Prognostic Factors of Neonatal Sepsis Mortality in Developing Country

  • 목적: 패혈증은 개발도상국에서 연간 사망률의 30-50%를 차지하는 신생아 사망의 가장 흔한 원인이다. 본 연구는 신생아 패혈증 사망률의 예후인자를 알아보고자 하였다. 방법: 2021년 4월부터 2021년 9월까지 R. Sosodoro Djatikoesoemo 주지사 병원 신생아 중환자실에서 패혈증을 진단받은 121명의 신생아를 대상으로 후향적 코호트 연구로 진행되었다. 연구대상자 선정기준은 신생아 중환자실에 입원하고 패혈증을 진단받은 생후 0-28일된신생아였다. 임상 기록이 불완전한 경우와 선천적 기형을 가진 경우는 제외하였다. 성별, 재태주령, 분만방식, 출생체중, APGAR 점수, 출생지, 혈액배양에 대해 카이제곱 검정을 시행하였고 백혈구, 림프구, 호중구, 혈소판, C반응단백 (C-reactive protein, CRP) 및 체류 기간에 대하여서는 정규성 검정을 한 후 Mann-Whitney 테스트로 분석하였다. 결과: 출생체중 (P=0.038), 임신주수 (P=0.009), 혈액배양 (P=0.014)은 신생아 패혈증 결과에 유의한 상관관계를 보였고, Mann-Whitney 검사는 혈소판 (P=0.018), CRP (P=0.002) 및 재원기간 (P<0.001)에서 유의한 차이를 보였다. 다변량 분석에서 신생아 패혈증 사망률과 관련된 세 가지 예후 인자는 미숙아 (오즈비 [odds ratio, OR], 3.906; 95% 신뢰구간 [confidence interval, CI], 1.344-11.356; P=0.012), 저체중 출생 (OR, 2.833; 95% CI, 1.030-7.790; P=0.044), 그람 음성 박테리아 (OR, 4.821; 95% CI, 1.018-22.842; P=0.047)인 것으로 나타났다. 결론: 미숙아, 저체중아, 그람 음성균 감염이 신생아 패혈증의 예후와 관련이 있었다.

비선형 자기회귀모형을 이용한 남방진동지수 시계열 분석 (Nonlinear Autoregressive Modeling of Southern Oscillation Index)

  • 본 연구에서는 조건부 핵밀도함수와 CAFPE(Corrected Asymptotic Final Prediction Error) 차수결정 방법에 근거한 비매개변수적 비선형 자기회귀 (Nonlinear AutoRegressive, NAR) 모형을 소개하고 이를 SOI(Southern Oscillation Index)에 적용하였다. SOI 자료에 대해서 선형 AR 모형을 적용하였으나 잔차에 대한 검정결과 이분산성(heteroscedasticity)을 나타내었다. 또한 BDS(Brock-Dechert-Sheinkman) 검정에서 비선형성이 존재함을 확인하였다. 따라서 NAR 모형에 SOI 자료를 적용시켰다. CAFPE를 이용하여 가장 적합한 모형으로 지체 1, 2와 4가 선택되었으며 조건부 평균함수를 추정하여 SOI 자료를 모의한 결과 잔차에 대해서 정규성과 이분산성 가정이 Jarque-Bera 검정과 ARCH-LM 검정에서 각각 기각되었으며 또한 조건부 표준편차함수의 최적 차수로 3, 8과 9가 CAPFE를 통해 선택되었다. 조건부 평균함수와 표준편차함수를 모두 고려한 모형에 대한 잔차 검정 결과 잔차의 I.I.D 가정을 만족하였으며 특히, BDS 검정에서 신뢰구간 95%와 99%에서 모두 만족한 결과를 나타내었다. 마지막으로 전체의 15%에 해당하는 SOI 자료에 대해서 One-Step 예측을 수행하였으며 선형 모형에 비해 평균제곱예측오차가 7% 적게 나타났다. 따라서, NAR 모형은 여타의 매개변수적 방법과 달리 모형 선택에 있어 자유로우며 비선형성을 고려할 수 있는 모형으로서 SOI 자료와 같은 비선형 자료를 위한 모의방법으로 선형 모형에 비해 많은 장점을 가지고 있다.

