Effect of local anesthesia on postoperative pain and hemostasis after dental rehabilitation under general anesthesia in pediatric patients: a randomized control trial

  • Background: This study aimed to investigate the effect of local anesthesia (LA) on postoperative pain and hemostasis after dental rehabilitation under general anesthesia (DRGA) in pediatric patients. Methods: A total of 43 patients, aged 3-7 years and rated ASA I or II, who had a definitely negative rating on Frankel's behavior rating scale, were included in this two-arm, parallel-design, single-blinded, randomized, controlled study. The patients were allocated equally into two main groups receiving both restorative treatments and tooth extractions. Two pain scales and one bleeding scale were used. In Group A, the treatment was done with LA, and in Group B, the treatment was done without LA. Results: The statistical analysis revealed no significant differences in the pain scores between the groups. It also revealed significant differences in the bleeding scores between the groups but no significant differences in the duration of bleeding. Conclusion: Within the limitations of this study, the use of LA in pediatric dental patients undergoing DRGA had no effects on postoperative pain reduction or bleeding duration after teeth extraction. We also observed that the use of LA had an impact on the reduction in the bleeding scores in pediatric dental patients undergoing DRGA.

  • Hemangioma is a lesion characterized by vascular proliferation of endothelial origin, manifesting neoplastic features. The occurrence of central hemangioma in the oral and maxillofacial region is exceptionally rare, and in two-thirds of cases, it is predominant in the mandible rather than the maxilla. The main symptoms and signs associated with central hemangiomas include pulsation, bone expansion, bruit, teeth displacement, and root resorption of the adjacent teeth. Bleeding may manifest periodically from the sulcus surrounding the affected teeth, particularly when accompanied by hypermobility in the primary dentition. One of the most noteworthy complications is the potential for severe bleeding during tooth extraction or surgical procedures conducted in proximity to unrecognized hemangiomas. Such situations may pose a life-threatening risk. Taking this into consideration, we present two cases of central hemangiomas in adolescent patients who sought consultation, with subsequent embolization performed by the Department of Radiology in Chosun University Hospital.

Lupus anticoagulant hypoprothrombinemia syndrome associated with a hemorrhagic ovarian cyst in a girl with systemic lupus erythematosus: a case report

  • Lupus anticoagulant hypoprothrombinemia syndrome (LAHPS) is a rare entity characterized by the presence of lupus anticoagulant (LA) and prothrombin (factor II) deficiency. It may cause severe bleeding contrary to classical antiphospholipid syndrome. Here, we report a case of LAHPS presenting with a hemorrhagic ovarian cyst in a 17-year-old girl with systemic lupus erythematosus (SLE) nephritis. She had been followed up for 8 years. Her first manifestation of SLE was prolonged gingival bleeding after tooth extraction at 9 years of age. During the follow-up period, she had neither severe bleeding nor thrombotic complications despite a positive LA and a prolonged activated partial thromboplastin time (aPTT). At this visit, the patient presented with colicky abdominal pain, a hemorrhagic ovarian cyst, a prolonged prothrombin time, a prolonged aPTT, a low factor II level, and a positive LA, leading to the diagnosis of LAHPS. While a hemorrhagic ovarian cyst resolved completely in 3 months, she received oral pill, transfusions of red blood cells and plasma, and intravenous cyclophosphamide pulse therapy in combination with glucocorticoids due to persistent menorrhagia, anemia, prolonged aPTT, and lupus flaring. Thus, LAHPS needs to be considered in SLE patients with positive LA and prolonged aPTT.

ERUPTION PATTERN OF A CYST-ASSOCIATED MANDIBULAR PREMOLAR AFTER MARSUPIALIZATON OF A DENTIGEROUS CYST (함치성 낭의 조대술 후 이환치아의 맹출 양상)

  • The purpose of this study was to evaluate the eruption pattern of a cyst-associated mandibular premolar after marsupialization of a dentigerous cyst in children. The result from the twenty two pairs of normalized panoramic radiographs of twenty two patients who underwent neither extraction nor orthodontic traction of the cyst-associated mandibular after marsupialization were as follows. 1. The eruption speed of a cyst-associated premolar was 3.5 times faster than that of the normal contra-lateral premolar(p<0.05). The angulation change of test group was an average of $2.7^{\circ}$ per month. 2. In the change of the level of root formation, the group which had a little root maturity tended to be faster in the eruption speed(p>0.05) and the R1/4 group had a great change in the angulation change(p>0.05). 3. In the change to be with the cusp position index, the eruption speeds were increased to the 30% deviation groups. But, the eruption speed was decreased above that(p>0.05). 4. Group with cyst diminishment rate of more than $80mm^2$ per month showed the fastest eruption speed of all(p<0.05). 5. The mesial angulated teeth in the test group were decreased more rapidly than the distal angulated teeth in the eruption speed(p>0.05). But, increased in the angulation change(p<0.05). 6. There was a tendency of faster tooth eruption with less deviation of tooth axis. Group with tooth axis deviation of less than $15^{\circ}$ showed the fastest angulation change of all(p<0.05). 7. The eruption speed and the angulation change rapidly decreased during the first 6 months following marsupialization. Based on the results of this study, a cyst-associated mandibular premolar erupted more rapidly after marsupializaton. We thought so that it's prognosis was good if the tooth had a little root maturity, a little devitation and cyst lesion healed faster. We recommend that if there is enough space for eruption, orthodontic traction and surgical treatment of the cyst-associated tooth should be postponed 6 months after marsupialization.

