Utilization Behaviour and Influencing Factors of Complementary and Alternative Medicine Therapies Among the Elderly (일부 노인의 보완대체요법 이용행태와 영향요인)

  • Objectives: The aim of this study is to analyze the utilization behaviour and influencing factors of complementary and alternative medicine (CAM) therapies among elderly people with chronic diseases. Methods: The study population was selected among 9 welfare facilities for the aged in 2 cities among Daegu Metropolitan City & Gyeongsangnam Province by two-stage cluster sampling. 250 senior citizens participated in a face-to-face interview using a structured questionnaire. Logistic regression analysis was performed to examine the association between the utilization of CAM therapies and various characteristics of the elderly. All statistics were analyzed using the PASW (ver 18.0). Results: Among 233 elderly peoples, 70.4% (164 persons) had used various kinds of CAM therapies (including nutritional methods, pharmacologic and biologic treatments, etc.) more than once during the last year. 48.8% (64 persons) ~ 60.7% (88 persons) of the elderly used CAM therapies without health and medical experts' counsel. The elderly with perceptions of CAM therapy used it 2 times more than those without knowledge of CAM therapies. The number of chronic diseases was more likely to increase the usage of nutritional methods (OR=2.92, 95% CI: 1.07-7.97), manipulative and body-based practices (OR=5.85, 95% CI: 1.97-17.34), pharmacologic and biologic treatments (OR=2.92, 95% CI: 1.49-8.17). Elderly with diabetes used nutritional methods 3.76 (95% CI: 1.49-9.47) more than elderly without diabetes. Conclusions: CAM therapies use in the aged with chronic diseases appears common. The findings suggest that the clinical efficacy and safety of CAM therapies on medical management of chronic disease may be investigated and that patient-physician communication need to be strengthened.

Research Trend of Acupuncture Therapy on Migraine - Focus on Pubmed Searches from 2012 to 2017 (편두통의 침 치료에 대한 임상 연구 동향 분석 - 2012년도 이후 Pubmed 검색 중심으로)

  • Objective: To compare recent domestic and overseas research trends about acupuncture therapies on migraines. Method: Case reports and randomized controlled trials were searched via Pubmed. Searches were limited to publications between 2012 and 2017. Results: Four case reports and twelve randomized controlled trials were searched from Pubmed. Research showed that acupuncture therapies are more significantly effective than other therapies, such as sham acupunctures and Western medicines. Conclusion: There has been more active overseas research about acupuncture therapies on migraine compared to domestic studies. To examine the efficacy and suitability of acupuncture therapies on Koreans with migraines, domestic studies and research should be conducted more actively.

  • Obesity is a serious medical disorder because it can cause a myriad of health problems, such as heart disease, hypertension, and adult-onset diabetes. While conventional medical treatment for obesity has limitations, there is enormous public enthusiasm for complementary and alternative treatments of obesity. This article reviews currently commonly used complementary and alternative therapies for obesity, such as medicinal herbs, nutritional supplements and acupuncture. Beneficial effects and possible adverse effects associated with these treatment are discussed.

  • 1) Objective ${\lceil}Wai-Tai-Mi-Yao{\rfloor}$ had been made by Wang-Dao(王燾) in Tang Dynasty(唐朝). It included fumigation therapy and aroma therapy. Therefore we would like to bring out use sphere and detailed method of Fumigation therapy and Aroma therapy in Tang Dynasty and before period. 2) Conclusions (1) Fumigation therapies of ${\lceil}Wai-Tai-Mi-Yao{\rfloor}$ have contained boil and burn. The effects of fumigation therapy are made by cooperation effect of medicine and heat. (2) Aroma therapies of ${\lceil}Wai-Tai-Mi-Yao{\rfloor}$ have been used to cure infectious, internal, psychologic, dental, pediatric, dermatologic and surgical diseases. Especially these therapies have a good effect on cough. (4) Aroma therapies of ${\lceil}Wai-Tai-Mi-Yao{\rfloor}$ have been used to cure infectious, internal, psychologic, dental, ophthalmic, otolaryngologic, obstetrics, gynecologic, dermatologic and surgical diseases. Expecially this therapy has an good effect on nightmare.

Engineering Cell Therapies for Autoimmune Diseases: From Preclinical to Clinical Proof of Concept

  • Autoimmune diseases are caused by a dysfunction of the acquired immune system. In a subset of autoimmune diseases, B cells escaping immune tolerance present autoantigen and produce cytokines and/or autoantibodies, resulting in systemic or organ-specific autoimmunity. Therefore, B cell depletion with monoclonal Abs targeting B cell lineage markers is standard care therapy for several B cell-mediated autoimmune disorders. In the last 5 years, genetically-engineered cellular immunotherapies targeting B cells have shown superior efficacy and long-term remission of B cell malignancies compared to historical clinical outcomes using B cell depletion with monoclonal Ab therapies. This has raised interest in understanding whether similar durable remission could be achieved with use of genetically-engineered cell therapies for autoimmunity. This review will focus on current human clinical trials using engineered cell therapies for B cell-associated autoimmune diseases.

