Study of gwol(厥) and gwoleumbyeong(厥陰病) from "Sanghanron (傷寒論)" ("상한론(傷寒論)"의 궐(厥)과 궐음병(厥陰病)에 대한 고찰)

  • From pathological view, Gwoleumbyeong(厥陰病) may be explained as extreme reduction of Gwoleumgyeonggi(厥陰經氣) due to immoderate Wihan(胃寒). Also, concerning regions of human body, Gwoleumbyeong has close relation to lower abdomen and genital, which are the origins of Gwoleum meridian[厥陰經]. Therefore, it is appropriate that the nature of Gwoleumbyeong be described as Hangeukyangul(寒極陽鬱), rather than Sangyeolhahan(上熱下寒), Hanyeolchakjap(寒熱錯雜). Sangyeolhahan, Hanyeolchakjap does not represent Gwoleumbyeong to the full extent, in a sense that the term views Hanyeol(寒熱) as two equal rank, whereas pathogenesis of Gwoleumbyeong depends on the sole extremity of Wihan(胃寒). The reason that the nature of sanghallon(傷寒論) be regarded as Sangyeolhahan, Hanyeolchakjap has been lying on the presupposition that whole symptoms of Hangwol (寒厥), Yeolgwol(熱厥), diarrhea[下利], vomiting[嘔吐] and hiccup, from "Gwoleumpyeon(厥陰篇)",may be categorized into Gwoleumbyeong. However, the symptoms described above do not show Gihwa(氣化) characteristic of Gwoleum(厥陰) and follow the regional distribution of Gwoleumbyeong, from which it can be pointed that those symptoms have no relation with Gwoleumbyeong. Ever since the theory of Yukgigihwa(六氣氣化) was used to comprehend and interpret "Sanghallon", Gwoleumbyeong has been thought as equivalence of Gwol(厥), which led to misunderstanding of Sangyeolhahan, Hanyeolchakjap. However, Gwoleumbyeong from "Sanghallon" has been argued in specific, through the historical practice of Sanghan(傷寒) as pathogen, particular state of meridian and correlation of meridian and Byeongjeung(病證).

Incidence of Colon Cancer Related to Cigarette Smoking and Alcohol Consumption in Adults with Metabolic Syndrome: Prospective Cohort Study (대사증후군 성인의 흡연 및 음주 상태가 대장암 발생 위험에 미치는 영향: 전향적 코호트 연구)

  • Purpose: The purpose of this study was to identify the impact of cigarette smoking and alcohol consumption on the incidence of colon cancer in adults with metabolic syndrome. Methods: This study employed a longitudinal study design and utilized secondary data drawn from the Korean Genome and Epidemiology Study (KoGES). The data of a sample of 2,327 adults with metabolic syndrome tracked every two years from 2001 to 2014 were used in this study. Statistical data analyses of the frequency, number of cases per 100,000 person-years, log-rank test, Kaplan-Meier curve, and Cox's proportional hazards regression were performed using IBM SPSS statistics version 24. Results: During the observation period, the number of colon cancer cases was 46, and the total person-years were 252,444. The incidence of colon cancer was higher in current, over 10 pack-year smokers when compared to non-smokers (hazard ratio=3.38, 95% confidence interval=1.09~8.42). Conclusion: Excessive and long-term smoking should be avoided to prevent colon cancer, especially in adults with metabolic syndrome, since it might exacerbate the risk factors of colon cancer. Particularly, health professionals need to provide individualized smoking cessation interventions to those at high risk of colon cancer.

Effects of Virtual Reality Exercise Program on Balance, Emotion and Quality of Life in Patients with Cognitive Decline

  • Purpose: In this study, we investigated the effectiveness of a 12-week virtual reality exercise program using the Nintendo Wii console (Wii) in improving balance, emotion, and quality of life among patients with cognitive decline. Methods: The study included 30 patients with cognitive decline (12 female, 18 male) who were randomly assigned to an experimental (n=15) and control groups (n=15). All subjects performed a traditional cognitive rehabilitation program and the experimental group performed additional three 40-minute virtual reality based video game (Wii) sessions per week for 12 weeks. The berg balance scale (BBS) was used to assess balance abilities. The short form geriatric depression scale-Korean (GDS-K) and the Korean version of quality of life-Alzheimer's disease (KQOL-AD) scale were both used to assess life quality in patients. Statistical significance was tested within and between groups before and after treatment, using Wilcoxon signed rank and Mann-Whitney u-tests. Results: After 36 training sessions, there were significant beneficial effects of the virtual reality game exercise on balance (BBS), GDS-K, and KQOL-AD in the experimental group when compared to the control group. No significant difference was observed within the control group. Conclusion: These findings demonstrate that a virtual reality-training program could improve the outcomes in terms of balance, depression, and quality of life in patients with cognitive decline. Long-term follow-ups and further studies of more efficient virtual reality training programs are needed.