뉴로피드백 훈련이 고등학생의 일반적 건강수준, 정신건강과 문제행동 및 뇌기능 지수에 미치는 영향 (The Effects of the Neurofeedback training on the General health status, Mental health and problem behavior, and Brain function quotient among High school students)

  • 최근 청소년기의 이환율과 사망률, 정신건강과 문제행동의 위험 수준이 심각하나 적절한 중재가 없는 실정이다. 이에 본 유사실험 연구는 뉴로피드백 훈련이 고등학생의 일반적 건강수준, 정신건강과 문제행동 및 뇌기능 지수에 미치는 영향을 확인하였다. 뉴로피드백 훈련은 2012년 9월부터 12월, 주 2회(총 20회)에 걸쳐 방과 후 교내 컴퓨터실에서 훈련된 연구자 1인에 의해 진행되었으며 구조화된 설문지와 뇌기능 프로그램을 이용하여 자료수집하였다. 대상자는 S시 I고등학교에 재학 중인 고등학생으로, 대조군 6명과 실험군 5명이 최종 분석되었다. 자료는 SPSS 20.0을 이용하여 정규성 검정 후 paired t-test와 Wilcoxon signed rank test로 효과 검증하였다. 뉴로피드백 훈련 결과, 일반적 건강수준, 정신건강과 문제행동은 유의미한 차이는 없었으나 통계적으로 향상되었고, 뇌기능 지수 가운데 정서지수와 좌 우 육체적, 정신적 스트레스 저항지수가 두 군 간에 통계적으로 유의미한 차이가 있는 것으로 나타났다.

12주 복합운동이 비만 여중생의 렙틴과 인슐린에 미치는 영향 (A study on the effects of a 12-week compound exercise program on obese middle school girls' leptin and insulin levels)

  • 본 연구는 체지방률이 30%이상인 비만 여중생을 대상으로 복합운동집단 (n=20)과 통제집단 (n=20)으로 비교분석하여 12주 복합운동 (유산소운동+저항운동)이 비만 여중생의 운동 전과 후의 렙틴과 인슐린에 미치는 영향을 구명하는데 그 목적이 있다. 본 연구의 자료처리는 SPSS 19.0 통계프로그램을 이용하여 각 변인 간에 평균과 표준편차를 산출하여 샤피로-윌크 검정 값을 이용해 정규성을 검정한 후 독립표본 t-검정을 실시하였다. 12주간 복합운동에 따른 전 후 전변인의 차이를 비교하기 위하여 대응표본 t-검정을 이용하였다. 모든 통계적 분석을 위한 유의수준은 ${\alpha}=0.05$로 설정하였다. 본 연구 결과에서 렙틴과 인슐린은 복합운동집단에서 모두 유의하게 감소하였다. 규칙적인 12주 복합운동이 비만 여중생의 렙틴과 인슐린에 긍정적인 영향을 미친 것으로 사료된다.

Midfacial soft tissue changes after maxillary expansion using micro-implant-supported maxillary skeletal expanders in young adults: A retrospective study

  • Objective: The aim of this retrospective study was to assess the midfacial soft tissue changes following maxillary expansion using micro-implant-supported maxillary skeletal expanders (MSEs) in young adults by cone-beam computerized tomography (CBCT) and to evaluate the correlations between hard and soft tissue changes after MSE usage. Methods: Twenty patients (mean age, 22.4 years; range, 17.6-27.1) with maxillary transverse deficiency treated with MSEs were selected. Mean expansion amount was 6.5 mm. CBCT images taken before and after expansion were superimposed to measure the changes in soft and hard tissue landmarks. Statistical analyses were performed using paired t-test and Pearson's correlation analysis on the basis of the normality of data. Results: Average lateral movement of the cheek points was 1.35 mm (right) and 1.08 mm (left), and that of the alar curvature points was 1.03 mm (right) and 1.02 mm (left). Average forward displacement of the cheek points was 0.59 mm (right) and 0.44 mm (left), and that of the alar curvature points was 0.61 mm (right) and 0.77 mm (left) (p < 0.05). Anterior nasal spine (ANS), posterior nasal spine (PNS), and alveolar bone width showed significant increments (p < 0.05). Changes in the cheek and alar curvature points on both sides significantly correlated with hard tissue changes (p < 0.05). Conclusions: Maxillary expansion using MSEs resulted in significant lateral and forward movements of the soft tissues of cheek and alar curvature points on both sides in young adults and correlated with the maxillary suture opening at the ANS and PNS.

