Multiple Endocrinologic Complications in Thalassemia Major

  • Thalassemia major is a genetic disorder with a defective synthesis of either the alpha or the beta chain of hemoglobin A. Blood transfusion is crucial for the survival in these patients. Unfortunately, endocrine dysfunction is a very common complication in these patients and is principally due to excessive iron overload as a result of frequent blood transfusions. Although regular blood transfusion may increase life expectancy, disturbances in growth and pubertal development, abnormal gonadal functions, impaired thyroid, parathyroid and adrenal functions, diabetes, and disorderly bone growth are common side effects. We hereby present a case of a 23-year-old, unmarried woman with beta thalassemia major presenting with primary amenorrhea, poor development of secondary sexual character, and short stature. Thorough history, clinical examination, and laboratory investigation, including dynamic function test (insulin tolerance test) were conducted. These tests confirmed that she had multiple endocrinopathies, including hypogonadotropic hypogonadism, growth hormone deficiency, and subclinical adrenal insufficiency, which were caused by iron overload. She required hormone replacement therapy. Early recognition of possible deficiencies in hypothalamo-pituitary-end organ hormones caused by iron overload in thalassemia patients that undergo frequent blood transfusion procedures is essential. Appropriate treatments, including transfusion regimen and chelation therapy, as well as specific treatment of each complication are the crucial for the successful management and improvement of quality of life these patients.

  • Although primary surgical excision is the treatment of choice for the vast majority of patients with nasopharyngeal angiofibroma, radiation therapy is also employed as a primary tool or in the cases of tumor recurrence or massive intracranial extension. But, especially in young patients, postradiation sequelae such as osteomyelitis, atrophic rhinitis, thyroid or bone malignancy are of concern. Postirradiation osteosarcoma is a rare, but well-recognized complication of radiotherapy, which comprise roughly 2 to 5 percent of all osteogenic sarcoma. Recently we experienced a case of postirradiation osteosarcoma of the mandible who had treated by radiation therapy for the nasopharyngeal angiofibroma 30 years ago, presently he is ongoing chemotherapy and relatively being well.

A case of hypothyroidism concurrent with myxedema coma in a Cocker spaniel dog

  • A 4-year-old female Cocker spaniel weighing 7.74 kg was presented with a seven-month history of lethargy and skin problems. On the basis of clinical history, physical examination, laboratory tests, electrocardiogram, radiography, ultrasonography, and thyroid function tests, the dog was diagnosed as hypothyroidism. The dog was administered levothyroxine and presented a good response to the therapy. However, uncontrolled for about 6 months, the dog was re-admitted to hospital with depression and three seizure episodes. The dog was diagnosed as hypothyroidism with concurrent myxedema coma and gallbladder mucocele. With fluid therapy and administration of levothyroxine, the dog was successfully controlled for 3 months after starting treatment. This report describes the clinical features and successful treatment of a Cocker spaniel dog with concurrent hypothyroidism and myxedema coma.

A Case of Tracheal Cancer Arising 3 Years after Radiation Therapy for Laryngeal Cancer (후두암에 대한 방사선 치료 3년 후 기관암이 발생한 환자 1례)

  • Tracheal cancer is rare and accounts for approximately 0.03% of all malignancies. Because of atypical symptoms, tracheal cancer can be misdiagnosed as obstructive lung disease, or tumors of thyroid or lung. Among patients of previous head and neck cancer, other primary cancer may accompany which called "econd primary cancer". We report a case of patient with tracheal cancer 3 years after definite radiation therapy of laryngeal cancer with a review of related literatures.

Usefulness of $^{18}F$-FDG PET/CT in Locoregional Recurrence of Differentiated Thyroid Cancer: Comparison PET/CT to PET and Neck Ultrasonography for Biopsy-proven Lesions (갑상선유두암 재발 진단에서 $^{18}F$-FDG PET/CT와 경부초음파검사 병용의 유용성: PET, 경부초음파검사 그리고 혈청 티로글로불린 (thyroglobulin)의 비교)

  • Purpose: The aim of this study was to investigate the usefulness of $^{18}F$-FDG PET/CT with neck ultrasonography (neck US) in patients with recurrent, papillary thyroid cancer. Material and methods: This retrospective study (December 2006 to April 2008) enrolled sixty-one patients (ninety-one lesions) who underwent high-dose $^{131}I$-ablation therapy after total thyroidectomy, and evaluated recurred papillary thyroid cancer. All lesions were confirmed by histopathology and compared histopathologic findings to PET, PET/CT, and neck US findings. Results: In sixty-one patients (57 women, 4 men; age range, 24-81 years, mean 49 years; 61 papillary carcinomas), the sensitivity, specificity, accuracy of $^{18}F$-FDG PET/CT was 87.2%, 64.0%, 78.1% on a patient basis and 92.3%, 66.7%, 80.9% on a lesion basis, respectively. The sensitivity, specificity, accuracy of $^{18}F$-FDG PET was 71.8% (p=0.03), 59.0% (p=1.00), 67.2% (p=0.03) on a patient basis and 78.8% (p<0.01), 64.1% (p=1.00), 72.5% (p=0.02) on a lesion basis, respectively. The sensitivity, specificity, accuracy of neck US was 71.1% (p=0.07), 52.2% (p=0.75), 63.9% (p=0.05) on a patient basis and 71.2% (p<0.01), 61.5% (p=1.00), 67.0% (p=0.06) on a lesion basis, respectively. Combined $^{18}F$-FDG PET/CT with neck US improved the sensitivity, specificity, accuracy to 94.7% (p=0.50), 82.6% (p=0.13), 90.2% (p=0.03) on a patient basis and 96.2% (p=0.50), 89.7% (p<0.01), 93.4% (p<0.01) on a lesion basis, respectively. Conclusion: $^{18}F$-FDG PET/CT demonstrated significantly higher sensitivity than neck US for the detection of recurred papillary thyroid cancer lesions. Furthermore, combined $^{18}F$-FDG PET/CT with neck US showed more improved sensitivity, specificity, accuracy for diagnosis of recurrent papillary thyroid cancer.