Utilization Pattern of Complementary Therapy in Hypertension, Diabetes and Chronic Arthritis Patients Visited to Local Health Center (일개 보건소를 방문하는 고혈압, 당뇨 및 관절염환자의 보완요법 이용실태)

  • Objectives: The objective of this study is to investigate the use rate and some aspect of complementary therapies used by patients with chronic illness(hypertension, diabetic mellitus and chronic arthritis). Methods: 600 patients visiting the health center for one month(Jan. 2001) were interviewed on their complementary therapies used by the subjects for the previous year. Results: About fourteen-eight percent of the respondents used therapies; 35% of patients with hypertension, 44.6% of patients with diabetic mellitus and 62.9% of patients with chronic arthritis, which shows the highest rate among patients with three chronic disease. The use rate of complementary therapies indicates few meaningful differences according to the general characteristics of the interviewees. Hypertension patients used herb medication(31.0%) acupuncture(29.6%) and most of all the other therapies. Diabetic patients used dietary therapy(57.5%) and herb medication(35.1%). Chronic arthritis patients used acupuncture(85%) and herb medication(34.7%). 36.8% of all the patients who used complementary therapies tried more than two therapies. 18.3% of hypertension patients, 24.1% of diabetic patients and 55.9% of chronic arthritis patients used more than two therapies. Acupuncture(47%) was used most frequently, followed by herb medications(26.3%), health assistance utensils(21.8%). oriental therapy(21.8%), physical therapy(9.5%), health assistance food(8.4%), herb(7.7%), Korea hand acupuncture(3.2%), abdomen respiration(1.1%), and pore therapy(0.7%) Oriental clinic was visited most frequently(42.8%), which was used to cure diseases(61.8%), and to relieve symptoms(26.0%). (p<0.001) The cost spent on complementary therapies last year was 90,000 won(40.3%) and there are some cases of more than 500,000 won(31.2%). Most of the patients(56.1%) were satisfied with the complementary therapies, with 6% of them having side effects. 74% of the patients used complementary therapies answered that they would continue them and 56.1% of them also answered that they would continue them and 56.1% of them also answered that they would advise other patients to do them. Advantages(compared with those of orthodox medical treatment) are psychological comfort(28.1%), body protection(26.0%), effectiveness(20.0%). 34% of the patients using complementary therapies wanted to have informational orientation on complementary therapies. These findings reveal that a considerable number of patients with chronic illness(47.5%) tried a variety of complementary therapies. Though 6% of the patients using therapies had side effects, most of the subjects seemed satisfied with them and they are supposed to continue them. Conclusions: In conclusion, health center personnels and medical doctors should pay more attention to the complementary therapies used by patients with chronic illness. They also have to try their best to advise more scientific and informative complementary programs with less side effects and more help to improve their conditions.

Trends and Characteristics of Patients Who Have Been Received the Physical Therapy of Korean Medicine at an University Hospital: A Retrospective Medical Chart Review (일개 대학병원 내 한방물리요법을 받은 환자들의 후향적 의무기록 분석)

  • Objectives The aim of this study was to offer the fundamental data for the physical therapies of Korean medicine through analyse the database of one university hospital. Methods As this study was retrospective analysis, following items were selected and analysed in the electronic medical record (EMR) database. (1) sex, (2) inpatient or outpatient, (3) medical department, (4) diagnosis, (5) kind of insurance. Results Although all kind of physical therapies were used, interferential current therapy (ICT) was the most used physical therapy. And department of rehabilitation medicine of Korean medicine prescribed physical therapies most among the 8 specialty departments. As physical therapies were used in various kinds of diseases, they were especially used in musculoskeletal diseases and nervous system diseases. Conclusions The analysis of actual condition of using physical therapies in a real clinical setting of Korean medicine could be a useful fundamental data for the application of modernized physical therapies.

  • Backgrounds : Since December 1, 2009, National Health Insurance began to reimburse for three physical therapies of Korean medicine. The extension of insurance coverage is demanded by Korean medicine societies. Objectives : This study aimed to measure the resource-based relative value scale (RBRVS) scores for thirty four physical therapies of Korean medicine and to estimate the insurer's expenditure for them in National Health Insurance. Methods : To measure the physician's work and the practice expense, the 'magnitude estimation method (MEM)' and the 'fully distributed cost - attributable cost method (FDC-ACM)' were applied respectively. We collected the frequency data of physical therapies from Korean medicine hospitals and clinics to estimate the total expenditure. Results : The resource-based relative value scale scores of physical therapies were measured from 23.44 to 160.66. Total insurer's expenditure was calculated to be 95.5 billion won as of 2009. Conclusions : Based on the result that showed minor increase of total expenditure, most physical therapies of Korean medicine need to be reimbursed in the National Health Insurance.

The Perception on Alternative Therapy of Adults in Some Areas (일부 지역 성인의 대체요법에 대한 인식도)

  • This study is carried in the goal to investigate the individual awareness rate about alternative therapy among the unspecified persons and their experience of using it, and to promote the diffusion of alternative therapy. The questionnaire research for this study has been conducted among 300 unspecified persons who live in some areas. The following is figured out in this study: 1. To the question of knowledge grade about alternative therapies, 44.4% of respondents answer they 'know a little.' 2. Regarding the experience of using alternative therapies, 22.6% of respondents answer 'Yes.' This rate is very low, the reason of it can be perceived that the names of categories in alternative therapies has not been showed to them and they don't have pre-knowledge about it. 3. The 166 persons of 248 reply that they use alternative therapies. This rate indicate their recognition of categories of alternative therapies, and the gap of the question of experience and using nowadays is interpreted to result from the gap of the pre-knowledge. 4. Regarding the field of the alternative therapies that the respondents want to learn, the first field the male respondents choose is acupuncture(41 persons, Res in needle, bloodletting, etc.), and the female respondents choose acupuncture firstly(20 persons). Therefore the learning courses of alternative therapies are more effective when the preference of men and women is considered. Consequently, both the research about the given kinds of alternative therapies with demonstration of their medical effect and the systematic promotion and education about alternative therapy is considered to be needed.
