Objective : Essential hyperhidrosis is a pathological condition of excessive sweating beyond that required to cool the body, though poorly understood, originating from a dysfunction of the sympathetic nervous system. Thoracoscopic sympathectomy is the most popular treatment for upper limb hyperhidrosis, because it is a safe, effective, minimally invasive, and time-saving method. However, the common complication is the compensatory hyperhidrosis in other areas of the body, notably on the back, chest, abdomen, and buttocks. Compensatory hyperhidrosis is severe enough for some people, especially those living in a warm climate or engaging in heavy physical activities, to regret ever having had operation. The pathophysiological mechanisms underlying compensatory hyperhidrosis are incompletely understood, even though it is thought to be a truly compensatory feature related to thermoregulation of the body. Materials and Methods : we studied the clinical features of total 233 patients who were diagnosed as essential hyperhidrosis and treated with thoracoscopic sympathectomy or sympathicotomy from March 1992 to July 2000. Results : The success rate of thoracoscopic sympathetic surgery(sympathectomy or sympathicotomy) was 98.7%. The global rate of compensatory hyperhidrosis was 77% ; 84% in group T2, 3 sympathectomy, 76% in group T2 sympathectomy, 43% in group T2, 3 sympathicotomy and 59% in group T2 sympathicotomy. The rate of embarrassing or disabling compensatory sweating was significantly higher in T2 sympathectomy and in T2, 3 sympathectomy than in T2 sympathicotomy and T2, 3 sympathicotomy with significancy in statistic analysis(p<0.01). The precipitating factors of compensatory hiperhidrosis, including heat(warm weather), anxiety, stress, and exertion were noted. The compensatory hyperhidrosis was the main cause of patient dissatisfaction after thoracoscopic sympathectomy. Conclusion : The degree of compensatory hyperhidrosis is closely related to the extent of thoracic sympathectomy.
일차성 다한증에 대한 흉강경을 이용한 흉부교감신경절제술은 효과적이면서도 기존의 방법에 비해 미용상의 장점이 있다고 알려져 있지만, 5 mm 또는 10 mm 기구를 이용한 흉강경 교감신경절제술은 트로카 부위의 통증과 상처의 문제를 여전히 갖고 있었다. 최근에 2 mm 흉강경 기구가 이용되기 시작하였는바, 서울대병원 흉부외과에서는 1997년 1월부터 4월까지 연속적으로 46명의 수장부 다한증 환자에서 2 mm 기구를 이용하여 양측성 교감신경절제술을 시행하였다. T2 신경절을 절제하였고 해부학적 위치가 불분명한 환자에서 T1 신경절의 하부 3분의 1을 함께 절제하였으며 액와부 발한도 호소한 4명의 환자들에서는 T3 신경절도 함께 절제하였다. 폐의 재팽창후 흉관삽입없이 트로카를 제거하였고 트로카 부위는 봉합없이 sterile tape 만 붙였다. 수술직후 전례에서 수장부 발한이 소실되었다. 수술수기에 관련된 합병증인 호너증후군, 혈흉, 상완신경총손상 등은 없었으며 아홉명(19.6%)에서 소량의 기흉이 있었으며 이 중 두 명에서는 needle aspiration이 필요하였다. 대부분의 환자에서 진통제가 필요없었으며 모든 환자가 수술당일에 퇴원하였다. 2 mm 흉강경 기구를 이용하여 심각한 합병증없이 다한증의 교감신경절제술을 안전하게 시술 가능하였으며, 미용상의 만족과 술 후 통증 감소의 결과를 얻었기에 2 mm 흉강경 기구가 기존의 5 mm나 10 mm 흉강경과 기구들에 비해 우월하다.
Thoracoscopic sympathectomy is a common technique used to treat plamar hyperhiodrosis. The complications of thoracoscopic sympathectomy are rare. Recently, we experienced a complex regional pain syndrome(CRPS) after thoracoscopic sympathecotomy in a patient with hyperhidrosis. The treatment of this complication was chemical epidural sympathetic block and conservative pain control. The result of this treatment was good. The patient was recovered after one month.
연구배경 : 안면부 다한증의 경우 주로 T1 교감 신경절에서 분포하는 것으로 알려져있어 안면다한증의 경우 T1 교감신경절의 절제가 치료의 원칙이었다. 그러나 T1 교감신경절을 절제하는 과정에서 성상신경절의 손상에 의한 Horner's 증후군의 유발가능성이 많기 때문에 수술방법이 어렵고 수술후의 일시적인 Horner's 증후군의 발생율이 높았다. 재료 및 방법 : 연세대학교 의과대학교 영동세브란스 병원 흉부외과에서는 1997년 3월부터 1998년 3월까지 안면부 다한증환자에서 T1 sympathectomy 한군(group I)과 T2 sympathicotomy 한군(group II)을 비교하였다. 결과 : 수술전 모든 환자에서 안면의 심한 발한증상이 있었으나 수술에 실패한 2례를 제외하고는 수술직후 전례에서 증상소실을 보였다. Group I과 II 의 수술에 대한 만족도, 결과 그리고 보상성다한증의 발생율에서의 유의 있는 차이는 없었으며 Group I 에 비하여 Group II에서 수술 시간이 짧았으며 수술후 합병증, 특히 Horner's 증후군에 대한 발생이 Group I에서 7례 인반면 Group II 에서는 전혀 없었다. 수술후 2개월에서 13개월까지의 추적조사에서 재발은 없었다. 결론 : 안면부 다한증 환자의 치료에서 T2 sympathicotomy는 수술의 결과와 만족도는 T1 symapthectomy 와 같으면서 수술시간의 단축과 Horner씨 증후군과 같은 심각한 합병증을 예방하는 간편하고 안전한 치료방법으로 생각된다.