The Effects of Community-based Fall Prevention Exercise Program on Lower Extremity Muscle Strength, Balance Ability and Fall Efficacy in Older Adults (낙상예방 운동 프로그램이 노인주간보호센터 재가노인의 하지 근력, 균형 능력 및 낙상효능감에 미치는 효과)

  • Purpose: This study aimed to verify the effects of a community-based fall prevention exercise program for older adults on lower extremity muscle strength, balance ability, and fall efficacy. Methods: This study recruited 30 participants, and the program was conducted for 10 weeks. The program included sessions on strength and balance exercises, conducted for 50 minutes a day and at least three times a week. The collected data were analyzed using non-parametric (Wilcoxon signed-rank test, generalized estimated equation) and parametric statistics (paired t-test, rmANOVA). Results: After 12 weeks of exercise program, lower extremity muscle strength and static-dynamic balance ability significantly increased. The fall efficacy increased from an average score of 2.48 points before the test to an average score of 2.91 points after the test. Regarding general characteristics, there were no significant differences in effect before and after the program, except for fall efficacy. Conclusion: The above results showed that the intervention of a community-based fall prevention exercise program for older adults was effective. Therefore, it is suggested that this exercise program be organized regularly within the center to provide periodic and long-term services.

  • Purpose: The government of India has initiated the Covid-19 Vaccination drive from early January 2021. Vaccination is identified to be best option to protect the people across the globe. However, owing to fast wide spread of the Covid-19, the Vaccine Distribution is a major challenge owing various issues like temperature control, infrastructure, hesitancy, geographical diversity, and other critical factors. Various research is carried out globally to understand and study the Vaccine Distribution issues based on the respective country issues and factors. Research Design, Data, and Methodology: This research paper attempts to explore prominent factors that could be taken up on priority for better and effective vaccination program. The study tries to rank various factors and sub-factors affecting vaccine distribution in India. AHP methodology based on feedback from 22 experts from the Vaccine industry has been deployed to get the desired results. Result: The results show that factors vaccine approval process, geographical prioritization, power supply, infrastructure maintenance costs for vaccine storage, and vaccine pricing are the prominent factors of effective vaccination in the country. Conclusion: The role and need for district-level health officers towards vaccine storage has been brought forward. A long-term effective vaccination policy is needed for optimum vaccine distribution.

Association Modeling on Keyword and Abstract Data in Korean Port Research

  • Purpose - This study investigates research trends by searching for English keywords and abstracts in 1,511 Korean journal articles in the Korea Citation Index from the 2002-2019 period using the term "Port." The study aims to lay the foundation for a more balanced development of port research. Design/methodology - Using abstract and keyword data, we perform frequency analysis and word embedding (Word2vec). A t-SNE plot shows the main keywords extracted using the TextRank algorithm. To analyze which words were used in what context in our two nine-year subperiods (2002-2010 and 2010-2019), we use Scattertext and scaled F-scores. Findings - First, during the 18-year study period, port research has developed through the convergence of diverse academic fields, covering 102 subject areas and 219 journals. Second, our frequency analysis of 4,431 keywords in 1,511 papers shows that the words "Port" (60 times), "Port Competitiveness" (33 times), and "Port Authority" (29 times), among others, are attractive to most researchers. Third, a word embedding analysis identifies the words highly correlated with the top eight keywords and visually shows four different subject clusters in a t-SNE plot. Fourth, we use Scattertext to compare words used in the two research sub-periods. Originality/value - This study is the first to apply abstract and keyword analysis and various text mining techniques to Korean journal articles in port research and thus has important implications. Further in-depth studies should collect a greater variety of textual data and analyze and compare port studies from different countries.

Text Summarization on Large-scale Vietnamese Datasets

  • This investigation is aimed at automatic text summarization on large-scale Vietnamese datasets. Vietnamese articles were collected from newspaper websites and plain text was extracted to build the dataset, that included 1,101,101 documents. Next, a new single-document extractive text summarization model was proposed to evaluate this dataset. In this summary model, the k-means algorithm is used to cluster the sentences of the input document using different text representations, such as BoW (bag-of-words), TF-IDF (term frequency - inverse document frequency), Word2Vec (Word-to-vector), Glove, and FastText. The summary algorithm then uses the trained k-means model to rank the candidate sentences and create a summary with the highest-ranked sentences. The empirical results of the F1-score achieved 51.91% ROUGE-1, 18.77% ROUGE-2 and 29.72% ROUGE-L, compared to 52.33% ROUGE-1, 16.17% ROUGE-2, and 33.09% ROUGE-L performed using a competitive abstractive model. The advantage of the proposed model is that it can perform well with O(n,k,p) = O(n(k+2/p)) + O(nlog2n) + O(np) + O(nk2) + O(k) time complexity.

