Does the Pain Associated with Temporomandibular Disorder Increase on Rainy Days?

  • Purpose: Patients who suffer from rheumatic arthritis, fibromyalgia, other various inflammatory diseases and musculoskeletal disorders, which are all similar to temporomandibular disorders (TMD), have been complaining about changes in the level and type of pain in response to changes in weather conditions for a long time. Through an investigation about pain perception in TMD patients in response to weather conditions, our primary objective was to develop base materials for future studies on change in pain in response to meteorological factors. Methods: Among patients who presented with TMD to Department of Oral Medicine, Pusan National University Dental Hospital from August to October 2016, one hundred consecutive TMD patients diagnosed with TMDs according to Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) were recruited for the study and 28 patients were excluded according to exclusion criteria. Survey was done with the questionnaire and investigated whether there was any difference in incidence and level of pain in TMD patients between non-rainy and rainy days. Results: Among a total of 72 samples, 4 patients reported change in pain on rainy days rather than non-rainy days. Two patients from chronic group (joint and complex subgroup) reported increased pain on rainy days rather than non-rainy days but it was not statistically significant (p>0.05). One patient from chronic/muscle group reported the change in pain characteristics while pain intensity remained unchanged. One patient from acute/complex group reported decreased pain intensity. In comparison of the patients who reported increased pain on rainy days between acute and chronic groups, there were two reported cases and were both from chronic group only. There was a significantly higher chance of reporting increased pain on rainy days in chronic group than acute group (p<0.001). Conclusions: It is considered that TMD patients couldn't perceive the change in pain well in response to weather change on rainy days but some chronic patients could perceive the increase in pain in rainy days.

  • The aim of this study is to evaluate the relative signal intensity of TMJ retrodiscal tissue in T2-weighted MRI as diagnostic marker of temporomandibular disorder(TMD). 58 temporomandibular joints from 29 TMD patients (14 men & 15 women) were evaluated. The relative signal intensity of retrodiscal tissue in T2-weighted MRI was referenced to brain gray matter same size of the region of interest(ROI). The collected data was compared to disc positions (normal, anterior disc displacement with reduction, anterior disc displacement without reduction), the presence of joint effusion. The relative signal intensity of retrodiscal tissue was significantly increased when the disc was displaced without reduction. And the relative signal intensity of retrodiscal tissue was significantly increased when joint effusion was present. The results suggest that evaluating the relative signal intensity of TMJ retrodiscal tissue using a T2-weighted MRI is valuable as a non-invasive tool for diagnosing the procession of TMD.

임신부에서 측두하악장애의 진단과 치료 (Diagnosis and Treatment of Temporomandibular Disorder in Pregnant Women)

  • In this case report, I discussed the diagnosis and treatment of two pregnant women with temporomandibular disorders(TMD) who visited the Department of Oral Medicine, PNUH. Also, I reviewed some investigations of diagnosis and treatment of TMD in pregnant women. The obtained results were as follows; 1. No single X-ray diagnostic procedure for TMD results in radiation dose that threatens the well-being of the developing embryo and fetus. 2. Most non-steroidal anti-inflammatory drugs(NSAIDs) have commonly used because these drugs are considered to be nonteratogenic, but these agents are not recommended for routine use after 3rd trimester. 3. Electro-acupuncture stimulation therapy(EAST) is contraindicated for 1st trimester, and ultrasonic deep heat therapy, microwave deep heat therapy, low level laser therapy, myo-monitor are not contraindicated for pregnant women but clinician must consider some risk of adverse fetal effects. 4. The occlusal stabilization splint may be used for pregnant women, if it is fabricated indirectly. 5. Surgical treatment is contraindicated for pregnant women.

측두하악장애 환자에서 치아 교모도에 의한 연령감정의 유효성 (The Effectiveness of Age Estimation Method by Occlusal Tooth Wear in Temporomandibular Disorder(TMD) Patients)

