Clinical Study of Tetracycline HCl Conditioning in the Treatment of Gingival Recession. A Comparative Study (치근 피개 술식시 치근에 도포된 테트라사이클린의 효과에 관한 임상적 연구)

  • Histological studies indicate a tetracycline HCl similar to citric acid to induce connetive tissue repair in animals. When tetracycline HCl was used as a root conditioning agent in humans, there was a trend toward more connective tissue attachment than in root planing alone. The purpose of this study was to evaluate clinical effect of tetracycline HCl in the treatment of gingival recession. 44 teeth in 12 patients with bilaterally gingival recession & Miller classification I, II gingival recession were selected and 22 teeth were treated with 125mg/ml tetracycline HCl , the others was not treated with tetracycline HCl. Gingival recession, pocket depth, clinical attachment level, width of keratinized gingiva were observed at baseline, postoperative 4, 12, 20weeks. Both groups were statistically analyzed by Wilcoxon signed Ranks Test & Wilcoxon's rank sum test(Mann-whitney test) using SPSS program.(5% significance level) The results were as follows: 1........The change of gingival recession, clinical attachment level, keratinized gingiva in both groups were increased significantly at 4, 12, 20 weeks. 2.......The pocket depth exhibited no marked changes throughout the entire investigation in both groups. 3........The change of gingival recession in tetracycline group was increased significantly than control group at 4, 12, 20 weeks and the percentage of root coverage was 93% in tetracycline group and 83% in control group. 4........The change of clinical attachment level, pocket depth, keratinized tissue from baseline to 4, 12, 20 weeks was not differ significanltly in both group.

Effects of Remote Ischemic Conditioning Methods on Ischemia-Reperfusion Injury in Muscle Flaps: An Experimental Study in Rats

  • Background The aim of this study was to investigate the effects of remote ischemic conditioning on ischemia-reperfusion injury in rat muscle flaps histopathologically and biochemically. Methods Thirty albino rats were divided into 5 groups. No procedure was performed in the rats in group 1, and only blood samples were taken. A gracilis muscle flap was elevated in all the other groups. Microclamps were applied to the vascular pedicle for 4 hours in order to achieve tissue ischemia. In group 2, no additional procedure was performed. In groups 3, 4, and 5, the right hind limb was used and 3 cycles of ischemia-reperfusion for 5 minutes each (total, 30 minutes) was applied with a latex tourniquet (remote ischemic conditioning). In group 3, this procedure was performed before flap elevation (remote ischemic preconditoning). In group 4, the procedure was performed 4 hours after flap ischemia (remote ischemic postconditioning). In group 5, the procedure was performed after the flap was elevated, during the muscle flap ischemia episode (remote ischemic perconditioning). Results The histopathological damage score in all remote conditioning ischemia groups was lower than in the ischemic-reperfusion group. The lowest histopathological damage score was observed in group 5 (remote ischemic perconditioning). Conclusions The nitric oxide levels were higher in the blood samples obtained from the remote ischemic perconditioning group. This study showed the effectiveness of remote ischemic conditioning procedures and compared their usefulness for preventing ischemiareperfusion injury in muscle flaps.

SHOCK-ABSORBING BEHAVIOR OF TEMPORARY SOFT DENTURE LINERS (임시 연성 의치상 이장재의 충격 흡수에 관한 연구)

  • Temporary soft liners can be used to prevent chronic soreness from dentures or to aid in its treatment are as adjuncts in tissue conditioning, for temporary obturators, and to stabilize baseplate or surgical stent. The purpose of this study was to evaluate the shock absorption properties of several temporary soft denture liners using a free drop test with an accelerometer. The materials tested inclued Coe-comfort, Softone, Tissue conditioner and Viscogel. The specimens were fabricated with the thickness of 1, 2, 3mm and were stored in distilled water at $37^{\circ}C$ for a day, 1, 2, and 3 weeks. Six samples were made with each material for each test condition and the shock-absorbing behavior was evaluated according to material, thickness and duration. The results were as following : 1. Softone of 3mm thickness stored for a day showed the most excellent shock absorbability. 2. The shock absorbing behavior of duration according to materials and thickness showed a day to be the highest and decreased in 1 week, 2 weeks and 3 weeks in that order(p<0.05). And there was no significant difference between durations in Tissue conditioner. 3. The shock absorbability of thickness according to materials and duration showed 3mm to be highest and decreased in the order of 2mm, 1mm(p<0.05). 4. In comparison of the shock absorbability of temporary soft denture liners according to thickness, there was statistically significant difference between Softone and Visocgel, Tissue conditioner, Coe-comfort / Viscogel and Tissue conditioner, Coe-comfort in 1,2mm thickness, and between Softone, Viscogel and Tissue conditioner, Coe-comfort in 3mm thickness (p<0.05).

