Background: Women receiving mastectomy usually prefer a single-stage surgical procedure without the need for additional surgery. Hence, nipple sparing mastectomy was introduced, and the follow-up data on the aesthetic outcome and recurrence of breast cancer were investigated in this study. Materials and Methods: The study subjects comprised 22 patients who received nipple-sparing mastectomy and immediate breast reconstruction using the free transverse abdominal rectus abdominis myocutaneous flap between June of 2007 and June of 2012. The patients' aesthetic outcomes were measured with 2 methods for the objective result: Breast size measurements and breast volume calculation both at preoperative phase and postoperative 1 years phase. Also, the patients' satisfaction was evaluated at postoperative 1 year with the self-assessment questionnaire. Follow up check for assessing cancer recurrence was performed for an average period of postoperative 1063 days. Results: First, in objective aesthetic outcome, there were no significant differences between the preoperative and postoperative results on both the breast size and the volume. Second, the patient satisfaction analysis scores were graded as very good in 15 patients (68.2%), and as good in 6 patient (27.3%). Most of the patients were very satisfied with our surgery method. Last, there was no local or distant recurrence in these 22 patients during the follow-up period. Conclusion: In this study, the nipple-sparing mastectomy achieved satisfactory results for the breast scar and shape with a single-stage surgical procedure, and the cancer recurrence rate was not significantly different from that of the conventional mastectomy. Besides, the nipple-sparing mastectomy is more cost-effective than the conventional mastectomy since it reduces the need for additional procedures. However, we think that it is necessary to determine the long-term outcomes about the recurrence rate.

The effectiveness and the sensitivity of a Wireless impedance/Admittance Monitoring System (WiAMS) for the prompt damage diagnosis of two single-storey single-span Reinforced Concrete (RC) frames under cyclic loading is experimentally investigated. The geometrical and the reinforcement characteristics of the RC structural members of the frames represent typical old RC frame structure without consideration of seismic design criteria. The columns of the frames are vulnerable to shear failure under lateral load due to their low height-to-depth ratio and insufficient transverse reinforcement. The proposed Structural Health Monitoring (SHM) system comprises of specially manufactured autonomous portable devices that acquire the in-situ voltage frequency responses of a network of twenty piezoelectric transducers mounted to the RC frames. Measurements of external and internal small-sized piezoelectric patches are utilized for damage localization and assessment at various and increased damage levels as the magnitude of the imposed lateral cycle deformations increases. A bare RC frame and a strengthened one using a pair of steel crossed tension-ties (X-bracing) have been tested in order to check the sensitivity of the developed WiAMS in different structural conditions since crack propagation, damage locations and failure mode of the examined frames vary. Indeed, the imposed loading caused brittle shear failure to the column of the bare frame and the formation of plastic hinges at the beam ends of the X-braced frame. Test results highlighted the ability of the proposed SHM to identify incipient damages due to concrete cracking and steel yielding since promising early indication of the forthcoming critical failures before any visible sign has been obtained.