산후 여성의 심박변이도 특성 연구 (Comparison research of HRV between Postpartum Women and Normal Women)

  • Objective : This research was designed to study the characteristics of heart rate variability in postpartum women compared with normal women. Method : HRV data of postpartum women were gathered from 255 women who was hospitalized for oriental postpartum treatments(Age : 25-35). HRV data of comparison group were gathered from 327 women who visited hospital to check up their health(Age : 25-35). The SPSS 12.0 for windows was used to analyze the date and the independent samples t-test was used to verify the result. Result : 1. Mean-RR and SDNN of postpartum women group significantly decreased compared with that of normal women group. But, Heart Rate of postpartum women group significantly increased compared with that of normal women group. 2. HRV-Index, RMSSD and SDSD of postpartum women group significantly decreased compared with that of normal women group. pNN50 of postpartum women group significantly increased compared with that of normal women group. 3. Ln(TP), Ln(VLF), Ln(LF) and Ln(HF) of postpartum women group significantly increased compared with that of normal women group. 4. There were no significant differences in Normal LF, Normal HF and LF/DF Ratio between postpartum women group and normal women group. Conclusion : The result suggest that the function of heart of postpartum women group significantly decreased compared with that of normal women group. Futhermore although the ANS maintained the balance in the range of normality, the sympathetic nervous system frequently revitalized which caused increasing the heart of pulsation.

야전 치명고장자료를 이용한 함정전투체계 신뢰성 분석 및 활용 방안 (A Study of Reliability Analysis and Application on Naval Combat System Using Field Critical Failure Data)

  • Naval combat system developed in-country is progressing at an alarming rate since 2000. ROK navy will be achieved all vessels that have combat system in the near future. The importance of System Engineering and Integrated Logistics Support based on reliability analysis is increasing. However, reliability analysis that everyone trusted and recognized is not enough and applied practically for development of Defense Acquisition Program. In particular, Existing Reliability Analysis is focusing on reliability index (Mean Time Between Failure (MTBF) etc.) for policy decision of defense improvement project. Most of the weapon system acquisition process applying in the exponential distribution simply persist unreality due to memoryless property. Critical failures are more important than simple faults to ship's operator. There are no confirmed cases of reliability analysis involved with critical failure that naval ship scheduler and operator concerned sensitively. Therefore, this study is focusing on Mean Time To Critical Failure (MTTCF), reliability on specific time and Operational Readiness Float (ORF) requirements related to critical failure of Patrol Killer Guided missile (PKG) combat system that is beginning of naval combat system developed in-country. Methods of analysis is applied parametric and non-parametric statistical techniques. It is compared to the estimates and proposed applications. The result of study shows that parametric and non-parametric estimators should be applied differently depending on purpose of utilization based on test of normality. For the first time, this study is offering Reliability of ROK Naval combat system to stakeholders involved with defense improvement project. Decision makers of defense improvement project have to active support and effort in this area for improvement of System Engineering.

Effect of biofilm formation, and biocorrosion on denture base fractures

  • PURPOSE. The aim of this study was to investigate the destructive effects of biofilm formation and/or biocorrosive activity of 6 different oral microorganisms. MATERIALS AND METHODS. Three different heat polymerized acrylic resins (Ivocap Plus, Lucitone 550, QC 20) were used to prepare three different types of samples. Type "A" samples with "V" type notch was used to measure the fracture strength, "B" type to evaluate the surfaces with scanning electron microscopy and "C" type for quantitative biofilm assay. Development and calculation of biofilm covered surfaces on denture base materials were accomplished by SEM and quantitative biofilm assay. According to normality assumptions ANOVA or Kruskal-Wallis was selected for statistical analysis (${\alpha}$=0.05). RESULTS. Significant differences were obtained among the adhesion potential of 6 different microorganisms and there were significant differences among their adhesion onto 3 different denture base materials. Compared to the control groups after contamination with the microorganisms, the three point bending test values of denture base materials decreased significantly (P<.05); microorganisms diffused at least 52% of the denture base surface. The highest median quantitative biofilm value within all the denture base materials was obtained with P. aeruginosa on Lucitone 550. The type of denture base material did not alter the diffusion potential of the microorganisms significantly (P>.05). CONCLUSION. All the tested microorganisms had destructive effect over the structure and composition of the denture base materials.