  • 본 연구는 측두하악장애 환자에서 이갈이의 유무 및 심도에 따른 추정연령과 실제 연령과의 차이(추정오차)를 평가하여 그 관계를 살펴보고, 측두하악장애 환자의 이갈이가 치아 교모도를 통한 연령감정에 미치는 영향에 대해 알아보고자 하였다. 교모도를 이용한 Takei의 연령감정법을 163명(대조군: 56명, 환자군: 107명)의 대상자에게 적용하였다. 저자는 추정연령과 실제연령의 차이를 통해 대상자의 교모도를 분석하여 다음과 같은 결론을 얻었다. 1. 연령에 따라서는 측두하악장애 환자군이 대조군에 비해 모든 연령군에서 교모도가 더 큰 경향을 나타냈으며, 특히 20대에서 유의성 있는 차이가 나타났다. 2. 성별에 따라서는 측두하악장애 환자군이 대조군에 비해 교모도가 더 큰 경향을 나타냈으며, 특히 남성에서 유의성 있는 차이가 나타났다. 3. 이갈이가 있는 측두하악장애 환자군은 대조군에 비해 교모도가 더 큰 경향을 나타냈으며 유의성 있는 차이를 나타냈다. 4. 측두하악장애 환자군간에는 이갈이 심도와 교모도에 대한 유의성 있는 상관관계를 발견하지 못하였다. 5. Takei에 의한 방법은 한국에서 여전히 유용한 연령감정법으로 평가된다. 따라서 측두하악장애 환자에서 교모도를 이용한 연령감정의 경우, 이갈이 심도에 대한 평가와 연령, 성별, 지역에 따른 차이에 대해서도 고려해야 할 것으로 생각된다.

The Analysis of Incidental Findings on Temporomandibular Joint Magnetic Resonance Imaging

  • Purpose: The aim of this study was to investigate the types and frequency of the various incidental findings (IFs) on magnetic resonance images (MRI) taken from the patients with temporomandibular disorder (TMD) symptoms. Materials and Methods: Temporomandibular joint (TMJ) MRI taken from 1,013 patients with TMD symptoms were evaluated retrospectively. IF was defined as imaging features that were accidentally or unexpectedly found, rather than degenerative bony changes of TMJ complex or disc derangement. They were classified into two groups as TMJ site-specific findings and unexpected findings at other regions. The frequency of the sub groups was analyzed. Result: A total of 26 (2.57%) cases with IFs were classified into 13 cases with TMJ site-specific findings and 13 cases with unexpected findings at other region. TMJ site-specific findings included synovial chondromatosis in 6 cases, synovial cyst in 6 cases and osteochondroma in one case. Unexpected findings included salivary gland tumor in 3 cases, developmental cyst in 3 cases, vascular malformation in 2 cases, mastoiditis in 4 cases and sialadenitis on parotid gland in one case. Conclusion: When diagnosing TMD through TMJ MRI, clinicians should carefully read the image, considering the possibility of IFs because TMJ MRI can provide pathologic information in TMJ region and other oral and maxillofacial region.

측두하악 증후군에 대한 침치료 군과 추나치료 군의 임상적 고찰 (Comparative Clinical Study between the Acupuncture Treatment and the Chuna Treatment on Temporomandibular Disorder)

  • Objectives : The purpose of study was to compare and analyze the effects of the distance Acupuncture and the Chuna on Temporomandibular Disorder. Methods : Clinical studies were done on 31 patients who were treated with Temporomandibular Disorder at Dept. of Acupuncture & Moxibusition of Hospital of Ha-na Oriental Medicine from May 23 2005 to Apr 30 2006. Subjects were divided Into two group, Distance acupuncture group and Chuna group. In Distance acupuncture group, we treated patients with Wijungyug(胃正格) or Damjungyug(膽正格). In Chuna group, we treated patients with Distraction & Translation technique. For evaluating Function of Temporomandibular joint of Patients, we use the Modified Craniomandibular Index(mCMI) before treatment and after 6th treatment. Results : In Mandibular Movement(MM) of patients, the patients of Acupuncture Group were more effective than Chuna group. In TMJ noise(TN) of patients, the patients of Chuna Group were more effective than Acupuncture group. Conclusions : The Distance acupuncture may be better effect on Mandibular Movement and The chuna maybe better effect on TMJ noise.

두개하악장애 환자의 하악운동에 관한 연구 (A STUDY ON THE MANDIBULAR MOVEMENTS OF TMD PATIENTS)

  • The purpose of this study was to evaluate the mandibular movements of TMD patients comparing to normal persons. Sirognathograph was used to measure five parameters of mandibular movements of twenty normal persons and eight TMD patients. Five parameters were (1) Maximum opening during maximum opening and closing, (2) Mean velocity during maximum opening and closing, (3) Maximum opening during unilateral chewing, (4) Mean velocity during unilateral chewing, (5) Consistency of mandibular movement during unilateral chewing. Based on above results, new Mandibular Movement Index(M.M.I.) was formulated and compared to Helkimo's Clinical Dysfunction Index by measuring two indices before treatment and 2 weeks, 4 weeks, 6 weeks, 8 weeks after treatment. The conclusions were as follows: 1, The amounts of maximum opening and mean velocity during maximum opening and closing of TMD patients were less than those of normal persons(p<0.01). 2. The amounts of maximum opening and mean velocity during unilateral chewing of TMD patients were less than those of normal persons(p<0.05) (P<0.01). 3. Although TMD patients showed limited mandibular movements during unilateral chewing, the consistency of mandibular movements was better than that of normal persons(p<0.05). 4. Both mandibular movement index and Helkimo's Clinical Dysfunction Index, were useful in diagnosing TMD patients.