The Evaluation of Images with Various Filters in I-131 SPECT/CT (I-131 SPECT/CT에서 Ringing Artifact 감소를 위한 다양한 Filter값의 적용)

  • Purpose: After I-131 therapy, SPECT/CT is useful in identifying location of thyroid remnants and metastasis of thyroid cancers. An excessive uptake of thyroid leads to a ringing artifact in the SPECT images. The aim of this study is to investigate and suggest a proper post filters to remove ringing artifact and produce better image quality. Materials and Methods: A low-cost, customized thyroid-mimicking phantom, consisting of an acrylic bottle and a hollow sphere was used for SPECT/CT Discovery (GE Healthcare, USA). It was filled with I-131 solution. The ratio of hollow sphere to background were varied as 50:1, 200:1, 1000:1 and 4000:1. Acquired images were reconstructed by OSEM (2 iterations, 10 subsets) with and without Evolution (resolution recovery correction, GE). Three different post-filters were applied; Butterworth (cut off: 0.38 to 0.58 with intervals of 0.05), Hanning (cut off: 0.8 to 1 with intervals of 0.05) and Gaussian (FWHM: 3 to 5 with intervals of 0.5) filters. Contrast, background variability, air area variability, and full width half maximum (FWHM) were compared. Results: Higher contrasts were obtained from the SPECT images with Evolution than without Evolution. In the case of images without Evolution, image distortion such as star artifact was generated. For all sphere-to-background ratio, the Butterworth filter showed better constrasts and FWHMs than other two filters, but the ringing artifact was still generated in all studies except 50:1 and it was decreased as cutoff value was increased. The ringing artifact didn't appear with Hanning and Gaussian filters at all studies, however constrats and FWHMs with Gaussian was worse than Hanning filter. For the images having ringing artifacts, the background variability and air area variability were increased. Conclusion: In this study, we suggested that it is desirable to use Hanning filter when the ringing artifact is generated and to use Butterworth filter when ringing artifact is not generated in I-131 SPECT.

A Case of Subclinical Hyperthyroidism Treatment with Herbal Medicine (한약 단독치료로 호전된 불현성 갑상선기능항진증 증례 보고)

  • Objective: This study was performed to evaluate the effects of herbal medicine on an elderly patient with subclinical hyperthyroidism who could not use antithyroid medication due to an adverse reaction to methimazole. Methods: Herbal medicine was administered and a thyroid function test was used to evaluate the effects of the treatment. Results: After treatment with herbal medicine and therapy, the patient showed improvements in TSH levels. Conclusion: This case suggested that herbal medicine can be used for patients who have adverse reactions to antithyroid medication; however, additional studies with more patients are required for verification of this finding.

  • 고에너지 의료용 선형가속기를 이용한 폐암 방사선치료 시 갑상선에 미치는 선량을 평가하였다. 산란광자의 영향은 3DCRT 시 평균 27.9mSv, IMRT에 있어서는 43.6mSv로 평가 되었다. 광중성자의 영향은 3DCRT 시 평균 3.2mSv, IMRT에 있어서는 평균 4.7mSv로 평가 되었다. 산란광자와 광중성자 모두 3DCRT 보다 IMRT가 높은 것으로 평가 되었다. 본 연구를 통하여 고에너지를 이용한 방사선치료 시 인접한 정상조직에 상당한 양의 산란선량이 영향을 주는 것으로 평가된 바와 같이 방사선을 이용한 암 치료 종사자는 이러한 내용을 충분히 인지하여 피폭 저감화를 위한 노력이 필요할 것으로 사료된다.

A Surgical Case of Lymphocytic Hypophysitis with Progressive Visual Worsening

  • Lymphocytic hypophysitis is a clinically rare disease, and it has been known to be an autoimmune disease which mainly affects pregnant women at the end of gestation or right after delivery. The authors experienced a case of lymphocytic hypophysitis in a 29-year-old pregnant woman with rapid progressing visual disturbance. Sella MRI showed a mass-like lesion of hypophysis and hypertrophy of pituitary stalk with evidences of hypopituitarism. Cesarean section was done and then TSA was performed. The pathologic diagnosis was lymphocytic hypophysitis. After TSA, visual acuity was improved and visual field defect was recovered. She was given thyroid hormone replacement therapy because of transient partial hypopituitarism for 6 months after surgery. One must consider the probability of lymphocytic hypophysitis, if there are alteration of visual acuity and visual field defect which aggravate rapidly during pregnancy due to mass effect, decreased serum hormonal levels shown in hypopituitarism and sella MRI findings of hypertrophy of pituitary stalk and enlargement of pituitary gland.
