후측개흉술과 근육보존수직개흉술의 임상고찰 (Posterolateral Thoracotomy versus Muscle-sparing Vertical Thoracotomy)

  • 후측개흉술은 흉근을 절단하는 단점은 있으나 훌륭한 수술시야를 제공하므로 표준개흉술로 이용되고 있다. 이에 반하여 근육보존수직개흉술은 흉근이 보존되며 팔을 내리면 액와부 수술 상흔이 감추어지는 미용상의 장점이 있다. 그러나 수직개흉술은 흉부의 정측면에 절개선이 이루어지므로 측와위에서 수직으로 수술시야를 내려다보게 되어 흉강내의 구조물, 특히 폐문부나 첨부 및 하부의 길이가 멀어져 수술조작이 불편하다. 본 인하대병원 흉부외과에서는 수술조작에 관련된 차이를 알아보고자 후측개흉술(15례)과 수직개흉술(14례)을 이용해 폐쐐기절제술 이상의 수술을 실시한 29례에서 수술과 관련된 임상지표들을 비교관찰한 결과, 수술시간, 수술 1일과 2일의 흉관 배액량, 흉관 거치기간, 수혈 수와 수혈량, 수술 중 실혈량, 수술 후 합병증 등에서 양군간에 차이가 없었다. 이상의 결과를 볼 때 근육보존수직개흉술도 후측개흉술과 마찬가지로 폐절제술시 안전하게 사용될 수 있다고 본다.

우측 쇄골하 동맥 기시이상으로 인한 연하 곤란 - 수술 치험 1례 - (Dysphagia Due to an Aberrant Right Subclavian Artery)

  • An aberrant right subclavian artery is a rare congenital anomaly that usually does not produce symptoms. Symptomatic patients require surgical intervention. Ligation of the aberrant artery through a left thoracotomy has been advocated as the operation of choice. If development of vertebrobasilar insufficiency is anticipated, division and ligation of he aberrant artery and its anastomosis to the right common carotid artery or aortic arch are performed at a second operation. Experience with successful surgical treatment of a patient with an aberrant subclavian artery is described. A right thoracotomy incision was utilized for division of the subclavian artery and for reestablishment of arterial continuity with Dacron graft. Postoperative arteriography demonstrated a good reconstruction and normal blood flow was established to the right upper extremity.

간농양에 속발한 기관지늑막루의 치험 6례 (Broncho-pleural Fistula due to Liver Abscess: A Review of 6 Cases)

  • Six cases of broncho-pleural fistula due to complicated liver abscess were experienced at the department of chest surgery, Seoul National University Hospital from October 1967 to March 1970. Amebic liver abscess was the primary cause in the 5 cases and the remaining one case was due to pyogenic liver abscess. Involved lung was right side in all case. The clinical manifestation was fever,chill,cough, sputum, dyspnea, chest pain, hemoptysis and shoulder pain. The methods of the treatment employed were closed thoracotomy [1], thoracotomy & drainage [2],decortication [1],and right lower lobectomy[3]. The average duration of the post-operative course was 19.6 days. There was no operative mortality.

만성 교약성 심낭염의 외과적 치료 -4례 보고- (Surgical Treatment of Chronic Constrictive Pericarditis -Report of 4 ases-)

  • Constrictive pericarditis is often accompanied with ~brothorax and deterioration of cardiac, hemodynamic functions. Surgical relief of fibrous peel causes remarkable improvement in pulmonary, cardiac, hemodynamic function, and subjective symptoms. We experienced 4 cases of constrictive pericarditis combined with bilateral ~brothorax after bilateral pleural effusion caused by tuberculosis and non-specific inflammation. Pleural decortication and pericardiectomy were done at the same time through anterolateral thoracotomy with sternal transection[3 patients] and median sternotomy incision[l patient]. Low cardiac output was the most common complication. With left anterolateral thoracotomy, we could prevent the hypotension from massive retraction for dissecting by median sternotomy, which was good for dissecting from anterior wall of left ventricle to posterior wall of left ventricle and surrounding phrenic nerve. It was enough to dissect the portion being through hard to dissect, right atrium, SVC and IVC.

소아에서의 우측 쇄골하동맥 기시이상 -수술치험 2례- (Aberrant Right Subclavian Artery in Children -Report of Cases-)

  • Aberrant right subclavian arteries were experienced in two young children. This is a rare aortic arch anomaly that usually does not produce symptoms. Symtomatic patients require surgical interventions.We describe two young children who had aberrant right subclavian artery with symptoms of difficulty in swallowing and respiratory problems. Diagnosis was made by esophagography, aortography and 3-Dimension chest CT. Operation had been advocated through right thoracotomy without difficulty for ligation, division and anastomosis to the ascending aorta. Dysphagia lusoria was immediately relieved and postoperative course was uneventful. We consider that the right thoracotomy is the choice of operative approach and noninvasive diagnosis by 3-Dimension chest CT is easily made for infant with aberrant right subclavian artery.

