국소적으로 절제불가능한 췌장암의 치료 (Treatment of Locally Unresectable Carcinoma of the Pancreas)

  • 1981년 1월부터 1985년 12월까지 서울대학교병원 치료방사선과에서 운2명의 극소적으로 절제 불가능한 췌장암 환자를 방사선과 악물요법으로 치료하였다. 방사선은 2주의 간격을 두고 2000cGy씩 총 4000cGy를 조사하였으며 5-FU (5-fluorouracil)는 방사선 조사의 각 course의 첫 3일에 걸쳐 주입하였다. 방사선치료 종료 4주 후부터 FAM (5-FU, Adriamycin, Mitomycin)을 지속요법으로 사용하였다. 통증 완화는 $22\%(4/18)$에서 완전관해를, $39\%(7/18)$에서 부분관해를 보였다. 생존기간의 중앙치는 31주였으며 치료 전 Performance status가 중요한 예후인자였다. 국소적으로 절제불가능한 췌장암의 치료에 있어서 방사선과 FAM regimen의 병용은 통증완화 및 생존기간의 연장에 기여할 수 있는 바 이에 대한 보다 많은 연구와 고찰이 없어야 차겠다.

신장이식환자와 혈액투석환자의 삶의 질 비교 (Comparison of Quality of Life between Kidney Transplant and Hemodialysis Patients)

  • Purpose: The purpose of this study was to discover kidney transplant and hemodialysis patients' quality of life(QOL) and provide basic data to improve their QOL. Method: One hundred two hemodialysis patients and 106 kidney transplant patients were given a self-administered questionnaire from Mar. 6 to Mar 31, 2006. The instrument consisted of demographic variables, therapeutic-related characteristics and QOL. Collected data was processed using the SPSS 12.0 statistical program for real numbers, percentages, ANCOVA, t-test, ANOVA, Stepwise multiple regression and the Scheffe test. Results: Kidney transplantpatients' QOL was higher than hemodialysis patients. The demographic variables which showed a significant difference in overall QOL were religion, children and monthly income for kidney transplantpatients and educational background for hemodialysis patients. The therapeutic-related variable which showed a significant difference in overall QOL was the patients' perceived health condition for both groups. The influencing factor on overall QOL for both groups was the health condition perceived by themselves. The total variance of the variable for QOL was 42% for kidney transplant patients and 19% for hemodialysis patients. Conclusion: This study revealed that kidney transplant patients have a higher QOL and how patients perceive their health is the strongest influencing factor for QOL. However, there is a large difference between the demand and supply of kidney donors. To solve this problem the standards for donation should be reviewed and revised.

Sulfasalazine attenuates tamoxifen-induced toxicity in human retinal pigment epithelial cells

  • Tamoxifen, a nonsteroidal estrogen receptor (ER) antagonist, is used routinely as a chemotherapeutic agent for ER-positive breast cancer. However, it is also causes side effects, including retinotoxicity. The retinal pigment epithelium (RPE) has been recognized as the primary target of tamoxifen-induced retinotoxicity. The RPE plays an essential physiological role in the normal functioning of the retina. Nonetheless, potential therapeutic agents to prevent tamoxifen-induced retinotoxicity in breast cancer patients have not been investigated. Here, we evaluated the action mechanisms of sulfasalazine against tamoxifen-induced RPE cell death. Tamoxifen induced reactive oxygen species (ROS)-mediated autophagic cell death and caspase-1-mediated pyroptosis in RPE cells. However, sulfasalazine reduced tamoxifen-induced total ROS and ROS-mediated autophagic RPE cell death. Also, mRNA levels of tamoxifen-induced pyroptosis-related genes, IL-1β, NLRP3, and procaspase-1, also decreased in the presence of sulfasalazine in RPE cells. Additionally, the mRNA levels of tamoxifen-induced AMD-related genes, such as complement factor I (CFI), complement factor H (CFH), apolipoprotein E (APOE), apolipoprotein J (APOJ), toll-like receptor 2 (TLR2) and toll-like receptor 4 (TLR4), were downregulated in RPE cells. Together, these data provide novel insight into the therapeutic effects of sulfasalazine against tamoxifen-induced RPE cell death.

