Change Patterns on Subjective Distress Level in Gynecological Postoperative Patients with Different Levels of State Anxiety (불안수준에 따른 부인과 수술환자의 불편감지각의 변화 - 통증 자가 조절기 사용 중인 환자를 중심으로 -)

  • Purpose: A descriptive study was conducted to identify the change patterns on the level of perceived distress during the early postoperative period with regard to state anxiety in patients using patient controlled analgesia. Method: One hundred women who underwent elective hysterectomy procedures or other gynecologic surgeries completed a series of questionnaires measuring state anxiety, and subjective distress assessed by visual analog scales at 8, 24, & 48 hours postoperatively. Data were analyzed with frequencies, percentages, means, ANOVA, Repeated Measures ANCOVA, and Scheff'e post test utilizing SPSS WIN 11.0. Result: There was a gradual decrease in levels of total distress and pain over the three points in time after surgery regardless the levels of state anxiety. However, women with higher levels of anxiety their pain curve rose at 48 hours post-op. In addition, over the first two points in time, women in the higher anxiety group showed higher levels of distress than those in the lower anxiety group, but no such group differences were observed in the levels of pain, revealing higher pain levels only at 8 hours post-op in both groups. Conclusion: Irrespective of effective pain management modality, most postoperatively experienced distress in gynecological patients was derived from anxiety and pain. These findings have implications for pain management, especially in patients with emotionally charged surgeries like hysterectomy.

Evaluation of physicochemical and textural properties of myofibrillar protein gels and low-fat model sausage containing various levels of curdlan

  • Objective: Curdlan has been widely used as a gelling agent in various food systems. This study was performed to evaluate the rheological properties of pork myofibrillar protein (MP) with different levels of curdlan (0.5% to 1.5%) and its application to low-fat model sausages (LFS). Methods: MP mixtures were prepared with 0.5%, 1.0%, and 1.5% of curdlan. Cooking loss (%), gel strength (gf), shear stress (Pa), and scanning electron microscopy were measured. Physicochemical and textural properties of LFS containing different levels of curdlan were measured. Results: The shear stress of MP mixtures increased with increasing levels of curdlan. MP gels with increased levels of curdlan decreased cooking loss and increased gel strength (p<0.05). The MPs with 1.0% and 1.5% of curdlan were observed more compact three-dimensional structure than those with 0.5% curdlan. Increased curdlan level in LFS affected redness ($a^{\star}$) and yellowness ($b^{\star}$) values. Although expressible moisture of LFS did not differ among curdlan levels, LFSs with various levels of curdlan decreased cooking loss as compared to control sausages. Hardness values (2,251 to 2,311 gf) of LFS with 0.5% and 1.0% curdlan was increased and differ from those (1,901 gf) of control sausages. Conclusion: The addition of 1.0% curdlan improved the functional and textural properties of LFS.

A Study on the Effects of Supportive Touch and Patient Educative on the Patient이s Post-operative Pain (지지적 접촉과 환자교육이 수술후 동통에 미치는 영향에 관한 연구)