EARLY CONNECTIVE TISSUE ATTACHMENT ON DENTIN SURFACE TREATED WITH CITRIC ACID, TETRACYCLINE AND FIBRIN SEALANTS (구연산, 테트라싸이클린, 섬유소 전색제로 처리된 상아질면에 대한 결체조직의 조기부착)

  • The purpose of this study was to observe early connective tissue attachment on dentin surface treated with citric acid, tetracycline, and fibrin sealants and compare their conditioning effects on dentin surface. Experimental dentin blocks conditioned with citric acid, tetracycline or fibrin sealant, and only root planned control block were surgically implanted in the pouch under buccal mucoperiosteal flaps of left mandible, right maxilla, left maxilla, right mandible of 18 male rabbits. Rabbits were sacrificed after 1 and 6 hours, 1, 3, 7 and 14 days after implantation and then specimens including dentin block and surrounding soft tissue were obtained, and prepared for light and transmission electron microscopic examination. 1 and 6 hours after dentin block implantation, there was plasma proteins adsorption followed by fibrin clot formation and no differences among specimens. At the 1-day observation interval, delicate fibrin network was observed in the all groups, and there were proliferative fibroblasts, angiogenesis and macrophage in the all 3-day specimens. Cellular aggregates and abundant connective tissue adhered dentin surface and tetracycline or citric acid treated group showed much proliferative fibroblast and abundant collagen fibers at 1 week. But at 2 week, citric acid treated group showed much proliferative fibroblast and abundant collagen fibers. These observations suggested that new connective tissue attachment to dentin was initiated by the adsorption of plasma proteins to the dentin surface and followed by fibrin clot formation. Tetracycline and citric acid seemed to make dentin surface more biologically favorable for the connective tissue attachment.

  • This study was performed to understanding on guidelines for using therapeutic modalities according to injury phases of soft tissue. Clinical decisions on how and when therapeutic modalities may be used should be based on recognition of signs and symptoms. as well as some awareness of the time frames associated with the various phases of the Healing process. The physical therapist must have a sound understanding of that process in terms of the sequence of the various process of healing stage. The results of this study are as follows: 1. Once an acute injury has occured, the healing process consists of the imflammatory response phase, the fibroblastic-repair phase, and the maturation-remodeling phase and can impede by various pathologic factors. 2. Modality use in the initial acute injury phase and the inflammatory response phase should be directed toward limiting the amount of swelling and reducing pain. 3. Modality use in the Fibroblastic repair phase may be change from cold to heat. The purpose of heat is to increase circulation to the injured area to promote healing. 4. During the Maturation-Remodeling phase, some type of heating modalities, ultrasound, or short wave and microwave diathermy should be used to increase circulation to the deeper tissue. In this phases, physical therapists must control training and conditioning habits to allow the injury to heal sufficiently.

Biological effects of a root conditioning agent for dentin surface modification in vitro

  • Purpose: Connective tissue reattachment to periodontally damaged root surfaces is one of the most important goals of periodontal therapy. The aim of this study was to develop a root conditioning agent that can demineralize and detoxify the infected root surface. Methods: Dentin slices obtained from human teeth were treated with a novel root planing agent for 2 minutes and then washed with phosphate-buffered saline. Smear layer removal and type I collagen exposure were observed by scanning electron microscopy (SEM) and type I collagen immunostaining, respectively. Cell attachment and lipopolysaccharides (LPS) removal demonstrated the efficiency of the root conditioning agent. Results: SEM revealed that the smear layer was entirely removed and the dentinal tubules were opened by the experimental gel. Type I collagen was exposed on the surfaces of the dentin slices treated by the experimental gel, which were compared with dentin treated with other root planing agents. Dentin slices treated with the experimental gel showed the highest number of attached fibroblasts and flattened cell morphology. The agar diffusion assay demonstrated that the experimental gel also has effective antimicrobial activity. Escherichia coli LPS were effectively removed from well plates by the experimental gel. Conclusions: These results demonstrated that this experimental gel is a useful tool for root conditioning of infected root surfaces and can also be applied for detoxification of ailing implant surface threads.

