THE EFFECTS OF IRRADIATION AND CALCIUM-DEFICIENT DIET ON DENTIN AND CEMENTUM FORMATION OF RAT MOLAR (방사선조사와 저칼슘식이가 백서구치의 상아질과 백악질형성에 미치는 영향에 관한 실험적 연구)

  • The present study was designed to elucidate the effects of the Co-60 γ irradiation and/or calcium­deficient diet on the dentin and cementum formation of rat molar. The pregnant three-week old Sprague­Dawley rats were used for the study. The experimental group was divided into two groups, irradiation/normal diet group and irradiation/calcium-deficient diet group. The control group was non­irradiation/normal diet group. The abdomen of the rats at the 19th day of pregnancy were irradiated with single absorbed dose of 350cGy. The rat pups were sacrificed on the 14th day after delivery and the maxillae including molar tooth germ were taken. The specimens including the 1st molar tooth germ were prepared to make tissue sections for light and transmission electron microscopy. Some of tissue sections for light microscopy were stained immunohistochemically with anti-fibronectin antibody. The results were as follows; 1. The Hertwig's epithelial root sheath cells, which are related to the differentiation of the tooth-forming cells, showed irregular cellular arrangement, decrease of intercellular junctional complex, and decreased immunoreactivity to the fibronectin after irradiation. These were more severe in the irradiation/calcium-deficient diet group. 2. The cementoblasts at the cementum-forming area showed chromatin clumpings after irradiation. The immu noreactivity to the fibronectin was weaken after irradiation, especially irradiation/calcium-deficient diet group. 3. The odontoblasts at the dentin-forming area showed increase of lysosomes in the cytoplasm and destruction of intercellular junctional complex. The irradiation/calcium-deficient diet group showed decrease of number and density of the electron dense particles and a large number of vacuoles scattered in the dentin matrix. The immunoreactivity was weaken.

Analysis of PDL Fibroblast Change During Mechanical Stimuli in the Rats (기계적 자극에 대한 백서 치주인대 섬유아세포의 변화)

  • Periodontal disease is characterized by inflammation and subsequent loss and/or damage to tooth-supporting tissues such as bone, cementum,and periodontal ligament. Periodontal ligament and cementum are the key tissues in the initial process of regeneration following periodontal disease. Therefore, studies on cementoblasts, which form cementum are emphasized. It is still unclear which cells cementoblast differentiate from. This study was conducted under the hypothesis that PDL fibroblast can differentiate into either cementoblast or osteoblast depending on the conditions of surrounding tissue. Clinically, with excessive traction force of orthodontic appliances or excessive occlusion hypercementosis is observed, and this has been confirmed histologically. Consequently, activation of cementoblast can be expected in rats when mechanical stimuli are given to PDL fibroblast. Therefore, the purpose of this article is to prove that PDL fibroblast differentiates into cementoblast in rats under mechanical stimuli using histologic and molecular methods. In this study, twenty rats were given hard diet. Ten of them were sacrificed after 1 week, and the others were sacrificed after two weeks. Slides were made from tooth specimen, and they were studied under the microscope. In addition, PDL fibroblast and cementum from the extracted teeth were analyzed with Northern blotting. In histologic examination, as time passed, PDL fibroblast migrated to the dentin side, differentiated into cementoblast, and formed new cementum. In Northern blotting, it was found that mRNA expression of cementoblast-specific proteins such as BSP, OC, OPN, and type I collagen were more prominent in rats sacrificed after 2 weeks of hard-diet than rats sacrificed after 1 week. From these findings we can conclude that PDL fibroblast can differentiate into cementoblast under mechanical stimuli. We think that 'Rat Models' used in this study will be beneficial to future studies regarding cementoblast.