  • The purpose of this study was to assess the value-of post-operative treatment in terms of nursing care in the allevation of pain. More specifically, the effects of supportive touch and patient education were examined. On the date before each patient's operation, the Bevels of anxiety, depression and affiliation were examined. The post-operative treatment was adminis. tered after an initial measurement of the patient's fain, according to the experimental category for the three days following the operation. On the final day, the level of anxiety and depression were again measured. The subjects of this study were 138 patient from the general surgical and gynecological wards in H university Hospital in Seoul. The study was conducted over a three-month period from June 24, 1984 to September 18, 1984. All patients had undergone laparatomies. Various standard instruments were used to measure the pain, anxiety, depression and affiliation levels. for pain, 5 grate Simple Descriptive Scale, and the Mclachlan four-range Observational Pain Scale were employed. For anxiety and depression, respectively the Spielberger State Anxiety Inventory and Beck Depression Inventory (B.D.I.) were used. Lastly, the affiliation was determined by the Mehrabian Affiliation Scale. The outcome of the research was as follows: 1. The first hypothesis concerning the existence of lower pain levels of Experimental Group A and Experimental Group B than the pain levels of Control Croup C was not supported. 2. The second hypothesis concerning the existence if lower anxiety levels of Experimental Group A and Experimental Group B than the anxiety levels of Control Group C was supported at the level of F=3.58 (p=.03). 3. The third hypothesis concerning the existence of lower depression levels of Experimental Group A and Experimental Group B than the depression levels of Control Group C was not supported. 4. The fourth hypothesis concerning the existence of different levels of pain in accordance with the levels of affiliation in Experimental Group A was not supported. 5. A positive correlation did exist between pain and anxiety after surgery (r=.34, p=.0001). Thus, the fifth hypothesis was supported. 6. A positive correlation did exist between pain and depression following surgery(r=.36, p=.0001). Thus, the sixth hypothesis was supported. Based on the above results, it was found that supportive touch and patient education either through human sources or via tape recorder do influence the anxiety of a patient after surgery, that a Positive correlation between pain, anxiety and deparession exist, that affiliation does not alter the influence of supportive touch, and that the graph on which pain levels were depicted indicates the possibility of development even if the effects of supportive touch did not reach a meaningful level. Thus it can be concluded that patient education, regardless of its form, is essential for the patient after surgery and that supportive touch, when reasonably modified and supplemented, can be an effective method of alleviating pain.

Effects of Dietary Protein and Lipid Levels on the Growth Performance, Feed Utilization and Innate Immunity of Juvenile Red Seabream Pagrus major (사료 내 단백질과 지방 수준이 참돔(Pagrus major) 치어의 성장, 사료효율 및 비특이적 면역력에 미치는 영향)

  • A $3{\times}3$ factorial study was conducted to investigate the effects of dietary protein and lipid levels on the growth, feed utilization and innate immunity of red seabream Pagrus major. Nine diets consisting of three protein levels (42%, 46% and 50% crude protein) and three lipid levels (10%, 14% and 18% crude lipid) were formulated. Triplicate groups of red seabream were fed the experimental diets to apparent satiation (5-6 times a day, from 08:00 to 18:00 h at 2-h intervals) for 10 weeks. At the end of the feeding trial, the weight gain and specific growth rate of fish fed P46L14 (46% protein and 14% lipid), P50L10 (50% protein and 10% lipid) and P50L14 (50% protein and 14% lipid) were significantly (P<0.05) higher than those of fish fed P42L18 (42% protein and 18% lipid). The feed conversion ratios (FCR) of the fish were affected by dietary lipid levels (P<0.039), but not dietary protein levels. The FCR tended to increase with increasing dietary lipid levels from 10% to 18% with the 46% and 50% protein levels. The weight gain, protein efficiency ratio, specific growth rate, feed intake and survival of fish were not affected by either dietary protein or lipid levels. Myeloperoxidase activity in the group fed P50L14 (50% protein and 14% lipid) was significantly higher than that in the group fed P42L10 (42% protein and 10% lipid) or P50L18 (50% protein and 18% lipid). However, the myeloperoxidase activity of fish was not affected by either dietary protein or lipid level. The fish fed P46L14 (46% protein and 14% lipid) and P46L18 (46% protein and 18% lipid) showed significantly higher superoxide dismutase activity than did the fish fed P46L10 (46% protein and 10% lipid), P50L10 (50% protein and 10% lipid) of P50L18 (50% protein and 18% lipid). In conclusion, the optimum protein and lipid levels for the growth and feed utilization of juvenile red seabream were 46% and 14%, respectively, and the optimum dietary protein to energy ratio was 27.4 g/MJ.

The Strength and Length Change Properties of Recycled Aggregate Concrete(RAC) by Compressive Strength Levels (압축강도 수준별 순환골재 콘크리트의 강도와 길이변화 특성)

  • This paper addresses mechanical properties and length change performance of the recycled aggregate concretes(RAC) in which natural coarse was replaced by recycled coarse aggregate(RCA) by compressive strength levels(20, 35, 50 MPa). A total of 9 RAC were produced and classified into three series, each of which included three mixes designed with three compressive strength levels of 20 MPa, 35 MPa and 50 MPa and three RCA replacement ratios of 0, 50 and 100%. Physical/Mechanical properties of RAC were tested for slump test, compressive strength, and length change. The test results indicated that the workability of RC could be improved or same by RCA replacement ratios, when compared with that containing no RCA. This is probably because of the RCA shape improving the workability of RAC. Also, the test results showed that the compressive strength was decreased by 9~10% as the RCA replacement ratios increase. However, the length change ratio by the RCA replacement ratios increased regardless of compressive strength levels. At 20 MPa level, the length change ratio was 8~40% which was much higher than that of 4~17% at both 35 and 50 MPa levels. Therefore, it was considered that such admixture addition preventing dry shrinkage is required in order to improve the properties of the RAC at 20 MPa level.

