Delayed Diaphragmatic Hernia after Left Nephrectomy -2 cases report- (좌신 절제후 발생한 지연성 횡격막 탈장 2례)

  • Since diaphragmatic injuries are difficult to diagnose, those that missed may present with latent symptoms.Delayed diaphragmatic hernia is very rare and occurs commonly after penetrating or blunt trauma. Recently we had experience with two cases of delayed diaphragmatic hernia after left nephrectomy.They were operated by herniolysis and closure of diaphragm using prosthetic patch[Dura, Dacron through the left thoracotomy. Postoperative courses were uneventful.

Surgical immobilization using Judet`d strut for flail chest with multiple rib fractures (다발성 늑골골절에 의한 동요흉에서 늑골고정판을 사용한 늑골고정술)

  • We experienced 2 cases of surgical immobilization with Judet`s strut for flail chest with multiple rib fractures. The patients were undertaken explorative thoracotomy for unevacuated hematoma and chest wall fixation for paradoxically moving segments of ribs after initial trials of internal fixation, i.e. mechanical ventilation with endotracheal intubation. Immediately after operation, the patient`s general condition and respiratory status were improved remarkably. Mechanical ventilation and endotracheal intubtion were removed on postoperative second day without any events.

Esophageal leiomyoma combined with achalasia; report of 1 case (아칼라지아와 동반된 식도 평활근종;수술 치험 1례 보고)

  • We experienced a case of esophageal leiomyoma combined with achalasia that is very rare. Patient had suffered from severe dysphagia and postprandial vomiting and diagnosis was accomplished by esophagography, esophagoscopy, chest CT, and esophageal motility test. The operative treatment was done through left lateral thoracotomy by enucleation of the submucosal tumor and esophagomyotomy. By histopathological findings, the diagnosis of leiomyoma was confirmed and LES biopsy revealed absence of the ganglion cells of myenteric and Auerbach`s plexus. Symptoms of the patient were completely relieved and postoperative course was uneventful.

  • Authors experienced a case of typical carcinoid in a patient of 46 year-old female who has been suffered from hemoptysis intermittently for 13 years. Exploratory thoracotomy and middle and lower lobectomy was carried out. The tumor was located in the right intermediate bronchus. Tissue pathology shows a glandular tumor with focal proliferation of Kulchitzky cells in the bronchial epithelium by H&E stain and membrane-bounded neurosecretory granules in the cytoplasm by electron microscope. Authors report the case with a literature reviews.

  • Posterior mediastinal goiter extending to carotid sheath posteriorly is rare. Recently we experienced two cases of posterior mediastinal goiter presenting dyspnea due to tracheal compression. The one was a 48-year-old female with mediastinal tumor shadow on chest roentgenogram . The other was a 54-year-old female with palpable mass on neck and huge mediastinal mass. These masses were resected completely through the right posterolateral thoracotomy and median sternotomy respectively. The postoperative courses were uneventful.

  • Air space disorders are usually considered medical diseases, although some patients with air space disorders can benefit from surgical intervention. Recently we experienced two cases of bullous emphysema. One case is large bilateral apical bullae and the other is infected large bulls of RUL with bullous emphysema. The patient with large bilateral apical bullae underwent simultaneous operation via bilateral thoracotomy and other patient underwent simultaneous bilateral operation via median sternotomy. Postoperatively, the patient with large bilateral apical bullae showed subjective as well as objective improvement and other patient is resulted subjective improvement.

Treatment of flail chest with Judet`s struts (Judet`s strut를 이용한 흉벽요동 치료;6례 보고)

  • Six patients with flail chest were performed operative stabilization with Judet`s Struts.The indications of opertive stabilization were exploratory thoracotomy or laparotomy in 4 patients, and severe chest pain due to displaced ribs which deteriorated respiratory pattern and gas exchange in 2 patients. After operation, all patients became comfortable and complained less pain.Two patients restored spontaneous breathing without ventilator therapy and 2 patients were ventilated during 4 days and 5 days, respectively.There were no morbidity and mortality related to operative stabilization.

Giant Leiomyoma of the Esophagus - A Case Report - (거대 식도평활근종 수술치험 1예)

  • A 42-year old male was seen with symptoms of dysphagia, chest pain and dyspnea of 9 months duration. A benign tumor of the posterior mediastinum was diagnosed from x-ray studies and a transthoracic needle aspiration biopsy which was inconclusive. A left thoracotomy revealed a huge mass occupying the retrocardiac space and the contra-lateral mediastinum. It was resected by blunt dissection and, during this process, a 3.0 cm laceration was created in the esophageal wall. This was repaired with Tevdec sutures and staplers and was reinforced with an intercostal muscle flap to prevent leakage. The postoperative course was entirely uneventful.

  • This is a report on the 4 cases of benign mediastinal teratoid tumor in the Department of Thoracic Surgery Chonnam University Hospital during the period from August, 1961 to August ,1972. All the tumors were teratomas which had three germinal layers and located in the anterior mediastinum. All the cases had symptoms such as Pancoast syndrome. exertional dyspnea, middle lobe syndrome with fistulous Connection to the cyst and retrosternal discomfort. X-ray studies are essential to recognize the tumor and its location. It`s believed that a exploratory thoracotomy is recommended because of the complications of the tumors and a possibility of malignancy.

  • Extramucosal esophagomyotomy by thoracotomy, first described by Heller in 1913 was the principal therapy for achalasia. Recently however, video-assisted thoracoscopic surgery has been implied in esophageal surgery, and we have experienced 23 year old male patient with dysphagia and was diagnosed as achalasia who underwent thoracoscopic esophagomyotomy. The esophagogram showed typical bird beak appearance and the lower esophageal sphincter pressure was increased to 35 mmHg. Thoracoscopic surgery was done twice due to incomplete myotomy in first operation and the patient was released of dysphagia in the second operation.