Conventional thoracoscopic sympathectomy is an effective method in treating palmar-axillary hyperhidrosis. However, this may result in a postoperatively compensatory hyperhidrosis. Conservative treatments of compensatory hyperhidrosis consist of aluminum chloride, anticholinergics, iontrophoresis, and botulinum toxin A injections. Surgical treatments in compensatory hyperhidrosis include excision of axillary tissue, liposuction, and thoracoscopic sympathectomy. Intradermal injection of botulinum toxin A has used to treat focal axillary or palmar hyperhidrosis. Botulinum toxin A bestows significant benefits with few side-effects and is well-tolerated, with beneficial results lasting from 4-16 months. We report a case illustrating the beneficial use of botulinum toxin A in a 25-year-old healthy male patient with compensatory sweating of the flank after thoracoscopic sympathectomy. Modified Minor's starch iodine test was used to allow accurate assess the impact of hyperhidrosis on the patient. In conclusion, Botulinum toxin type A is a valuable therapy for compensatory sweating after endoscopic thoracic sympathectomy.
Background: Essential hyperhidrosis is a condition with excessive sweating, which may be localized in any parts of the body. Thoracic sympathectomy has been a surgical procedure for the management of hyperhidrosis. Methods: We studied 30 ASA I and II patients suffering from severe hyperhidrosis. Bilateral upper thoracoscopic sympathectomy of $T_{2-4}$ was performed in 30 patients under general anesthesia. Anesthesia was induced with 2.5% thiopental sodium 5 mg/kg and succinylcholine chloride 1 mg/kg and was maintained with enflurane 1~2 Vol% and $N_2O-O_2$ mixture adjusted to maintain $SpO_2$ greater than 96%. During anesthesia, invasive arterial pressure, heart rate, EKG, $SpO_2$ and capnography were monitored. Skin temperature was measured with thermister probes attached to the index finger of each hand. An increase in temperature after cautery confirmed success of the sympathectomy. Results: There were 14 men and 16 women whose ages ranged from 16 to 46 years old (mean age 22.2). Of these patients, 13 patients had complained of palm-sole hyperhidrosis, 9 of palm-sole-axilla hyperhidrosis, 4 of palm-sole-face hyperhidrosis and 4 of palm-sole-axilla-face hyperhidrosis. The provocative factors of excessive sweating were tension and stress from interpersonal relationships. There was positive familial history in 37%. The most common complication was compensatory hyperhidrosis in 23 patients comprising 76%. Other complication included peumothorax (4 patients), hemothorax (1 patient), ipsilateral Horner's syndrome (1 patient) and paresthesia of right arm (1 patient). The degree of satisfaction was graded as good, fair and poor with 15, 12 and 3 patients, respectively. Conclusions: Thoracoscopic sympathectomy with VATS is an efficient, safe and minimally invasive surgical procedure for essential hyperhidrosis.
Objectives : Essential hyperhidrosis is a common condition characterized by excessive body sweating. Excessive sweating beyond what is necessary to maintain normal body temperature need not be considered pathological unless it interferes with one's occupation and/or life-style. The existing non-operative therapeutic options seldom give sufficient relief or show a transient effect. In this regard, the thoracic sympathectomy may provide a definitive cure. In the past, surgical procedures were highly invasive and caused significant morbidity, but the minimally invasive thoracoscopic procedure provided detailed visualization of sympathetic ganglia and is associated with minimally postoperative morbidity. Nowadays, thoracoscopic transthoracic sympathectomy is accepted as the treatment of choice for essential hyperhidrosis. In palmar hyperhidrosis, however, the level of sympathetic chain to be blocked has been somewhat obscure. It is assumed that the incidence of compensatory hyperhidrosis may closely related to the extent of thoracic sympathectomy. Material & Methods : To compare the results of posterior midline approach with endoscopic sympathectomy, and the results of T2 with T2, 3 sympathectomy or sympathicotomy, we retrospectively studied 62 patients treated for palmar hyperhidrosis between September 1993 and May 2000. We reviewed medical records and recently interviewed the patients by telephone calls. Results : The treatment effect of T2 sympathectomy is no different from T2, 3 sympathectomy. But, the incidence of compensatory hyperhidrosis is less in the T2 sympathectomy group than the T2, 3 sympathectomy group. Conclusion : Thoracoscopic sympathectomy is considered a simple, safe, and effective method for treating palmar hyperhidrosis, with a shorter operation time, fewer hospital days, and a better cosmetic result, as compared with the open approaches. However, sympathicotomy seems to provide the advantages of a limited extent of denervation and the resultant decrease of compensatory hyperhidrosis compared to sympathectomy.
