  • A 45-year-old man presented with lower back pain and pain in the right leg of 3years duration. A plain radiographic examination revealed grade I isthmic spondylolisthesis, with instability at L4-5. Computed tomography and magnetic resonance imaging demonstrated bilateral foraminal stenosis, with soft foraminal disc herniation on the right side at the L4-5 level. He underwent anterior lumbar interbody fusion[ALIF] with percutaneous posterior fixation[PF] at the L4-5 level. Without removing the posterior bony structures, removal of foraminal disc herniation and reduction of spondylolisthesis were successfully performed using ALIF with percutaneous PF. When there is no hard disc herniation or lateral recess stenosis, ALIF with percutaneous PF can be one of the treatment options for isthmic spondylolisthesis, even in the presence of foraminal disc herniation, as in our case.

  • We report an interesting case in which overt aortic dissection mimicked two episodes of aortic intramural hematoma (IMH) (Stanford A, DeBakey I). This took place over the course of four days and had a major influence on the surgical treatment strategy. The first episode of IMH regressed completely within 15 hours after it was clinically diagnosed and verified using imaging techniques. The recurrence of IMH was detected three days thereafter, resulting in an urgent surgical intervention. Overt aortic dissection with evidence of an intimal tear was diagnosed intraoperatively.

  • The purpose of this study was to compare the body alignment during standing on level and wedge board. Twenty healthy college students (8 females, 12 males) were evaluated in this study. Diagnostic contourline potographic imaging system (Model JTC-1, Jodang Trading Co.) was used to measure body alignment. Sagittal and frontal plane images were used to analyze the body alignment. The result showed that the cervical and lumbar lordotic curve significantly decreased during standing on wedge board when compared with standing on level. On the other hand, thoracic kyphosis significantly increased during standing on wedge board. There was no significant difference in body alignment according to gender, weight, and height. Clinically, patients with low back pain and severe lordosis may be affected by heel wedge. Further study is needed to identify whether the standing on wedge board can change the body alignment in patients with low back pain and spinal deformity.

  • Cleidocranial dysplasia is a rare, autosomal dominant congenital disorder. A 12-year old female visited with chief complaint of unerupted permanent teeth. Also her father showed severe class III malocclusion. The extraoral radiography and computed tomography showed delayed closure of the cranial sutures and underdevelopment of maxilla, maxillary sinuses, and frontal sinus. Both clavicles were underdeveloped and thoracic rib cage was bellshaped. Both zygomatic process appeared as hypoplastic feature. There were many unerupted permanent and supernumerary teeth in the maxilla and mandible. We examined location and number of the unerupted teeth using 3D CT. Finally we could conclude this case was cleidocranial dysplasia based on the clinico-radiologic findings.

  • Mechanical valve thrombosis is a serious and potential lethal complication unless early diagnosis & prompt therapy are made. We have been experienced 3 cases of valve thrombosis. From Aug. 1988 to July 1994, reoperations of mitral valve replacement [MVR] with mechanical prostheses [all mitral] were performed in three patients[2 men, 1 woman] due to valve thromboses. All three patients were diagnosed by means of cineradiography. Preoperative status of was shock status and he was applied intra-aortic balloon pump [IABP]. All three cases of prosthetic valve failure [PVF] were treated by Redo-MVR. Time intervals of reoperations were 5months, 40months, and 35months, respectively. In all cases, valve thromboses were excised successfully. Cineradiography provided an accurate diagnosis in all cases, which was utilized as safe, reliable & noninvasive imaging modalities. There were no operative death & complication. All three patients were fully recovered and returned to their employements, and active lives.

  • Neurogenic tumors of brachial plexus are rare lesions. Recently 1 experienced a case of Schwannoma arising from the brachial plexus. Thirtyfour-year-old man presented with a slow-growing mass on the left supraclavicular area. Magnetic resonance imaging revealed a well demarcated solid mass on posterosuperior aspect of the left subclavian artery. During operation, a well-encapsulated mass was seen beneath the brachial plexus. 1 performed intracapsular enucleation of the tumor from the none in an effort to avoid damaging none fibers as much as possible. Post-operative neurological deficit was not found.

  • Glomus tumors are uncommon lesions of glomus cell origin with structural and immunohistochemical features of smooth muscles. Primary glomus tumors of the lung and bronchus are extremely rare and only several cases have been reported. A 16-year-old woman was admitted for the complaint of productive cough, fever, and dyspnea. Imaging studies revealed a protruding mass in the left main bronchus and the mass was completely resected via a left thoracotomy incision. We report this case with literature review.

  • From July 1966 to July 1983, sixteen atrial myxomas in fifteen patients were seen at Severance Hospital. Fifteen of the sixteen myxomas were located in the left atrium and one in the right atrium. All the cases except three were correctly diagnosed preoperatively. Presently, M-mode and two-dimensional echocardiography are utilized as safe, reliable, and noninvasive imaging modalities. Echocardiography provided an accurate diagnosis in twelve cases since November 1977. In all cases, myxoma were excised successfully. On patient had reoperation and mitral valve replacement on postoperative first day due to persist mitral regurgitation after excision of left atrial myxoma. One patient had recurrence requiring reoperation 37 months after primary operation. Follow up results of each patient were excellent.

  • Lymphangioma(or cystic hygroma) of the chest wall is rare case. We experienced one such case in a 16-year-old girl who complained of a large recurrent mass on her right upper post erolateral chest wall which had developed several years ago. The diagnosis was made following a physical examination, chest magnetic resonance imaging(MRI), and radio isotope (RI) lymphangiogrphy and was confirmed by a histopathological examination. We performed total excision of the lesion followed by a repeated sclerosing therapy with intralesional injection of Vibramycin.

  • From August 1990 to October 1991, we performed operation on 5 patients with cardiac myxoma, which was located in the left atrium, There was 1 male and 4 female patients ranging in age 45 years to 61 years. Chief complaints included exertional dyspnea, palpitation, cough, orthopnea, general weakness. M-mode and two dimensional echocardiography provided an accurate diagnosis in all cases which was utilized as safe, reliable and noninvasive imaging modalities. In all cases, myxoma was excised successfully. Follow up is current. No recurrent myxoma has been identified clinically or by echocardiography. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum.
