항암치료 받는 악성 뇌종양 환자의 재발여부에 따른 지질프로필 비율의 비교 (Comparison of Lipid Profile Ratios in Patients with High-grade Brain Cancers according to the Presence of Recurrence during Cancer-related Therapy)

  • Purpose: The purpose of this study was to identify the lipid profile ratios as factors affecting disease progress in patients with high-grade primary brain cancers undergoing concurrent chemoradiotherapy (CCRT) and adjuvant chemotherapy. Methods: The levels of lipid profile ratios were evaluated by looking at the total cholesterol (TC) to high-density lipoprotein (HDL)-cholesterol (TC/HDL-c), low-density lipoprotein (LDL)-cholesterol to HDL-cholesterol (LDL-c/HDL-c), and triglycerides to HDL-cholesterol (TG/HDL-c). This descriptive research was conducted 7 months after the initiation of CCRT and adjuvant chemotherapy. Results: A total of 36 patients with newly diagnosed primary malignant brain cancer were included in the study. The levels of lipid profile ratios such as TC/HDL-c, LDL-c/HDL-c, TG/HDL-c were significantly different between the patients with and without disease progress at 7 months after initiation of CCRT and adjuvant chemotherapy. Conclusion: The lipid profile ratios were indicators affecting disease prognosis with tumor-related factors at 7 months after initiation of CCRT and adjuvant chemotherapy. Therefore, lipid profile ratios indicating hyperlipidemia in patients with high-grade brain cancers should be carefully monitored during and after cancer-related therapy.

방풍통성산가미방(防風通聖散加味方)이 비만유도(肥滿誘導) 백풍(白嵐)와 지방세포(脂肪細胞) 분화(分化)에 미치는 영향(影響) (Anti-obestic Effects of Bangpoongtongsungcankamibang(BTSK) in 3%3-L1 preadipocyte cells and the Lipid Metabolism of Obesity induced Rats)

  • Objective : This experimental study was designed to determine the anti-obestic effects of Bangpoongtongsungcankamibang(BTSK). Method : In vitro, BTSK extracts of various concentration (50,100, 200 $200{\mu}g/ml$)were added examination. The protein and mRNA expression of $C/EBP{\alpha}$ and $PPAR{\gamma}$ receptor was measured by western blot assay and RT-PCR. In vivo, BTSK extracts of various concentration (100, $200{\mu}g/ml$) were orally administered to induced hyperlipidemic rats by poloxamer-407 for consecutive four weeks and serum triglyceride, total cholesterol were measured. This method applied to induced hyperlipidemic rats by triton WR-1339, too. Obesity induced rats by the high fat-diet for six weeks were orally administered BTSK extracts of various concentration (100, $200{\mu}g/ml$) and serum triglyceride, total cholesterol, LDL-cholesterol, triglyceride, LDL-cholesterol, triglyceride, HDL-cholesterol, hydroxy radical, superoxide dismuatse activity were measured. Results : I. In vitro 1. The 3T3-L1 cells' differentiation was significantly decreased by BTSK. 2. expression of $C/EBP{\alpha}$ and $PPAR{\gamma}$ was was significantly decreased by BTSK. II. In vitro 1. BTSK significantly reduced serum triglyceride, total cholesterol, contents in poloxamer-407 treated rat. 2. BTSK significantly reduced serum triglyceride, contents in Triton WR-1339 treated rat. But Total cholesterol did not show a significant change. 3. BTSK significantly reduced body weight gain of rat and adipose tissue mass of rats and serum triglyceride, LDL-cholesterol, contents and significantly increased HDL-cholesterol, HTR(HDL-cholesterol/Total-cholesterol) in rats with obesity induced by the high fat-diet. 4. BTSK reduced blood lipid peroxide, hydroxy radical and increased superoxied dismuatse(SOD) activity.

