침형 경피신경 전기자극법과 전통적인 경피신경 전기자극법의 교차효과 비교 (The Comparison of the Acupuncture-Like Transcutaneous Electrical Nerve Stimulation and Conventional Transcutaneous Electrical Nerve Stimulation in Crossover Effect)

  • The purpose of this study was to compare acupuncture-like transcutaneous electrical nerve stimulation(ALTENS) with conventional transcutaneous electrical nerve stimulation(C-TENS) for crossover effect in healthy subjects. Forty subjects recieved ALTENS(20 persons), C-TENS(20 persons) to one upper extremity. Each technic was applied to the motor point of the wrist extensor muscle group for twenty-minutes. With the subject placed in supine the technics were applied at 80 Hz, 2-10 mA(ALTENS) and 10 Hz, 4-12 mA(C-TENS). Results revealed: (1) a significant difference between the pretreatment and posttreatment in each group(p<.05) (2) no significant difference between ALTENS and C-TENS(p>.05). In conclusion, there was no difference between ALTENS and C-TENS for crossover effect.

  • Prostaglandins are generated through two isoforms of the enzyme cyclooxygenase, constitutively expressed cyclooxygenase(COX)-1 and COX-2, which is induced at sites of inflammation. Inhibition of COX-2 is desirable as this may avoid side effects seen with NSAIDs. We examined the effects of transcutaneous electrical nerve stimulation and cold therapy on the levels of muscle cycloooxygenase-2 mRNA in rats of carrageenan-induced inflammatory. The method of behavioral assessment were paw withdrawal latency(PWL) and tail flick test(TFT). The COX-2 mRNA levels were quantified by reverse transcription-polymerase chain reaction (RT-PCR). Following the transcutaneous electrical nerve stimulation and cold therapy, PWL and TFT were increased and COX-2 mRNA expression in gastrocnemius muscles were decreased. These results suggest that a transcutaneous electrical nerve stimulation and cold therapy were good therapy for a muscle pain.

  • Transcutaneous electrical nerve stimulation (TENS) is one of the representative physiotherapical modalities used for the treatment of various musculoskeletal disorders by the application of electrical stimuli. In dental practice, it has long been used in the treatment of acute and chronic orofacial pain conditions including temporomandibular disorders. TENS is the delivery of therapeutic electrical stimuli with a variety of electrical intensity, frequency and duration to stimulate peripheral nerve through surface electrodes with various form and placement. While controversy still remains over the clinical effectiveness and application of TENS, basic understanding of its electrical properties and the expected biological reactions is important to increase the therapeutic effect and decrease the risk of possible side effects. This review, therefore, focuses on basic understanding of TENS including its underlying mechanisms and stimulation parameters.

경피신경전기자극과 초음파가 전기생리학적 반응에 미치는 영향 (Effects on Electrophysiologic Responses to the Transcutaneous Electrical Nerve Stimulation and Ultra Sound)

  • The purpose of this study was to investigate the influnce of afferent stimuli, transcutaneous electrical nerve stimulation and ultra sound, on the electrdiagnostic study of normal subjects. Electrodiagnostic study was performed before and after the application of afferent stimulation of the right popliteal fossa on 18 healthy female volunteers. After the transcutaneous electrical nerve stimulation, there is no significantly change of latencies and amplitudes of SEP, H-reflex, peroneal nerve F-wave, and sensory nerve conduction. After the ultra sound, there is no significantly change of latencies and amplitudes of SEP, H-reflex, peroneal nerve F-wave, and sensory nerve conduction. Tibial nope F-wave and motor nerve shows prolonged latency after TENS and US (p<0.01). Ultrasound may have a similar mechanism of action compared to transcutaneous electrical nerve stimulation by having localized inhibitory effects of the peripheral nerve. However, further investigation is needed to assess their mechanism of action and the precise relevance of stimulation modality.

