Factors to consider for surgical in elderly patients with oral cancer

  • At present, in Korea, due to developments in medicine and technology and an increasing mean lifespan, the expected lifetime for elderly people is increasing (at 70 years, the expected additional lifespan is 14.8 years for males and 18.3 years for females). However, among all causes of death, malignant neoplasm is ranked first for all ages. Further, the incidence rate of oral cancer tends to increase with age. Thus, oral and maxillofacial surgeons have increased opportunity to experience the 'oldest old' patients with oral cancer. Elderly patients commonly have several comorbidities, poor general condition, limited socioeconomic support, fear of various postoperative complications, and perception of short for the rest of their life. In this situation, the patient, caregiver, and surgeon often choose undergraded treatment rather than standard treatment for oral squamous cell carcinoma owing to patient age. In elderly patients with oral cancer, ablation of tumor or reconstructive surgery is challenging for surgeons. Oral and maxillofacial surgeons must evaluate carefully the patient's medical condition and make a decision regarding treatment plans after sufficient discussion with patient and caregivers. We review the literature to consider the factors involved for deciding on a treatment plan regarding surgery in elderly patients with oral cancer.

  • This study was undertaken to evaluate the living condition and the reality of visiting part-time workers through the survey. On the matter of living condition, the most difficult problem is financial problem followed by residential problem. On the matter of female's household labor, there was a considerable difference according to the houseband's age and academic background, and family monthly income. On the question of family relationship, it was found that there was rare communication between children and father because father was rigid to talk with. In the case of mother and children, they could not find time to talk with each other. In the group of the old aged, they work to make their living. On the other hand, in the group of younger generations and the higher educated, they replied that they should work whatever the work is. It was found that they were subjected to their work because of their financial situation and that they were under the double yoke of family and work because there were no legal protection and organizational devices.

유치원 유아의 모-자간 애착유형분류와 애착에 대한 유아기질의 영향 (Classifications of Attachment to Mother at Age 5 and 6 and the Relationship between Temperament and Attachment)

  • The purpose of the present study was to classify the attachment to mothers of kindergarten children based on a system for classifying attachment organization developed by Main and Cassidy(1988) and to examine the relationship of children's temperament with attachment. 32 children and their mothers were observed and videotaped in the strange situation. Mothers were administered the children's temperament questionnaire revised by Chun (1992). The data were analysed by t-test. The results showed similarity between the attachment classifications of Main and Cassidy (1988) and those of Korean children. These results can be considered partial validation of Main and Cassidy's classification system for the measurement of the attachment of 5-to 6-year-old Korean children. Children's temperament has significant correlation to the security of attachment.

대중음악이 뇌파에 미치는 영향 (Effects of Auditory Stimuli Using Pop Music on EEG)

  • The purpose of this study is to analyze the effects of the auditory stimulation using pop music on EEG. The subjects in the study were eight healthy university students; five men and three women, age group of 22-28 years old. EEGs were measured for one minute at 19 channels, according to the international 10-20 system method, after both pop musics which the subject likes and not were provided with 100% and 50% volume of standard value, respectively, and the ratio of $\beta$/$\alpha$ was obtained. As a result, when the subjects heard their favorite music, the values of $\beta$/$\alpha$ ratio at the Right-Parietal.Temporal.Occipital lob(T6, O2) showed to be lower than stable state in the situation with 50% volume of standard value.

타인양육 영아의 어머니에 대한 애착 (The Effects of Infant Child Care on Infant-mother Attachment)

  • This study investigated whether infant child care experience, alone or in combination with mother/child factors, is associated with attachment security. Participants were forty 12-to 18-month-old infants and their mothers. Infants were observed in the Strange Situation to assess the pattern of infant-mother attachment; the Observational Ratings of the Caregiving Environment was used to assess the caregiving environment. Mothers were interviewed with the questionnaires and observed in the laboratory "living room". There were significant main effects of maternal sensitivity and responsiveness and of stability of care on attachment security and on insecure/avoidance. Significant interaction effects revealed that infants were more likely to be secure when high maternal sensitivity/responsiveness was combined with good quality child care, non-maternal care initiated prior to six months of age, or care by close relatives.