Long Term Therapeutic Plan for Patients with Non-Small Cell Lung Cancer Harboring EGFR Mutation

  • Non-small cell lung cancer harboring epidermal growth factor receptor (EGFR) sensitizing mutations has a distinct disease entity. Patients with this cancer have better prognosis, and frequently achieve long-term survival. EGFR-tyrosine kinase inhibitor (TKI) is the drug of choice for this cancer; but the disease inevitably progresses, after durable response. The tumor is a mixture of EGFR-TKI sensitive clones and resistant clones, regardless of their molecular mechanisms. EGFR-TKI sensitive clones are very susceptible to this drug, but rarely eradicated; so, withdrawal of the drug permits rapid regrowth of drug sensitive clones, possibly causing "disease flare." Re-administration or continuation of EGFR-TKI can effectively suppress the expansion of drug sensitive clones, even when the total tumor volume continuously increases. Chemotherapy can definitely prolong the survival of patients experiencing EGFR-TKI failure. Prospective clinical trials are warranted to compare efficacies of chemotherapeutic agents. A few retrospective studies suggested that a taxanebased regimen may be superior to others. Here, we reviewed therapeutic options and clinical evidence about this unique disease entity.

A comprehensive review of the therapeutic effects of Hericium erinaceus in neurodegenerative disease

  • Mushrooms are considered not only as food but also for source of physiologically beneficial medicines. The culinary-medicinal mushrooms may important role in the prevention of age-associated neurological dysfunctions, including Alzheimer's and Parkinson's diseases. Hericium erinaceus (H. erinaceus), is edible mushrooms, is a parasitic fungus that grows hanging off of logs and trees and well established candidate for brain and nerve health. H. erinaceus contains high amounts of antioxidants, beta-glucan, polysaccharides and a potent catalyst for brain tissue regeneration and helps to improve memory and cognitive functions. Its fruiting bodies and the fungal mycelia exhibit various pharmacological activities, including the enhancement of the immune system, antitumor, hypoglycemic and anti-aging properties. H. erinaceus stimulates the synthesis of Nerve Growth Factor (NGF) which is the primary protein nutrient responsible for enhancing and repairing neurological disorders. Especially hericenones and erinacines isolated from its fruitin body stimulate NGF, synthesis. This fungus is also utilized to regulate blood levels of glucose, triglycerides and cholesterol. H. erinaceus can be considered as useful therapeutic agents in the management and/or treatment of neurodegeneration diseases. However, this review focuses on in vitro, in vivo and clinical trials for neurodegerative disease.

Synergistic Anti-tumor Effect of KLF4 and Curcumin in Human Gastric Carcinoma Cell Line

  • Kr$\ddot{u}$ppel-like factor 4 is a transcription factor which plays an important role in development and progression of various carcinomas. Curcumin characterized by excellent anti-cancer properties is regarded as a serviceable natural compound used in carcinoma therapy. This study aimed at exploring the impact of KLF4 overexpression in cooperation with curcumin on the proliferation, apoptosis and invasion of human gastric carcinoma BGC-823 cells. Flow cytometry analysis, CCK-8 assays, transwell assays and Western blot results showed that KLF4 overexpression combined with curcumin had significant anti-proliferation, pro-apoptosis and anti-invasion effects on BGC-823 cells. We also found that KLF4 had synergistic effects with curcumin, better promoting apoptosis and inhibiting proliferation and invasion of gastric carcinona cells. These results indicate that KLF4 could be used as a potential therapeutic target; curcumin could act as an auxiliary and provide a promising therapeutic strategy in stomach cancer.

Analysis of Biological Experiments on the Anti-inflammatory and Antipyretic Effects of Hwangryeonhaedok-tang

  • Objectives: To establish scientific and objective evidence for the use of a Korean medicine, articles regarding Hwangryeonhaedok-tang (HRHDT), a herbal medicine frequently used in Korean medical clinics and hospitals, were gathered and analyzed. Methods: The articles were classified as being from domestic or international journals, and by their year of publication. The mechanisms of the anti-inflammatory and antipyretic effects of HRHDT were investigated. Results: Of the 25 articles analyzed, 7 were published from Korea, 7 were from China, and 11 were from Japan. HRHDT showed anti-inflammatory and antipyretic effects through the regulation of the expression of Th1 cytokines including interleukin-2 (IL-2), IL-8, interferon-${\gamma}$ (IFN-${\gamma}$), and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$); and Th2 cytokines including IL-4, IL-6, and IL-12, which inhibit leukotriene B4 (LTB4), cyclooxygenase-2 (COX-2), prostaglandin E2 (PGE2), and inflammatory cells. It also lowered preprodynorphin (PPD), and corticotropin-releasing factor (CRF) in the peripheral nerve system and hypothalamus. Conclusions: We speculate that the anti-inflammatory and antipyretic effects could be related to the therapeutic efficacy of HRHDT in removing pathogenic fire and heat.