경피전기자극을 이용한 연접 전 억제가 만성 뇌졸중 환자의 global synkinesis에 미치는 영향 (The Effect of Presynaptic Inhibition Using the Transcutaneous Electrical Stimulation for Global Synkinesis on the Post-Stroke Hemiplegic Patients)

  • Purpose : This study was performed as follows in order to investigate the effect of presynaptic inhibition mechanism using the transcutaneous electrical stimulation (TES) for global synkinesis (GS) on the post-stroke hemiplegic patients. Methods : The subjects consist of 38 post stroke hemiplegic patients; experiments were performed on thirty patients excluding eight. The experiment was performed on sham group, sensory level stimulation group, and motor level stimulation group for 20 minutes a day 5 times a week for 6 weeks total. We compared the differences in GS levels and walking ability. The measurements were carried out pre, immediated, post 10th, and 20th, for a total of four measurements. Results : The GS level using sEMG found significant differences between groups at the post 10th and post 20th in dorsiflexion, and post 20th in plantarflexion (p<0.05, p<0.01). The motor level group indicated more significant differences when the number of electrical stimulations increased. TUG and 10 m walking test indicated a significant difference at immediated, post 10th, and post 20th. The motor level group showed more significant decreasing tendency than the sensory level group. Conclusion: From these results, electrical stimulation using presynaptic inhibition mechanism of transcutaneous electrical stimulation (TES) had positive effects for walking ability on inhibition of muscle tone in lower extremity. The motor level stimulation group experienced a more significant effect than the sensory level stimulation group. Therefore, the transcutaneous electrical stimulation (TES) is considered to be effective on walking ability increasing through inhibition of muscle tone in lower extremity for rehabilitation of post stroke hemiplegic patients.

고빈도 경피신경전기자극의 자극강도에 따른 정상 성인여성 교감신경성 반응의 변화 (Changes in Sympathetic Nervous System Responses of Healthy Adult Women with Changes in the Stimulus Intensity of High Frequency Transcutaneous Electrical Nerve Stimulation)

  • Purpose: The purpose of this study was to investigate the change in sympathetic nervous system responses of healthy adult women with changes in stimulus intensity of high frequency transcutaneous electrical nerve stimulation. Methods: Twenty-four healthy subjects (women) received high frequency electrical stimulation of the forearm. The subjects were randomly assigned to one of two groups; a low intensity stimulation group (n=12) and a high intensity stimulation group (n=12). The electrode attachment was arranged on the forearm of the dominant arm and the electricity stimulus time was 20 minutes. Measured items included skin conductance, pulse rate, skin temperature, and respiration rate. Each was measured at 4 times. Results: Skin conductance and skin temperature showed significant group by time interactions, though there were no significant group and time effects. There were no significant differences according to time, group effect, and a group by time interaction in pulse and respiration rates. Conclusion: High frequency and high intensity electrical stimulation may be helpful for the improvement of sudomotor function through the activation of the sympathetic nervous system. Also, high frequency and low intensity electrical stimulation may be helpful for the reduction of sudomotor function via inhibition of the sympathetic nervous system.

경피신경전기자극이 지연성근육통의 교감신경활동에 미치는 영향 (The Effects of Transcutaneous Electrical Nerve Stimulation on Sympathetic Nerve Activity in Delayed Onset Muscle Soreness)

  • This study conducts eccentric exercise ti non-dominant elbow flexor of 20 persons in order to examine the effects of transcutaneous electric nerve stimulation on sympathetic nerve activity in delayed onset muscle soreness, induces delayed onset muscle soreness, divides them into 10 persons respectively as experimental and control groups. And a stimulation for 2 min. with 100 pps is given to elbow flexor after repeated three times of 10 minutes rest, temperature, blood pressure and pulse are measured and as a result of two-way ANOVA, change of temperature didn't show a significant difference according to the elapse of times(p>0.05) and systolic pressure and pulses in showed a significant difference between experimental and control groups(p<0.05). These results suggest that transcutaneous electrical nerve stimulation has a direct or indirect influence on sympathetic nerve activity in delayed onset muscle soreness under a restricted condition of electrical stimulation.

경흉교감신경절부의 전기자극의 체표면 체열변화에 미치는 영향 (Influencing of Electrical Stimulation to Cervicothoracic Sympathetic Ganglion on the Temperature Change of Body Surface)

