Accuracy and radiation exposure from image-guidance in Tomotherapy Hi-Art System (토모테라피 Hi-Art System의 영상유도 정확성 평가와 환자 피폭에 관한 연구)

  • IGRT(Image Guided Radiation Therapy) in radiation therapy is a very useful technique in order to increase setup of patient and position reproducibility. Tomotherapy can increase accuracy of setup to take IGRT by MVCT, but it be for verified accuracy of Image guided, and MVCT occurs the exposure of patient. Through this study, IGRT accuracy of Tomotherapy is very accurate within 1.0mm. When MVCT using Tomotherapy phantom for QA, QC be taken, exposure dose is Fine(2mm Slice thickness) 3cGy, Normal(4mm Slice thickness) 1.5cGy, Corse(6mmSlice thickness) 1.0cGy. Measurement value of spatial resolution using AAPM CT performance phantom did't cause a big difference. As a result, ability of IGRT in Tomotherapy is very accurate. While obtaining image for IGRT, we should minimize expose range because patient's be exposed to radiation. We should make an effort to do accurate radiation therapy to minimize exposure of patient by selecting the appropriate thickness of MVCT depending on patient's body and treat area.

A Comparison for Treatment Planning of Tomotherapy and Proton Therapy in Prostate Cancer (전립선암에 대한 토모치료와 양성자치료의 치료계획 비교)

  • The prostate cancer is the most common malignant tumor in males. Prostate cancer is the most common malignant tumor that occurs in the male in Korea in 2007 to an annual average of 5,292 cases and 3.3% of the total cancer incidence seventh occurred. Our study compared property for tomotherapy and proton therapy in radiotherapy of prostate cancer patients. We analyzed DVH(Dose Volume Histogram) and dose distribution for prostate, bladder and rectum for radiation treatment planning of prostate cancer with 11 patients in Ilsan K cancer hospital from June to November 2011. There was no differences between tomotherapy and proton therapy in the purpose of prostate cancer therapy for PTV. The adjacent organs of bladder and rectum of average dose-volume were 2port proton therapy that it was low dose treatment comparing with tomotherapy and 5port proton therapy. $H{\cdot}I$ of proton therapy was less than $H{\cdot}I$ of tomotherapy. Also, 5port was less than 2port in $H{\cdot}I$ of proton therapy. However, 2port proton therapy has more advantage over 5port proton therapy that the bladder and rectum of average dose-volume and control time of equipment in radiotherapy of prostate cancer.

Evaluation of the Lens-absorbed Dose of the Scattered Radiation Generated During Tomotherapy IMRT to the H&N Cancer Patient

  • This paper uses a glass dosimeter to evaluate the lens-absorbed dose of scattered radiation generated in tomotherapy intensity modulated radiation therapy (IMRT). The head and neck portion of the rando phantom was subjected to a CT scan. The tomotherapy plan was designed to ensure delivery of the prescribed total 70 Gy day 2.2 Gy. With the lens portion of the glass dosimeter, a 5mm bolus was subjected to the scattered radiation treatment, and the dose was measured in each of the three megavoltage CT (MVCT) modes. The result is multiplied by 30 times and was determined once as the mean value. The measurement at the MVCT Coarse mode is RT mode 10.797 mGy, that for the Normal mode is 13.360 mGy, for the Fine mode is a maximum of 22.872 mGy, and for the treatment mode is 895.830 mGy. A small amount of scattered radiation in the MVCT is measured in the lens scattered radiation, but scattered radiation during treatment was measured to be near 1 Gy on the lens. Compared to a one-time radiation treatment of 2.2 Gy, the survey showed something unexpected in that it was half the value of that research to the patient. Therefore, will be aware of how much of an influence there will be on sensitive organs, such as the lens by scattered radiation generated during intensity modulated radiation therapy.

