안구내 악성종양에 대한 저준위 방사선요법에 관한 연구 (A study of Brachytherapy for Intraocular Tumor)

  • 연구의 목적 및 중요성 안구에 발생하는 악성종양의 치료에 일반적으로 사용되는 적출술이나 외부 방사선치료법은 시력상실 및 충분한 종양선량을 얻을 수 없는 단점이 있다. 최근 들어 기존치료법의 단점들을 줄이면서 치료 효과를 높이기 위한 치료법으로서 방사성동위원소가 삽입된 기구를 이용한 근접 방사선요법이 외국에서 개발되어 시행되고 있다. 본 연구에서는 안구내 악성종양의 새로운 치료법인 근접 방사성요법을 시행하기 위하여 접합한 형태의 기구를 개발하고, 기구에서 방출되는 방사선의 선량 및 선량분포측정을 통하여 종양 및 주변조직에 분포되는 선량을 보다 정확하게 계산해 보고자 하였다. 연구의 내용 및 범위 안구내 악성종양의 치료를 위한 근접 방사선요법의 연구를 위하여, 1. 안구의 모델을 결정하고, 적당한 방사성동위원소를 선택하였으며, 2. 적합한 형태의 기구를 설계 및 제작하고, 3. 제작된 기구의 방사선 투과계수 및 기구에서 방출되는 방사선의 선량분포를 측정하였으며, 4. 이들 측정 결과와 기존의 컴퓨터 선량계산모델들의 계산 결과와 비교하였다. 연구 결과 및 활용에 대한 건의 본 연구를 통해 다음과 같은 결과를 얻었다. 1. 안구의 모델은 지름 25mm인 구형으로 결정하였으며, 근접 치료에 사용될 방사성동위원소는 크기, 반감기, 발생 방사선의 에너지 및 구입의 용이성 등을 고려하여 Ir-192가 가장 적합한 것으로 판단되었다. 2. 기구는 인체 조직과의 생물학적 반응 및 피폭 선량 차폐를 고려하여 순금으로 제작하였으며, 기구의 크기는 지름이 15mm, 17mm 및 20mm인 3종류로 하였고, 두께는 모두 1.5mm로 하였다. 3. 기구의 방사선 투과계수를 TLD 및 Film으로 측정한 결과 표면에서 모두 0.71, 표면의 1.5mm거리에서 각각 0.45, 0.49이었다. 4. Ir-192가 삽입된 기구의 선량분포 측정 결과를 Gary Luxton 등의 컴퓨터 선량계산모델 및 원자력병원에서 보유하고 있는 방사선 치료계획장치인 CAP-PLAN의 선량계산모델의 계산 결과와 비교하여, 흡수선량은 ${\pm}10\%$ 이내에서 거리로는 0.4mm 이내에서 대부분 일치하였다. 최대 오차는 각각 $11.3\%$ 및 0.8mm로 나타났다. 이상의 결과들로부터 새로 개발한 금으로 된 기구와 Ir-192 seed를 이용한 근접 방사선 치료법으로 안구의 악성종양에 대한 치료를 보다 효과적으로 시행할 수 있을 것으로 기대된다.

연(鉛)필터의 투과선량을 이용한 15 MV X선의 에너지스펙트럼 결정과 조직선량 비교 (Compare the Clinical Tissue Dose Distributions to the Derived from the Energy Spectrum of 15 MV X Rays Linear Accelerator by Using the Transmitted Dose of Lead Filter)

  • 최근의 방사선 치료선량 계획시스템은 대체로 커널빔을 컨볼루션하여 조직선량을 구하고 있다. 본 연구에서는 광자선 빔에 따른 심부선량과 임의의 깊이에서 프로파일 선량을 구하기 위하여 반복적 수치해석을 통해 투과 필터에 의한 감쇠선량으로부터 에너지 스펙트럼을 구성하였다. 실험은 15 MV X선(Oncor, Siemens사)과 이온선량계 0.125 cc (PTW T31010)을 이용하여 납필터를 투과한 선량을 측정하여 이루어졌다. 15 MV X선의 에너지스펙트럼은 0.25 MeV 간격으로 납필터 0.51 cm에서 8.04 cm의 감쇠선량으로 실측치와 비교하여 구하였다. 실험 연산에서 15 MV X선의 최대유량은 3.75 MeV에서 나타났으며, 평균에너지는 4.639 MeV를 보였으며, 투과선량은 평균 0.6%의 오차인 반면에 최대오차는 납두께 5 cm에서 2.5%를 보였다. 조직선량은 에너지에 크게 의존하므로, 평탄형 필터의 중심과 Tangent 0.075와 0.125인 가장자리의 에너지를 구하였으며, 각각 4.211 MeV와 3.906 MeV로 나타났다. 심부선량과 프로파일 선량은 상업화로 공급되고 있는 선량계획시스템에 중심 선속과 가장자리의 각 에너지스펙트럼을 적용하여 구하여 실측선량률과 비교하였다. 생성된 심부선량 곡선은 조사면 $6{\times}6cm^2$에서 $30{\times}30cm^2$까지 실측치와 비교한 결과 1% 이내의 거의 일치하는 값을 얻었으며, 프로파일 곡선은 $10{\times}10cm^2$에서 1% 이내의 오차를 보였으나, $30{\times}30cm^2$와 같이 큰 조사면의 얕은 깊이에서는 2%의 오차를 보였다. 따라서 투과선량을 연산으로 구한 에너지 스펙트럼이 조직선량을 평가하는 데 상당히 적은 오차범위 내에서 정량적이고 정성적으로 얻을 수 있음을 알 수 있다.