  • Cementoblastoma is a benign ectomesenchymal odontogenic neoplasm that forms a mass of cementum or cementum-like tissue continuous with the tooth root. Cementoblastoma represents 1% to 6.2% of all odontogenic tumors, and occurs more than 75% arise in the mandible, with most cases arising in the molar and premolar regions. About 75% arises before the age of 30. Radiographically, it appears as a radiopaque mass with a thin radiolucent rim attached to the roots of a tooth. The recurrence rate is 37% in the current study and cortical expansion and perforation of the cortex are common findings in lesions that subsequently recurred. It is apparent that recurrence rate depends largely on the completeness of removal than any other factor. Here we present a 20 year old, female case of an unusual multiple recurrent cementoblastoma around a previous lesion which had been surgically excised 2 years ago.

THE EARLY EFFECT OF DIFFERENT ROOT TREATMENT MODALITIES ON CONNECTIVE TISSUE ATTACHMENT IN RATS (치근면 처치시 결합조직 부착에 관한 초기효과의 조직병리학적 연구)

  • An essential fact in the regeneration of new periodontal tissue after periodontal therapy is the reattachment of collagen fibers to the tooth. Two phenomena play a fundamental role in preventing new connective tissue attachment to the exposed root surface ; 1) The apical migration of the junctional epithelium 2) The contamination of cementum by toxic substances, especially endotoxins. Authors have used rat submucosal implantation of root sections to study the connective tissue healing to periodontally diseaed root, previously planed and demineralized with citric acid and tetracycline- HCl. The results were obtained as follows. 1. The connective tissue attachment was increased in tetracycline, citric acid, non disease, scaling and root planing order and inflammatory reaction was seen in the rat teeth, no treatment group. 2. Collagen fiber attachment at the dentin surface was more increased than cementum surface 3. In 2 week of citric acid and tetracycline-HCl specimens, osteoid was seen near the fibrotic band. 4. In the MT view, collagen fiber formation was increased with time and the numerous collagen fiber and connective tissue was more densly attached to the tooth surfaces in the tetracycline-HCl group than the citric acid group.

Isolation and characterization of bovine cementoblast progenitor cells

  • Dental follicle is the mesenchymal tissue which surrounds developing tooth germ. During tooth root development, periodontal components such as cementum, periodontal ligament and alveolar bone are considered to be created by progenitors present in the dental follicle. However, little is known about these progenitors. Previously we observed that cultured bovine dental follicle cells (BDFC) contained putative cementoblast progenitors. To further analyze the biology of these cells, we have attempted to immortalize BDFC by expression of the polycomb group protein Bmi-1 and human telomerase reverse transcriptase (hTERT). The BDFC expressing Bmi-1 and hTERT showed extended life span by 90 population doublings more than normal BDFC, and still contained cells with potential to differentiate into cementoblasts upon implantation into immunodeficiency mice. Among them, we established a clonal cell line designated as BCPb8, which formed cemetum-like mineralized tissue reactive to anti-cementum specific monoclonal antibody, 3G9, and expressed mRNA for bone sialoprotein, osteocalcin, osteopontin and type I collagen upon implantation. Thus with the combination of hTERT and Bmi-1, we succeeded in immortalization of cementoblast progenitor in BDFC without affecting differentiation potential. The BCPb8 progenitor cell line could be a useful tool not only to study cementogenesis but also to develop regeneration therapy for periodontitis.

GENERALIZED HYPERCEMENTOSIS WITH ARRESTED DENTAL ERUPTION IN A CEREBRAL PALSY PATIENT : A CASE REPORT (뇌성마비 환자의 지연맹출을 동반한 다발성 과백악질증 : 증례보고)

  • Hypercementosis is an excessive deposition of secondary cementum on the root of a tooth. It is mostly presented as a solitary lesion or in rare cases as a generalized type, but which is seldom recognized; typically it is discovered during regular dental X-ray. Increased thickness of cementum is not uncommon but generalized hypercementosis on impacted permanent teeth which may cause delayed eruption is rarely reported. This case report discusses a patient with cerebral palsy, epilepsy and mental retardation that presents multiple hypercementosis with delayed eruption. On intraoral examination, multiple retained primary molar teeth were found. As there was no any further symptoms, regular dental checkup had been done for several years. In 2015, a surgical opening was performed in the second molar area, but there was no specific change. Panoramic view showed multiple impacted permanent teeth with increased thickness of roots due to excessive deposition of cementum. Hypercementosis was also observed in the root of the erupted tooth. Several laboratory test results including hormone, urine, complete blood count test were reviewed. The patient was also diagnosed with subclinical hypothyroidism, impaired fasting glucose and had been taken valproic acid($Orfil^{(R)}$) for 10 years. However, none of them clearly explained generalized hypercementosis or delayed eruption. The patient is now 24 years old and regular dental checkups and radiographs are taken to confirm that there is no change in the lesion.