Ginsenoside Rg2 inhibits osteoclastogenesis by downregulating the NFATc1, c-Fos, and MAPK pathways

  • Ginsenosides, among the most active components of ginseng, exhibit several therapeutic effects against cancer, diabetes, and other metabolic diseases. However, the molecular mechanism underlying the anti-osteoporotic activity of ginsenoside Rg2, a major ginsenoside, has not been clearly elucidated. This study aimed to determine the effects of ginsenoside Rg2 on receptor activator of nuclear factor-κB ligand (RANKL)-induced osteoclast formation. Results indicate that ginsenoside Rg2 inhibits RANKL-induced osteoclast differentiation of bone marrow macrophages (BMMs) without cytotoxicity. Pretreatment with ginsenoside Rg2 significantly reduced the RANKL-induced gene expression of c-fos and nuclear factor of activated T-cells (Nfatc1), as well as osteoclast-specific markers tartrate-resistant acid phosphatase (TRAP, Acp5) and osteoclast-associated receptor (Oscar). Moreover, RANKL-induced phosphorylation of mitogen-activated protein kinases (MAPKs) was decreased by ginsenoside Rg2 in BMM. Therefore, we suggest that ginsenoside Rg2 suppresses RANKL-induced osteoclast differentiation through the regulation of MAPK signaling-mediated osteoclast markers and could be developed as a therapeutic drug for the prevention and treatment of osteoporosis.

홍화자약침(紅花子藥鍼)의 항염증능이 백서의 LPS로 유발된 류마티스성 관절염에 미치는 영향 (Anti-inflammatory Effect of Hong Hua Za Pharmacupuncture on Rheumatoid Arthritis that Caused by Lipopolysaccharide in Mouse)

  • Objectives : This study is to evaluate Effect of Hong Hua Za acupuncture anti-inflammatory on rheumatoid arthritis that caused by Lipopolysaccharide in mouse. Methods : Hong Hua Za(Carthami Tinctorii Fructus) acupuncture(HA) was prepared and tested for therapeutic potential of rheumatoid arthritis, by measuring the inhibition of inducible nitric oxide synthase (iNOS) expression, nitric oxide(NO) production in vitro experiment, by measuring Tumpr Necrosis Factor-$\alpha$(TNF-$\alpha$) & Nuclear Factor kappa B(NF-$\kappa$B) p65 activation, synovial hyperplasia, angiogenesis and fibrosis in synovial membrane of knee joint of mice. Results : HA showed therapeutic effects on Lipopolysaccharide(LPS)-induced rheumatoid arthritis. HA inhibited iNOS mRNA and NO in RAW264.7cell of LPS-induced rheumatoid arthritis in a dose dependent manner. HA also showed significant inhibition of TNF-$\alpha$ & NF-$\kappa$B p65, activation, synovial hyperplasia, angiogenesis and fibrosis in synovial membrane of knee joint of mice. Conclusions : These results suggest that HA has an therapeutic effects on LPS induced-rheumatoid arthritis by inhibiting TNF-$\alpha$ activation.

방사선에 의한 EL 4 백서 백혈병 세포 및 정상 백서 비장 임파구 DNA Single-Strand Breaks의 정량적 분석과 측정 (Quantitative Analysis of DNA Single-strand Breaks in EL 4 cells and Mouse Spleen Lymphocytes after Irradiation)

  • Filter elution 방법으로 EL 4백서 백혈병 세포 및 C57BL/6백서 유래의 비장 임파구에 대한 Co-60 $\gamma$ 선의 DNA single-strand breaks (SSB) 효과를 정량적으로 측정하였다. 임파구는 [${^3}H$]thymidine을 표지하기 위하여lipopolysaccharide (LPS, 20 $\mug/ml$)를 첨가하여 자극하고 부유 상태의 EL 4 세포 및 임파구를 $0^{\circ}C$에서 0 Gy, 1 Gy, 5 Gy, 10 Gy 또는 15 Gy조사하였으며, elution용액의 pH는 12.1로 하였다. $\gamma$ 선 조사에 따른 single-strand breaks의 수는 방사선 조사량에 따라 증가 하였으며 21 ml elution 양을 기준으로 한 strand scission factor (SSF)는 EL 4 세포에서 $0.01301\pm0.00096\;Gy^{-1}(n=5)$이었고, 임파구는 $0.01097\pm0.00091\;Gy^{-1}(n=5)$를 나타내므로 본 실험에서는 EL 4 세포가 정상 임파구에 비하여 방사선에 의한 DNA SSB가 민감함을 알 수 있었다(p<0.005). 본 연구 결과 DNA strand breaks의 측정법을 이용하여 방사선의 특성 및 생물학적 효과의 파악은 물론 나아가 기존의 방호제 및 새로운 약제의 DNA에 대한 효과를 판별할 수 있을 것이다.
