과배란유도 시 혈중 AMH와 난소 반응성과의 상관관계; 예측 인자로서의 효용성 (Correlation of Basal AMH & Ovarian Response in IVF Cycles; Predictive Value of AMH)

  • 목 적: 체외수정을 위한 과배란유도 시 난소 반응성의 예측 인자로서 항뮬러리안 호르몬 (AMH)의 효용성을 FSH와 비교하여 알아보고, 체외수정 결과와의 관계도 알아보고자 하였다. 연구방법: 2007년 1월부터 2007년 8월까지 단기 요법이나 GnRH antagonist 요법을 이용하여 체외수정을 시행하는 111명을 대상으로, 생리주기 3일째 혈중 AMH 및 FSH를 전향적으로 측정하였다. 과배란유도 후 채취된 난자 개수와 AMH 또는 FSH의 상관관계를 분석하였고, 채취된 난자 수 및 AMH 수치에 따라 과배란유도 결과와 체외수정 결과를 비교하였다. 저반응군 및 과반응군의 예측을 위한 AMH의 임계치를 알아보기 위해 ROC curve 분석을 시행하였다. 결 과: AMH는 채취된 성숙 난자 수와 밀접한 양의 상관관계를 나타냈으며 (r=0.792, p<0.001), FSH (r=-0.477, p<0.001) 보다 더 높은 상관관계를 보였다. 난소 반응성에 따라 저반응군 (성숙난자수 ${\leq}2$), 정상반응군 ($3{\sim}16$), 과반응군 (${\geq}17$)으로 나누어 AMH와 FSH 수치는 유의한 차이를 보였으며, 저반응군 (채취된 성숙 난자 수${\leq}2$) 및 과반응군 (채취된 성숙 난자 수${\geq}17$)을 예측하는데 ROC curve 상 AMH의 임계치는 각각 <0.50 ng/ml (sensitivity 88.9%, specificity 89.5%), 및 ${\geq}2.60\;ng/ml$ ((sensitivity 85.7%, specificity 87.0%)였다. AMH level에 따라 low AMH group (${\leq}0.60\;ng/ml$), normal AMH group ($0.60{\sim}2.60\;ng/ml$), high AMH group으로 구분하여, 각 군간 채취된 성숙 난자 수와 ($2.7{\pm}2.2$, $8.1{\pm}4.8$, $16.5{\pm}5.7$, p<0.001), 투여된 성선자극호르몬제의 용량에 ($3530.5{\pm}1251.0$, $2957.1{\pm}1057.6$, $2219.2{\pm}751.9\;IU$, p<0.001) 유의한 차이를 보였다. 성숙 난자의 비율과 수정율에 유의한 차이는 없었으며 임신율도 23.8%, 34.0%, 37.5%로 각 군간 유의한 차이는 없었다. 결 론: 혈중 기저 AMH는 과배란유도 시 채취되는 성숙 난자 수와 높은 상관관계가 있어 난소 반응성의 예측 인자로서 유용할 것으로 생각된다. AMH 수치에 따라 체외수정 결과에는 차이가 없었으나, 저반응군 및 과반응군의 예측에 유용하여 체외수정 시 주기의 취소나, 환자별 적정 용량의 결정 및 난소과자극 증후군의 위험을 감소시키는데 도움이 될 것으로 사료된다.

온도조건(溫度條件)이 벼멸구의 발육(發育) 및 산란(産卵)에 미치는 영향(影響)에 관한 연구(硏究) (Effects of Temperature Conditions on the Growth and Oviposition of Brown Planthopper, Nilaparvata lugens $St{\aa}l$)

