NOTES (Natural orifice transluminal endoscopic surgery) is a new technique in minimally invasive procedure, surgeon use normal orifice and hollow viscus for procedurs as approaching route, Currently the NOTES are performed for diagnostic and therapeutic purpose by some investigators in research setting. Functional gastrointestinal disorders are among the most common medical problems in children. Abdominal pain-related functional gastrointestinal disorders can be categorized as functional dyspepsia, irritable bowel syndrome, abdominal migraine and childhood functional abdominal pain according to the Rome III criteria for pediatric functional gastrointestinal disorders. The aim of this paper was to examine the evidence supporting the use of the range of therapeutic options available for functional gastrointestinal disorders.

Having an understanding of the properties of cytokines is essential for the immunologist, the researcher and the medical practitioner who need to understand immunologic diseases and immunological therapeutic approaches. Cytokines are redundant in their actions on target cells and promiscuous in their receptor reactions. (ED note: That is some cool use of English!) Moreover, many cells concomitantly produce several cytokines that have overlapping actions. Here this review provides conceptual framework to understand the intriguing aspects of the cytokine system.

Light therapy (also called light treatment or phototherapy) involves scheduled exposure to bright artificial light. Evidence-based treatments for sleep disorders especially for circadian rhythm sleep disorders include light therapy and pharmacotherapy. In clinical practice, many of patients with sleep problems tend to impair circadian rhythmicity. Considering that light is the most potent entraining agent of circadian rhythm, careful use of light therapy can be recommended for patients with several kinds of sleep disorders. I briefly review the possible therapeutic mechanisms and clinical applications of light therapy, focusing on circadian sleep disorders.

PegIntron is the pegylated form of human recombinant interferon alfa-2b (IFN${\alpha}$2b). IFN ${\alpha}$2b, known as Intron A, has been in approved clinical use since the 1980's for various cancer indications, and for the treatment of Hepatitis C. In the mid 1990's, several clinical investigators reported that combination therapy with ribavirin and Intron A dramatically increased the therapeutic efficacy for treatment of Hepatitis C.(omitted)

한국에너지연구소 원자력병원 싸이클로트론에 의해 생산되는 중성자를 임상에 적용시키기 위해, 이의 물리적 특성을 알기 위하여 방사선선량 측정실험을 시행하였다. 여기서 얻은 결과를 외국의 다른 치료기관에서 얻은 데이타와 비교 분석하였다. 중심축 선상의 심부선량백분율, build-up곡선, open과 쐐기등선량 곡선의 값이 4MV와 6MV X-ray값의 중간에 위치하였다. 최대선량의 build-up은 피부아래 1.35cm에 위치했으며 입사 선량은 약 $40\%$였다. 출력인자는 $6\times6cm$의 조사야에서 0.894, $30\times30cm$의 조사야에서는 1.187이었다. 중성자선의 X-ray오염도는 $10\times10cm$ 조사야에서 심부 2cm에서 $4.9\%$였다.

Background: Mizoribine (MZR) is an imidazole nucleoside isolated from Eupenicillium brefeldianum. MZR is currendy in clinical use for patients who have undergone renal transplantation. Therapeutic efficacy of MZR has also been demonstrated in rheumatoid arthritis and lupus nephritis. MZR has been shown to inhibit the proliferation or lymphocytes by interfering with inosine monophosphate dehydrogenase. Since the exact mechanism by which MZR benefits rheumatoid arthritis (RA) is not clear, we investigated the ability of MZR to direct its immunosuppressive influences on other antigen presenting cells, such as macrophages. Methods: Mouse macrophage RAW264.7 cells were stimulated with lipopolysaccharide in the presence of MZR. To elucidate the mechanism of the therapeutic efficacy in chronic inflammatory diseases, we examined the effects of MZR on the production of pro-inflammatory cytokines, nitric oxide (NO) and prostaglandin $E_2\;(PGE_2)$ in macrophages. Results: MZR dose-dependendy decreased the production of nitric oxide and pro- inflammatory cytokines such as tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), interleukins $1{\beta}$ (IL-${\beta}$ and IL-6 $PGE_2$. Examination of gene expression levels showed that the anti-inflammatory effect correlated with the down-regulation of inducible nitiric oxide synthase expression, cycloxygenase-2 expression and TNF-${\alpha}$ gene expression. Conclusion: In this work, we resulted whether MZR $(1.25{\sim}10{\mu}g/ml)$ inhibited macrophage activation by inhibiting secretion of pro-inflammatory cytokines, NO and $PGE_2$. These findings provide an explanation for the therapeutic efficacy of MZR in chronic inflammation-associated diseases.

Objectives The aim of this study was to develop a new Korean psychotherapeutic model in terms of emotion.Methods We compared the five minds mutual restriction therapy with short-term dynamic psychotherapy (STDP). We summarized 4 similarities and 4 differences of these two models of psychotherapy.Results 1. Short-term dynamic psychotherapy (STDP) needs active intervention of the therapist, with a restricted therapeutic focus and session. It blends with the present state of oriental neuropsychiatric clinics, and is suitable for Korean medical psychiatrists to apply short-term dynamic psychotherapy (STDP) to their clinical settings. 2. Short-term dynamic psychotherapy (STDP) and five minds mutual restriction therapy are similar, in that they both mainly use the emotions, needing restricted therapeutic focus and sessions, and active intervention of the therapist. 3. Short-term dynamic psychotherapy (STDP) and five minds mutual restriction therapy are different in their methods of arousing therapeutic emotion, the necessity of the patient being unconscious, the standard of patient selection, and the importance of follow up.Conclusions It is necessary to develop a new Korean psychotherapeutic model, which amalgamates the advantages of both methods of psychotherapy, and considers the present state of clinic. It is necessary that the Korean medical psychiatrists introspect themselves, since they are not merely therapists but also a role model for the patient.

유방암의 근치적 또는 수술후 방사선 조사시 치료해야 할 부위는 유방과 주위임파 조직으로 매우 복잡한 3차인적 구조이다. 여러 방향의 조사에 의한 조사부위의 중복을 피하고 부작용을 최소화하기위한 여러 방법이 시도돼 왔으나 계산과정이나 치료방법이 매우 복잡하여 여러가지 문제점이 있었다. 이에 저자들은 Asymmetric jaw를 이용하고 gantry, collimator와 couch를 적절히 회전시켜 조사야가 만나는 부분에서 beam의 divergence를 없애줌으로써 가장 이상적으로 치료할 수 있는 방법을 소개하고자 한다. 5문 조사 방법은 다음과 같은 장점을 가진다. 1. 모든 조사야의 set up이 같은 위치에서 시행되므로 환자의 위치변동에 의한 target volume의 변화를 막을수 있다. 2. Half beam block을 사용하지 않아도 된다. 3. 조사야가 만나는 선(match line)에서 beam의 divergence가 없으므로 정확하다. 4. 계산이 쉽고 환자 set up이 용이하다. 5. 매일 치료를 같은 방법으로 정화하게 반복할 수 있다.
