Traumatic Aneurysm of Descending Thoracic Aorta -A Case Report- (외상성 하행 흉부 대동맥류 -치험 1 례 보고-)

  • We experienced a case of traumatic aneurysm of descending thoracic aorta by an automobile accident. The patient was 23-year-old-male with a traumatic aortic aneurysm [6x12cm] on the descending thoracic aorta just distal to the origin of the left subclavian artery. Exposure was obtained through a left posterolateral thoracotomy incision in the fourth intercostal space and then partial femoro-femoral cardiopulmonary bypass was established.After aortic cross- clamping, the aneurysmal sac was opened and repaired with interposition of Dacron vascular graft and aortic cross-clamping period lasted for 100 minutes. Postoperative bleeding and vocal cord paralysis were complicated, but bleeding was controlled by reoperation and vocal cord paralysis was improved. Follow up was continued for 14months and postoperative course was uneventful.

A Single Left Lung Transplantation in Dog -One Case Report- (황견에서 좌측 폐이식수술 -1례 보고-)

  • We performed post-operative hemodynamic evaluation and lung perfusion scan after left lung transplantation in dog. The heart & lungs were extracted from donor dog while the both lungs were flushed with 4oC Euro-Collin`s solution after heparinization & infusion of prostaglandin E1.In the recipient dog, anastomosis of the left atrial cuff was performed by continuous 4-0 Prolene everted suture, and bronchial anastomosis by telescope method with 4-0 Prolene continuous suture. The end-to-end anastomosis of left main pulmonary arteries was performed with continuous 6-0 Prolene suture. After closure of left thoracotomy incision, the lung perfusion scan was performed post operative 2 days for evaluation of the function of the transplanted lung which showed good perfusion. The dog was sacrificed in the post-operative 5 days for autopsy.

  • Bronchial artery aneurysm is a rare but life-threatening disease when it ruptures, and it requires prompt treatment to prevent from catastrophic ruptures. A 78-year-old woman was referred to our hospital to evaluate mediastinal massfound on chest computed tomography. We did a selective bronchial artery angiography and diagnosed as bronchial artery aneurysm. Aneurysmectomy with division of afferent and efferent arteries was performed through standard thoracotomy without cardiopulmonary bypass. The patient recovered uneventfully and was discharged on postoperative 8th day without any complications.

Glomus Tumor in Left Main Bronchus -A Case Report- (좌측 주기관기시부의 사구종양 1례)

  • Glomus tumors are uncommon lesions of glomus cell origin with structural and immunohistochemical features of smooth muscles. Primary glomus tumors of the lung and bronchus are extremely rare and only several cases have been reported. A 16-year-old woman was admitted for the complaint of productive cough, fever, and dyspnea. Imaging studies revealed a protruding mass in the left main bronchus and the mass was completely resected via a left thoracotomy incision. We report this case with literature review.

Clinical Analysis of Chest Trauma; Analysis of 247 patients (흉부 손상의 임상적 고찰)

  • Clinical analysis were performed on 247 cases of thoracic trauma, those were admitted & treated at the department of thoracic & cardiovascular surgery,Hanyang University Hospital during the period from Jan,1989 to June,1992. Age distribution of those was from 2 to 80 years old & mean age was 38 years old. The ratio of male to female patient was 186:61 [3:1].This ratio revealed high incidence in male patient. The most common cause of trauma was traffic accident in this series.The modes of injury were as follows: traffic accident 124 cases[50.2%],fall down 52 cases[21.05%], stab wound 47 cases[19.03%] and gun-shut wound 1 case.Ellapse time from accident to admission were 141 cases [57.09%] under 6 hr.Rib fracture were observed in 159 cases[64.37%], hemo or pneumothorax were observed 134 cases[54.25%] of total cases and location distributed Right:Left:Both[74:112:37], in left predominant. Conservative,non-operative treatment were performed in 128 cases and operation[open thoracotomy] 32 cases.Mortality was 1.6%[4 cases] & most common cause of death were due to irreversible shock with brain edema. Conclusively, more evaluation & co-operation of other department were expected treatment & better prognosis.

Localized Fibrous Tumor of Pleura; A report of a case (흉막에 발생한 국소성 섬유성 종양;1례 보고)

  • Localized fibrous tumor of pleura is submesothelial origin and related terms with localized mesothelioma, giant sarcoma of visceral pleura, post-inflammatory tumor of the pleura, pleural fibroma, submesothelial fibroma. This tumor is rare. We experienced a case of localized fibrous tumor.This 66 years old female was admitted with 2 years left persistant flank pain and mild dyspnea. Chest X-ray and CT scan showed a 12x10cm well-defined huge mass in the left subpulmonic area, and not metastatic lesion of any organs.Exploratory thoracotomy was done and a 14x10x8cm [650gm weight] sized mass was excised.The patient was discharged without any complications postoperatively.

  • Percutaneous closure of atrial septal defect (ASD) has become an increasingly common procedure. Serious complications of the procedure, such as cardiac migration, are rare, and usually occur <72 hours after device placement. In this report, we present the case of a patient who underwent successful surgical treatment for the migration of an ASD occluder device to the thoracic aorta 12 months after ASD closure.

Surgical Treatment of Bronchobiliary Fistula with Pulmonary Resection and Omentopexy

  • Bronchobiliary fistula is a rare disease defined as an abnormal connection between the biliary tract and the bronchial tree. We report the successful surgical repair of bronchobiliary fistula. A 78-year-old man underwent surgery and several rounds of transcatheter arterial chemoembolization and radiofrequency ablation as treatment for hepatocellular carcinoma. He presented with greenish sputum and chronic cough for several months, and his symptoms did not resolve after endoscopic treatment. We performed lobectomy of the right lower lobe and omentopexy for bronchobiliary fistula under laparotomy and thoracotomy. The bronchobiliary fistula was successfully closed, and the bilious sputum disappeared after surgery.

Persistent left cranial vena cava with congenital heart defect in two dogs

  • The purpose of this report is to introduce persistent left cranial vena cava (PLCVC) with persistent right aortic arch (PRAA) and patent ductus arteriosus (PDA). Case 1 was a Cocker Spaniel with PRAA and case 2 was a Maltese with PDA. PLCVC was enclosed at the sites of PRAA and PDA surgery; therefore, it was lifted dorsally during PDA and PRAA surgery. Surgery to repair congenital heart defects including PRAA and PDA is recommended for dogs that do not die of PLCVC at a young age.

Mediastinoscopy (종격동경 검사에 관한 연구: 56례 보고)

  • Fifty six consecutive cases of mediastinoscopy are analysed. The procedure was performed without any complication and mortality. It is concluded that the mediastinoscopy is a safe procedure and causes little disturbance to the patient. The value of mediastinoscopy as a diagnostic procedure for mediastinal and pulmonary hilar masses is shown. In my series, mediastinoscopy was performed in 17 cases, which could not be diagnosed by other means, for purpose of diagnostic procedure. A histologic diagnosis was obtained in 11 of the 17 patients [64.7%]. The findings are 6 cases of tuberculosis, 2 cases of metastatic carcinoma, 1 case of primary carcinoma, and one case of noncaseating granuloma. The histologic diagnosis of carcinoma in patients believed to have non-resectable tumors could be obtained in 4 of the 7 cases [57.1%]. Three cases out of 32 patients with presumably resectable bronchial carcinoma were found to have mediastinal lymph node metastases. Two patients of 29 cases refused to be operated on. The remaining 27 cases had thoracotomy and resection was undertaken in all but 9 cases [33.3%].
