Mild symptom patients' improvement rate and moderate to severe symptom patients' improvement rate was similar. Mean PASI score was increased until treatment week 4 to 8, and after that period, mean PASI score was decreased. • Objectives : The purpose of this study is to assess Traditional Korean Medicine (TKM) doctors' awareness and utilization of the case report, based on the opinion that careful observation on the case sometimes provides us with the important information as clinical trial especially in the traditional medicine research field. Methods : A questionnaire study was conducted among TKM doctors who participated in the annual continuous maintenance education (CME) held at 5 regions of the Republic of Korea. Results : Almost 60% of the respondents had read case reports published in medical journals and 67% had openly shared their clinical cases with their colleagues. Of the respondents, 18.6% had been educated on reporting cases, and only 16% had the experience of reporting cases on one's own. However, 32.6% of the respondents had the intention to report cases in the future. These results show significant differences between general physicians who holds a license but no hospital training experience and board certified TKM doctors who have training experience. Conclusions : A majority of TKM doctors have read case reports but holds little experience of having been properly trained. Through this research, it has been found that awareness of case reports is rising in hospital training. Thus, the objective of case report education for TKM doctors who have hospital training experience should be set on encouraging them to do more whereas for doctors without hospital training experience, the objective should be set on making them more exposed to case reports to heighten one's awareness.

  • Background: The recent outbreak of the novel strain of influenza A (H1N1) virus has raised a global concern of the future risk of pandemic. Traditional Korean Medicine(TKM) has been combatting against contagious diseases and developed its own particular and efficient way in treating those diseases. Objectives: Provide a distinctive and effective TKM method in understanding the principles of treatment, prevention, and contraindications against influenza A through case histories. Method: We revised case histories of eminent doctors of Ming and Qing dynasties according to their clinical manifestations similar to those of influenza A. We also verified prescriptions of the "Dong-uibogam(東醫寶鑑)" through examining clinical practices of China today. Results: 1. The subtypes of Warm disease; Wind Warmth and Pestilence has the most similar clinical manifestation in comparison to Influenza A. Specifying these terms is important in recognizing and classifying various diseases under the name of Warm disease. 2. TKM considered not only external factors but also individual factors like general condition, diet, emotion, constitutional types, etc in the treatment of febrile diseases. 3. TKM developed a new way in the treatment of contagious diseases. considering the characteristics of each pathogen. TKM described the Warm pathogen being light as a feather, which enters through the nose, principally affecting the Lung. Therefore, they emphasized treatment with Pungent Sweet Cooling Moistening[辛甘凉潤] method. 4. As the conservation of fluid and humor is the primordial concern in the treatment of Warm disease, they restricted release of the exterior with pungent-warm and purgation method. The purgation methods was used exclusively to decrease fever and preserve fluid and humor. 5. The only differentiating characteristics of Influenza A with seasonal influenza are vomiting and diarrhea. Case research revealed the possibility of these manifestations to be a mechanism of restoration. 6. TKM provides alimento prevention method like Mint Pear Porridge, mung bean, and etc also combination of herbal medicine. Also emphasized in the conservation of essence for the prevention of contagious diseases. Conclusions: TKM developed its unique way in understanding the pathogenesis, diagnosis, treatment, and prevention of contagious diseases and formed its independent scheme of Warm Disease. This knowledge in febrile contagious disease is relevant today in providing diverse treatment and prevention for influenza A.

  • Objectives: After the discontinuance of the Women's Health Initiative (WHI)clinical trial in the U.S. in July 2002, I thought that the domestic medical field would come to a crisis in hormonal treatment for perimenopausal and postmenopausal women. However, the domestic condition of HRT has barely changed. This study was designed to investigate world research trends in HRT and bring forward the response of the Traditional Korean Medicine (TKM) Research Group. Methods: I investigated recent domestic ;md world research trends about HRT and climacteric syndrome. Based on this analysis, I predicted western medical research trends and direction for HRT and medical care of perimenopausal and postmenopausal women. Results and Conclusions: I propose that we (TKM Research Group) have to explain the side effects of HRT for postmenopausal women and try to suggest complementary therapy for it. For that aim, the Association of Korean Medicine's effort is very important, as well as personal practitioner's actions.

  • Objectives : The purpose of this study is to investigate the practice patterns of traditional Korean medicine (TKM) doctors and the acceptance of payment model in order to develop a new TKM health insurance payment model linked with TKM clinical practice guidelines (CPGs). Methods : Lumbar herniated intervertebral disc (HIVD) and idiopathic facial palsy (IFP) were selected as a test diseases to develop a new TKM payment model. The level of benefit coverage in the National Health Insurance (NHI) was designed. The survey asked 228 TKM doctors about their practice patterns in HIVD and IFP patients and acceptance of new payment model. Results : Mean of medical cost for treatment of HIVD was 441,000 KW, mean of treatment period ranged from 4.9 to 17.5 weeks, and mean of number of treatment ranged from 14.6 to 50.4 HIVD patients. In the case of IFP, mean of medical cost for treatment of IFP was 468,000 KW, mean of treatment period was at least 4.2 and up to 15.9 weeks and mean of number of treatment ranged from 14.2 to 52 IFP patients. Conclusions : Current study suggests that mixed payment model of per-visit and episode-based model seem to be proper. The model 1 bundles both items which were covered and not covered by NHI in a rational way. The model 2 is based on the development and application of critical pathway. Lastly, model 3 suggests bundling of items covered by current NHI. Acceptance of TKM doctors is expected to be highest in the model 3.

And all the symptom of uterine myoma, Gonadotropin-releasing hormone(Gn-RH)'s side effects were improved. The metrorrhagia of the second case was stopped, and was not repeated during six month. The second symptoms of metrorrhagia were improved. So, the two cases don't need to have a TAH. Conclusion : The results indicate that TKM can be effective for the management of uterine myoma for TAH without operation. Further study will be investigated in TKM for healing uterine myoma for TAH completely.
