Medical Image Segmentation: A Comparison Between Unsupervised Clustering and Region Growing Technique for TRUS and MR Prostate Images

  • Prostate cancer is one of the most diagnosed malignancies found across the world today. American cancer society in recent research predicted that over 174,600 new prostate cancer cases found and nearly 31,620 death cases recorded. Researchers are developing modest and accurate methodologies to detect and diagnose prostate cancer. Recent work has been done in radiology to detect prostate tumors using ultrasound imaging and resonance imaging techniques. Transrectal ultrasound and Magnetic resonance images of the prostate gland help in the detection of cancer in the prostate gland. The proposed paper is based on comparison and analysis between two novel image segmentation approaches. Seed region growing and cluster based image segmentation is used to extract the region from trans-rectal ultrasound prostate and MR prostate images. The region of extraction represents the abnormality area that presents in men's prostate gland. Detection of such abnormalities in the prostate gland helps in the identification and treatment of prostate cancer

부뜸이에 의한 전립선암 치료 1례 보고 (Clinical Report of One Chronic Several Patient on the Cancer of the Prostate Gland by Using BUDDEUMI;equipment with using Combination of Moxibustion and Cupping Literatures)

  • "BUDDEUMI is a new remedy of the synchronous combination of cupping boil and moxibustion for the exclusion of human body's waste product. It wishes the reach for improving the effect of cupping boil and moxibustion by the synchronous combination. As the negative pressure of cupping and the hot cure of moxa are made up of BUDDEUMI structural character. In order to determine the depth-hot effect of BUDDEUMI, clinical study was performed in the cancer of the prostate gland for several years. The targets of study was one person with the cancer of the prostate gland for several years. The BUDDEUMI was operated 3-4 times/ day, during 12 months on the Gowan-Won, Jang-Wan, Jeon Jung and the abdominal region & Jang-Gang, Meong-Moon, Dae-Chue and the back region. Therefore we compared PSA(prostate singular antigen: June, 2006/ June, 2007) before and after BUDDEUMI therapy. PSA significantly decreased in a comparison of before-treatment (30.76) and after 12 months (0.76). And the times of enuresis significantly decreased in a comparison of before-treatment (5-10 times) and after 12 months(1-2 times). The effects of BUDDEUMI are observed on the treatment of the cancer of the prostate gland. As "BUDDEUMI and BUDDEUMI's treatment" is expected to relieve pain, and for the healthy article of Senior-Friendly.

Fiducial Gold Marker가 삽입된 전립선암 양성자 치료 시 Digital Image Positioning System (DIPS)을 이용한 전립선의 위치변화에 따른 보정에 관한 연구 (A Study of the Adjustment and Treatment Depending on the Change of Prostate Location Using DIPS in Proton Beam Therapy for Prostate Gland in which a Fiducial Gold Marker was Inserted)

  • 목 적: Fiducial Gold Marker가 삽입된 전립선암 양성자 치료 시 DIPS를 이용하여 전립선의 위치변화를 관찰하고, 전립선의 움직임에 따른 보정 및 양성자치료에 관한 연구를 하고자 한다. 대상 및 방법: 2008년 4월 이후로 본원에서 양성자치료를 받은 10명의 전립선암환자를 대상으로 하였다. 전립선의 위치변화를 관찰하기위해 치료 전에 전립선에 3개의 Fiducial Gold Marker를 삽입하였고, 전립선의 움직임을 최소화하기 위해 치료 전에 소변을 보고, 매일 정량의 물을 섭취하며, Rectal balloon을 함께 시행하였다. 환자의 처음 치료자세는 CT 모의치료 때와 같고, DIPS (Digital Image Positioning System)를 이용하여 AP, Lat 촬영을 하여 얻은 영상과 치료계획에서 얻은 PA, Lat 영상을 비교하여, Fiducial Gold Marker 위치변화(X, Y, Z) 관찰하였고, 이에 따른 보정 후에 양성자치료를 시행하였다. 결 과: 치료계획에서 얻은 영상과 DIPS를 이용하여 얻은 영상의 각각의 Fiducial Gold Marker의 위치변화 차이는 10명 환자에 있어서 치료 중심을 기준으로 평균 X축: $\pm$0.116 cm, Y축: $\pm$0.19 cm, Z축: $\pm$0.176 cm이었다. 10명의 환자 중 X축으로 최고 RT: 0.24 cm, 최저 RT: 0.04 cm의 위치변화 차이가 나타났고, Y축으로 최고 Sup: 0.42 cm, 최저 Inf: 0.03 cm 위치변화 차이가 나타났고, Z축으로 최고 Ant: 0.35 cm, 최저 Post: 0.05 cm의 위치변화 차이가 관찰되었다. 결 론: 전립선의 움직임을 최소화 하기위해 치료 전에 전처치로 소변을 보고, 매일 정량의 물을 섭취하며, Rectal balloon을 시행하였으나, 모든 환자의 경우에서 전립선에 삽입된 3개의 Fiducial Gold Marker의 매일 움직임을 관찰할 수 있었다. 위 자료를 바탕으로 보다 정확한 치료 중심 설정이 요구되는 양성자치료에서 Gold Marker Matching System의 필요성을 확인할 수 있었으며, 본 연구에서 사용된 DIPS를 이용하여 전립선의 위치변화에 따른 치료 중심을 보정함으로써 보다 정밀한 양성자치료 효과를 향상시킬 수 있을 것으로 사료된다.