HISTOPATHOLOGIC STUDY ON THE HEALING PROCESS OF APICAL WOUND IN APPLYING SEVERAL MATERIALS (치근단 손상부의 치유에 영향을 미치는 제재에 관한 병리조직학적 연구)

  • The purpose of this study was to evaluate the effect of several materials on the healing process of apical wound. Sixteen mandibular premolars obtained from 4 healthy dogs were used for this study. Under general anesthesia, the pulpal chamber of each tooth was opened and the pulps were extirpated. The root canals were then instrumented with H-file and irrigated with physiologic saline solution ; the apices were purposely perforated and enlarged with the engine K-reamer. In the experimental groups, apical wounds were filled with one of calcium hydroxide, hydroxylapatite, and tricalcium phosphate materials, mixture of each materials and physiologic saline solution, with a lentulo spiral. In the control group, apical wounds were not filled with any material. All the root canals were filled by the lateral condensation technique with gutta-percha cone and ZOE sealer. The access opening of all the teeth were closed with amalgam. On the 10, 20, 40 and 60th day after experiment, experimental animals were sacrificed. Segments of jaws, each containing one tooth, were fixed in 10% formalin solution and decalcified in Plank-Rychlo solution. The specimens were embedded in paraffin and serially sectioned to an average thickness of $6{\mu}m$. The sections were stained by hematoxylin-eosin and Masson's trichrome stain method and examined under light microscope. The results were as follows : 1. In the experimental groups, the new bone formations were observed in apical wounds. 2. Fourty days later, apical wounds were healed by granulation tissue in the experimental groups, but were not healed by granulation tissue in the control group, and the healing process of experimental groups were more rapid than that of control group. 3. Sixty days later, chronic inflammation disappeared in the experimental groups, and the materials used showed biologic affinity to the periapical tissue. 4. In all the groups, the resorption of cementum appeared on the 10th and 20th day after experiment, and the deposition of cementum appeared on the 40th and 60th day after experiment, especially showing narrowness of apical foramen due to newly formed cementum in calcium hydroxide group. 5. Calcum hydroxide and tricalcium phosphate particles were gradually resolved, but hydroxylapatite particles were not resolved through the experimental period.

A STUDY OF ROOT RESORPTION AND ALVEOLAR BONE CHANCES DURING TOOTH MOVEMENT AFTER TREATMENT WITH ETIDRONATE DISODIUM (Biphosphonate (Etidronate disodium) 투여 후 치아이동에 따른 치근흡수 및 치조골 변화에 관한 연구)

  • Many studies has been conducted concerning prevention of unnecessary complications such as root resorption during orthodontic too th movement under various mechanical forces. Nowadays, the cause of the root resorption is not thought to be confined only to mechanical forces. But the factor that affects bone metabolism are thought to be major one of the predisposing factors. The light microscope and scanning electron microscope were used to the effects of 60 gm, and 100 gm of tipping force on root resorption of cats, which were treated with Etidronate disodium. The results were as follows: 1. In the 60gm control group, hyalinization on the compression site of periodontal ligament appeared after first week and second week. In the 60gm experimental group, it appeared after first week with low frequency. In the 100gm control group it appeared with high frequency by first and second week while in 100gm experimental group, it appeared with low frequency. 2 In the 100gm control group, resorption of the cementum and the alveolar bone rapidly increased after second week. In the 60gm experimental group, resorption or formation of alveolar bone and cementum didn't appear all through the experimental period. 3. In the 100gm control group, formation of cementum and alveolar bone appeared after first week while in the 100gm experimental group, formation of cementum and alveolar bone appeared after second week and fourth week respectively. In the 60gm control group, formation of the cementum didn't appear all through the experimental period. 4. In the control group, the root resolution of 100gm group was higher than that of 60gm group after second week, while in experimental group, root resorption didn't appear regardless of the forces.