The Effect of a Forest Healing Program on Cognitive Function, Depression, Anxiety, and Cortisol Levels in Elderly People

  • Increasing life expectancy leads to a rise in the prevalence of aging-related mental diseases and a concomitant increase in the financial and societal pressures related to their prevention and management. This issue is attracting increasing attention from researchers. Therefore, this study aimed to determine whether a healing program in a forest environment is effective for improving cognitive function, depression, anxiety, and stress in elderly people. A nonequivalent control group pretest-posttest design was used. The experimental group consisted of 18 males and females aged 60 to 80 years who participated in a forest healing program for seven days, while the comparison group comprised 10 people with similar demographics. For the process of data analysis, this study performed a cross-analysis to determine the homogeneity of the data and carried out the Wilcoxon signed-rank test, a nonparametric statistical test, to verify the significance of each group. Participation in the forest healing program led to a significant improve-ment in cognitive function (p=0.030). The levels of depression decreased, but this result was not statistically significant. The levels of anxiety decreased significantly (p=0.004). The mean cortisol score, a measurement of stress, decreased, but it was not statistically significant. These findings confirmed the effectiveness of this program for improving certain mental conditions of elderly people, and affirmed that the use of long-term forest healing programs can be expected to alleviate national pressures created by aging societies.

The Family Caregivers' Stress Pathways by Types of Long Term Care Services for the Elderly (장기요양보호서비스 유형별 가족 부양스트레스 경로)

  • The purpose of this study is to analyse the family caregivers' stress pathways by types of long term care services for the elderly, and then to discuss the findings of analysis. For this research, primary caregivers that provide care the elderly requiring long term care services sanctioned by National Health Insurance Corporation were drawn and surveyed. Among collected data, data for 258 primary caregivers by type of long term care services were used for this study. The results of this study can be summarized as follows. First, on average, the elders that utilize care service in institutions for the elderly were higher proportion of women, older, higher rate of bereavement, more children than the elders that utilize in-home care service, but some cases were vice-versa. Second, the elders that utilize care service in institutions more ADL dependency, higher proportion of severe dementia or severe stroke, higher care rank by National Health Insurance Corporation than the elders that utilize in-home care service on average. Third, primary caregivers with elders that utilize in-home care service were higher proportion of women, older, lower education level, higher rate of spouse and daughter-in-law in relationship with care recipient, less health, lower income than primary caregivers with elders that utilize care service in institutions. Fourth, subjective indicators representing caregivers' reactions to caregiving for the elderly significantly impact on caregivers' stress(ie, depression), and pathway of caregivers' stress are differentiated by type of long term care services. Fifth, stressors that have direct impacts on depression as caregiving family are differentiated by type of long term care services. Therefore, policies or programs to reduce negative mental health or stress of caregivig families should be designed differently by reflecting pathway of various stressors and stress by use types of long term care services for the elderly.

Prognosis and Management for Gallbladder Cancer with Hepatic Invasion: Long-term Results of 139 Patients from a Single Center in China

  • Objective: To improve the diagnosis of primary gallbladder carcinoma (GBC) with/without hepatic metastases by analyzing our experience of different GBC treatment in our patients. Methods: A retrospective study was carried out to analyze the clinical data of the 139 patients with GBC who underwent hepatic resection in our unit from January 2003 to December 2007. Patients were divided into two groups according to whether they demonstrated hepatic invasion. Tumor presentation, surgical modes, and prognosis of each patient were retrospectively reviewed. Kaplan-Meier curves and log-rank tests were employed to compare the survival rates of those patients undergoing different surgical procedures. Results: Of the 139 patients, 46 were men and 93 were women with the male to female ratio of 1:2.0. Their ages were ranged from 35 to 86 years with a mean age of $62.8{\pm}10.4$ years. There were 73 patients complicated with hepatic invasion (group A), and no hepatic invasion occurred in the other 66 patients (group B). Compared with the group B, the patients with hepatic invasion suffered lower differentiation of tumor (p=0.000), more advanced Nevin staging (p=0.008) and poorer prognosis (p=0.013). Radical resection were more frequently performed in group B (75.76%) than in group A (45.20%) with better outcomes (p=0.000). Conclusion: GBC patients complicated with hepatic invasion had poorer prognosis than those without invasion in long-term follow-ups. Radical resection might result in a satisfied prognosis in patients without hepatic invasion, but appears less favorable than palliative resection in those who were complicated with hepatic invasion.
