Supernumerary tooth is developed from overgrowth of dental laminar and is mainly observed in the maxillary anterior area, named 'mesiodens. Mesiodens cause many problems such as malposition of permanent anterior tooth, diastema, formation of cyst, and eruption to oral or nasal cavity. Extraction, therefore, is generally recommended choice of treatment. This case report describes that the patient is a 9 years and 8 months old boy, whose unilateral maxillary incisor was missing due to trauma, and he had a mesiodens. We practiced autotransplantation of the mesiodens to the missing maxillary central incisor area and then restored with prosthetics, substantially kept out alveolar bony resorption and provided esthetic appearance. Mesiodens was too small to be used as an abutment because its length is totally 14mm, so that poor prognosis was anticipated due to bad crown/root ratio. However, we observed clinically acceptable result during the observation period of 8 months after operation.

The ectopic eruption is defined rise to be abnormal eruption, which gives to displacement of the teeth and abnormal root resorption of adjacent teeth. The ectopic eruption of first permanent molar is approximately 2-4% of the population, most of them are in the maxilla, rarely in the mandible. In the case of the second permanent molars, most of them are found in the mandible for the reason of the arch length discrepancies and large size of the teeth. The ectopic erupted molars should be treated early in order to coordinate normal growth pattern and to obtain good occlusal support. So, this should be early diagnosed and treated. But, ectopically erupting molars are often self-corrected, hence periodic follow-up is required. The methods of the treatment are largely classified into surgical, surgical-orthodontic, and orthodontic method. Especially in orthodontic method, they are divided as follows; appliances that is positioned at the contact point for unlocking and the distal movement, fixed and removable appliance that is connected to more than one tooth, and occlusion guiding method after disking or extraction of the second deciduous molar. The report presents the good results in treating patients, whose chief complaint was ectopic eruption of mandibular permanent molars.
Objectives: To determine the effect of size and insertion depth of irrigation needle on the amount of apical extruded debris and the amount of penetration depth of sealer using a confocal laser scanning microscope (CLSM). Materials and Methods: Twenty maxillary premolars were assigned to 2 groups (n = 10), according to the size of needle tip, 28 G or 30 G. Buccal roots of samples were irrigated with respective needle type inserted 1 mm short of the working length (WL), while palatal roots were irrigated with respective needle type inserted 3 mm short of the WL. Prepared teeth were removed from the pre-weighed Eppendorf tubes. Canals were filled with F3 gutta-percha cone and rhodamine B dye-labeled AH 26 sealer. Teeth were transversally sectioned at 1 and 3 mm levels from the apex and observed under a CLSM. Eppendorf tubes were incubated to evaporate the irrigant and were weighed again. The difference between pre- and post-weights was calculated, and statistical evaluation was performed. Results: Inserting needles closer to the apex and using needles with wider diameters were associated with significantly more debris extrusion (p < 0.05). The position of needles and level of sections had statistically significant effects on sealer penetration depth (p < 0.05 for both). Conclusions: Following preparation, inserting narrower needles compatible with the final apical diameter of the prepared root canal at 3 mm short of WL during final irrigation might prevent debris extrusion and improve sealer penetration in the apical third.
The purpose of this study was to compare the sealing ability of root canal obturation with or without the treatment of smear layer. Eighty extracted human teeth with one canal were selected Instrumentation was performed with crown-down technique. After instrumentation, root canals of the NaOCl group and NaOCl-6 group were irrigated with 3% NaOCl. EDTA group and EDTA-6 group were irrigated with 17% EDTA. Then all teeth were obturated using continuous wane obturation technique NaOCl group and EDTA group were immersed in methylene blue solution for 84hours. NaOCl-6 group and EDTA-6 group were immersed in methylene blue solution for 6months. The teeth were sectioned at 1.5 mn (Level 1), 3.0 mm (Level 2) and 4.5 mm (Level 3) from the root apex. The length of dye-penetrated inter-face and the circumferential length of canal at each level were measured using Sigma-Scan Pro 5.0. 1. The mean leakage ratio was decreased cervically. 2. NaOCl group showed higher mean leakage ratio than EDTA group at each level. But there was significant difference at level 1 only (p < 0.05). 3. NaOCl-6 group showed higher mean leakage ratio than EDTA-6 group at each level. But there was significant difference at level 1 only (p < 0.05). 4. NaOCl-6 group showed higher mean leakage ratio than NaOCl group at each level. But there was significant difference at level 1 only (p < 0.05). 5. EDTA-6 group showed higher mean leakage ratio than EDTA group at each level. But there was no significant difference. 6. In NaOCl group and NaOCl-6 group, scanning electron micrographs of tooth sections generally covered with smear layer. In EDTA group and EDTA-6 group, tooth sections showing the penetration of sealers to opened dentinal tubules. The results suggest that removal of smear layer was effective to reduce the apical microleakage of the root canal.