  • 온도조건이 벼멸구(Nilaparvate lugens Stal)의 발육과 산란에 미치는 영향을 정밀하게 조사하여 벼 포장에서의 온도변화에 따른 벼멸구의 발육경과를 예측함으로서 방제적기를 포착하는데 필요한 기초자료를 얻고자 정온과 변온의 식물 생장조정항온기 내에서 벼멸구의 발육속도 및 산란력을 측정하는 실험을 수행한 바 그 결과를 요약하면 다음과 같다. 1. 정온조건에서의 발육 및 산란 벼멸구 난의 부화율은 $25^{\circ}C$에서 가장 높았으며 그 이하나 이상의 온도로 변함에 따라 낮아지는 경향이었다. 난기간은 $27.5^{\circ}C$에서 가장 짧았으며 온도가 낮아짐에 따라 난기간이 길어지는 경향이었고 $30^{\circ}C$ 이상의 고온에서도 난의 발육이 지연되었다. 우화율은 $27.5^{\circ}C$에서 가장 높았고 온도가 낮아짐에 따라 낮아져 $32.5^{\circ}C$$35^{\circ}C$에서는 전혀 우화하지 못하였다. 약충기간은 $27.5^{\circ}C$$30^{\circ}C$에서 가장 짧았으며 온도가 낮아짐에 따라 길어지는 경향이었다. 우화성충의 수명은 온도가 낮아짐에 따라 길어지는 경향이었고 수컷은 $27.5^{\circ}C$에서 수명이 가장 짧았다. 산란전기는 $32.5^{\circ}C$에서 가장 짧았으며 온도가 낮아짐에 따라 길어져 $17.5^{\circ}C$에서는 $32.5^{\circ}C$에서보다 약 6.5배에 달했다. 산란수는 $25^{\circ}C$에서 가장 많았으며 그 이하나 이상의 온도로 변함에 따라 적어지는 경향이었다. 2. 변온조건과 정온조건에서의 발육 및 산란 같은 유효적산온도에서, 난의 부화율은 정온 조건에서보다 변온조건에서 약 10% 정도 높았으나 난기간은 큰 차이가 없었다. 우화율은 $22^{\circ}C$ 동일 유효적산온도 조건의 경우 변온에서 약 8% 정도 높았으나 $28^{\circ}C$ 동일 유효적산온도의 경우 반대로 정온조건에서 약 8% 정도 높았다. 약충기간은 정온조건에서보다 변온조건에서 약 $4{\sim}6$일 정도 길었다. 같은 유효적산온도에서, 성충의 수명과 산란수는 성충기의 온도가 정온인 것보다 변온인 것에서 길었으며 산란수도 많았다. 한편, 난기에서부터 약충기까지 노출시켰던 온도를 고려해 보면 약충기까지 $28^{\circ}C$ 정온에서 노출시켰던 구에서 각 온도 공히 성충의 수명이 길어져 성충의 수명은 약충기의 온도에 영향을 받았다. 산란전기에는 약충기와 성충기의 온도가 함께 영향을 주었으며 산란수에는 성충기의 온도가 큰 영향을 주었다. 3. 각 발육단계별 발육임계온도 및 유효적산 온도 본 실험결과 계산된 발육임계온도는 난기에서 $14.12^{\circ}C$, 약충기에서 $14.76^{\circ}C$, 성충기에서 $9.12^{\circ}C$, 산란전기에서 $15.95^{\circ}C$로 나타났다. 각 발육단계별 유효적산온도는 난기에서 141.25 온일도(Degree-Day), 약충기에서 167.83 온일도, 성충기에서 349.64 온일도, 그리고 산란전기에서 58.60 온일도 이었다.

감정예측모형의 성과개선을 위한 Support Vector Regression 응용 (Application of Support Vector Regression for Improving the Performance of the Emotion Prediction Model)