The Relationship between Risk Factors for Cardiovascular Disease and Levels of Plasma Total Homocysteine, Folate and Vitamin {TEX}$B_{12}${/TEX} in Koreans

  • The elevation of total plasmahomocysteine is now an established risk factor for cardiovascular disease. Plasma folate and vitamin {TEX}$B_{12}${/TEX} influence Hcy metabolism as cofactors. In this study, we studied the relationship of major risk factors for cardovascular disease, including advanced age, male gender, obesity, hypertension, hyperglycemia, and dislipidemia and plasma homocyteine, folate and vitamin {TEX}$B_{12}${/TEX} levels in Koreans. A total of 195 adult Koreans participated. The subjects were divided into three groups according to how many major conventional risk factors of cardiovascular disease they had: no risk, low risk (1~3 risk factors) and high risk (>3 risk factors) groups. As the number of risk factors increased, the plasma homocysteine levels significantly increase, while the plasma folate levels significantly decreased. The plasma homocysteine levels re higher in males than in females. The subjects with hyperglycemia had higher plasma homocysteine levels than the subjects without the risk factor. Also the subjects with dislipidemia had higher plasma homocysteine levels than the subjects without the risk factor. The plasma folate and vitamin {TEX}$B_{12}${/TEX} levels were significantly lower in males tan females. However, there were no significant differences in plasma folate and vitamin {TEX}$B_{12}${/TEX} levels between the subjects with or without other risk factors. These results indicate that plasma homocysteine levels were positively related with risk factors for cardiovascular disease and plasma folate levels were negatively related with the risk factors for cardiovascular disease. Also, we conclude that plasmahomocysteine levels might be related to the combination of risk factors, rather than an individual risk factor.

Influence of Food Allowance on Nutrient Utilization of Oat (Avena sativa) Hay by Goats and Feeding Value of Leftovers from High Levels of Offer in Sheep

  • Two experiments were conducted to determine the effect of feeding levels on intake and nutrient utilization of oat (Avena sativa) hay (OH) by goats and potential feeding value of leftovers from high levels of offer in sheep. In experiment 1, the goats (15) were offered OH at three levels of feeding to give leftovers of about 20% (T-1), 35% (T-2) and 50% (T-3) of DM offered. A marked effect of refusal rate of OH on intake and digestibility of nutrients was evident. Allowing selective consumption at higher levels (T-2 and T-3), the intake of DCP and TDN from OH was found not only to meet the maintenance requirement but provided surplus nutrients for moderate production. In experiment 2, Sheep (3) were offered one of the three dietary treatments viz OH, refusals left uneaten by goats in T-2 and T-3 (OHR) and 3% urea treated oat hay refusals (UTR) in a $3{\times}3$ latin square design. Though the potential feeding value of leftovers (OHR and UTR) was lower than OH, the results confirm the possibility of their effective use. It was concluded that feeding above the conventional ad libitum level could be an alternative to improve the performance of ruminants fed feeds like oat hay. The potential impact of the strategy would, however, depend on the effective reutilization of leftovers for feeding other animals.

Bruising in Slaughter Cattle and Its Relationship with Creatine Kinase Levels and Beef Quality as Affected by Animal Related Factors