  • Fusion and gemination are irregularities in tooth development. It is often difficult to differentiate between gemination and fusion and it is common to refer to these anomalies as 'double teeth'. The deciduous teeth are most commonly involved, but in 0.1% of cases permanent teeth are affected, in which case aesthetic, funtional and periodontal problems can result. Double teeth present great difficulties in management and required a multidisciplinary approach. The central groove on the labial and palatal surfaces of a double tooth is extremely prone to caries, therefore early 'fissure sealing' is essential. In permanent dentition, surgical separation of fused teeth may be possible with subsequent orthodontic alignment and restorative treatment as needed to reshape the crown. Reshaping or reduction of a double tooth with a single canal may be attempted by modifying the appearance of the labial groove and the use of composite tints but is often impossible and extraction may be the only alternative. Orthodontic treatment and prosthetic replacement is then required. Implants may be an option for adolescents. The present study describes three clinical cases of double teeth in the position of the maxillary permanent incisors. The first case demonstrates an example of multidisciplinary care including surgical intraoral hemisection, root canal therapy, restorative and orthodontic treatment. The second and third cases describe the external and internal morphology of the two fused teeth by means of three dimensional dental computer tomography.

A photoelastic study of the stress distribution on canine retraction by segmented TMA T-loop spring (Segmented TMA T-loop spring에 의한 견치 후방이동시의 응력분포에 관한 광탄성법적 분석)

  • The segmented TMA T-loop spring, used for reciprocal space closure and described by Burstone, was used to achievebodily movement of canine. Photoelastic analysis is a technique for the transformation of internal stress into visible light patterns. The two-dimensional photoelastic stress analysis was performed, and stress distribution was recorded by photography. The purpose of this study was to visualize photoelastically the distribution of forces transmitted to the alveolus and surrounding structures using new segmented TMA T-loop spring for canine retraction. The results were as follows: 1. Decreased activation produced decreased stress of upper 1st. premolar extraction site and increased intrusive stress of upper 1st. molar, regardless of T-loop position. 2. At 5mm activation, More posterior positioning of T-loop Produced an increased stress in upper 1st. premolar extraction site. 3. At 3mm activation, More posterior positioning of T-loop produced an increased stress in upper 1st. premolar extraction site and mesial lower half of upper 1st. molar mesio-buccal root. 4. At 1mm activation, More anterior positioning of T-loop produced an increased stress in upper mesial and blew apex area of upper canine root. 5. 0.25 B/L ratio and 3mm activation produced bodily movement of canine. To summarize, desired tooth movement and anchorage requirement is possible by altering the activation and mesio-distal position of the T-loop spring.

EFFECT OF PATIENT'S BREATHING PATTERN ON THE STRESS CHANGES IN THIRD MOLAR EXTRACTION (제 3 대구치 발치시 환자의 호흡 패턴이 스트레스 변화 양상에 미치는 영향)

  • Purpose: Heart rate variability(HRV) is the clinical consequence of various influences of the autonomic nervous system(ANS) on heart beat. HRV can estimate the potential physiologic rhythm from the interval between consecutive beats(RR interval or HRV data). It is known as RSA which represents respiration-related HR rhythmic oscillation. Previous studies demonstrated a specific breathing pattern(0.1Hz, 6breaths/min) to improve a physiological body condition related to the stress. In this paper, the level of stress would be evaluated in terms of three phases of the dental treatment, combined with 6breaths/min. Methods: These phases include before, during and after tooth extraction or anesthesia or something.36 patients' stresses were assessed using HRV stress analyzer in each phase in Kangdong Sacred Heart Hospital, and Chuncheon Sacred Heart Hospital, Hallym University Medical Center from Jun. to Sept. of 2007. HRV 5-min data collected were analyzed in time-domain and frequency-domain to evaluate the activity of autonomic nervous system(ANS) which represents the level of stress. Results: All HRV parameters including HF(high frequency), LF(low frequency) and LF/HF ratio showned a significant change affecting the ANS balance. There was a 6.4% difference between R(LF/HF)s on general breathing pattern for balance of Autonomic nervous system, but on controlled breathing pattern, 0.1Hz, was made narrow till 1.4%. The activity of ANS has increased by 1.4% on general breathing pattern, and by 2.9% on controlled breathing pattern, 0.1Hz. Conclusion: After analysis of preoperative stress changes and effect of breathing pattern of 0.1 Hz on the stress in 36 patients who have undergone third molar extraction, following was concluded. In the preoperative stage, the sympathetic change was the greatest?after the?anesthetic injection, and stress was relieved by controlling the breathing pattern to a frequency of 0.1Hz.