  • 오늘날 정보사회에서는 정보에 대한 가치를 인식하고, 이를 위한 정보의 활용과 수집이 중요해지고 있다. 얼굴 표정은 그림 하나가 수천개의 단어를 표현할 수 있듯이 수천 개의 정보를 지니고 있다. 이에 주목하여 최근 얼굴 표정을 통해 사람의 감정을 판단하여 지능형 서비스를 제공하기 위한 시도가 MIT Media Lab을 필두로 활발하게 이루어지고 있다. 전통적으로 기존 연구에서는 인공신경망, 중회귀분석 등의 기법을 통해 사람의 감정을 판단하는 연구가 이루어져 왔다. 하지만 중회귀모형은 예측 정확도가 떨어지고, 인공신경망은 성능은 뛰어나지만 기법 자체가 지닌 과적합화 문제로 인해 한계를 지닌다. 본 연구는 사람들의 자극에 대한 반응으로서 나타나는 얼굴 표정을 통해 감정을 추론해내는 지능형 모형을 개발하는 것을 목표로 한다. 기존 얼굴 표정을 통한 지능형 감정판단모형을 개선하기 위하여, Support Vector Regression(이하 SVR) 기법을 적용하는 새로운 모형을 제시한다. SVR은 기존 Support Vector Machine이 가진 뛰어난 예측 능력을 바탕으로, 회귀문제 영역을 해결하기 위해 확장된 것이다. 본 연구의 제안 모형의 목적은 사람의 얼굴 표정으로부터 쾌/불쾌 수준 그리고 몰입도를 판단할 수 있도록 설계되는 것이다. 모형 구축을 위해 사람들에게 적절한 자극영상을 제공했을 때 나타나는 얼굴 반응들을 수집했고, 이를 기반으로 얼굴 특징점을 도출 및 보정하였다. 이후 전처리 과정을 통해 통계적 유의변수를 추출 후 학습용과 검증용 데이터로 구분하여 SVR 모형을 통해 학습시키고, 평가되도록 하였다. 다수의 일반인들을 대상으로 수집된 실제 데이터셋을 기반으로 제안모형을 적용해 본 결과, 매우 우수한 예측 정확도를 보임을 확인할 수 있었다. 아울러, 중회귀분석이나 인공신경망 기법과 비교했을 때에도 본 연구에서 제안한 SVR 모형이 쾌/불쾌 수준 및 몰입도 모두에서 더 우수한 예측성과를 보임을 확인할 수 있었다. 이는 얼굴 표정에 기반한 감정판단모형으로서 SVR이 상당히 효과적인 수단이 될 수 있다는 점을 알 수 있었다.

Auto-PEEP이 존재하는 환자에서 호흡 일에 대한 External PEEP의 효과 (The Effect of External PEEP on Work of Breathing in Patients with Auto-PEEP)

  • 연구배경: Auto-PEEP 혹은 intrinsic PEEP은 호기말에 폐용적이 전체 호흡기계의 이완 용적으로 돌아오지 않음으로써, 증가된 호흡기계의 탄성반도압만큼 호기말 폐포내압(alveolar pressure) 이양의 값을 보이는 것을 말한다. Auto-PEEP 이 존재하는 만성폐쇄성폐질환 환자에게 externa1 PEEP을 적용하면 환자의 호흡 일을 줄일 수 있어서, 질환의 급성악화시 혹은 기계호흡으로부터의 이탈시 환자의 자발호흡을 보조하기 위한 요법으로 제시되고 있다. 이에 기계호흡중인 환자에서 auto-PEEP의 존재가 호흡 일에 미치는 영향을 알아보고, externa1 PEEP의 사용이 auto- PEEP에 의해 증가된 호흡 일을 줄이는지를 알아보기 위해 본 연구를 시행하였다. 방법: 호흡부전으로 기계호흡을 하고 있는 환자 15명을 대상으로 연구가 이루어 졌으며, 이들 7명에서 auto-PEEP이 관찰되었고(auto-PPEP군) 8명에서 auto-PEEP이 auto-PEEP군). 양군 간의 환자의 호흡역학적 지표의 차이를 조사하였으며, auto-PEEP이 존재하는 환자들에 대해 3cm $H_2O$의 external PEEP을 적용한 뒤 호흡역학적 지표들의 변화를 조사하였다. 호흡역학적 지표는 상시호흡량(tidal volume, 이하 $V_T$), 분당 호흡수, 분당환기량 (minute ventilation 이하 $V_E$), 최고흡기유량(peak inspiratory flow rate, 이하 PIFR), 최고호기유행peak expiratory flow rate, 이하 PEFR), 최고흡기압(peak inspiratory pressure, 이하 PIP), $T_I/T_{TOT}$, auto-PEEP, 폐 동적탄성 (dynamic compliance of lung, 이하 Cdyn), 호기 기도저항(expiratory airway resistance, 이하 RAWe), 평균 기도저항(mean airway resistance, 이하 RAWm), $P_{0.1}$, 환자에 의해 수행되는 호흡 일 (work of breathing performed by patient, 이하 호흡 일), pressure-time product(이하 PTP)등이었다.