In this study, transverse relaxation time (T2) measurement and the evaluation of the characteristics of the spectral peak related to stomach tissue metabolites were performed using ex vivo proton magnetic resonance spectroscopic imaging (MRSI) at 1.5-T MRI/S instruments. Thirty-two gastric tissues resected from 12 patients during gastric cancer surgery, of which 19 were normal tissue and 13 were cancerous tissue, were used to measure the $T_2$ of the magnetic resonance spectroscopy (MRS) peaks. The volume of interest data results from the MRSI measurements were extracted from the proper muscle (MUS) layer and the composite mucosa/submucosa (MC/SMC) layer and were statistically analyzed. MR spectra were acquired using the chemical shift imaging (CSI) point resolved spectroscopy (CSI-PRESS) technique with the parameters of pulse repetition time (TR) and echo times (TE) TR/(TE1,TE2)=1500 msec/(35 msec, 144 msec), matrix $size=24{\times}24$, NA=1, and voxel $size=2.2{\times}2.2{\times}4mm^3$. In conclusion, the measured $T_2$ of the metabolite peaks, such as choline (3.21ppm) and lipid (1.33ppm), were significantly decreased (p<0.01 and p<0.05, respectively) in the cancerous stomach tissue.
Objective: To compare three-dimensionally the midfacial hard- and soft-tissue asymmetries between the affected and the unaffected sides and determine the relationship between the hard tissue and the overlying soft tissue in patients with nonsyndromic complete unilateral cleft lip and palate (UCLP) by cone-beam computed tomography (CBCT) analysis. Methods: The maxillofacial regions of 26 adults (18 men, 8 women) with nonsyndromic UCLP were scanned by CBCT and reconstructed by three-dimensional dental imaging. The frontal-view midfacial analysis was based on a $3{\times}3$ grid of vertical and horizontal lines and their intersecting points. Two additional points were used for assessing the dentoalveolar area. Linear and surface measurements from three reference planes (Basion-perpendicular, midsagittal reference, and Frankfurt horizontal planes) to the intersecting points were used to evaluate the anteroposterior, transverse, and vertical asymmetries as well as convexity or concavity. Results: Anteroposteriorly, the soft tissue in the nasolabial and dentoalveolar regions was significantly thicker and positioned more anteriorly on the affected side than on the unaffected side (p < 0.05). The hard tissue in the dentoalveolar region was significantly retruded on the affected side compared with the unaffected side (p < 0.05). The other midfacial regions showed no significant differences. Conclusions: With the exception of the nasolabial and dentoalveolar regions, no distinctive midfacial hard- and soft-tissue asymmetries exist between the affected and the unaffected sides in patients with nonsyndromic UCLP.

This paper investigates the vibrational damping characteristics of laminated plates composed of elastic, viscoelastic and elastic layers by theoretical and experimental methods. Laminated plates are in cylindrical bending and visco-elastic adhesive layer is assumed as the visco-elastic spring which takes damping effect through both shear and normal deformations. Governing equations oof laminated plates are derived in the form of simultaneous first order differential equations, which account for the longitudinal displacements, rotary inertia and shear deformations of elastic base plate and elastic constraining plate. The numerical calculations of the equations are illustrated by the applications to the cantilever beam in transverse vibration. The results of the solutions agree well with the experimental measurements in general. The damping effects due to the shear and thickness deformations in the adhesives are analyzed and it is shown that for thicker adhesives, the damping effect due to thickness deformation becomes significant and for thinner adhesives, due to shear deformation.
Background Pincer nail deformity is a transverse overcurvature of the nail. This study aimed to define the anatomical characteristics of pincer nail deformity and to evaluate the surgical outcomes. Methods A retrospective review was conducted on 20 cases of pincer nail deformity of the great toe. Thirty subjects without pincer nail deformity or history of trauma of the feet were selected as the control group. Width and height indices were calculated, and interphalangeal angles and base widths of the distal phalanx were measured with radiography. We chose the surgical treatment methods considering perfusion-related factors such as age, diabetes mellitus, kidney disease, and peripheral vascular disease. The zigzag nail bed flap method (n=9) and the inverted T incision method (n=11) were used to repair deformities. The outcomes were evaluated 6 months after surgery. Results The interphalangeal angle was significantly greater in the preoperative patient group ($14.0^{\circ}{\pm}3.6^{\circ}$) than in the control group ($7.9^{\circ}{\pm}3.0^{\circ}$) (P<0.05). The postoperative width and height indices were very close to the measurements in the control group, and most patients were satisfied with the outcomes. Conclusions We believe that the width and height indices are useful for evaluating the deformity and outcomes of surgical treatments. We used two different surgical methods for the two patient groups with respect to the perfusion-related factors and found that the outcomes were all satisfactory. Consequently, we recommend taking into consideration the circulatory condition of the foot when deciding upon the surgical method for pincer nail deformity.
Purpose: This study examined thickness changes in abdominal muscles according to difficulty level of core stabilization exercise in healthy subjects. Methods: Thirty healthy subjects (age range: 21-30 years) volunteered under three conditions. In the first condition, the subjects performed an abdominal draw-in maneuver (ADIM). In the second condition, they performed the ADIM during quadruped exercise using a suspension device without extending their lower limbs. In the third condition, the subjects performed the ADIM during quadruped exercise using a suspension device while extending both lower limbs. The changes in thickness of transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles were measured by ultrasonography (US) imaging during the three experimental conditions, and US was used to measure the improvement ratio of muscle thickness at rest. The interventions were conducted over three trials in each condition, and measurements were performed on each subject by one examiner. Results: Our results showed a significantly greater increase in the muscle thickness of TrA and IO muscles after performance of quadruped exercise using a suspension device without knee extension (p<0.05) compared to the other conditions. The results also showed a significantly greater increase in the thickness changes of EO muscle in those who performed the ADIM during quadruped exercise using a suspension device with knee extension (p<0.05) compared with the ADIM only. Conclusion: These findings demonstrated positive evidence that a low-level core stabilization exercise could improve thickness of abdominal muscles.

