  • A coronary artery fistula was surgically ligated in a 38-year-old woman via a left anterior mini-thoracotomy without the use of cardiopulmonary bypass. In selected cases, this surgical approach can provide an excellent surgical exposure for coronary artery fistula ligation. It also offers an excellent cosmetic result and shorter hospital stay.

  • The review of 68 patients, who were diagnosed as spontaneous pneumothorax during the period from Dec, 1991. to Jul, 1992. were performed thoracoscopy of 70 cases under local anesthesia with 1% lidocaine at the department of thoracic & cardiovascular surgery, HanYang University Hospital. Clinical data on distribution of Age & Sex, Location, Frequency of Reccurrence and other aspects of pneumothorax were summerized.37 cases were treated by thoracoscopic management and closed thoracostomy. As thoracoscopic management, Electrocauterization of bullae or blebs[37 cases], Endo-clip application [2 cases], Removal of foreign body[1 case] were performed. 31 cases were cured by open thoracotomy. The thoracotomy indications under thoracoscopic finding were followed as: 1. Severe pulmonary adhesion and destroyed lung parenchyme 2. multiple bullae or blebs on several areas 3. finding of pulmonary tuberculous caseous lesion 4. persistant air leakage after 7 days from thoracoscopic management Excision, wedge resection of bullae or blebs was performed in most cases [22 cases], 2 cases by median sternotomy and Segmentectomy of 7 cases were carried out depending on the pathologic change of lung. There was no operative mortality and Follow-up for all patients were showed good results.

  • In this study, 186 cases of spontaneous pneumothorax experienced at the Dept. of thoracic and Cardiovascular surgery, chosun University Hospital during from Jan. 1978 to Mar. 1987 were analysed retrospectically. The results were as follows: 1. The sex ratio of the stadied patient was 2.5:! in male prominence and the incidence and the incidence was highest in the adolescence between 20-29 year of age. 2. The clinical symptoms were frequently Dyspnea (52.2%) and Chest pain (38.7%). The etiologic factors were as follows, Tuberculous origin in 55.9%, Unknown origin in 17.2%, Bullae in 11.8%. 3. In the method of treatment, closed thoracotomy was the most effective procedure in achieving the expansion of collapsed lung in 87.1%. Other methods were bed rest with high oxygen inhalation, thoracentesis and open thoracotomy. 4. The incidences of complication were developed in 12.4% and recurrent rates were seen in 9.7%.

  • Substernal goiter may be defined as any thyroid enlargement that has 50 to 100 % of its mass inferior to the thoracic inlet. Ectopic substernal goiters are rare, and most substernal gaiters arise from cervical thyroid gland. Fifteen to fifty percent of these patients are asymptomatic. Symptoms, when present, are usually the result of tracheal or esophageal compression. Symptoms are often positional. Standard chest roentgenograms are often diagnostic, but computed tomographic or radioactive iodine scans may be helpful. In symptomatic patients or those in whom explorations are undertaken for diagnostic purpose or exclude carcinoma, surgical removal is indicated. Although cervical thyroids with substernal extension may be safely and successfully removed through a cervical incision, primary substernal goiters by definition derive their blood supply from within the thorax and are better approached by splitting the sternum or through a posterolateral thoracotomy. Recently we experienced a child fist sized secondary posterior mediastinal goiter in 55-year old female. The mass was completely removed through right posterolateral thoracotomy without any complications. The postoperative courses were uneventful.

  • In this study, 26 cases of bilateral spontaneous pneumothorax experienced at the department of thoracic and cardiovascular surgery, Chungnam National University Hospital during from 1985 to August 1989 were analyzed retrospectively. The results were as follows; 1. The incidence of bilateral spontaneous pneumothorax was 13.5 % and sex ratio was 7.7:1 with male preponderance. 2. Among the nonsimultaneously occurring cases of bilateral pneumothorax, 12 patients [75 %] were developed at contralateral side within a year. 3. The most patients [65.4%] belonged to the age group between 15 and 25 year-old, and among 57 male patients suffered spontaneous pneumothorax in the same age group, 16 cases[28%] developed bilaterally. 4. The etiologic factors were as follows; blebs or bullae; 65.4 %, tuberculosis; 15.4 %, unknown; 19.2 %. 5. In the method of treatment, 15 patients were treated by closed tube thoracotomy and underwater-seal drainage only, 10 patients were treated by open thoracotomy. One patient died of respiratory failure due to severe destructed lung.