Impacted molars are not usual with the prevalence rate of less than 1%, however problems such as infraocclusion, extrusion of the opposite tooth and lack of space are caused. If molars are impacted, radiographic check-up and observations are needed as they play an important role in skeletal development and mastication. Causes of impaction are underdevelopment of roots, supernumerary teeth, cysts, odontomas, ankylosis and etc. In our first case, we present impacted molar which was surgically exposed and orthodontically traded. As the developmental state of the root was less than half at first visit, we observed until the root was 2/3 in its length and traction was carried out. The second case is orthodontic traded molar with compleate enucleation of the existing cysts. When trading impacted molars, direct bonding of attachments to the tooth and light continuous forces are recommended so as to ankylosis, external absorption and periodontal attachment loss avoid. Fixed appliances tract the teeth on arch and obtain eruption spaces without patient's cooperation. We report surgically exposed and orthodontically traded molars which resulted in good occlusion and patients satisfaction.
Background: The concept of the ideal morphology for the alveolar bone form is an important element to reconstruct or restore the in maximizing esthetic profile and functional alveolar bone restoration. The purpose of this preliminary study is to evaluate the normal alveolar bone structure to provide the standard reference and guide template for use in diagnosing for implant placement, determining the correct amount of bone augmentation in actual clinical practice and producing prostheses based on three-dimensional imaging assessment of alveolar bone. Methods: This study was included 11 men and 11 women (average age, 22.6 and 24.5 years, respectively) selected from among 127 patients. The horizontal widths of alveolar bone of maxilla and mandible were measured at the crestal, mid-root, and root apex level on MDCT (multi-detector computed tomography) images reconstructed by medical imaging software. In addition, tooth dimensions of the central incisors, canines, second premolars, and first molars of maxilla and mandible, including the horizontal width of the interdental alveolar bone crest, were also measured and statistically analyzed. Results: The horizontal alveolar bone width of the palatal side of maxilla showed a distinct increment from the alveolar bone crest to the apical region in both anterior and posterior areas. The average widths of the maxillary alveolar ridge were as follows: central incisor, 7.43 mm; canine, 8.91 mm; second premolar, 9.57 mm; and first molar, 12.38 mm. The average widths of the mandibular alveolar ridge were as follows: central incisor, 6.21 mm; canine, 8.55 mm; second premolar, 8.45 mm; and first molar, 10.02 mm. In the buccal side, the alveolar bone width was not increased from the crest to the apical region. The horizontal alveolar bone width of an apical and mandibular border region was thinner than at the mid-root level. Conclusions: The results of the preliminary study are useful as a clinical guideline when determining dental implant diameter and position. And also, these measurements can also be useful during the production of prefabricated membranes and customized alveolar bone scaffolds.