The Effect of Occlusion on the Reorganization of Periodontal Fibers during Retention Periods after Tooth Movement in Rats (백서에서 치아이동 후 보정기간 동안 교합이 치주조직섬유의 재형성에 미치는 영향)

  • The Purpose of this study was to evaluate the effect of occlusion on the mechanical strength of periodontal fibers during retention periods after experimental tooth movement. In the Sprague-Dawley male rats weighing 200g or more, the ntraoral elastics were inserted into the both right and left interproximal space between upper first and second molars for tooth movement. kiter 4 days later, the left lower first, second, and third molars were extracted for differentiating the non-occlusal side from the occlusal side in the same mouth. At the same time the elastics were removed and then light cured resin was Placed in the space between upper first and second molars following undercut was made for retention bilaterally. From the beginning of retention, 7 rats were sacrificed at 0, 4, 8, 12, 16, 20 days respectively. For evaluating of magnitude on the mechanical strength of periodontal tissue, the maximal shear load of the upper first molars were measured bilateraly during extraction using Instron Universal Testing Machine. The results of this study were obtained as follows : 1. In the occlusal side, the maximal shear load was increased from no retention to retention 20 days group as time was going and statistically difference was shown from retention 12 days group (p<0.05). 2. In the non-occlusal side, the maximal shear load was increased slightly from no retention to 20 days group as time was going but there was no statistically difference (p>0.05). 3. The result compared with the maximal shear load between occlusal and nonocclusal side showed no statistically difference until retention 8 day group (p>0.05), but showed statistically difference from retention 12 day to 20day group (P<0.05). These results show that occlusion had an effect on mechanical strength of the periodontal fibers during retention periods after experimental tooth movement; therefore, it is suggested that occlusion should be considered while the retainer types and retention period are planned.

Survey on the Dental Health Status & Dental Health Behaviors of Workers (근로자의 구강건강실태 및 구강보건행동에 관한 조사)

  • The purpose of this study was to obtain database for development of education and programs of dental examination in industries. survey and dental examination was conducted by dental hygienist and dentist, and the data from 1016 respondents were analyzed. The result obtained was as follows: 1. Dental disease was 58.2% among 1,016, there was most a lot of peridontal disease, and overlapping disease, dental caries, missing tooth, fracture and abrasion, in order. 2. Utilization of dental care was 40% for last six months, it was significant high that age and working periods, married in marriage state, and unhealthy in subjective dental health state. 3. It was significant high that woman, fourty ages, middle economic state and working periods in above three times of tooth brushing a day. 4. Utilization of dental hygiene auxiliary devices was 41.8%, it was significant high that age, marriage state, economic state and working periods. 5. As for relationship between dental disease status and dental health behaviors, a person without periodontal disease was significant high in above three times a day, and a person without extraction, a person with fracture and abrasion, a person without overlapping disease was significant high in utilization of dental hygiene auxiliary devices.

RESTORATION OF MAXILLARY PRIMARY INCISORS USING POLYETHYLENE FIBER-REINFORCED POST (폴리에틸렌 섬유 강화형 포스트를 이용한 상악 유전치의 수복)

  • Early childhood caries which mainly affect maxillary anterior area, defined as 'the presence of 1 or more decayed, missing, or filled tooth surfaces in any primary tooth in a child 71 months of age or younger'. Extraction of teeth when early childhood caries affected in maxillary primary incisors often develops progressively, result in severe destruction of crowns, acute or chronic pulpitis, and periapical abscess formation. Maxillary primary incisors are need to preserve as possible, because the early loss of maxillary primary incisors may lead to various functional, esthetical, and psychological problems. It is necessary to the availability of an easy to perform technique capable of providing efficient, durable, functional, and esthetic restorative methods. Polyethylene fiber-reinforced post can be used in strengthen of composite resins that is esthetic and good physical and mechanical properties. $Ribbond^{(R)}$ is made from an polyethylene fiber, has numerous usages, its surface is treated to enhance adhesion to resins, ease of manipulation, relatively cost effective. We report this case, had restored of maxillary primary incisors with severe coronal destruction due to affecting severe early childhood caries, using polyethylene fiber-reinforced posts, composite resin cores, and celluloid crowns. We could obtain good result of treatment.