Evaluating the Usefulness of Rice Bolus Phantom in Tomotherapy: Phantom Study (토모테라피에서 쌀 볼루스 팬텀의 유용성 평가: 팬텀연구)

  • The purpose of this study was to evaluate the usefulness of the rice bolus for upper-lower extremity radiation therapy by Tomotherapy. The computed tomography images were obtained for air, water, and rice bolus. The average and standard deviation of the Hounsfield unit (HU) were measured for image evaluation. The conformity index (CI) and homogeneity index (HI) were calculated for dose distribution of the planning target volume (PTV) which was treated by direct mode with gantry angle (90 and 270 angle). The point dose of a total of ten axial planes was measured to confirm the different regions. The mean of HU was -999.72 ± 0.72 at the air. The water and rice bolus were -0.13 ± 1.65 and -170 ± 27.2, respectively. The CI (HI) of PTV was 0.96 (1.36) at the air. 0.95 (1.04) at the water bolus, and 0.95 (1.04) at the rice bolus. The maximum dose for air was 136 cGy which is about 32% higher than 103 cGy for water and 104 cGy for rice bolus. There was a statistical difference for point dose between air and water including rice bolus (p=0.04), however, no statistical difference between water and rice bolus (p=0.579).The rice bolus phantom for extremities radiation therapy could be not only the optimized dose distribution but also the convenience and equipment safety at Tomotherapy. However, additional research will be necessary to more accurately verify the clinical usefulness of rice bolus phantom due to not enough examination.

Effect of Iterative-metal Artifact Reduction (iMAR) at Tomotherapy: a Phantom Study (토모테라피에서 반복적 금속 인공물 감소 알고리즘의 유용성 평가: 팬톰 실험)

  • We evaluated the effect of high-density aluminum, titanium, and steel metal inserts on computed tomography (CT) numbers and radiation treatment plans for Tomotherapy. CT images were obtained using a cylindrical TomoPhantom comprising cylindrical rods of various densities and metal inserts. Three CT image sets were evaluated for image quality as the mean CT number and standard deviation. Dose evaluation also performed. The reference values did not significantly differ between the CT image sets with the corrected metal inserts. The higher-density material exhibited the largest difference in the mean CT number and standard deviation. The conformity index at Iterative-Metal Artifact Reduction (iMAR) was approximately 20% better than that of non-iMAR. No significant target or organ at risk dose difference was observed between non-iMAR and iMAR. Therefore, iMAR is helpful for target or organ at risk delineation and for reducing uncertainty for three-dimensional conformal radiation therapy in Tomotherapy.

Evaluation on Usefulness of Abdomen and Chest Motion Control Device (ABCHES) for the Tumor with a Large Respiratory Motion in Radiotherapy (호흡으로 인한 움직임이 큰 종양의 방사선치료 시 Abdomen and Chest Motion Control Device (ABCHES)의 유용성 평가)

  • Purpose: It is essential to minimize the respiratory-induced motion of involved organs in the Tomotherapy for tumor located in the chest and abdominal region. However, the application of breathing control system to Tomotherapy is limited. This study was aimed to investigate the possible application of the ABCHES system and its efficacy as a means of breathing control in the tomotherapy treatment. Materials and Methods: Five subjects who were treated with a Hi-Art Tomotherapy system for lung, liver, gallbladder and pancreatic tumors. All patients undertook trained on two breathing methodes using an ABCHES, free breathing methode and shallow breathing methode. When the patients could carry out the breathing control, 4D-CT scan was a total of 10 4D tomographic images were acquired. A radiologist resident manually drew the tumor region, including surrounding nomal organs, on each of CT images at the inhalation phase, the exhalation phase and the 40% phase (mid-inhalation) and average CT image. Those CT images were then exported to the Tomotherapy planning station. Data exported from the Tomotherapy planning station was analyzed to quantify characteristics of dose-volume histograms and motion of tumors. Organ motions under free breathing and shallow breathing were examined six directions, respectively. Radiation exposure to the surrounding organs were also measured and compared. Results: Organ motion is in the six directions with more than a 5 mm displacement. A total of 12 Organ motions occurred during free breathing while organ motions decreased to 2 times during shallow breathing under the use of Abches. Based on the quantitative analysis of the dose-volume histograms shallow breathing showed lower resulting values, compared to free breathing, in every measure. That is, treatment volume, the dose of radiation to the tumor and two surrounding normal organs (mean doses), the volume of healthy tissue exposed to radiation were lower at the shallow breathing state. Conclusion: This study proposes that the use of ABCHES is effective for the Tomotherapy treatment as it makes shortness of breathing easy for patients. Respiratory-induced tumor motion is minimized, and radiation exposure to surrounding normal tissues is also reduced as a result.