In order to explore the influence of tooth root cracks on the dynamic characteristics of multi-stage planetary gear transmission systems, a concentrated parameter method was used to construct a nonlinear dynamic model of the system with 30-DOF in bending and torsion, taking into account factors such as crack depth, length, angle, error, time-varying meshing stiffness (TVMS), and damping. In the model, the energy method was used to establish a TVMS model with cracks, and the influence of cracks on the TVMS of the system was studied. By using the Runge- Kutta method to calculate the differential equations of system dynamics, a series of system vibration diagrams containing cracks were obtained, and the influence of different crack parameters on the vibration of the system was analyzed. And vibration testing experiments were conducted on the system with planetary gear cracks. The results show that when the gear contains cracks, the TVMS of the system will decrease, and as the cracks intensify, the TVMS will decrease. When cracks appear on the II-stage planetary gear, the system will experience impact effects with intervals of rotation cycles of the II-stage planetary gear. There will be obvious sidebands near the meshing frequency doubling, and the vibration trajectory of the gear will also become disordered. These situations will become more and more obvious as the degree of cracks intensifies. Through experiments, the theoretical results are in good agreement with experimental results, verifying the correctness of the theoretical model. This provides a theoretical basis for fault diagnosis and reliability research of the system.
The purpose of this study was to compare the apical leakage of the root canal filled with the System B and the EndoTwinn(the combined application of heat and ultrasonic vibration). Sixty extracted premolars with straight root were cleaned and shaped to size 35. Group SB was obturated using System B and Group ET was filled with EndoTwinn. A size 35 of 0.06 tapered gutta-percha and Adseal were used and the plugger which could be introduced to 4 mm short of working length was selected in the obturation procedure. As the positive control, Group PC was not filled. In Group SB, ET and PC, all external surfaces of each tooth were coated with nail varnish leaving only 1 mm area around the apical foramen. In the negative control of Group NSB and Group NET, all of external tooth surface including apical foramen was coated with the nail varnish. The specimens were immersed in methylene blue dye solution for 2 days. Then the specimens were sectioned at each 1 mm from apex to 5 mm level. The final score of one specimen was given by summing up of the points at all levels. The dye leakage of Group ET was significantly less than that observed in Group SB (p < 0.05). And the frequency of gutta-percha pulling out from root canal when the plugger was removed was more often with the System B than with EndoTwinn but there was no significant difference.

Cantilever bridge is widely used by mny clinicians, but its worst mechanical character, so called Class I lever system, makes dentists hesitate to restore the missing tooth with it. Therefore it is important to study stress of the cantilever bridge. In this study, two models of cantilever bridges that restores the missing mandibular second molar with two abutment teeth were constructed. One model was a type of cantilever bridge supported by a normal alveolar bone, the other one was supported by an alveolar bone resorbed to its 1/3 of root length. Maximum bite force(550N) and funtional maximum bite force(300N) were vertically applied to the distal end of the pontic, distal 1/3, and distal half of the pontic. And each force was also applied to centric occlusal contacts as a distributed force. Total 16 loading cases were compared and analyzed with 3-dimensional finite element method. The results were as follows: 1. The stress was concentrated on the joint of the pontic and the retainer, grooves, and distal cervical margin of the posterior retainer. 2. In case of maximum bite force(550N) at the end of the pontic, the risk of fracture at the joint of the pontic and the retainer was high. 3. In case of distributed force in centric occlusion and functional maximum bite force(300N), the stresses were less than the yield strength of the type VI gold for any loading cases. 4. In case of alveolar bone resorption, the occlusal force to the cantilever pontic caused more stress on the root apex and less stress on the alveolar crest region of the distal surface of the posterior abutment. 5. In case of alveolar bone resorption, the displacement was larger than that of normal alveolar bone in all loading cases.

Purpose: The purpose of this study was to evaluate the push-out bond strength of glass-fiber post cemented with different adhesive systems and surface treatments. Materials and methods: 160 tooth samples made from 48 human maxillary single-rooted teeth with similar root length were divided into 4 groups according to the adhesive system (no adhesive, Adper Single Bond 2, Clearfil SE Bond, Clearfil S3). Each group had 4 subgroups according to the post surface treatment methods (no treatment, sandblast, silane, sandblast and silane). Posts (Parapost Fiber White) were cemented with Rely X Unicem. The teeth were sectioned perpendicular to their long axis into 1-mm thick sections. The push-out tests was performed at a speed of 0.5 mm/min. The results were evaluated by 2-way ANOVA, 1-way ANOVA and multiple comparison procedures (Tukey test) (${\alpha}=0.05$). Results: Tukey test showed that the adhesive system significantly influenced the push-out strength. The Clearfil SE Bond group showed the highest value. Post surface treatments showed no significant effect. Conclusion: Bond strength of glass-fiber post cemented with self-adhesive resin cement using Clearfil SE Bond showed significantly higher values compared to other adhesive systems.