타인양육 영아의 정서조절: 격리 및 타인양육 특성과 어머니 및 영아 특성의 영향 (Emotion Regulation Among Nonmaternal Care Infants: The Effect of Separation/Nonmaternal Care Experiences and Child/Mother Variables)

  • The purpose of this investigation was to examine whether early child-care experience, alone or in combination with mother/child factors, was associated with emotion regulation. Participants were forty 12-18 month-old infants, their mothers and their caregivers. Infants were observed in the Strange Situation to assess the pattern of emotion regulation. Mothers were interviewed, given Questionnaires, and observed in play. Caregivers were observed in child care to assess the caregiving environment. There were significant main effects of good quality child care on open emotion expression and heightening emotion expression. Significant interaction effects revealed that infants were more likely to be open emotion expression when high maternal sensitivity /responsiveness was combined with good quality child care, or nonmaterial care initiated prior to six month of age. Infants were more likely to be minimizing emotion expression when low maternal sensitivity/responsiveness was combined with nonmaternal care initiated after six month of age, or more than one care arrangement. Difficult temperament infants received nonrelative care were somewhat more likely to be minimizing emotion expression. Infants were more Likely to be heightening emotion expression when high maternal sensitivity/responsiveness was combined with relative care. Boys experiencing nonrelative care were more Likely to be heightening emotion expression.

영아를 위한 대화식 그림책읽기 교사교육 프로그램의 효과 (A Dialogic Picturebook Reading Program : Effects on Teacher-Toddler Interactions and on Toddler Language)

  • Subjects were 88 two-year-old-toddlers(25-36 months of age) and 32 teachers in 13 childcare centers; they were randomly assigned to experimental or control groups. The researcher observed teacher-toddler interaction in the picturebook reading situation. Analysis of patterns of teachers' verbal behavior and coding of toddlers' verbal and nonverbal behaviors were based on Senechal, et al.(1995) and Whitehurst, et al.(1988), respectively. In comparison with the control group, toddlers of the experimental group showed significant differences in verbal behavior; they acquired nouns occurring in the picturebooks and more expressive and comprehensive language. After training intervention, teachers of the experimental group showed changes in quality and quantity of verbal behavior.

Using nasal cannula for sevoflurane deep sedation in emergency dental treatment

  • Background: Emergency room doctors run into difficulties in treating injured pediatric patients because usually they fell into panic after trauma. In these situation, deep sedation with sevoflurane is fully recommendable method. The conventional way can interrupt common dental treatment procedure. Methods: In the present study, nasal cannula was used for sevoflurane deep sedation in 11 dental emergency treatment. Age ranged from 0 to 3 years old (average of 1.8 years). Results: Treatment duration was from 10 to 35 minutes (average of 16.7 minutes). Average duration of sedation was 25.5 minutes ranging from 15 to 45 minutes. Conclusions: It has advantages to use nasal cannula for sevoflurane deep sedation rather than conventional intubation; saves time and secures good operation field.

By alternative about these problem, including Biometrics, several authentication systems through sign(Signature), Smart Card, Watermarking technology are developed. Therefore, This paper shows that extract factor that efficiency can get into peculiar feature in physical features for good fingerprint recognition algorithm implementation with old study finding that take advantage of special quality of these fingerprint.

가상적 딜레마 상황에 대한 만 4세 및 5세 유아의 사회적 지식과 대인간 문제해결 전략의 관계 (The Relationships between Social Knowledge about Hypothetical Social Dilemmas and Interpersonal Problem Solving Strategies of 4- and 5-year-olds)

  • This study investigated 4- and 5-year-olds' social knowledge and interpersonal problem solving strategies according to their sex, age, and hypothetical situation, and the relationships among them. Subjects were 116 preschoolers (73 boys and 43 girls; 68 four and 48 five-year-olds). Results showed that preschoolers had the higher scores of freindliness in potential entry situations than in support situations. Also, 4-year-old children were higher in forceful problem solving strategies than 5-year-olds. Furthermore, children's social knowledge assertiveness scores were positively related to their relevant problem solving strategies in conflict situations, while children's social knowledge friendliness scores were negatively related to their forceful problem solving strategies in conflict and potential entry situations.
