A verification on the physical effectiveness of therapeutic horseback riding exercise: Focused on the EMG analysis

  • Various studies related to therapeutic horseback riding have been reported to be positive for the therapeutic effect of patients with cerebral palsy; however, most of the previous studies focused on to muscle development with training period related to the physical effects of therapeutic horseback riding. To identify the causes and phenomena of muscular activation of the body through actual therapeutic horseback riding exercise and to promote the excellence of physical effects of therapeutic horseback riding. This study was a nonrandomized prospective positive-controlled trial design. Twelve teenaged males with cerebral palsy were selected who had experienced riding exercise for 8-12 months. This study measured 8 muscle activities of the pectoralis major muscle (PM), biceps brachii (BB), rectus abdominis muscle (RA), latissimus dorsi muscle (LD), spinal erector muscle (SE), rectus femoris muscle (RF), anterior tibial muscle (AT), and external gastrocnemius muscle (EG) by using electromyography (EMG). Muscle activity was significantly higher in horse riding position than sitting on the common chair in all muscles (PM, BB, RA, LD, SE, RF, AT, and EG). The activity of the body muscles according to the difference of horse walking method (walk: WA; sitting trot: ST; and riding trot: RT) of therapeutic horse riding showed the highest muscle activity in the PM muscle at ST, and the highest activity at BB, RA, LD, SE, and AT muscles at ST and RT, and showed the highest muscle activity in RF and EG muscle at RT. The results of this study suggest that intervention for the treatment of cerebral palsy patients can use therapeutic riding exercise as a rehabilitation method.

고지혈증 환자에서 사상체질 치료식의 임상적 효능 (The Clinical Effect of Sasang Constitutional Diets for the Hupercholestrolemic Patients)

  • In Sasang constitutional medicine, a part of oriental medicine, there are beneficial foods of harmful foods according to the each constitutions. Until now, most of the studies have investigated the classification of foods according to the each constitutions, The clinical usage of the constitutional diets in now in the beginning. The purpose of this study is to investigate the therapeutic effects of the constitutional diets in comparison with general therapeutic diets in the patients with hyperlipidemia. From January to August, 1999, the 65 hyperlipidemic patients admitted to Kyung Hee Oriental Medical Center were studied. Therapeutic diet for hyperlipidemic patients or each constitutional diets were given to the subjects 3 meals a day for 6 weeks. The Sasang constitutional classification, food habits were assessed. The anthropometric assessment and blood analysis were carried out before and after taking each experimental diets. The results are as folow ; 1) The mean age of the objects was 62.4${\pm}$6.5 years, the distribution of their constitution were Tae-eumin :63.0%, So-yangin :26.2% So-eumin: 10.8%. 2) The mean body weight and BMI of Tae-eum were significantly higher than those of So-eum and So-yang. 3) Food habits of each constitutional groups were very similar to those described by sasang medicine. 4) The therapeutic and constitutional diets lowered the serum levels of total lipid(from 756.4mg/dl to 692.3mg/dl) triglyceride(from 244.4mg/dl to 212.mg/dl) and VLDL-cholesterol(from 48.9mg/dl to 42.5mg/dl). The therapeutic diet decreased the HDL-cholesterol level(from 49.0mg/dl to 41.7mg/dl) but the constitutional diet did not. 5) The effects of the therapeutic and constitutional diets were the highest in Tae-eum group. In the So-yang group, the constitutional diet lowered the levels of total lipid and LDL-cholesterol but the therapeutic diet did not. But the therapeutic and constitutional diets did not change the blood lipid levels significantly in the So-eum group.(Korean J Nutrition 33(8) : 824-832, 2000)

Injectable hydrogels delivering therapeutic agents for disease treatment and tissue engineering

  • Background: Injectable hydrogels have been extensively researched for the use as scaffolds or as carriers of therapeutic agents such as drugs, cells, proteins, and bioactive molecules in the treatment of diseases and cancers and the repair and regeneration of tissues. It is because they have the injectability with minimal invasiveness and usability for irregularly shaped sites, in addition to typical advantages of conventional hydrogels such as biocompatibility, permeability to oxygen and nutrient, properties similar to the characteristics of the native extracellular matrix, and porous structure allowing therapeutic agents to be loaded. Main body: In this article, recent studies of injectable hydrogel systems applicable for therapeutic agent delivery, disease/cancer therapy, and tissue engineering have reviewed in terms of the various factors physically and chemically contributing to sol-gel transition via which gels have been formed. The various factors are as follows: several different non-covalent interactions resulting in physical crosslinking (the electrostatic interactions (e.g., the ionic and hydrogen bonds), hydrophobic interactions, ${\pi}$-interactions, and van der Waals forces), in-situ chemical reactions inducing chemical crosslinking (the Diels Alder click reactions, Michael reactions, Schiff base reactions, or enzyme-or photo-mediated reactions), and external stimuli (temperatures, pHs, lights, electric/magnetic fields, ultrasounds, or biomolecular species (e.g., enzyme)). Finally, their applications with accompanying therapeutic agents and notable properties used were reviewed as well. Conclusion: Injectable hydrogels, of which network morphology and properties could be tuned, have shown to control the load and release of therapeutic agents, consequently producing significant therapeutic efficacy. Accordingly, they are believed to be successful and promising biomaterials as scaffolds and carriers of therapeutic agents for disease and cancer therapy and tissue engineering.