정상 뇌 혈류 영상에서 방사성의약품에 따라 혈류 분포에 차이가 있는가: 통계적 파라미터 지도를 사용한 분석 (Different Uptake of Tc-99m ECD and Tc-99m HMPAO in the Normal Brains: Analysis by Statistical Parametric Mapping)

  • 목적: 이번 연구는 두 명의 경험 있는 핵의학과 의사에 의하여 정상이라고 판독된 뇌 혈류 단일광자방출전신화단층촬영 영상에서 사용한 방사성의약품에 따라 혈류 분포에 차이가 있는지를 통계적 파라미터 지도를 사용하여 분석하여 보았다. 대상 및 방법: 정상이라고 판독된 뇌 혈류 단일광자방출전산화단층촬영 53 증례를 후향적으로 분석하였고, 그 중 32증례는 Tc-99m ECD를 방사성의약품으로 사용하였으며 나머지 21 증례는 Tc-99m HMPAO를 방사성의약품으로 사용하여 획득한 영상이었다. 모든 뇌 혈류 단일광자방출전산화단층촬영 영상은 통계적 파라미터 지도 분석에 적합하도록 미리 표준화 시킨 후 분석을 시행하였고 분석결과 통계적으로 두 그룹간에 corrected P value가 0.05 이하인 경우에 서로 의미 있는 차이라고 해석하였다. 결과: 정상으로 판독된 뇌 혈류 단일광자방출전산화단층촬영 중에서도 방사성의약품을 어느것을 사용하느냐에 따라 분명한 혈류 분포의 차이를 나타내었다. Tc-99m ECD를 사용한 뇌 혈류 단일광자방출전산화단층촬영에서는 Tc-99m HMPAO를 사용하였을 때 보다 전두엽, 측두엽, 후두엽, 기저핵, 시상 및 소뇌의 상부 등 광범위한 부위에서 높은 혈류분포를 보였으며 반대로 Tc-99m HMPAO를 방사성의약품으로 사용한 경우에는 Tc-99m ECD를 사용하였을 경우보다 전두엽의 피질하 부위, 측두엽 일부 및 소뇌의 하부 등 국소적인 부위에서 상대적으로 증가된 혈류분포가 관찰되었다. 결론: 통계적 파라미터 지도를 사용하여 분석하여 본 결과 정상으로 판독된 뇌 혈류 단일광자방출전산화단층촬영에서 사용된 방사성의약품에 따라 유의한 혈류분포의 차이를 나타내었다. 따라서 뇌 혈류를 평가하는데 있어서는 어떠한 방사성 의약품을 사용하였는가를 반드시 고려하여야 한다.손상 환자들의 정신의학적 후유 장애에서 임상심리평가에 보조적 역할을 한다.n{\pm}S.D.$) 이다. 결론적으로 기존의 신장깊이를 구하는 여러 방법들과는 달리 본 연구를 통해 핵의학 영상만으로도 개인차가 고려된 신장깊이를 얻어낼 수 있게 되었다. 결론: 이 연구를 통해 도출된 신장깊이 계산 방정식을 이용하여 구한 신장깊이는 방사성동위윈소 주입 후 감마카메라를 이용하여 집적된 계수를 이용하여 구하는 것이 되므로 개개인의 개인차는 물론 좌신과 우신에 따른 차이도 고려될 수 있다. 더 나아가 이연구에서 개발된 신장깊이 계산 프로그램의 임상 응용에서의 적용을 위해서는 신장에 도달하는 방사성 동위윈소의 방사능양과 효율적일 관심영역 크기에 대한 연구가 이루어지며 좀더 정확하고 개인차가 고려된 신장깊이가 계산될 것이라고 사료된다.발한 핵의학 영상의 정량적 분석을 통한 신장기능 분석 프로그램을 사용하여 신장 기능을 분석한 결과, 타당성 있는 결과를 도출하여 그 유용성을 입증하였다. 이 개발 프로그램은 좀 더 다양한 임상응용 목적의 분석기능을 사용자가 직접 개발, 추가하기가 용이하여 기존 상용 프로그램보다 연구적 활용범위가 크다고 사료된다.2.2{\pm}0.4\;(1.6{\sim}3.2){\mu}g/ml$와 $1.4{\pm}0.2\;(0.8{\sim}1.6){\mu}g/ml$로서(p=0.16), 표준균주 3종 모두에서 Infecton의 MBC 또한 ciprofloxacin에 비해 $2{\sim}4$배가 높았다. 결론: Tc-99m Infecton은 ciprofloxacin 보다는 약하였지만 표준균주에 대해 생체외 항균력을 보였다.를 보였고 또한 다변량분석에서 휴식기의 관류지수는 좌심실 확장을 예측할 수 있는 유일한 지표였다. 또한 작은 수의

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.