외상성 횡경막 Hernia: 2례 보고 (Traumatic Diaphragmatic Hernia: A Report of Two Cases)

  • Two cases of traumatic diaphragmatic hernia are reported, who were operated on in this department during the last 12 months` period. The first case, a 34 year old male, fell from 6 meters` height while he was working on electric pole. He sustained rib fractures, left 8th, 9th and 10th rib, left diaphragmatic rupture and ileal perforation. In the pleural cavity, there were stomach, omentum, left lobe of liver, transverse colon and ileum, which were reduced into the peritoneal cavity, and the diaphragmatic aperture was closed through anterolateral thoracotomy. After closure of the thoracic incision, median abdominal incision was made and closed the ileal perforation by primary suture. The second case was a 19 year old tyre repairman, who felt abrupt severe abdominal pain during lifting a heavy lyre. A barium study revealed a marked displacement of the stomach into the left pleural cavity. Immediately, thoracotomy was performed and closed the ruptured diaphragm after reduction of the herniated stomach, omentum, transverse colon, spleen and small intestine. The size of the diaphragmatic aperture were measured 17cm. in first case and 12cm. in the other respectively. Both cases discharged after uneventful recovery.

종격동 종양에 있어서 종격동경 및 흉강경 검사의 진단적 의의 (Diagnostic Significance of the Mediastino-& Thoracoscopy in the Mediastinal Tumors)

  • Thoracoscopy, mediastinoscopy and/or mediastinotomy and explo-thoracotomy were performed and confirmed diagnosis of the 26 cases of the mediastinal tumors, those were admitted and treated at the Department of Thoracic and Cardiovascular surgery, Chonbuk National University Hospital, from June,1976 to September, 1986. We experienced 7 thoracoscopies, 7 mediastinoscopies &/or mediastinotomies, and 12 explo-thoracotomies. The most common histologic type was teratoma and lymphoma [6 cases]. Of the 26 cases of the histologically confirmed mediastinal tumor, 19 were benign tumors and 7 were malignants. 19 cases of benign tumors were surgically resected with good result but one case of the surgically resected malignant thymoma was recurred 4 months later. 6 cases of malignant lymphoma and one case of recurred malignant thymoma were treated with chemo-and radiation therapy. Thoracoscopy and mediastinoscopy with tissue biopsy were simple in procedure and had a tissue diagnosis with obtaining the pathologic tissue in a acceptable complication rate and reduced exploratory thoracotomy.

자연기흉의 임상적 고찰: (Clinical evaluation of spontaneous pneumothorax:a review of 360 cases)

  • We have observed 360 cases of spontaneous pneumothorax from Jan. 1971 to Dec. 1981 at the department of Thoracic and Card iovascular Surgery, Yonsei University College of Medicine. The patients age ranged from 2 days to 95 year-old. The associated pulmonary lesions were shown pulmonary tuberculosis in 158 cases[43.9%], bullae in 35, pulmonary emphysema In 32, pneumothorax in 10, paragonimiasis In 7 and unknown underlying pathology in 109 patients. 70 [51.1 %] out of 1 37 cases who received conservative medical treatment Including thoracentesis were cured completely, but the 67 cases [48.9 %] of remaining uncured patients were treated by surgical procedures. The 290 patients who received surgical management were recovered without recurrent pneumothorax. The surgical procedures were closed thoracotomy drainage or explothoracotomy. The choice of treatment should be based on the extent of pneumothorax or the presence of underlying pulmonary disease. Tube thoracotomywas the most effective procedure in achieving the expansion of collapsed lung. On the other hand, open thoracotomy could be a good approach to recurrent pneumothorax, persistent air leakage, incomplete expansion of the lung and bilateral pneumothorax. The minithoracotomy Is the best procedure to recurrent pneumothorax.

식도의 평활근종 -수술치험 1례 보고- (Leiomyoma of the Esophagus -A Case Report-)

  • Esophageal leiomyoma is a very rare disease. We present a patient who underwent enucleation of esophageal leiomyoma through a left thoracotomy. The patient was suffered from substernal pain and chest discomfort for 4 months.The esophagogram revealed irregular ovoid smooth filling defect in just proximal portion of G-E junction with the normal mucosal folds. Chest CT demonstrated well-defined, polypoid tumor mass on the anterolateral wall of the distal esophagus. Esophagoscopy revealed normal intact mucosal patterns with swollen hard protruded tumor mass lesion from the just proximal portion of G-E junction. In June, 1993, patient underwent enucleation of esophageal leiomyoma through the left thoracotomy. A horseshoe and spiral shaped, whitish firm tumor mass was noted on the distal esophagus, and the tumor mass was enucleated by blunt dissection carefully. The esophageal leiomyoma was confirmed with histopathological examination. The postoperative course was smooth and uneventful.
