보는 제한적이다. 우리나라의 천연림 관리 및 복원을 위해서는 다양한 자생 수종의 천연갱신에 대한 연구가 필요하다.

복잡 심기형 환자에서 `REV`술후 우심실 출구 성장에 대한 고찰 (Growth of Right Ventricular Outflow Tract after "REV" Operation in Complex Congenital Heart Disease)

  • From February 1988 to December 1990, 42 patients underwent so called REV operation for pulmonary stenosis or atresia with or without anomalies of ventriculoarterial connection and truncus arteriosus. The principles of operative technique are mobilization of pulmonary arterial tree beyond the pericardial reflection, transection of pulmonary trunk between the pulmonary ventricle and pulmonary artery, suture of distal pulmonary arterial stump to the upper margin of Pulmonary ventriculotomy site with absorbable suture, and anterior patch with 0.625% glutaraldehyde fixed autologous pericardium with monocusp inside it. Age at operation ranged 3-156months [mean 41.8 month] with twelve of whom infants. Operative indications were pulmonary atresia, with ventricular septal defect[16], and pulmonary stenosis with double outlet right ventricle[8], with ventricular septal defect[16], with double outlet right ventricle[8], with complete transposition of the great arteries[8], with corrected transposition of the great arteries[6], with Fallot`s tetralogy[3], and truncus arteriosus[1]. There were six hospital deaths[14%] and no late death. Twenty-four of 36 survivals were followed up more than 12 months with good clinical results. Postoperative angiocardiogram was performed in fifteen patients. Hemodynamically, two patents had residual pressure gradients along the pulmonary outflow tract, one patient showed severe pulmonary regurgitation; morphologically, there were six significant stenosis of left pulmonary arterial tree, two of whom showed significant pressure gradients. Our present experience with REV operation suggests that this technique make it possible to perform anatomic repair in a wide variety of congenital anomalies of abnormal ventriculoarterial connection associated with pulmonary outflow tract obstruction without using the prosthetic material, even in infants, with relatively low mortality and morbidity.

최근 국내에서 유행하는 돼지 유행성 설사 바이러스 nucleocapsid 유전자의 다양성 및 계통 분석 (Genetic diversity of nucleocapsid genes of recent porcine epidemic diarrhea viruses isolated in Korea)

  • Porcine epidemic diarrhea virus (PEDV), a porcine enteropathogenic coronavirus, causes lethal watery diarrhea in piglets, resulting in large economic losses because of high mortality. In November 2013, PEDV reemerged in Korea, and these outbreaks have since continuously occurred. In the present study, we determined the full-length nucleocapsid (N) gene sequences of three Korean PEDV field isolates collected in 2014-2015. Sequence and phylogenetic analysis of N genes revealed that recent prevalent Korean PEDV isolates were very closely related to the US PEDV isolates in 2013. Interestingly, the phylogenetic tree based on the nucleotide sequencing of the PEDV N gene was similar to the tree topology of the PEDV complete genomes. Therefore, our data provide a better understanding of the genetic diversity and contribute to the accurate diagnosis and development of vaccines against PEDV.

청개구리 배아를 활용한 농약류(살균제, 살충제, 제초제)의 독성평가 연구 (Studies on Toxicological Evaluation of Pesticides(Fungicide, Insecticide, Herbicide) using Tree Frog Embryos, Hyla japonica)

  • 국내에 서식중인 청개구리의 배아를 활용하여 농약류 3종의 독성평가를 위해 FETAX(Frog Embryo Teratogenesis Assay-Xenopus) 기법에 따라 청개구리(Hyla japonica)의 배아를 배양하면서 Benomyl(살균제), Carbofuran(살충제), Thiobencarb(제초제)의 효과를 probit 분석법으로 조사하였다. 그 결과, Benomyl, Carbofuran, Thiobencarb의 농도에 의존하여 유생의 체장 길이는 감소하고 치사율과 기형율은 증가하였다. Benomyl, Carbofuran, Thiobencarb의 teratogenic concentration($EC_{50}$)은 각각 1.00, 0.58, 4.75mg/L을 나타내어 Carbofuran이 기형유발에 가장 민감하게 반응하였으며, embryo lethal concentrations($LC_{50}$)은 7.04, 28.71, 16.12mg/L을 나타내어 Benomyl이 가장 낮은 농도에서 배아를 치사하는 것으로 나타났다. Teratogenic index($TI=LC_{50}/EC_{50}$)는 Benomyl 7.04, Carbofuran 49.50, Thiobencarb 3.39를 나타내어 TI값이 모두 기형유발물질로 판단하는 기준인 1.5 이상을 나타내어 시험에 사용된 농약류 3종은 최기형성 물질로 판단되며 Carbofuran이 가장 강력한 최기형성물질로 작용함을 알 수 있었다.

Comparison of stand structure and growth characteristics between Korean white pine plantation and oak-dominated natural deciduous forest by thinning treatment

  • Background: Korean white pine (Pinus koraiensis) is a major commercial species, and the importance of the oak trees (Quercus spp.) is increasing due to various factors such as environmental and ecological values. However, more information is required to clearly understand the growth characteristics of these species especially regarding thinning intensity. This study was performed to provide the basic information to develop the silvicultural guideline and field manual by analyzing tree and stand characteristics in line with thinning intensity in the Korean white pine plantation and oak-dominated natural deciduous forest. Results: Diameter at breast height (DBH) and volume changes by the thinning intensity in the Korean white pine plantation were significantly different from those in the oak-dominated deciduous natural forest. In particular, DBH distribution in the pine stand appeared that there were more large diameter trees as the thinning intensity was higher. DBH periodic annual increment (PAI) of the pine stand was higher as the thinning intensity was stronger and the growth period was shorter. This trend was similarly shown in the natural deciduous forest, but the amount of PAI was smaller than in pine stand. The volume PAI after thinning was not decreased over time. In each stand type, the PAI tended to be lower as stand density was higher. The volume PAI in the pine stand was significantly higher than that in the oak-dominated natural deciduous forest. Dead trees occurred the most in the unthinned plots of each stand type, and those were higher in the natural deciduous forest. Ingrowth trees were observed only in the natural deciduous forest, and its distribution was the lowest in unthinned plots; Korean white pine as ingrowth occurred the most frequently among many tree species. Conclusions: Different effects of thinning treatment on DBH and volume PAI, mortality, and ingrowth were observed for each stand. With respect to forest growth, Korean white pine plantation was superior to the oak-dominated natural deciduous forest. The results of this study offer fundamental information for the development of silvicultural guidelines for Korean white pine plantations and oak-dominated natural deciduous forests in Korea.