A comparative study of the effect of root planing methods and various conditioning agents (치근면 활택 방법 및 수종의 치근면 처리 약제의 효과에 대한 비교 연구)

  • The purpose of this study was to compare the effect of tetracycline HCL, Citric acid and PrefGel applied on the root surfaces that planed with periodontal curret with Roto bur. In this study, 20 extracted teeth with advanced periodontal disease were used. The teeth were root planing with periodontal curette and Roto bur. Following root planing, each agents was burnished on the prepared root surface for 3 minutes to find opened dentinal tubules. And then, each specimens were investigated using scanning electron microscope. Amount of remained cementum by loss of tooth substance index and the number of opened dentinal tubules were evaluated to each specimens The results were as follows. 1. Groups treated with periodontal curette were almost seemed no removed. Other groups treated with Roto bur showed partially opened dentinal tubule orifices. 2. Loss of tooth substance index were compared between groups. There was no statistically difference between periodontal curette groups. Between Roto bur groups was alike. But there were statistically differences between periodontal curette and Roto bur groups. 3. At comparing with various root conditioning agents, Tetracycline HCL group took statistically higher than Citric acid and PrefGel in opened dentinal tubules. On the other hand, there was no statistically difference between Citric acid group and PrefGel group. As a result of this study, groups treated with Roto bur showed more cementum removed than groups treated with periodontal curette. In a treatment for regeneration of periodontal tissue, it was regarded that Roto bur should be used and that Tetracycline HCL would be more effective as chemical root conditioning agent.

Transformation of Chinese Cabbage Glutathione Reductase (GR) gene into Lettuce (Lactuca sativa L.) with Particle Bombardment (유전자총을 이용한 상추 내로의 배추 Glutathione Reductase (GR)유전자의 도입)

  • The cytosolic glutathione reductase(GR) gene of chinese cabbage was introduced into Lettuce (Lactuca sativa L.) with particle bombardment method. When cotyledon explants were treated with osmoticum-conditioning medium (0.6 M sorbitol/mannitol) 4 hours prior to and 16 hours after bombardment, it was identified by GUS assay that this condition was the most efficient one for introduction of foreign genes into cotyledon tissue of lettuce with particle bombardment. The stable integration of a GR gene was confirmed by the PCR analysis. 0.3, 0.6, 1.5 kbp PCR fragments of transgenes were obtained by three types of primers designed on the basis of GR sequence. To know whether the expression of the GR gene of pBKs-GR 1 can be stably maintained in the next generation, T$_2$selfing seeds obtained from the transformed mother plants were sowed on MS medium supplemented with 200 mg/L kanamycin sulfate. 70% of seedlings showed resistance to kanamycin.

  • The purpose of this study was to compare effects of demineralization of citric acid and tetracycline HCI on periodontally involved root surface. Twelve periodontally involved single rooted teeth were used. After scaling and root planing, root conditioning with citric acid and tetracycline HCI were carried and the teeth were processed scanning electron microscopic observation.The results were as follows: The scaled root surface was covered by much debris and calculus. The effect of demineralization of citric acid and tetracycline HCI was more reduced on scaling group than root planing group, because of hypermineralization of cementum surface and demineralization effect on root surface of tetracycline HCI showed tendency to reduction. The root planed group displayed more smooth root surface than scaling group, the surface was covered by smear layer, thus no exposure of dentinal tubule opening and collagen fiber, especially after root planing, citric acid and tetracycline HCl treated group showed exposure of dentinal tubule and collagen fiber, thus it was thought that new connective tissue attachment could be acquired.