  • Traumatic injuries of the diaphragm are not an infrequent occurrence with rise in violence and increasing use of automobiles, more diaphragmatic injuries may be seen. Fifty cases from Severance hospital were reviewed of these there were 27 injuries secondary to blunt trauma and 23 penetrating injuries, and occurred commonly in male. In blunt trauma, right and left diaphragmatic injuries occurred equally. Chest X * ray were normal in 18 cases [36 %], a hemo-and/or pneumothorax was present in 22 cases [44 %], and only 12 cases[24 %] were diagnosed or suspected as diaphragmatic injuries preoperatively. Seventeen cases underwent thoracotomy alone, 19 cases required laparotomy only, and 14 had combined thoracotomy and laparotomy. There were 5 deaths [10 % mortality], and all deaths related to the severity of associated injuries. It was concluded that injuries of the diaphragm should be suspected in all patients with severe blunt torso trauma or penetrating injuries near the diaphragm.

  • For most surgeons, stomach and colon are the first choice for reconstruction of the esophagus, as well as for bypass. When the esophagogastric or esophagocolonic anastomosis is made in the neck, cervical anastomosis site leakage is the main complication. In our most recent four patients who underwent a transhiatal & posterior mediastinal esophagogastric or esophagocolonic anastomoses following esophageal resection, we performed the cervical anastomoses with a circular EEA stapler. No leaks have developed at the anastomosis site. In these four patients the cancer was tiny and was located on the upper or middle third of the thoracic esophagus. A total esophagectomy was performed by blunt resection without thoracotomy. Surgical staplers have been used previously for esophagogastric anastomosis through a right thoracotomy with a very low rate of leakage. When the esophagogastric or esophagocolonic anastomosis is performed in the neck, the prevalence of leakage does not increase the postoperative mortality, but it can increase significantly the duration of hospitalization and morbidity. The use of the circular stapler allowed us to perform four consecutive cervical esophagogastric & esophagocolonic anastomoses without any leakage and to shorten the operating time.

  • The author made a clinical study of IIO cases of empyema thoracis who were diagnosed and treated at department of chest surgery, chosun university hospital, during the period of December 1979 through June 1983. 1. In age and sex distribution, 45 cases [41%] was under the age of 15 years, 65 cases [59%] was above the age of 15 years. The ratio of male to female was 2.6:1. 2. The predisposing factors were pneumonia 45 cases [41%] and pulmonary tuberculosis 40 cases [36.5%]. 3. The cardinal symptoms were dyspnea, chest pain, fever, cough in order. 4. Etiologic organisms were confirmed in 69% which requested in 87 cases. Staphylococcal infection were 19 cases, Streptococcal infection were 13 cases, pneumococcal infection were 11 cases. 5. In treatment of empyema, thoracentesis 4 cases, closed thoracotomy 50 cases, open drainage 29 cases, decortication 14 cases and thoracoplasty 13 cases. In children, only thoracentesis and closed thoracotomy was favorable result in treatment. 6. 103 cases were discharged with recovery and improvement but 7 cases were early discharged by their economic or personal condition without improved.

  • To determine the role of mediastinoscopy in the preoperative staging of the primary lung cancer, we studied 23 patients from January 1993 to December 1993 and compared the results of mediastinoscopy and computed tomographic scanning with the findings at thoracotomy. Mediastinoscopy was performed in 14 patients when they had larger than 10mm mediastinal nodes at computed tomographic scanning. Six of them were found to have metastatic nodes. Nine patients, who had negative computed tomographic scanning and negative mediastinoscopic results, underwent thoracotomy. One of them was found to have a metastatic mediastinal lymph node. When results from both series of patients were compared, the sensitivity and specificity of computed tomographic scanning showed 83.3% and 47.1% respectively with an accuracy of 56.5%. The results of mediastinoscopy showed that the sensitivity was 85.7%, the specificity was 100% and the accuracy was 95.7%.Because of the low accuracy rate of computed tomographic scanning, a more routine use of mediastinoscopy seems to be justified.

  • Esophageal atresia(EA) with a double tracheoesophageal fistula(TEF) is rare. It accounts for only 0.7% of all cases of EA and TEF. A male newborn weighing 2860g was born by normal vaginal delivery at 41 weeks' gestation to a 27-year-old mother who had a normal pregnancy. But immediately developed recurrent choking and respiratory distress. Feeding tube was inserted and chest X ray showed the feeding tube coiled in the proximal pouch(T-2 level). With a preoperative diagnosis of EA and distal TEF, the thoracotomy was performed on the third day of life. At thoracotomy, TEF was proved to be a double fistula. Both fistulas were divided and an esophageal anastomosis was performed. The postoperative course was uncomplicated until the eighth postoperative day when a minor anastomotic leak. The patient was discharged at 103 days of age.