THE ROOT RESORPTION AND THE PERIODONTAL TISSUE CHANGE AFTER ORTHODONTIC TOOTH MOVEMENT OF THE VITAL AND ENDODONTICALLY TREATED TOOTH IN THE DOG (실활치의 교정적 치아이동시 치근 및 치주조직의 변화에 관한 실험적 연구)

  • The objective of this experiment is to observe structural differences in the othodontic movement of vital (control group) and devitalized (experimental group) teeth in the mongrel dogs. The 5 utilized dogs in this experiment were approximately 1 year of age and their average weight was about 12 Kg. Endodontic therapy was performed on the 2nd premolars in upper & lower jaws of each animal under the general anesthesia by intravascular injection of 25mg/kg of pentobarbital sodium. The canals of the teeth were obturated by using gutta percha in conjunction with root canal sealer (AH26 Densply). One of the roots in the 2nd premolars was hemisected to make an extraction space for the devitalized teeth to be moved. The edgewise technique was employed for the movement of the teeth. Orthodontic models and intraoral roentgenograms were taken before and after orthodontic tooth movement. The open coil springs (.010 x .040) were used at interbraket space in order to provide equal forces (75gm) between the teeth in each arch wire. After 13 weeks of active orthodontic tooth movement, dogs were sacrified and the experimental results w ere examined through the intraoral radiography, microscopic examination and scanning electromicroscopic examination at the root sulfate. From the results of the study, the following conclusions may be drawn: The root resolution and cemental deposition were observed within the pressure and tension site in both group. 2. The root resorbed lacunae were observed in the cementum and/or into the dentin in both group. 3. The prominent osteoblastic activities were observed on the alveolar margin in the tension site in both group. 4. A few of blood vessels were observed in the pressure site, but also lots of blood vessels were observed in the tension site especially in the periphery of the alveolar bone in both group. 5. In the pressure site, resorbed lacunae were formed with deep and narrow cavity in the control group; the shallow and wide cavity in the experimental group. 6. In the pressure site, the repaired cementum or cementoid tissue was lined on cementum in the experimental group, but not in the control group. 7. There was no significant difference between external root resolution of endodontically and vital teeth when both were subjected to orthodontic forces.

THE EFFECTS OF FORMOCRESOL AND GLUTARALDEHYDE ON THE PERFORATED INTERRADICULAR TISSUES AND TOOTH GERMS OF PRIMARY TEETH IN DOGS (Formocresol, Glutaraldehyde가 유견 계승치 치배 및 주위조직에 미치는 영향에 관한 병리조직학적 연구)

  • The purpose of this study is to observe the effect of formocresol and glutaraldehyde to tooth germs and periapical tissues after perforation of interradicular portion of pulpal floor and application of physiological saline solution in control groups, formocresol and glutaraldehyde in experimental groups. The following results were obtained 1. In control groups, normal healing processes were seen, and, on the sixteenth day, the epithelization of injured areas was completed. Inflammatory reactions were limited to the injured surface, and the underlying alveolar bone were normal and successive tooth germs were normal. 2. In both formocresol groups and glutaraldehyde groups, tissue reactions were identical. Inflammatory reactions were slightly compared with control groups, but the surface epithelizations were delayed compared with control group. 3. In both formocresol and glutaraldehyde groups, necrosis was seen in superficial tissue of bone marrow, and, at 24th day, center area of bone marrow on the successive tooth germs were losed and replaced with connective tissue, and superficial soft tissue of the injured area was connected with soft tissue on the successive tooth germ. In remaining alveolar bone, osteoclastic reaction was remarkable. 4. In both formocresol and glutaraldehyde groups, there is no injury to the successive tooth germs. 5. In both formocresol and glutaraldehyde groups, periodontal membrane was normal, but the partial resorption of cementum and dentin near the injured area were seen.