Validity for Use of Non-HDL Cholesterol Rather than LDL Cholesterol

  • NonHDL cholesterol values have been suggested as a risk marker for cardiovascular disease. NonHDL cholesterol values were calculated, using a very simple measurement [nonHDL cholesterol=serum total cholesterol-HDL cholesterol]. This formula is very useful as a screening tool for identifying dyslipoproteinemias, risk assessment, and assessing the results of hypolipidemic therapy. The data from the 2009 Korean National Health and Nutrition Examination Survey were used. Analysis was done for 1,992 subjects with lipid panels (Cholesterol, HDL, LDLdirect and Triglycerides) results. We studied the relationship between nonHDL cholesterol and LDL cholesterol. As a result, nonHDL cholesterol values were plotted against the LDL direct and calculated values. The linear regression equation for nonHDL cholesterol and direct LDL cholesterol was $nonHDLchol=23.60+1.03{\times}LDLdirect$ (p<0.0001, $r^2=0.80$) in all subjects. The subjects were classified into triglyceride values. When triglycerides are below 400 mg/dL, the linear fit to LDL direct is found to be $[nonHDLchol=17.34+1.07{\times}LDLdirect]$ (p<0.0001, $r^2=0.88$) and to the Friedewald LDL calculation is $[nonHDLchol=23.10+1.02{\times}LDLcalc]$ (p<0.0001, $r^2=0.82$). For triglycerides above 400 mg/dL, the linear fit equation is $[nonHDLchol=87.57+0.92{\times}LDLdirect]$ (p<0.0001, $r^2=0.50$) and to the LDL calculated, it is $[nonHDLchol=142.70+0.50{\times}LDLcalc]$ (p<0.0001, $r^2=0.32$). This study provides examples of the utility of nonHDL cholesterol concentrations in clinical medicine.

  • Ojectives : Cheonggukjang(natto) is known to have anti-hyperlipidemic action. This study was designed to investigate whether Herbal-cheonggukjang (HC, Herbal-natto) is more effective than cheonggukjang only, Methods : We investigated anti-hyperlipidemic effects of HC in hyperlipidemic rats induced by high-fat diet in terms of observation on levels of total cholesterol, HDL-cholesterol and triglyceride in serum. HC was made by cheonggukjang added Codonopsis Lanceolata, Houttuynia cordata and Lentinus edodes in indicated concetrations. Results : In this experiment, hyperlipidemic rats showed elevated levels of total cholesterol and triglyceride, and also showed lowered level of HDL-cholesterol. Oral administration of HC decreased levels of total cholesterol and triglyceride, Cheonggukjang(natto) did not affect levels of total cholesterol, HDL cholesterol and triglyceride. Conclusions : In these results, oral administration of HC decreased total cholesterol and triglyceride effectively. These results demonstrate that HC is useful to modulate cholesterol and triglyceride in serum.

중년여성의 에어로빅댄스의 수행기간이 혈청 HDL-Cholesterol에 미치는 영향 (Effects of Exercise Periods of Aerobic Dance on the Serum HDL-Cholesterol in Middle - Aged Women)

  • This study was designed to find out the effects of aerobic exercise on the serum lipids in the middle-aged women. The effects of aerobic dancing on serum total cholesterol(TC), triglyceride(TG) and HDL-cholesterol (HDL-C) were studied in eight sedentary women(control group) and twenty seven aerobic exercising women(aerobic group) , aged 35∼45%. Aerobic exercising subjects were divided into 3 group; 2 to 3 months exercising group(Al), 4 to 10 months exercising group(AH), over 1 year exercising group(AIII) according to the periods of exercise. The serum lipid levels of aerobic exercising groups(AI, AII, AIIII) were compared with those of control group. The results statistically analyzed were summarized as follows : 1) The serum TG levels of aerobic groups decreased very significantly compared with those of control group(p< 0.001) and tended to be lower with the increase in aerobic periods. But there were no significant differences among aerobic groups by ANOVA. 2) The serum TC levels of aerobic groups decreased compared with those of control group and tended to be lower with the increase of aerobic periods. But there were no significant differences among groups. 3) Serum HDL-cholestrol level of A I group was elevated significantly compared with that of control group and significant difference was observed according to the aerobic periods : AR group exhibited higher serum HDL-cholesterol values than AH group, and AH group higher than A I group.

스테로이드에 반응을 보인 신증후군 환아에서 재발 예측인자로서 고지혈증의 중요성 (The Significance of Hyperlipidemia as a Predictive Factor of Relapse in Corticosensitive Nephrotic Syndrome)

