  • Objectives : The purpose of this study is to analyze the characteristics of tinnitus patients according to the presence of headache. Methods : The medical records of 110 tinnitus patients who visited oo Korean Medicine Clinic from June 2019 to June 2021 were retrospectively reviewed. Clinical characteristics including gender, age, onset, cause, tinnitus site, severity of tinnitus and accompanied symptoms were analyzed. Results : Among all patients, 40s and 50s and chronic tinnitus over 1 year was the most frequent group. After an average of 2 months of treatment, 61 patients(55.5%) showed improvement, and there were prescribed more frequently 50s and 60s and with tinnitus grade mild to moderate group. The proportions of female(65%, p=0.012) and herbal dialectics which 'tonify and replenish' and 'tranquilize'(32.5%, p=0.043) were prescribed more frequently in patients with headache than those without headache. Dizziness, ear pain, heart symptoms, fatigue, and neck stiffness were more frequent in patients with headache(p<0.05). In patients with improvement in tinnitus, tinnitus grade improved significantly in patients with improvement in headache compared to those with no improvement in headache(1.83±0.79 vs 0.80±0.84, p=0.018). Conclusions : In tinnitus patients treated with Korean medicine, there was no significant difference in the improvement of tinnitus according to the presence or improvement of headache, but the patients with improvement of both tinnitus and headache showed significantly more degree of improvement in tinnitus severity than patients with only tinnitus improved. Based on these findings, further study is needed regarding the correlation headache and tinnitus in clinical setting.

  • Tinnitus is usually due to insufficiency of Kidney-essence, asthenia of the Spleen and Stomach, diseases caused by exogenous pathogenic factor, Liver fire(肝火) by severe stress, Phlegm -fire(痰火) by surfeit of high caloric food and alcoholic liquor. I observed and treated four patients who had been diagnosed with tinnitus of Liver-fire(肝火) and Phlegm-fire(痰火). After I checked the Inyeong(人迎) and the chon spot(寸口), all of their Inyeong were stronger than chon spot. Unbalance of the Inyeong(人迎) and the chon spot(寸口), and symptoms of tinnitus were removed by Acupuncture therapy of three Yang meridians of the hand(手三陽) and three Yang meridians of the foot(足三陽).

  • Objectives : The purpose of this study was to evaluate the effectiveness of Chuna manual therapy (CMT) for the treatment of tinnitus. Methods : We performed a literature search using eight electronic databases, using related keywords, from January 1990 until the end of April 2019. The randomized controlled trials (RCTs) which evaluated the effectiveness of CMT in the treatment of tinnitus were included in this study. The risk of bias were evaluated using the Cochrane risk of bias tool. Results : Five appropriate RCTs were included and analyzed. The efficacy rate of the CMT group was statistically significantly higher (P<0.03) versus the control group treated with Western medicine, acupuncture, and herbal medicine-only. Conclusions : In this literature review, there was sufficient evidence that CMT is more effective than conventional therapy (including Western medicine, acupuncture, and herbal medicine) in treating tinnitus. However, it should be considered that the included studies lacked any reference of the risk of bias

  • Objective The case report showed that sudden hearing loss and tinnitus after trigeminal schwannoma surgery were improved with Yeoldahansotang-gamibang. Method The patient was diagnosed with Taeeumin interior heat (Ganyeol) disease based on the original symptoms of Taeeumin. He was treated with herbal medicine and acupuncture treatment. Puretone audiometry (PTA), speech audiometry, tinnitus handicap inventory (THI) and the original symptoms were investigated before and after the treatment. Results Right side PTA score was significantly reduced, speech audiometry was improved in speech reception threshold (SRT), word recognition score (WRS) and most comfortable level (MCL) and THI score decreased from 40 to 0, which showed normalizing hearing function. And the patient revealed improvements in sleeping, digestion, stooling, perspiration and facial sensation after treatment. Conclusion This study suggests that Yeoldahansotang-gamibang is effective on sudden hearing loss and tinnitus after surgery by correcting the imbalanced energy of Taeeumin.

  • Objective : The aim of this study is to classify the patterns of Ryodoraku using cluster analysis on tinnitus patients. Method : 261 patients with tinnitus and 159 patients with other disease were analyzed. We used cluster analysis with SPSS 20.0 to classify the pattern of Ryodoraku. We searched the cluster characteristics and tried to interpret the specific patterns from previous studies. Result : We divided tinnitus patients to four groups and control patients to five groups. There were some shared patterns between tinnitus group and control group. One group on tinnitus patients showed lower age than other groups. Time, sex, total average did not show any differences among groups. Conclusion : Cluster analysis on tinnitus patients showed the possibility of presenting the specific Ryodoraku patterns. But because of weak Ryodoraku studies, clinical usefulness is unknown. Further study is needed.

  • Objectives : The purpose of this study is to analyze the various methods of sound therapy(ST) applied to tinnitus patients and their effectiveness, and to suggest an effective method that can be applied in clinical settings. Methods : Studies published from January 2018 to March 2023 were searched on 5 databases(Pubmed, RISS, OASIS, KISS, KCI). All RCTs that applied sound therapy as a main treatment method were included. Results : 14 studies were included. In 13 studies, sound therapy was statistically significant in improving tinnitus. Basic sound therapy was used in 6 studies(42%), followed by tinnitus rehabilitation therapy(TRT)(n=5, 35%). White noise(n=11, 75%) and nature sound(n=4, 28%) were the most frequently used sound sources. In the case of intensity, mixing point were the most common with 6 studies(42%). The mobile application(n=4) was the most frequently used implement. The application time of sound therapy per day was more than 3 hours(n=7), and the total treatment period was more than 3 months(n=9). Conclusions : Our findings indicate that sound therapy could be considered as an intervention for tinnitus patients. For better use, we suggest a basic type of sound therapy or TRT using white noise or nature sound at the mixing point level provided as a mobile phone application. In addition, the treatment period is recommended to be more than 3 hours/day for 3 months.

  • Hemangiomas are relatively common in the head and neck region, but their occurrence in the temporal bone are extremely rare. The hemangioma of the external auditory canal (EAC) is a rare otologic entity as only 22 cases of hemangioma of the EAC have been reported worldwide. Here we report a patient presenting with pulsatile tinnitus, which turned out to be caused by capillary hemangioma arising from the posterior medial portion of external auditory canal touching the tympanic membrane. Surgical excision of the tumor was successful. The clinical manifestations and management of benign vascular lesions of the EAC are discussed with a review of literature.