Feasibility of Novel Three-Dimensional Magnetic Resonance Fingerprinting of the Prostate Gland: Phantom and Clinical Studies

  • Objective: To evaluate the feasibility of a new three-dimensional (3D) MR fingerprinting (MRF) technique for the prostate gland by conducting phantom and clinical studies. Materials and Methods: The new 3D MRF technique used in this study enables quick data acquisition and has a high resolution. For the phantom study, the MRF T1 and T2 values in an in-house phantom were compared with those of goldstandard mapping methods using linear regression analysis. For the clinical study, we evaluated 90 patients who underwent prostate imaging with MRF for suspected prostate cancer between September 2019 and February 2020. The mean T1 and T2 values were compared in the peripheral zone, transition zone, and focal lesions using paired t tests. The differences in the T1 and T2 values according to cancer aggressiveness were evaluated using one-way analysis of variance. Results: In the phantom study, the MRF T1 and T2 values showed a perfect correlation with the gold-standard T1 and T2 values (R > 0.99). In the clinical study, the T1 and T2 values in the peripheral zone were significantly higher than those in the transitional zone (p < 0.001, both). The T1 and T2 values in prostate cancer were significantly lower than those in the peripheral and transitional zones. The higher the grade of cancer, the lower the T2 values. Conclusion: The T1 and T2 values obtained from the 3D MRF showed a perfect correlation with the gold standard values in the phantom study. Differences in the T1 and T2 values among the different zones of the prostate gland were identified using 3D MRF in patients.

Gastric Adenocarcinoma with Prostatic Metastasis

  • Metastasis of gastric adenocarcinoma to the prostate gland is extremely rare. Herein, we report a case of gastric adenocarcinoma in a 56-year-old man with prostatic metastasis diagnosed through the analysis of biopsy specimens from representative lesions in the stomach and prostate gland. Immunohistochemistry of the prostatic tissue showed positive staining for cytokeratin 7 and negative staining for prostate-specific antigen (PSA), whereas the serum PSA level was normal, confirming the diagnosis of prostatic metastasis from carcinoma of the stomach.

Significant Association of Metabolic Indices, Lipid Profile, and Androgen Levels with Prostate Cancer

  • Objectives: To compare the metabolic indices, lipid profile, androgens, and prostate specific antigen between prostate cancer and BPH and between grades of prostate cancer in a cross-sectional study. Materials and Methods: The study enrolled 95 cases of prostate cancer and 95 cases of benign prostatic hyperplasia (BPH). Prostate gland volume was measured using transrectal ultrasound. We compared insulin, testosterone, dihydrotestosterone, prostate specific antigen levels and lipid profile between prostate cancer of different grades and BPH. Further, prostate cancer patients were classified into low grade and high grade. Unpaired t-test for normally distributed variables and Man-Whitney U test for non-normal variables were used to assess differences. Results: We found that prostate cancer patients had significantly higher levels of insulin, testosterone, PSA, cholesterol, triglycerides, low-density lipoprotein (LDL) and very low-density lipoprotein (VLDL) in comparison to their BPH counterparts. Higher levels of these parameters also correlated with a higher grade of the disease. Conclusions: We conclude that higher levels of insulin, testosterone, PSA, and cholesterol correlate with a higher risk of prostate cancer, and also with a higher grade of the disease.

전립선 생검: 일반적 고려사항 및 체계적 생검 (Prostate Biopsy: General Consideration and Systematic Biopsy)

  • 한국은 급속하게 고령화 사회로 진입하고 있으며, 이로 인한 전립선암과 관련된 사회경제적 부담 증가는 불가피하다. 따라서, 전립선암의 조기 발견과 정확한 진단의 필요성은 더욱 높아지고 있다. 일반적으로 이상적인 생검은 최소 숫자의 생검 코어만으로 암을 정확히 찾아낼 수 있어야 한다. 그러나 전립선암은 영상에서 뚜렷한 이상 소견으로 드러나지 않을 수 있기 때문에, 이미지 유도 표적 생검만으로 진단에 충분하지 않다. 따라서 수십 년의 시행착오와 변천을 거쳐, 전립선암 진단은 실질 전반에 대한 무작위적이고 반복적인, 이른바 체계적 생검에 의존하고 있다. 이 종설을 통해 전립선암 진단법의 변천사를 살펴보고, 전립선 생검에 있어 일반적으로 고려해야 할 사항들과 생검 전후의 환자 관리에 대해서 다루고자 한다.