  • 목 적 : 신증후군 환아의 궁극적인 예후는 양호하지만 치료시의 어려움은 빈번한 재발이다. 스테로이드에 반응을 보였던 신증후군 환아의 재발 가능성의 예측인자에 대한 여러 보고가 있었는데 최근에는 스테로이드에 반응을 보이는 신증후군 환아에서 관해기까지의 기간이 짧을수록 재발의 빈도가 의미 있게 적음이 보고되었다. 본 연구에서는 신증후군으로 처음 진단 후 스테로이드에 반응을 보였던 환아들을 대상으로 스테로이드를 사용하기 전의 고지혈증의 정도와 환아의 신기능 및 관해 유도기간과의 연관성을 검증하여, 고지혈증이 관해유도의 지연 혹은 빈발 재발 환자를 조기에 선별하여 예측지표로 사용될 수 있는지 알아보고자 시행하였다. 대상 및 방법 : 1994년 10월부터 2000넌 8월까지 본원 소아과에서 입원했던 환아들 중 처음으로 신증후군으로 진단된 후 스테로이드 치료에 반응을 보였던 33명을 대상으로 이들의 입원 및 외래 기록지를 중심으로 후향적으로 분석하였다. 결 과 : 모든 대상 환아에서 고지혈증 소견이 관찰되었으며 이중 triglyceride, total cholesterol, LDL cholesterol, apoB, Lp(a), total cholesterol/HDL cholesterol은 증가하였으나 HDL cholesteol은 증가하지 않았다. 스테로이드를 사용하기 전의 혈청 알부민은 $1.7{\pm}0.8\;g/dL$의 감소되었고 단백뇨는 $3.8{\pm}3.2\;g/24hr$로 증가보이나 크레아티닌 청소율은 $121.7{\pm}47.7\;mL/min/1.73m^2$로 감소되지 않았으며 스테로이드 사용 후 관해 유도기간은 $15.6{\pm}11.0일$이 필요했다. 혈청 알부민은 total cholesterol (r = -0.5157, P<0.005), LDL cholesterol (r = -0.5543, P<0.005), total cholesterol/HDL cholesterol (r = -0.4506, P<0.01), Lp(a) (r = -0.4570, P<0.025), apoB (r = -0.5297, P<0.025), apoB/apoAl (r = -0.5851, P<0.01), apoB/HDL cholesterol (r = -0.4961, P<0.05)등과 음의 상관관계가 있었다. 단백뇨와 크레아티닌 청소율은 스테로이드를 사용하기 전 혈청지질, 지단백과 상관관계를 보이지 않았다. 관해 유도기간은 스테로이드를 사용하기 전 HDL cholesterol (r = +0.4511, P<0.05), apoB (r = +0.5190, P<0.05), apoB/HDL cholesterol (r = +0.7169, P<0.005)과 양의 상관관계가 있었다. 결 론 : 본 연구를 통해 신증후군의 특징인 고지혈증은 서로 다른 경과의 신증후군, 서로 다른 병리조직학적 소견과 지질대사 과정에 다양한 영향을 끼치는 요인에 의해 각각의 지질 및 지단백의 상승에 차이가 있을 수 있고 이런 지질 및 지단백의 상승은 주로 간에서 생성의 증가의 발생함을 알 수 있었다. 또한 지단백중 스테로이드를 사용하기전의 HDL cholesterol, apoB, apoB/HDL cholesterol등은 관해 유도기간과 의미 있는 양의 상관관계를 보여서 신증후군 환아의 재발을 예측 할 수 있는 지표로서 임상적인 의미를 부여할 수 있을 것이다.

이상지혈증을 동반한 제2형 당뇨병환자에서 미세화된 fenofibrate (Comparison of Efficacy Between Micronised- and Non-micronised Fenofibrate in Type 2 Diabetic Patients with Dyslipidemia)

  • Fenofibrate is a fibric acid derivative that is a strong reducer of triglyceride. Micronozed formulation of fenofibrate has improved bioavailability compared to non-micrornized formulation. This study performed a retrospective comparison of micrornized and non-micrornized fenofibrate (28 in micronized and 51 in non-micronized group) by comparing the means of changes in total triglyceride, total cholesterol, HDL-cholesterol and TC/HDL ratio in type 2 diabetics with dyslipidemia The result skewed that after 12 weeks of treatment both drugs produced a significant reduction in total triglyceride levels (62% with micronized, 37% with non-micronized). The mean decrease observed for total triglyceride levels were significantly lower for micronized fenofibrate (p<0.001). Both drugs showed a significant reduction for total cholesterol levels (-22% with micronized, -14% with non-micronized fenofibrate). The mean decrease observed for total cholesterol was not significantly different between the two drugs (p>0.05). HDL-cholesterol levels increased by 24% and 15%) with micronized and non-micronized, respectively and the differences from the baseline were statistically significant for both drugs (p<0.05). The mean change of HDL-cholesterol was not significantly different between the two drugs. There was a statistically significant reduction in TC/HBL-cholesterol ratio from baseline for both drugs (7.1 to 4.8 with micronized and 5.1 to 4.5 with non-micronized), and the reduction of TC/HDL-cholesterol ratio tended to be significantly greater with micronized fenofibrate (p<0.05). This study shows that short-term treatment with micronized fenofibrate is more effective than non-micronized fenosbrate in type 2 diabetes patients with dyslipidemia.