배경: 연세대학교 영동세브란스병원 흉부외과에서 1992년 다한증환자에서 흉곽내시경을 이용한 교감신경절제술을 시행한이래로 교감신경절제술은 원발성 다한증환자에서 간편하고 효과적인 치료방법이다. 수술직후의 만족도는 94∼98%로 보고되는 반면 6개월이 지난 장기 추적조사에서의 만족도는 약 66%로 보고되고 있고 만족도가 떨어지는 원인으로 보상성다한증에 의한 일상생활의 불편이 가장큰 요인이다. 대상 및 방법:연세대학교 영동세브란스 흉부외과에서 1992년부터 1998년 까지 783례의 다한증 수술환자중 T2 교감신경절단술 환자 89명(1군), T2 교감신경절제술 88명(2군), T2-4 교감신경절제술 138명(3군)을 대상으로 각각의 수술방법에 따라 보상상다한증의 발생 빈도와 정도을 비교하였다. 보상성다한증의 정도를 1, 없다(Absence); 2, 약간있다(Mild); 3, 눈으로 보일정도이나 생활에 지장이 없다(Embarrassing); 4, 생활에 불편할정도로 많으며 하루에 2-3번내의를 갈아입는다(Disabling)로 구분하였다. 결과: 모든 환자에서 증상이 소실되었으며 전체적인 보상성 다한증의 발생율은 1군이 64%, 2군이 73.8%, 3군이 87.8%였으며 Embarrassing 과 Disabling 의 경우 1군 15.7%, 2군 32.2%, 3군 58.0% 로 수술방법에 따른 보상성 다한증의 발생과 정도에 차이가 있었다(p<0.05) 결론: 이와 같은 결과로 보상성 다한증의 발생과 정도는 교감신경절제의 범위와 밀접한 연관이 있으며 보상성 다한증을 줄이고 장기 추적조사에서의 만족도를 증가 시키기 위해서는 T2 교감신경절제술 보다는 T2 교감신경절단술이 종 더 유용할 것으로 생각됩니다.
연세대학교 의과대학 영동세브란스병원에서는 1989년 3월부터 1997년 9월까지 외래와 전화추적이 가능하였던 654명(남자 354명과 여자 300명)의 다한증 환자를 대상으로 흉부교감신경절제술을 시행하였다. 다한증에 대한 흉강 내시경을 이용한 수술기법은 기존의 개흉술보다 선명한 수술시야를 확보할 수 있는 장점이 있다. 단순기관 삽관을 이용한 마취 유도기법으로 마취시간의 단축과 수술시간의 단축, 그리고 입원기간의 단축이 가능하게 되었다. 시간절약뿐만 아니라 수술기법이 쉽고 미용효과를 최대로 살릴수 있는 2 mm주사침 흉강경을 이용한 흉부 교감신경 절제술은 최근 다한증에 가장 확실한 치료법이 되었다. 수술기법의 변화는 수술의 결과를 향상시켰으며, 수술 후 만족도를 95%이상으로 향상시켰다.
손과 발에 정상 이상의 많은 땀이 나는 것은 정신적으로나 직업적으로 난처하고 때로는 무력한 상태를 만든다. 다한증은 자율신경계의 흔한 질병중 하나이다. 다한증은 특별한 치료법이 없었으며 따라서 액와 또는 경부 쪽에서 접근하는 절제술이 있었다. 최근에는 흉강경을 이용한 흉부수술(VATS)이 수장부와 액와부의 다한증에 가장 널리 사용되고 있다. 경희의료원에서는 1996년 3월부터 1997년 3월까지 양쪽 수장부에 다한증이 있는 15명의 환자를 양측 흉부 교감신경 절제술 (T2, T3, T4)을 시행하였고, 수술 전후로 컴퓨터 적외선 전신 체열 측정 (DITI)을 하였다. 이들 모두 개흉술로의 전환은 없었다. 3건의 술후 합병증으로 폐부종 1건, Horner씨 증후군 1건, 미각 다한증 1례가 있었다. 반 이상의 환\ulcorner에서 하복부, 둔부,배부, 대퇴부에 보상성 다한증이 나타났다. 결론적으로, 대부분의 환자는 수장부와 액와부에 땀이 나지 않고, 통증이 적고, 미용상 더 낫고, 족저부와 안면부에도 땀이 줄어 흉강경 수술 후의 결과에 대해 만족하였다. 또한 수술중에 수장부 온도를 측정함으로써 흉강경 교감신경 절제술의 성공 여부를 평가할 수 있었으며, 컴퓨터 적외선 체열 측정을 수술 전후로 시행함으로써 이 방법이 흉강경 교감신경 절제술의 성공적인 결과를 평가하는 객관적인 방법이 될 수 있었다고 사려된다.