전립샘암 환자의 방사선 치료 시 광중성자 선량으로 인한 암 발생률의 연구 (A Study of Cancer Incidence Rate due to Photoneutron Dose during Radiation Therapy for Prostate Cancer Patients)

  • 전립샘암의 방사선치료법인 3D CRT와 IMRT 5 portals, IMRT 9 portals에서 정상장기인 대장과 갑상샘의 광중성자 선량 피폭으로 인하여 암이 발생할 확률을 연구하고자 한다. 전립샘암 총 처방선량 6600 cGy로 1회당 220 cGy, 총 횟수는 30회의 분할조사를 적용하였다. 실험에 사용한 의료용 선형가속기의 치료테이블(couch)위에 Rando phantom을 setup 한 후에 phantom의 대장, 갑상샘의 해당부위에 중성자 광자극발광선량계 (Optically stimulated luminescence albedo neutron dosimeter)를 위치시켜 측정하였다. 전립샘암 3D CRT시 정상장기인 대장과 갑상샘의 광중성자 선량으로 인한 2차 암의 발생확률은 10,000명 당 1.8명 이었다. 그리고 IMRT 5 portals은 10,000명 당 8.7명으로, 3D CRT에 비해서 약 5배 컸다. IMRT 9 portals는 1,000명 당 1.2명이 암이 발생할 확률이 있음의 결과를 도출하였다. 본 연구를 바탕으로 방사선 치료 시 발생하는 광중성자 선량으로 인하여 2차 방사선 피폭선량의 위험성을 연구하여, 향후 방사선의 확률적 영향과 방사선 방호에 관련하여 유용한 자료로 활용될 것으로 사료된다.

Conditional PTEN-deficient Mice as a Prostate Cancer Chemoprevention Model

  • Background: We generated a mouse model of prostate cancer based on the adult-prostate-specific inactivation of phosphatase and tensin homolog (PTEN) using the Cre-loxP system. The potential of our mice as a useful animal model was examined by evaluating the chemopreventive efficacy of the anti-androgen, chlormadinone acetate (CMA). Materials and Methods: Six-week-old mice were treated subcutaneously with $50{\mu}g/g$ of CMA three times a week for 9 or 14 weeks and sacrificed at weeks 15 and 20. Macroscopic change of the entire genitourinary tract (GUT) and histologically evident prostate gland tumor development were evaluated. Proliferation and apoptosis status in the prostate were examined by immunohistochemistry. Results: CMA triggered significant shrinkage of not only the GUT but also prostate glands at 15 weeks compared to the control (p=0.017 and p=0.010, respectively), and the trend became more marked after a further five-weeks of treatment. The onset of prostate adenocarcinoma was not prevented but the proliferation of cancer cells was inhibited by CMA, which suggested the androgen axis is critical for cancer growth in these mice. Conclusions: Conditional PTEN-deficient mice are useful as a preclinical model for chemoprevention studies and serve as a valuable tool for the future screening of potential chemopreventive agents.

p63 Cytoplasmic Aberrance is Associated with High Prostate Cancer Stem Cell Expression

  • Introduction: Prostate cancer in Indonesia is the $3^{rd}$ ranking cancer among males and the $5^{th}$ rank for their cancer mortality. Prognostic markers that can identify aggressive prostate cancer in early stages and help select appropriate therapy to finally reduce the mortality are therefore urgently needed. It has been suggested that stem cells in the prostate gland have a role in initiation, progression, and metastasis of cancer, although controversy continues to exist. Maintenance of normal stem cell or reserve cell populations in several epithelia including prostate has been shown to be regulated by p63 and alteration of p63 expression is considered to have an oncogenic role in prostate cancer. We hypothesize that the expression of cytoplasmic aberrance of p63 is associated with high ALDH1A1 expression as a cancer stem cell marker, thus leading to progression of prostate cancer. Methods: Using a cross-sectional study during two years (2009-2010), a total of 79 paraffin embedded tissues of benign prostatic hyperplasia, PIN prostatic intraepithelial neoplasia, low and high Gleason score prostate cancer were investigated using immunohistochemistry. Associations between cytoplasmic p63 and ALDH1A1, as well as with pathological diagnosis, were analyzed by Chi-Square test using SPSS 15.0. Links of both markers with cell proliferation rate (KI-67) and apoptotic rate (cleaved caspase 3) were also analyzed by Kruskal-Wallis test. Results: The mean age of patient at the diagnosis is 70.0 years. Cytoplasmic aberrance of p63 was associated with ALDH1A1 expression (p<0.001) and both were found to have significant relationships with pathological diagnosis (including Gleason score), (p=0.006 and p<0.001 respectively). Moreover, it was also found that higher levels of cytoplasmic p63 were significantly associated with the frequency of proliferating cells and cells undergoing apoptosis in prostate cancers (p=0.001 and p=0.016 respectively). Conclusion: p63 cytoplasmic aberrance is associated with high ALDH1A1 expression. These components are suggested to have an important role in prostate cancer progression and may be used as molecular markers.