Effects of Wet Cupping (Al-Hijamah) on Cholesterol in a Sudanese Population

  • Background: Wet cupping (WC) is an efficient and cost-effective technique for removing metabolic waste from the bloodstream via the skin. The study aimed to examine the effect of WC on cholesterol levels including total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) in a Sudanese population. Methods: In this prospective cross-sectional study, 30 participants undergoing regular WC therapy were enrolled. Blood samples collected twice: pre-WC therapy (case group) and 10-14 days afterward (controls). Results: Of the participants, 56.67% were male and 43.33% were female, aged 24-69. Pre-WC TC and LDL-C levels were significantly higher than the post-WC control group (p = 0.001). Conversely, HDL-C levels decreased significantly in the pre-WC cases compared to controls (p = 0.001). No significant sex-based difference in mean cholesterol levels (p > 0.05). Conclusions: After WC, males and females experienced significant reductions in TC and LDL-C, and significant increase in HDL-C.

중년 남성 근로자에서 신체 적합도, 여가중 신체 활동과 혈중 지질 농도 (Physical Fitness, Leisure Time Physical Activity, and Serum Lipid Levels in Middle-Aged Male Workers)

  • 본 연구는 40세 이상 중년의 남자 근로자 253명을 대상으로 신체 적합도 및 여가중 신체 활동과 혈중 지질 중 심혈관질환 발생에 특히 중요한 혈중 총콜레스테롤 값, HDL 콜레스테롤 값 및 총콜레스테롤/HDL 콜레스테롤 비와의 관련성을 알아 보기 위하여 시도된 단면조사연구이다. 단순분석에서 혈중 총콜레스테롤 값과 유의한 상관 관계가 있는 변수는 신체 적합도(r=-0.27), 비만지수(r=0.27)였다. 여가중 신체 활동은 총콜레스테롤 값이 170mg/dl 이상군에서만 총콜레스테롤 값과 유의한 상관관계(r= -0.20)가 있었다. HDL 콜레스테롤 값과 유의한 상관관계가 있는 변수들은 신체 적합도(r=0.15), 비만지수(r=-0.22)및 1주간 음주량(r=0.14)이였다. 총콜레스테롤/HDL 콜레스테롤 비와 유의한 상관관계가 있는 변수들은 신체 적합도(r=0.23), 비만지수(r=0.32), 총흡연지수(r=0.13), 1주간 음주량(r=-0.13) 및 야채 선호도(r=0.13)이었다. 신체 적합도는 안정시 맥박수와 중등도의 상관관계(r=-0.66)가 있었고, 여가중 신체 활동(r=0.19)및 비만지수(r=-0.18)와도 유의한 상관관계가 있었다. 다중회귀분석(Multiple linear regression) 결과 혈중 총콜레스테롤 값과 유의한 관련성을 보이는 변수는 신체 적합도(beta=-0.23)와 비만지수(beta=0.18)이었다. HDL 콜레스테롤 값과 유의한 관련성을 보이는 변수는 비만지수(beta=-0.25)뿐이었다. 총콜레스테롤/HDL 콜레스테롤 비와 유의한 관련성을 보이는 변수는 비만지수(beta=0.30)와 신체 적합도(beta=-0.16) 및 야채선호도(beta=0.14)이었다. 결론적으로 중년 남성 근로자들에서 신체 적합도는 비만지수와 함께 혈중 총콜레스테롤 값, HDL 콜레스테롤 값, 총콜레스테롤/HDL 콜레스테롤 비의 중요한 관련 요인이었다. 여가중 신체 활동은 혈중 총콜레스테롤 값이 170mg/dl 이상인 군에서만 혈중 총콜레스테롤 값과 관계가 있었고, 신체 적합도와는 관련성이 유의하지만 크지 않았다. 따라서 신체 적합도가 여가중 신체 활동보다 혈중 지질 농도의 더 중요한 관련요인이며, 앞으로 신체 활동에 관련된 역학적 연구에서 신체 적합도가 독립적이고 중요한 요인으로 추가되어야 할 것이다.