  • The objective of the study was to determine the effects of animal related factors on bruising in slaughter cattle, creatine kinase (CK) and beef quality. Three hundred and twenty one cattle from three breeds (108 Bonsmara, 130 Beefmaster and 83 Brahman) were used in this study. The animals were grouped as follows: Group 1 (16 months old), Group 2 (18 months old) and Group 3 (24 months old). At exsanguinations, blood samples for CK determination were collected using disposable vacutainer tubes. Muscularis longissimus thoracis et lumborum (LTL) was collected 24 h after slaughter to determine the colour ($L^*$, $a^*$, and $b^*$) and ultimate pH ($pH_u$) of beef. Breed, sex and age had significant effects (p<0.05) on bruising score, CK levels and beef quality. Bonsmara breed had the highest (80%) bruising score percentage, CK ($705.3{\pm}80.57U/L$) and $pH_u$ ($6.3{\pm}0.05$) values while the Bonsmara had the highest $L^*$ ($24.8{\pm}0.78$) $a^*$ ($17.5{\pm}0.53$) and $b^*$ ($12.8{\pm}0.53$) values. Higher CK levels were also observed in winter compared to summer, spring and autumn respectively. Therefore, animal factors (sex, breed and animal age at slaughter) contribute to the development of bruises and have an effect on the levels of CK and meat quality. It was also concluded that there is no significant relationship between meat parameters (L,* $a^*$, and $b^*$) and CK levels.

A Comparative Study on the Dry Matter Yield and Nutritive value of Festulolium braunii (Festulolium braunii의 건물수량 및 사료가치 비교 연구)

  • This experiment was conducted to evaluate the forage yield and quality potential of Festulolium braunii (Festuca pratensis Huds. $\times$ Lolium multiflotum Lam) as forage source in Korea. The experimental design was a split plot design with four replications from Feb. 1994 to Jan. 1996 at Hohenheim uniwrsity. Main plots were three forage species {Fesruca pratensis(Cosmos), Lolium mult~~orum(Lemtal) and Festulolium braunii(Pau1ita)) and subplots were three N levels (150,300 and 450kgha). No differences in dry matter(DM) yields were noted between Festulolium braunii and Lolium multj7orum, but DM yields for Festulolium braunii was 138 more than for Fesruca prafensis(P<0.01 ). Total DM yields over all species increased linearly with increasing N levels. The peak of DM yields over 2 years mean at 450kglha N levels were higher for Lolium mulriJotum(l5,724kg) than for Festulolium braunii(14,730kg) and for Festuca pratensis(l3,68 l kg) (P<0.01). OMD, ME, NEL and other minerals of Fesrulolium braunii were slightly higher and CP, Ca and Mg slightly lower than thsoe of Fesruca pratensis, but CP, OMD, NEL and minerals were absolutly higher for Fesrulolium braunii than for Lolium mulriflorum(P<0.01). It appears that Festulolium braunii has higher potential for improvement of the forage quality than Fesruca prarensis or Lolium rnulrijlorum. The content of CP, CF and minerals were increased, wherease OMD, ME and NEL were decreased in all species with increasing N levels. Yields of CP, DOM and NEL over 2 yean mean were increased in all species with increasing N levels. A randomized, open labeled study comparing the serum levels of cobalamin after three doses of 500 mcg vs. a single dose methylcobalamin of 1500 mcg in patients with peripheral neuropathy

  • Background: Vitamin B12 deficiency has been associated with peripheral neuropathy, loss of sensation in the peripheral nerves, and weakness in the lower extremities. Methylcobalamin is the most effective analogue of vitamin B12 used to treat or prevent the complications associated with vitamin B12 deficiency. The current study aimed to compare the serum cobalamin levels after administration of two different regimes of methylcobalamin in peripheral neuropathy patients. Methods: The present study was a prospective, randomized, comparative study. The study consisted of two parallel groups, group A (methylcobalamin $500{\mu}g$ injection intramuscularly three times a week) and group B (methylcobalamin $1500{\mu}g$ injection intramuscularly once a week). A control group of healthy volunteers was also included. Results: A total of 24 patients (12 in each group) were included in the study. Five healthy volunteers were also included as a control in each group. At the end of treatment, serum cobalamin levels were significantly (P = 0.028) higher in group A ($1892.08{\pm}234.50$) as compared with group B ($1438.5{\pm}460.32$). The serum cobalamin levels in Group A healthy volunteers were also two times higher than that of group B (P = 0.056). Both the LANSS scale and DN4 questionnaire reported similar results at end of treatment. Conclusions: The $500{\mu}g$ methylcobalamin thrice weekly regime is more effective in increasing the serum cobalamin levels as compared to the $1500{\mu}g$ methylcobalamin once weekly regime.