  • This study was carried out to determine the influencing of electrical stimulation to cervicothoracic sympathetic ganglion(CTSG; stellate ganglion) u the sympathetic tone. For the purpose of this study. the stimulation was given to both the interferential current stimulation(ICS: AMF 100Hz) group consisting of 10 person(males 8, females 2) and the transcutaneous nerve electrical stimulation(TENS: 100 Hz) group of 10(males 7, females 3) in the right side of the trachea with probe electrodes. Then. the temperature changes on the surfaces of the forehead. cheek, neck and internal ear of cephalocervix, which is subject to the influence of the cervicothoracic sympathetic ganglion, and the palm. which is the end of the upper limbs. measured before. immediately alter. 10 minutes after and 20 minutes after experiment. The results are summarized as follows. 1. The emergence of remarkable Horner's symptoms which appear due to the changes of the tone of cervicothoracic sympathetic ganglion was not seen. However, in the interferential current stimulation group there were two felt the sense of warmth in the facial region and one person who felt it in the upper limbs, and in the transcutaneous nerve electrical stimulation group there was each one person who felt the sense of warmth in the facial region and in the upper limbs, respectively. Both groups have each one person who felt the sense of oppression in the eyelids. Three persons of the interferential current stimulation group and two persons of the transcutaneous nerve electrical stimulation group have the sense of hoarse voice or numbness in the neck. These are the symptoms that appeared during stimulation, so it is difficult for them to be considered as the direct effects of the changes of the tone of cervicothoracic sympathetic ganglion. 2. The t-test was performed to determine the significance between the right, which is the experimental side, and the left, which is the non-experimental side. Significance between the right, which is the experimental side, and the left, which is the non-experimental side. Significant changes were seen in the necks of the interferential current stimulation group and in the cheeks and internal ears of the transcutaneous nerve electrical stimulation immediately after experiment(P<.05). And the interferential current stimulation group showed a very high significance in the cheeks immediately after experiment and in the necks ten minutes after experiment(p<.01). Therefore, it could be seen that the electrical stimulation had an influencing on the changes of body temperature of the cephalocervix. 3. In both the interferential current stimulation group and the transcutaneous nerve electrical stimulation group, the forecheads, checks and necks of the cephalocervix in the experimental side(right) rather than the non-experimental side(left) had mostly a statistically significant rise in temperature immediately after experiment. The one-way ANOVA was carried out to determine the temperature change of on the surface of the body with the lapse of time; before, immediately after, ten minutes after and tewenty minutes after experiment. But no statistical significance was found from both the right and left sides.

미주신경 자극을 위한 새로운 방법 : 비침습적 TENS 적용에 대한 미주신경 자극의 치료적 효과 (New Methods of Vagus Nerve Stimulation : Therapeutic Effects of Non-Invasive Vagus Nerve Stimulation by TENS Application)

  • Purpose: The purpose of this study was to investigate the therapeutic effect of non-invasive vagus nerve stimulation by transcutaneous electrical nerve stimulation application on the autonomic nervous system of human body. Methods: Participants were seventeen healthy adults. Standard deviation of all normal N-N intervals(SDNN), root mean square of successive differences(RMSSD), low frequency(LF), high frequency(HF) were compared in pre and post Mean values after intervention. Data were analyzed in Wilcoxon's signed-ranks test. Results: The results of this study is that sistolic blood pressure and pulse rate decreased mean value after non-invasive vagus nerve stimulation by transcutaneous electrical nerve stimulation. High frequency, low frequency, SDNN, RMSSD increased mean value in heart rate variability after intervention. But that is not significant except for SDNN. Conclusion: Non-invasive vagus stimulation by transcutaneous electrical nerve stimulation effect on parasympathetic nerve stimulation, and then it might be effective method for autonomic nerve balance control.

휴대폰 내장을 위한 경피신경 자극치료기의 설계 및 구현 (Design and Implementation of Transcutaneous Electrical Nerve Stimulation System for the Integration with Mobile Phone)

  • 최근 휴대폰의 성능이 향상되면서 다양한 부가기능을 내장한 멀티미디어 폰이 등장했다. 특히 웰빙에 대한 관심이 증가하여 바이오 기능을 휴대폰에 내장 하기위한 다양한 시도가 있어 왔다. 여러 가지 바이오 기능 중 경피 신경 전기치료기는 혈액 순환의 개선과 통증 억제의 기능이 있으므로 이를 휴대폰에 내장시키면 언제 어디서나 쉽게 전기 자극을 통해 혈액 순환을 개선하고 통증 억제를 할 수 있는 장점이 있다. 경피 신경 전기치료기를 휴대폰에 내장시키기 위해서는 소형 및 저전력의 자극모듈이 필요하며, 모듈이 오동작할 경우에도 사용자의 안전을 보장할 수 있어야 한다. 본 논문에서는 소형의 승압기를 이용하여 휴대폰에 내장가능 한 전기자극기를 설계 및 제작하였으며, 사용자의 안전을 위해 자극기의 출력단에 충전된 전체 에너지가 한꺼번에 출력되어도 인체에 무해하도록 설계하였다. 구현한 자극기 모듈의 검증을 위하여 소형의 마이크로 프로세서를 이용하여 테스트 시스템을 구현하여 성능을 검증하였다.