PURPOSE: The purpose of this study was to investigate the effects of supplementary verbal encouragement according to the gender on the activation of abdominal muscles during performance of crunch exercise in healthy subjects. METHODS: A Total of 32 healthy subjects (15 male and 17 female) were randomly allocated to two conditions, crunch exercise with Ki-hap and verbal encouragement with same gender (CKVS) and crunch exercise with Ki-hap and verbal encouragement with different gender (CKVD). The interventions were conducted over three trials in each condition, and measurements were performed on each subject by one examiner in three trials. The activation of rectus abdominis, external oblique, internal oblique, and transverse abdominis muscles were evaluated using electromyography (EMG) during performance of crunch exercise with Ki-hap and verbal encouragement with same gender or different gender, respectively. RESULTS: The results showed that there were no significantly difference in the activation of all abdominal muscles in those of the CKVS compared with the CKVD (p>.05). The results also showed that there were no significantly difference in the ratio of muscle activation at global muscle to that at local muscle between two conditions (p>.05). CONCLUSION: These findings demonstrated that application of the supplementary verbal encouragement by gender does not affect to activate the abdominal muscles of subject to perform a movement, at the same time, it would suggest positive evidence for improving activation of abdominal muscles.

PURPOSE: This study was to determine the changes of abdominal muscles activation according to the heel contact methods of stair ascent in healthy young adults. METHODS: 33 healthy young subjects (mean age: $26.37{\pm}9.72$ years, age range: 20-32 years) volunteered under two conditions. They were performed stair ascent with heel contact and without heel contact. The effects of heel contact methods were assessed using the surface electromyography (sEMG) analysis during stair ascent for activation of abdominal muscles (internal oblique; IO, transverse abdominis; TrA, external oblique; EO rectus abdominis; RA). The interventions were conducted over three trials in each method, and measurements were performed on each subject by one examiner in three trials. RESULTS: Our results revealed that there were significantly greater increase in the EMG activation of IO and TrA muscles in the performance of stair ascent with heel contact (p<.05) compared to those of stair ascent without heel contact. The results also showed that there were greater decrease in the ratio of abdominal muscle activation in those of stair ascent with heel contact compared with stair ascent without heel contact. CONCLUSION: These findings demonstrated that the method of stair ascent with heel contact would suggest positive evidence for improving activation of abdominal muscles.
The Ting Kau Bridge in Hong Kong is a cable-stayed bridge comprising two main spans and two side spans. The bridge deck is supported by three towers, an end pier and an abutment. Each of the three towers consists of a single reinforced concrete mast strengthened by transverse cables and struts. The bridge deck is supported by four inclined planes of cables emanating from anchorages at the tower tops. In view of the heavy traffic on the bridge, and threats from typhoons and earthquakes originated in areas nearby, the dynamic behaviour of long-span cable-supported bridges in the region is always an important consideration in their design. Baseline finite element models of various levels of sophistication have been built not only to match the bridge geometry and cable forces specified on the as-constructed drawings but also to be calibrated using the vibration measurement data captured by the Wind and Structural Health Monitoring System. This paper further describes the analysis of axle loading data, as well as the generation of random axle loads and simulation of vibrations of the bridge using the finite element models. Various factors affecting the vehicular loading on the bridge will also be examined.