먼쪽 노뼈 골절환자에게 적용한 칼텐본-에반스 관절가동술의 적용 횟수가 노자관절의 운동성, 통증, 기능과 악력에 미치는 영향 (The Effects of Kaltenborn-Evjenth Joint Mobilization of Application Count on Joint Mobility, Pain, Functions and Grip Strength in Patients with Distal Radius Fracture)

  • Purpose : The purpose of this study was to investigate the effects of Kaltenborn-Evjenth (KE) joint mobilization of the distal radioulnar joints (RUJ) and proximal RUJ in distal radius fractures (DRFs) on range of motion (ROM), grip strength (GS), and patient-written wrist evaluation (PRWE) in each group once, thrice, or fivefold. Methods : Forty-two subjects participated in this study. We divided the subjects with DRFs into groups applying KE concepts RUJ mobilization once, thrice, and fivefold. The patients' ROM and GS were measured using a joint goniometer and dynamometer, respectively. Pain and function were also assessed using a PRWE. In the statistical analysis, all data were tested for normality using the Shapiro-Wilk test, and paired t-tests were performed for within-group before-and-after comparisons of each intervention. One-way analysis of variance was used for between-group comparisons of differences. All statistical significance levels were set at α=.05. Results : There were significant differences in the ROM in all three groups before and after the intervention (p<.05), but there were no significant differences between the groups. There were significant differences in the GS in the three groups before and after the intervention (p<.05), but there were no significant differences between the groups. In the pain part of the PRWE, all three groups had significant differences before and after intervention (p<.05), but there was no significant difference between the groups. In the functional part of the PRWE, there were significant differences in the three groups before and after intervention (p<.05), but no significant difference occurred between the groups. Conclusion : Based on the aforementioned results, there were no significant between-group differences in ROM, GS, and PRWE (pain and function) after the application of the K-E joint mobilization to DRFs once, thrice, and fivefold. Nevertheless, there were significant within-group differences in all the above.

가토의 수정란이식에 관한 연구 ―II. 동결융해난자의 발육단계별 생존성- (Studies on Embryo Transfer in Rabbit ―II. The viability of deep-frozen embryos at different developing stages―)

  • Present studies were conducted to investigate the developmental stage and the location of embryos in the reproductive tract at various times after ovulation, the morphologically normal after thawing of embryos preserved in liquid nitrogen, and the survival after transferring frozen-thawed embryos. The results obtained were as follows: 1. Embryo stage and location in the reproductive tract after hCG administration. For the investigation of embryo stage and location in the reproductive tract after ovulation, rabbits were laparotomized at 24, 40, 48, 72 and 120 hrs post hCG injection, simultaneously with mating. the oviducts and uteri were flushed out with PBS medium containing 50% rabbit serum, respectively. 1) Most of embryos was remained in the oviduct within 48 hrs, with the lapse of time, embryos were started to move to uterus and shifted in uterus at 72 hrs after hCG injection. 2) The representatives of embryos stage collected at 24, 40, 48, 72 and 120 hrs were 1-cell(60.4%), 8-cell to early morula (52.3, 39.3%), late blastocyst (95.5%) stages, respectively. 2. Morphological normality and survival of the frozen-thawed embryos. For the evalution of the quality and viability on the frozen-thawed embryos, immediately after thawing, embryos were assessed by morphologically normal under a dissecting microscope, and a further test of frozen-thawed embryos was made by transferring the morphologically normal embryos to the uteri of recipient rabbit induced pseudopregnancy by the injection of hCG at the time of hCG injection in donor rabbits. 1) The propotions of embryos which a, pp.ared morphologically normal was higher when 8-cell (85.7%) and morula(90.5%) were used for freezing than when 4-cell (66.7%) and blastocyst (75.8%) were used. 2) Preganacies were observed at Day 15 after transfer of frozen-thawed 8-cell (7/13), morula (19/42) and blastocyst (3/19) but not after transfer of embryos at 4-cell stage.