The Study of Dose Distribution according to the Using Linac and Tomotherapy on Total Lymphnode Irradiation (선형가속기와 토모치료기를 이용한 전림프계의 방사선 치료시 선량분포에 관한 연구)

  • In this study, compare and analyze the dose distribution and availability of radiation therapy when using a different devices to TNI(Total Lymphnodal Irradiation). Test subjects(patients) are 15 people(Male 7, Female 8). Acquire CT Simulation images of the 15 people using Somatom Sansation Open 16 channel and then acquired images was transferred to each treatment planning system Pinnacle Ver 8.0 and Tomotherapy Planning System and separate the tumor tissue and normal tissues(whole lung, spinal cord, Rt kidney, Lt kidney). Tumor prescription dose was set to 750 cGy. and then Compare the Dose Compatibility, Normal Tissue's Absorbed Dose, Dose Distribution and DVH. Statistical analysis was performed SPSS Ver. 18.0 by paired sample Assay. The absorbed dose in the tumor tissue was $751.0{\pm}4.7cGy$ in tomotherapy planning, $746.9{\pm}14.1cGy$ in linac. Tomotherapy's absorbed dose in the tumor was more appropriate than linac. and These values are not statistically significant(p>0.05). Tomotherapy plan's absorbed dose in the normal tissues were less than linac's plan. This value was statistically significant(p<0.05) excepted of whole lung. In DVH, appropriated on tumor and normal tissues in tomotherapy and linac but tomotherapy's TER was better than linac. Namely, a result of Absorbed dose in tumor and normal tissue, Dose distribution pattern, DVH, Both radiation therapy devices were appropriated in radiation therapy on TER. The Linac has a short treatment time(about 15-20 min) and open space on treatment time. It cause infant and pediatric patients to receiving uncomfortable treatment. So, In this case, it will be fine that Linac based therapy was restricted use. and if the patient was cooperative, it will be show a better prognosis that Tomotherapy using Radiation Therapy.

Evaluation of the usefulness of Halcyon VMAT treatment plan through comparison with tomotherapy in bilateral breast cancer radiation therapy. (양측 유방암 방사선치료 시 토모테라피와의 비교를 통한 Halcyon VMAT 치료계획의 유용성 평가)

  • Purpose: To evaluate usefulness of Tomotherapy and Halcyon VMAT treatment in radiation therapy for bi-breast cancer Materials and Methods: For 10 patients with bi-breast cancer, volumetric modulated arc therapy(VMAT) treatment plan was established using helical Tomotherapy(Accuray. USA) with field width of 5.0-cm and pitch of 0.287, and Halcyon(Varian Medical System, Palo Alto, CA, Version 3.0 USA) of 6arc and 8arc. Prescribed dose was 42.4 Gy/16, and V 40.3Gy of Planning Target Volume(PTV) was 90%. The quality of plan was evaluated by comparing the dose to tumor and normal organs, and the efficiency was evaluated by comparing total MU and beam on time. Results: About three treatment plans(Tomotherapy, Halcyon 6Arc VMAT, Halcyon 8Arc VMAT) , the mean homogeneity index(H.I) of PTV were 1.07, 1.10, 1.11, and the mean conformity index(C.I) of PTV were 1.21, 1.16, 1.17, respectively. The average value of the dose assessment item for a normal organ is V 5Gy(%) of both lung was 36.3, 31.2, 29.7, and V 15Gy(%) were 18.6, 15.5, 14.6, respectively. The mean heart dose(Gy) were 4.17, 2.69, 2.51. Total MU were (7498.6, 2494.2, 2471.5), and beam on time(sec) were 462.5, 195.4, 198.0. Conclusion: Halcyon VMAT showed similar quality of treatment plan compared to helical Tomotherapy, while also protecting normal organs. In addition, the efficiency of radiotherapy increased due to a decrease in Beam On Time and MU.
