Evaluation of Korean teenagers with temporomandibular joint disorders

  • Objectives: This study aims to evaluate the severity and pattern of symptoms exhibited by teenage Korean temporomandibular disorder (TMD) patients. Materials and Methods: Among patients with an association of TMDs, teenage patients (11-19 years) who answered the questionnaire on the research diagnostic criteria for TMD (RDC/TMD) were recruited. Results: The ratio of patients who visited our clinic with a chief complaint of clicking sound (34.5%) or temporomandibular pain (36.6%) at the initial diagnosis (examination) was the highest. In the evaluation of the depression index, 75.8% of the subjects were normal, 12.9% were moderate, and 11.3% were severe. With regard to non-specific physical symptoms (including pain), 66.5% of the subjects were normal, 17.0% were moderate, and 16.5% were severe. Concerning non-specific physical symptoms (excluding pain), 70.6% of the subjects were normal, 14.4% were moderate, and 15.0% were severe. In terms of the graded chronic pain score, high disability (grade III, IV) was found in 9.3% of the subjects. Conclusion: Among teenage TMD patients, a portion have clinical symptoms and experience severe psychological pressure; hence requiring attention and treatment, as well as understanding the psychological pressure and appropriate treatments for dysfunction.

측두하악장애의 수기치료에 관한 국내외 임상 연구 고찰 (A Review of Recent Clinical Studies for Manual Therapy on Temporomandibular Joint Disorder)

  • Objectives The aim of this review is to investigate clinical studies for manual therapy on Temporomandibular Joint Disorder and to propose for the better method of studies in the future. Methods We investigated recent clinical studies for manual therapy on Temporomandibular Joint Disorder via searching Pubmed, KISS, KISTI, KERIS, KMBASE and National Assembly Library. 21 domestic and overseas articles were analyzed and the journals, the authors, the published years, countries, methods, periods, techniques of chiropractic, outcome measures, and purpose of those articles were examined. Results Studies on manual therapy were mainly published via Cranio. The diagnostic criteria that most frequently adopted in the overseas articles were research diagnostic criteria for temporomandibular disorders, The technique of chiropractic that most frequently adopted in the articles was passive traction and translation of TMJ. Many of the studies were researched for effectiveness of the manual therapy as the secondary treatment. Conclusions Reviewing the recent clinical studies for manual therapy on temporomandibular joint disorder and figuring out the strong points and weak points of those studies are necessary to future studies. It is anticipated that this review would benefit the in-depth treatments for temporomandibular joint disorder in terms of Korean medicine.

Positional and morphologic changes of the temporomandibular joint disc using magnetic resonance imaging

  • Purpose: To evaluate displacement and morphologic changes of the temporomandibular joint (TMJ) disc in patient with internal derangement using magnetic resonance imaging (MRI). Materials and Methods: One hundred and forty five MR images of TMJs in 73 patients were evaluated. Positional and morphologic changes of the TMJ discs were assessed. Lateral or medial disc displacement was also evaluated on coronal images. Results: Among 63 discs with anterior disc displacement, 37 discs were assessed as a biconcave disc and 21 as a deformed disc. Rotational disc displacement was observed in 35 discs. Anteromedial disc displacement was observed in 29 discs, and anterolateral direction in 6 discs. Among 35 rotational displacement, 5 biconcave discs and 21 deformed discs were observed. Conclusion : Rotational and sideways displacement of TMJ discs were found to be common and an important aspect of internal derangement. This study also suggests that sagittal and coronal images of the TMJ have complementary abilities for an assessment of joint abnormality.

Synovial Chondromatosis in Temporomandibular Joint

  • Synovial chondromatosis (SC) in temporomandibular joint is a rare benign disorder characterized by cartilaginous metaplasia of the mesenchymal remnants of the synovial tissue. The etiology of the disease is unclear but may be associated with trauma, overuse, local infection, and embryologic disturbance. SC does not spontaneously resolve and respond to nonsurgical treatment. SC should be differentially diagnosed with other temporomandibular joint disorders such as arthralgia or osteoarthritis because surgery should be done for managing it. However, primary diagnosis of SC is not easy because of nonspecific symptoms and signs. For the patients with unsuccessful conservative treatment response, especially accompanied by crepitus, preauricular swelling or posterior open bite, computed tomography/cone-beam computed tomography or magnetic resonance imaging should to be performed to exclude SC. We discussed the importance of the early diagnosis and surgical treatment of SC from this case.

구강내과영역에서 측두하악장애 환자의 Spiral Taping 치료 (The Spiral Taping Treatment on Temporomandibular Disorder in oral Medicine)

  • 최근 저작, 개구장애, 두경부의 동통 및 관절 잡음을 나타내는 턱관절 기능장애 환자가 점차 증가 추세에 있다. 그러나 대부분의 치료가 기간대비효과 및 지속성이 떨어지는 단점을 가지고 있다. 또한, 고비용과 장기간의 치료와 구강 장착물의 이물감으로 인한 불편감 등으로 좋은 치료법으로 선호되기 힘든 상황이다. 테이핑 치료방법은 시행하기가 비교적 간단하고 부작용이 없어 안정성이 높으면서도 치료효과가 탁월하다. 본 연구에서는 최근 비교적 쉽게 환자를 치료하고, 치료에 따른 부작용 없는 테이핑 요법 중 Spiral Taping법을 이용하여 턱관절의 통증과 관절가동범위의 제한을 일으키는 근육에 비신축성 테이프를 적용하여 측두하악장애의 효율적인 치료의 기본적인 방향을 설정하여 구체적인 치료방법과 치료절차에 대해 알아보고자 하였다.