Comparative analysis of craniofacial asymmetry in subjects with and without symptoms of temporomandibular joint disorders: a cross-sectional study

  • Objectives: The aim of the study was to quantify and compare craniofacial asymmetry in subjects with and without symptoms of temporomandibular joint disorders (TMDs). Materials and Methods: A total of 126 adult subjects were categorized into two groups (63 with a TMDs and 63 without a TMDs), based on detection of symptoms using the Temporomandibular Joint Disorder-Diagnostic Index (TMD-DI) questionnaire. Posteroanterior cephalograms of each subject were traced manually and 17 linear and angular measurements were analyzed. Craniofacial asymmetry was quantified by calculating the asymmetry index (AI) of bilateral parameters for both groups. Results: Intra- and intergroup comparisons were analyzed using independent t-test and Mann-Whitney U test, respectively, with a P<0.05 considered statistically significant. An AI for each linear and angular bilateral parameter was calculated; higher asymmetry was found in TMD-positive patients compared with TMD-negative patients. An intergroup comparison of AIs found highly significant differences for the parameters of antegonial notch to horizontal plane distance, jugular point to horizontal plane distance, antegonial notch to menton distance, antegonial notch to vertical plane distance, condylion to vertical plane distance, and angle formed by vertical plane, O point and antegonial notch. Significant deviation of the menton distance from the facial midline was also evident. Conclusion: Greater facial asymmetry was seen in the TMD-positive group compared with the TMD-negative group. The mandibular region was characterized by asymmetries of greater magnitude compared with the maxilla. Patients with facial asymmetry often require management of temporomandibular joint (TMJ) pathology to achieve a stable, functional, and esthetic result. Ignoring the TMJ during treatment or failing to provide proper management of the TMJ and performing only orthognathic surgery may result in worsening of TMJ-associated symptoms (jaw dysfunction and pain) and re-occurrence of asymmetry and malocclusion. Assessments of facial asymmetry should take into account TMJ disorders to improve diagnostic accuracy and treatment outcomes.

  • 본 연구는 오늘날 한국에서, 현실적으로 많은 치과의사, 한의사, 물리치료사들이 관심을 갖고 치료를 시도하고는 있으나 그 학문적 근거가 미약하여 논란의 소지가 있는, 교합과 측두하악장애 및 관련된 전신 증상과의 관계를 규명하기 위한 사전 작업으로 진행된 문헌 검토에 그 바탕을 두고 있다. 가장 대표적인 다섯가지 이론인 (1) Craniosacral mechanism (2) Osteopathy (3) Myodontics (4) Chirodontics (5) Dental Distress Syndrome and Quadrant Theorem 들에 대한 조사를 토대로 각 이론에 대한 대략적인 개요를 소개하며, 향후 계속적으로 발표될 논문에서 각각의 이론에 대한 구체적인 배경과 근거를 알아보고자 한다.

Internal derangement as a predictor of provoked pain on mouth opening: A magnetic resonance imaging study

  • Purpose: This study investigated the relationship between pain and internal derangement in temporomandibular disorder(TMD) patients using magnetic resonance imaging (MRI). Materials and Methods: This study analyzed 356 TMD patients (712 temporomandibular joints [TMJs]). The inclusion criteria were the presence of spontaneous or provoked pain on one or both TMJs and having undergone MRI. The patients with provoked pain were divided into 3 groups: pain on palpation, pain on mouth opening, and pain on mastication. MRI was performed using a 1.5-T scanner. T1- and T2-weighted parasagittal and paracoronal images were obtained. According to the findings on the T1-weighted images, another 3 groups were created based on internal derangement: normal, disc displacement with reduction, and disc displacement without reduction. The MRI findings were independently interpreted by 2 experienced oral and maxillofacial radiologists at 2 different times. Statistical analysis was performed by the chi-square test using SPSS (version 12.0; SPSS Inc., Chicago, IL, USA). Results: Provoked pain on mouth opening was found to be correlated with internal derangement in TMD patients (P<.05). However, spontaneous pain and provoked pain on palpation or mastication were not associated with internal derangement(P>.05). Conclusion: These results suggest that internal derangement was a significant predictor of provoked pain on mouth opening.