뇌졸중 환자의 상지기능, 일상생활활동, 치료적 몰입이 삶의 질에 미치는 영향 (Influence of Upper extremity function, Activities of Daily Living, Therapeutic Flow and Quality of Life in Stroke patients)

  • 본 연구의 목적은 뇌졸중 환자의 상지기능, 일상생활활동, 치료적 몰입과 삶의 질 간의 상관관계를 알아보고, 삶의 질에 영향을 미치는 요인을 파악하기 위함이다. 총 13개 병원에서 입원중인 뇌졸중 환자 249명을 대상으로 상지기능, 일상생활활동, 치료적 몰입과 삶의 질을 치료사와 연구자가 평가하였다. 그 결과, 뇌졸중 환자의 상지기능(r=.312, p<.001), 일상생활활동(r=.605, p<.001), 치료적 몰입(r=.525, p<.001)이 양의 상관관계를 나타내었다. 그리고 뇌졸중 환자의 삶의 질에 영향을 미치는 요인을 분석한 결과, 치료적 몰입(${\beta}=.344$, p<.001), 일상생활활동(${\beta}=.293$, p<.001), 발병기간(${\beta}=.145$, p<.05)은 뇌졸중 환자의 삶의 질에 유의한 영향을 미치는 변수로 확인되었으며, 이들 요인은 삶의 질을 35.9% 설명하는 것으로 나타났다(F=35.736, p<.001). 이를 통하여 치료적 몰입은 삶의 질에 영향을 미치는 중요한 요인임을 확인할 수 있었으며, 향후 치료적 몰입에 대한 계속적인 연구가 요구된다.

재활승마에 대한 재활전문인력의 인식도 조사 (A Survey on the Recognition of Rehabilitation Specialists on Therapeutic Horse Riding)

  • 장애인을 위한 치료의 수단으로서 승마에 대한 관심이 높아지고 있는데 승마는 근육, 관절, 심폐 기능 등의 신체 조건을 향상시키고, 긴장을 완화시키며, 말과 같은 큰 동물을 통해 자신감을 키워주는 운동이다. 다양한 재활승마의 치료효과는 미국의 작업치료 및 물리치료협회 등을 포함한 많은 의학 전문가들에 의해 인정받고 있다. 반면 우리나라는 구체적인 질환별 치료적 효과나 임상적인 치료에 쓰기에 과학적 토대에 관한 연구는 아직 미비한 실정이다. 본 연구는 재활과 관련된 전문인력들의 재활승마에 대한 인식도를 조사함으로써 임상으로서의 가능성을 알아보고자 하였다. 재활승마에 대한 인지도와 사용경험은 전반적으로 낮았으나 효과성의 기대감이나 재활승마의 교육이 가정될 경우 활용하고자 하는 의사는 비교적 높았다. 이러한 결과를 토대로 향후 재활승마가 임상적 치료방법으로 인정될 가능성을 확인할 수 있다.

혈액투석 환자의 치료지시 이행정도와 생리적 지표와의 관계 (A Study of the Relationship between Compliance with Therapeutic Regimens and Physiological Parameters of Hemodialysis Patients)

  • Purpose: This study was done to investigate correlations between compliance and physiological parameters of hemodialysis patients. Method: The subjects were 102 patients on hemodialysis at 3 hospitals in B city. Data was collected using Shon(l986)'s questionnaire and measuring physiological parameters (serum urea nitrogen, creatinine, hemoglobin, albumin, potassium, phosphorus, interdialytic weight gain). Result: Mean scores of compliance with the therapeutic regimen was $4.00\pm$ 0.55 on a 5 point scale. The area of visiting hospitals and taking medicines . were shown to have high compliance with therapeutic regimens; on the other hand, the areas concerning diet and symptoms were shown to be low. Interdialytic weight gain and phosphorus were significantly related to the compliance with therapeutic regimens. Conclusion: Hemodialysis patients' therapeutic compliance was related to the physiological parameters(potassium, phosphorus, interdialytic weight gain). Therefore, these findings give hemodialysis patients useful information for raising their therapeutic compliance.