방사선 입체조형치료용 다엽콜리메이터의 특성과 조직내 선량분포 측정 (Dose Distribution and Characterization for Radiation Fields of Multileaf Collimateor System)

  • 목적 : 다엽콜리메이터를 이용하여 부정형의 치료부위를 편리하고 정확히 차폐하며 삼차원 입체조형치료를 원활히 수행하기 위하여 다엽콜리메이터의 차폐특성을 계측하고 치료계획을 위한 최적방법을 확정한다. 대상 및 방법 : 차폐폭이 1cm이고 길이가 15cm인 52개의 금속차폐엽들이 양방향 각각 26개로 구성되어 컴퓨터계획에 의거 여러모양의 차폐면을 구사할 수 있는 다엽콜리메이터(Multileaf collimator)를 대상으로 방사선 치료에 요구되는 고에너지 방사선의 출력선량율, 차폐 및 투과효과, 반음영, 조직내 선량분포등을 전리함측정기와 열형광측정기 및 필름 선량측정방법을 이용하여 계측하였다. 다엽콜리메이터의 계단식 반음영 측정은 고체팬텀과 비디오밀도측정기를 이용하여 실효반음영(Effective penumbra)과 일반적반음영(General penumbra)으로 구분 평가하였다. 결과 : 다엽콜리메이터의 출력선량율은 표준차폐조리개에 비교하여 출력 선량율이 $1-2\%$ 증가 되었으며 치료오차 이내로 평가되었다. 다엽콜리메이터의 방사선투라 선량율은 6-10MV X선에서 평균 $2\%$로 표준차폐조리개보다 컸지만 혼합차폐벽돌보다 적었다. 원형으로 형성된 차폐엽 끝면의 조사선량은 약 $20\%$증가 되었으며 엽과 엽사이의 선량은 6mm 폭의 $5\%$증가로 심부치료에 영향이 없었다. 다엽콜리메이터의 실효반음영($20-80\%$)은 약 6mm로서 혼합차폐벽돌과 거의 같았으나 중심선에서 벗어날수록 $2-3\%$증가되었다. 다엽콜리메이터에 의한 계단식 모양의 반음영은 6-10MV x-선에서 차폐면 경사각이 45도일 때 약 11mm로 가장 컸으며 방사선속에 평행히 절단된 차폐벽돌보다 약 5mm 더 증가되었다. 결론 : 다엽콜리메이터는 1cm폭의 차폐엽들로 구성되어 계단식 차폐면에 대한 반음영이 다소 증가되고 있지만 조직내부의 다문조사 경우 반음영이 완화되므로 부정형 조사면 차폐에 적당하며 방사선 입체조형치료를 수행할 수 있는 가장 적당한 차폐기구로 생각된다.

쐐기필터 사용 시 레디오크로믹 필름을 이용한 조직에 따른 선량분포 연구 (Dose Distribution According to the Tissue Composition Using Wedge Filter by Radiochromic Film)

