흉선 지방종[1례 보고] (Thymolipoma - One Case Report -)

  • The thymolipoma is rather rare benign tumor of the thymus. One case of huge thymolipoma, seen in a 11-year-old boy, is presented. It is about 2.16kg. He had some chest discomfort. The chest film showed homogeneous haziness fills the left hemithorax, On chest CT scan, multiple small amorphous soft tissue densities were recognized as islands within a large fatty mass. Tumor resection was performed through left anterolateral thoracotomy. The mass was yellowish soft, measured 29x19Xllcm, 12X7.5x3.5cm, 7.0X3.0X1.0cm. Microscopically, the tumor was comprised of abundant mature adipose tissue and normal thymic tissue.

양측에 발생한 외상성 횡격막 파열;1례 보고 (Bilateral Traumatic Rupture of Diaphragm - 1 Case Report -)

  • Bilateral traumatic rupture of diaphragm is very rare. One case due to car accident is reported. Preoperative chest X-ray revealed the diaphragm rupture in the left side and the hemothorax in the right side. During the completion of left diaphragm repair through left thoracoabdominal incision, right diaphragm rupture was found incidentally. Left diaphragm was repaired using pledgets which were anchored at the thoracic wall. Right diaphragm was also repaired by interrupted Halsted sutures through seperated right thoracotomy. Postoperative course was uneventful.

  • Pneumomediastium in a newborn baby is a rare condition and is usually manifested by respiratory and circulatory distress syndrome. We recently have experienced a newborn baby of severe tension pneumomediastinum associated with bilateral pneumothorax. The patient in this report was a day old female and the mother of the baby a lot of difficulties during her delivery and the aid of vaccuum was necessary. This patient was received closed thoracotomy and followed by explothoracotomy and excision of tension multiple air bubbles. The post-op. course is not uneventful.

식도천공의 임상적 고찰 (Clinical evaluation of the esophageal perforation: 8 cases report)

  • 8 cases of the esophageal perforations were treated at the department of thoracic surgery, Chungnam National University Hospital during the period from July, 1980 to Dec., 1982. The causes of the perforation were various; swallowed a piece of glass, stocking pin, coiled wire, compressed air blow, strenuous vomiting, dog bite, tiller accident, and endoscopic procedure. The perforation sites were cervical esophagus in 3 cases, upper thoracic in 2 cases and lower thoracic in remains. We have performed following surgical procedure; Incision and drainage for cervical abscess, closed thoracostomy, thoracotomy and debridement, esophagoscopy and gastrostomy. Two cases were died. The causes of death were massive bleeding and sepsis.

폐과오종을 동반한 전종격동 흉선낭종 1례 보 (Pulmonary hamartoma associated with thymic cyst: a case report)

  • 33-year old female was admitted chest surgery department for evaluation of mild chest pain. Chest plain film showed right anterior mediastinal mass and small metastatic daughter mass ipsilateral side. Exploratory thoracotomy was performed 24th Aug. 83 revealed that small nodular parenchymal mass at right middle lobe and large cystic mass at anterior mediastinum which was connected with anterior mediastinal fat. Histological examination confirmed diagnosis as pulmonary hamartoma and thymic cyst individually. We successfully treated these two masses by wedge resection and excision. Although postoperative course was uneventful, the cause and associated relationship between two tumor origin were obscure.

식도 선낭포종양치험 1예 (Adenoid Cystic Carcinoma of the Esophagus - A case report -)

  • Adenoid cystic carcinoma of the esophagus has been relatively an uncommon, slow growing tumor. A 51 year-old man patient had a tumor in the lower third of the esophagus which was incidentally found during an examination for UPPER C-I series, and resected successfully without Thoracotomy. The tumor exhibited a polypoid appearance covered by normal esophageal epithelium, localized entirely in the submucosal layer of the esophagus and morphologically identical to adenoid cystic carcinoma in the salivary glands.

유아 거대 긴장성 기관지 낭종 -수술치험 1례 보고- (Large Tension Bronchogenic Cyst in an Infant -A Case Report-)

  • A routine chest radiograph in a 10 months old male infant revealed a giant air filled cystic lesion of the left hemithorax under tension. At thoracotomy, a large left lower lobe intraparenchymal cyst required lobectomy and the pathological finding were consistent with a bronchogenic cyst. This kind of parenchymal bronchogenic cyst is uncommon lesion, and we have performed successful surgical resection. After this lobectomy, this patient was complete recovered and postoperative course was uneventful.

  • The recommended approach for dumbell trmor is a one-stage removal by a team work of both neurosurgeons & thoracic surgeons. Recently we removed a case of dumbell tumor in a 69 years old woman with standard posterolateral thoracotomy and concomitant laminectomy as single stage operation. Postoperative course was uneventful and there were no postoperative neurologic signs.

  • Video thoracoscopic surgery is a new modality that gains acceptance rapidly from thoracic surgeons. We have experienced two left lower lobectomies, one left upper lobectomy & one right upper lobectomy with using video thoracoscopy for the four patients with lung parenchymal disease from July 1992 to February 1993. The post-operative courses were uneventful. The final pathologic diagnosis were sclerosing hemangioma, adenocarcinoma, bronchiectasis, leiomyoma & the post-operative courses were short. These patients needed less analgesics because postoperative pain was reduced markedly, and hospitalization was shortened due to rapid recovery. We would like to prefer video thoracoscopic lobectomy to the lobectomy through standard thoracotomy in uncomplicated patients with simple pulmonary parenchymal diseases.

종격동 결핵성 농양: 2례 보고 (Mediastinal Tuberculous Abscess - Report of two cases -)

  • Tuberculosis is a chronic disease caused by Mycobacterium tuberculosis, which usually affects the lung but may cause lesions in any organ or tissue of the human body. Mediastinal lymph node involvement is common feature of intrathoracic tuberculosis in children. Sometimes the lymph node may be enlarged and it causes compressive symptoms. Recently we experienced two cases of tuberculous abscess at middle mediastinum. The abscess seemed to be originated from the mediastinal lymphadenitis, and caused the symptoms. Operation was performed by median sternotomy and by posterolateral thoracotomy incision respectively for the purpose of relieving symptoms and diagnosing the mediastinal mass. The symptoms were relieved completely and postoperative course was uneventful.