Comprehensive Identification of Tumor-associated Antigens via Isolation of Human Monoclonal Antibodies that may be Therapeutic

  • Although the success of trastuzumab and rituximab for treatment of breast cancer and non-Hodgkins lymphoma, respectively, suggests that monoclonal antibodies(mAbs) will become important therapeutic agents against a wider range of cancers, useful therapeutic Abs are not yet available for the majority of the human cancers because of our lack of knowledge of which antigens (Ags) are likely to become useful targets. We established a procedure for comprehensive identification of such Ags through the extensive isolation of human mAbs that may be therapeutic. Using the phage-display Ab library we isolated a large number of human mAbs that bind to the surface of tumor cells. They were individually screened by immunostaining, and clones that preferentially and strongly stained the malignant cells were chosen. The Ags recognized by those clones were isolated by immunoprecipitation and identified by mass spectrometry(MS). We isolated 2,114 mAbs with unique sequences and identified 25 distinct Ags highly expressed on several carcinomas. Of those 2,114 mAbs 434 bound to specifically to one of the 25 Ags. I am going to discuss how we could select proper target Ags for therapeutic Abs and candidate clones are therapeutic agents.

의약용 당단백질에 부가된 당사슬의 중요성 (Impacts of glycans attached to therapeutic glycoproteins)

  • High value-added therapeutic proteins have been leading the biologics industry and occupied major portion of the market. More than 60% of the currently available protein therapeutics are glycoproteins attached with glycans which play crucial roles for the protein folding, therapeutic efficacy, in vivo half-life and immunogenecity. This review introduces the process of glycosylation and the impacts of glycans in the aspects of therapeutics. The important glycan structures in therapeutic performances were also summarized focusing on three representative categories of glycoproteins, cytokines, therapeutic antibody and enzyme. Currently, mammalian expression systems such as Chinese hamster ovary cells are preferred for the production of therapeutic glycoproteins due to their ability to synthesize glycans having similar structures with human type glycans. However, recent advances of plant glycoengineering to overcome the limitation originating from different glycan structures will soon allow to develop more efficient and economic plant-based production systems for therapeutic glycoproteins.

Glyco-engineering strategies for the development of therapeutic enzymes with improved efficacy for the treatment of lysosomal storage diseases

  • Lysosomal storage diseases (LSDs) are a group of inherent diseases characterized by massive accumulation of undigested compounds in lysosomes, which is caused by genetic defects resulting in the deficiency of a lysosomal hydrolase. Currently, enzyme replacement therapy has been successfully used for treatment of 7 LSDs with 10 approved therapeutic enzymes whereas new approaches such as pharmacological chaperones and gene therapy still await evaluation in clinical trials. While therapeutic enzymes for Gaucher disease have N-glycans with terminal mannose residues for targeting to macrophages, the others require N-glycans containing mannose-6-phosphates that are recognized by mannose-6-phosphate receptors on the plasma membrane for cellular uptake and targeting to lysosomes. Due to the fact that efficient lysosomal delivery of therapeutic enzymes is essential for the clearance of accumulated compounds, the suitable glycan structure and its high content are key factors for efficient therapeutic efficacy. Therefore, glycan remodeling strategies to improve lysosomal targeting and tissue distribution have been highlighted. This review describes the glycan structures that are important for lysosomal targeting and provides information on recent glyco-engineering technologies for the development of therapeutic enzymes with improved efficacy. 저항운동치료 처방에 관한 연구 (A Study of Resistive Therapeutic Exercise Prescription)

  • Resistive therapeutic exercise prescription goal is to improve functional performance and capabilities through the development of increased muscular strengh endurance or power. Resistance can be applied to either dynamic or static muscle contractions. Resistive therapeutic exorcise can be carried nut concentrically, eccentrically, isometrically, isokinetically. Neurodevelopmental treatment has not resistive therapeutic exercise concept. But proprioceptive neuromuscular facilitate techniques have resistive therapeutic exercise concept with pattens and techniques. It is aid muscle contraction, motor control and increase strength. Manual muscle testing will help the therapist establish a qualitative and quantitative baseline level of strength. Manual resistance maybe applied a against controlled lengthening contraction re static contraction of a muscle. A repetition maximum is not easy to calculate and is not the most accurate method available today to measure strength before of after a resistive therapeutic exercise program. Plyometric training emphasize the development of muscular power and coordination. Quick bursts of force in functional movement patterns are often necessary of a patient is to return to high-demand occupational, recreational or sports related activities.