일부 남자 고등학생의 측두하악장애 관련 증상의 유병률과 구강악습관 및 컴퓨터 이용시간과의 관계 연구 (Relationship between the prevalence of temporomandibular joint disorders in some male high school students and computer utilization)

  • Objectives : The purpose of the study is to verify the correlation between teenagers' use of internet and temporomandibular joint disorders(TMJD) because TMJD patients gradually increase in high school students. Methods : Subjects were 221 male high school students attending liberal arts and commercial schools from 9 to 21 of April, 2012. Students completed self-reported questionnaire. Results : TMJD symptoms included crackling joint (32.1%), headache (18.6%), malocclusion (8.1%), and jaw fatigue and muscular aching (7.2%). Students having at least one symptom accounted for 49.8%. Students having at least one poor habit accounted for 83.7% and 34.0% of the students used internet for 3 hours.. Score of students experiencing shoulders and chest pain was $5.13{\pm}1.10$ (p<0.05). Score of TMD afflicting area revealed $4.67{\pm}2.73$ (p<0.05) in mastication pain ($4.13{\pm}2.73$ (p<0.05)) due to long hours of internet use. Score of jaw fatigue related to muscular pain was $4.31{\pm}1.96$ (p<0.01). Duration of computer use was related to perceived TMJD symptoms (r=.153). Long duration of computer use was related to poor oral cavity habits (r=.157). Conclusions : Male high school students can perceive slight TMD which needs self management and regular check-ups and try to prevent TMD by utilizing measurement tools on TMD. Also, this study helps to prevent from worsening TMD by shortening hours of computer utilization.

Effect of pain on cranio-cervico-mandibular function and postural stability in people with temporomandibular joint disorders

  • Background: Neck and jaw pain is common and is associated with jaw functional limitations, postural stability, muscular endurance, and proprioception. This study aimed to investigate the effect of jaw and neck pain on cranio-cervico-mandibular functions and postural stability in patients with temporomandibular joint disorders (TMJDs). Methods: Fifty-two patients with TMJDs were included and assessed using Fonseca's Questionnaire and the Helkimo Clinical Dysfunction Index. An isometric strength test was performed for the TMJ depressor and cervical muscles. The TMJ position sense (TMJPS) test and cervical joint position error test (CJPET) were employed for proprioception. Total sway degree was obtained for the assessment of postural stability. Deep neck flexor endurance (DNFE) was assessed using the craniocervical flexion test. The mandibular function impairment questionnaire (MFIQ) was employed to assess mandibular function, and the craniovertebral angle (CVA) was measured for forward head posture. Results: Jaw and neck pain negatively affected CVA (R2 = 0.130), TMJPS (R2 = 0.286), DNFE (R2 = 0.355), TMJ depressor (R2 = 0.145), cervical flexor (R2 = 0.144), and extensor (R2 = 0.148) muscle strength. Jaw and neck pain also positively affected CJPET for flexion (R2 = 0.116) and extension (R2 = 0.146), as well as total sway degree (R2 = 0.128) and MFIQ (R2 = 0.230). Conclusions: Patients with painful TMJDs, could have impaired muscle strength and proprioception of the TMJ and cervical region. The jaw and neck pain could also affect postural stability, and the endurance of deep neck flexors as well as mandibular functions in TMJDs.

Observation of bilaminar zone in magnetic resonance images of temporomandibular joint

  • Purpose: To observe the relationship of bilaminar zone of temporomandibular joint retrodiscal tissues to the disc condition. Materials and Methods : The upper and lower stratum of bilaminar zone were identified on magnetic resonance open mouth images of 148 joints from 74 patients with disc displacements. Results: Both strata were identifiable in 105 joints which had disc displacement with reduction. Lower stratum was not identifiable in 35 joints which had disc displacement without reduction but 12 of 35 had hyalinized posterior attachment where the disc was. The 8 joints which had partial disc displacement without reduction showed identifiable lower stratum at the reducing site which was medial. Conclusion: Disruption or no identification of lower stratum which corresponds to the condylar portion of posterior attachment may be the sign of disc displacement without reduction.

Correlation between internal derangement and osteoarthrosis in the temporomandibular joint using magnetic resonance imaging

  • Purpose: To evaluate the relationship between internal derangement and osteoarthrosis in the temporomandibular joint (TMJ) using magnetic resonance imaging (MRI). Materials and Methods: One hundred and six MR images of TMJs in 53 patients were evaluated. Disc displacements and osseous changes of the TMJs were assessed. Lateral and rotational disc displacements were also evaluated on coronal images. Results: No significant differences in the frequency of osseous changes of the TMJs between disc displacement with reduction and disc displacement without reduction groups were found. The erosion of the condylar head and the sclerosis of the articular eminence were more frequent in the internal derangement group than in the no disc displacement group. The flattening was the most frequently observed osseous change of both the condylar head and articular eminence. Conclusion : The relationship between internal derangement and osteoarthrosis is obscure, but it is thought that both disorders adversely affect each other.

악관절장애 환자 치험 2례 (Two Cases of Temporomandibular Joint Disorder)

  • Temporomandibular joint disorder is quite common dental problem. Research has shown that 5 to 15$\%$ of the population have a TMJ disorder requiring some type of treatment. TMJ disorder's symptoms are pains in the jaw and muscles of your face, limitation of opening or closing jaw and noises in jaw joint such as clicking or popping sounds. Two patients with TMJ symptoms visited our clinic. They complained pain, limitation of jaw opening and clicking sound. We treated them by stimulating trigger points on the TMJ muscle with acupuncture and had a good effects. The symptoms is reduced or disappeared.