  • 본 논문의 목적은 쐐기필터를 사용한 방사선조사면에서 다양한 조직전자밀도가 선량분포에 미치는 영향을 분석하고자 함이다. 구성된 물질에 따라 밀도가 다른 고체 팬텀, 코르크팬텀, 그리고 공기층에서 동적쐐기필터와 금속쐐기필터를 이용하여 선량분포의 변형정도를 평가하였다. 본 실험에서는 매질 내 삽입이 용이하고 우수한 선량특성을 가지고 있는 레디오크로믹 필름(Gafchromic EBT2, International Speciality Products, NJ)을 사용하였다. 선형가속기 6 MV 광자선을 이용해서 $10{\times}10cm^2$ 조사면에 400 MU를 조사하였다. 필름의 선량분포는 선량 분석프로그램을 이용하여 조사면 내 영역과 반음영 영역을 분석하였다. 조직의 밀도가 같을 때 동적쐐기필터와 금속 쐐기필터의 선량분포는 금속 쐐기필터 선량이 동적쐐기필터 선량보다 높게 나타났다. 조직전자밀도가 다른 부위에 쐐기필터의 종류에 따른 선량분포는 고체팬텀과 코르크 팬텀에서 2% 이내 차이를 나타내고 있었다. 그러나 공기층에서 선량분포는 고체팬텀이나 코르크 팬텀의 선량분포와 큰 차이를 보이고 있다. 공기층에서 쐐기필터의 선량분포는 쐐기 사용 효과가 나타나지 않고 있다. 쐐기필터의 두꺼운 부분과 얇은 부분 밖에서 반음영의 크기는 1 cm에서 2 cm 정도 크게 두꺼운 부분에서 크게 나타났다. 그리고 금속 쐐기필터에서 반음영이 동적쐐기필터 보다 평균 6.4%정도 높게 반음영이 나타났다. 본 실험을 통해 공기층과 같이 조직전자밀도 현저히 작은 매질에서는 쐐기필터의 효과가 크게 떨어지는 것과 불균질 물질에 따라 흡수되는 선량분포가 크게 변형되는 것을 알 수 있었다. 따라서 조직전자밀도의 차이가 큰 부위의 방사선치료계획 시 쐐기필터의 적용에 따른 적절한 보정이 이루어져야 한다.

Smart미디어시대 정보통신·미디어(ICT) 분야 규범체계의 재구조화에 관한 연구 (A Study on the Restructuration of Norm System in the Field of ICT for the Smart Media)

  • 이 논문에서는 최근 단행된 정부조직 개편의 결과 탄생한 미래창조과학부와 방송통신위원회 등의 소관 법률의 가장 최상위의 ICT관련 법제의 정비와 아울러 기본법으로서의 성격을 갖는 ICT특별법에 담겨야할 헌법적 차원이 기본 문제와 몇 가지 쟁점들에 대해 이론적 실제적 차원에서 논의하였다. 정보통신기술의 발전은 인류의 생활방식을 혁명적으로 변화 시켰으며, 전지구적 차원의 네트워크 사회의 출현은 경제적 사회적 기회와 혜택을 제공 하였지만 동시에 도전과 위협의 이슈가 끊임없이 제기되고 있다. 기술발전에 따른 많은 장점에도 불구하고 디지털 혁명으로 인하여 인류가 누리게 된 인터넷 등 온라인 매체를 통한 사이버공간은 현실적으로 해결해야 할 많은 문제들이 있다. 전송기술의 발전으로 인한 표현의 자유의 통로 확대와 경제적 기획 창출이라는 긍정적인 측면을 보다 발전시켜 나가기 위해서는 정보화로 인한 따른 부작용을 최소화하려는 노력이 절실하게 필요하다. 첫째, ICT특별법은 역사적으로 방송통신융합을 넘어선 미디어의 융합시대에 적헙하도록 규범화되어야 한다. 향후 정보화와 정보통신 기술 및 콘텐츠의 개발과 아울러 미디어 분야에서도 경제적 발전과 아울러 언론의 자유를 수호라는 두 가지의 정책을 모두 달성할 수 있는 근거규정이 마련되어야 한다. 둘째, 규범구조적 측면에서는 정부의 행위들은 모두 경제발전과 정보의 자유라는 목표를 달성하기 위한 것이라는 점을 깊이 명심해야 한다. 따라서 정치적 차원에서 말끔하게 해결되지 못한 조직개편의 문제점을 규범적 관점 해소함으로써 소모적 갈등의 발생가능성을 최소화할 수 있도록 노력해야 한다. 셋째, ICT특별법은 미래부가 소관하는 정보통신분야, 유료 방송분야, 디지털미디어 콘텐츠 분야를 모두 포괄하는 내용의 기본법적 성격을 가지는 규범이어야 한다. 넷째, 기술적인 관점에서는 ICT융합으로 인해 발생하는 망중립성의 문제, 디지털 콘텐츠를 둘러싼 이해관계의 대립 등 다양한 문제점들을 해결할 수 있어야 한다. ICT특별법은 정보화를 통해 일방적으로 효율성이나 산업의 발전만을 추구하는 것이 아니라, 정보화가 국민의 삶의 질을 높이고 우리 사회의 민주화를 촉진할 수 있도록 제도의 설계와 구축하는 것을 목표로 하여야 한다. 향후 이번 ICT특별법의 입안과정에서 논의하고 있는 다양한 관점들을 참고하여 정보통신기술의 강국임과 동시에 정보인권분야에서도 한 발 앞서가는 계기가 마련되어야 한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. The system based on PC-486DX was designed to acquire medical images produced by medical imaging instruments and related audio information, that is, doctors' reporting results. Input information was processed and analyzed, then the results were presented in the form of graph and animation. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction.
