Systemic Review of RCTs focusing on Chronic Fatigue

  • Objective: This study aimed to build an overview of randomized clinical controlled trials (RCTs) for chronic fatigue-related symptoms to extract the useful data for management of patients and development of therapeutics using Korean traditional medicine in the future. Methods: All RCT-derived papers for chronic fatigue-related symptoms were collected via PubMed Database. We surveyed elementary information of RCTs such as clinical question, study design, and its quality and results. Results: A total of fifty-three RCTs met these review criteria. Most of the RCTs were performed in Western countries, particularly the UK and USA. The major portion of RCTs focused on chronic fatigue syndrome using immune modulators, psychotherapeutic and anti-depressants. Five RCTs using complementary and alternative medicine, including herbal remedies, showed positive results. Conclusions: Fatigue-related symptoms are a main target of Oriental medicine. This study provides helpful information for planning clinical study of chronic fatigue-related symptoms using traditional Korean medicine.

A Case of Painful Trigeminal Neuropathy as a Complication of Progressive Systemic Sclerosis (진행성 전신성 경화증환자에게서 보인 동통성 삼차신경병증 1예)

  • Progressive systemic sclerosis (PSS) is a multi-systemic disorder characterized by abundant fibrosis of the skin, blood vessels, and visceral organs. But it rarely affects the peripheral nervous system. We report a 36-year-old man of painful trigeminal neuropathy as a complication of PSS. He was referred from Rheumatology for the evaluation of abruptly developed bilateral facial pain. He had facial hypesthesia and paresthesia on neurologic examinations. In the blink reflex, ipsilateral and contralateral R1 and R2 responses were not detected during bilateral supraorbital stimulation. But normal latency and CMAP amplitude of facial NCV were found. Under the impression of trigeminal neuropathy caused by PSS, steroid therapy was tried, and his clinical symptoms and electrophysiologic findings were improved. PSS could be the cause of the painful trigeminal neuropathy.

Determinants of Plant Growth-promoting Ochrobactrum lupini KUDC1013 Involved in Induction of Systemic Resistance against Pectobacterium carotovorum subsp. carotovorum in Tobacco Leaves

  • The plant growth-promoting rhizobacterium Ochrobactrum lupini KUDC1013 elicited induced systemic resistance (ISR) in tobacco against soft rot disease caused by Pectobacterium carotovorum subsp. carotovorum. We investigated of its factors involved in ISR elicitation. To characterize the ISR determinants, KUDC1013 cell suspension, heat-treated cells, supernatant from a culture medium, crude bacterial lipopolysaccharide (LPS) and flagella were tested for their ISR activities. Both LPS and flagella from KUDC1013 were effective in ISR elicitation. Crude cell free supernatant elicited ISR and factors with the highest ISR activity were retained in the n-butanol fraction. Analysis of the ISR-active fraction revealed the metabolites, phenylacetic acid (PAA), 1-hexadecene and linoleic acid (LA), as elicitors of ISR. Treatment of tobacco with these compounds significantly decreased the soft rot disease symptoms. This is the first report on the ISR determinants by plant growth-promoting rhizobacteria (PGPR) KUDC1013 and identifying PAA, 1-hexadecene and LA as ISR-related compounds. This study shows that KUDC1013 has a great potential as biological control agent because of its multiple factors involved in induction of systemic resistance against phytopathogens.

  • Objectives : The purpose of this study is to set up the standard for Classification and Diagnosis of Low Back Pain by through collecting bibliographic study on Low Back Pain and related general symptoms in classical literatures Methods : We investigated the contents of classical literatures about chronic low back pain and related general symptoms. With this contents, we established a systemic classification and diagnostic standard for Low back pain. Results : There are many different opinions on classification of low back pain and general symptoms in oriental medicine classical literature. Every opinion is reasonable, so it is difficult to establish a diagnosis of Low back pain. But it is necessary to set up the one-systemic classification and diagnostic technique of Low back pain. Conclusions : We conclude that the Ten type Low back pain classification of in Dong-Eui-Bo-Gam is a reasonable standard for diagnostic classification.

Relationship of Stress, Oral Habits and TMJ Symptoms in 20-30 ages Adults (20-30대 일부 성인의 스트레스, 구강악습관 및 턱관절 증상의 연관성)

  • Objectives : The purpose of this study was to examine the relationship of stress symptoms, oral habits and temporomanbibular joint symptom among 20-30 ages adults. And it's also meant to investigate the direct and indirect influence of these factors by using a path model to determine their causal relationship. Methods : The subjects in this study were 287 selected 20-30 ages adults, on whom a survey was conducted from June 15 to July 10, 2014. The data were analyzed using SPSS 18.0(SPSS 18.0 K for window, SPSS Inc USA) and IBM SPSS Amos 18.0(SPSS Inc, Chicago, IL, USA). Results : There were significant differences in oral habits, TMJ symptoms and stress according to gender. There were significant differences in oral habits, TMJ symptoms, physical and psycho-emotional symptoms according to Systemic disease. And stress, physical, psycho-emotional symptoms, oral habits and TMJ symptoms were correlated to one another. Stress exerted a direct influence on physical, psycho-emotional symptoms, and psycho-emotional symptoms had a direct impact on physical symptoms, oral habits and TMJ symptoms. Physical symptoms exercised a direct influence on oral habits and TMJ symptoms, and oral habits affected TMJ symptoms in a direct way. physical, psycho-emotional symptoms and oral habits served as parameters that produced partial mediation effects, and the two factors had an indirect impact on TMJ symptoms. Conclusions : It's found that stress exerted direct and indirect influence on oral parafunction and TMJ symptoms. Like other diseases, oral habits and TMJ symptom that stem from stress is likely to lead to chronic diseases if the two are not noticed at the right time. Therefore individual people should try to get rid of stress in a manner to be appropriate for their own characteristics in order to maintain their oral health.

A Survey on Parent's Recognition and Utilization Patterns of Oriental Medical Care of Preschool Students in Seong-Dong district (성동구내 어린이집 원아 부모의 소아 한방치료에 대한 인식 및 이용에 관한 조사 연구)

  • Objectives: The purpose of this study is to examine the parental recognition and utilizing patterns of oriental medical care in children who live in Seong-Dong district. Methods 802 day-care center children's parents participated in the survey on recognition and utilizing patterns of oriental medical care, and 702 of the surveys were analyzed. Results: 1. 38.33% has received previous oriental medical treatments and most were received at the children's age under 12-24 months. Majority of the treatments took place in the clinical settings. By recommendation, some also received their treatment at the medical centers. One of the reasons why they receive oriental medical treatments was that they prefer to balance their treatment with the orental medical treatments along with the western's. 2. 53.85% of those who received oriental medical treatments were satisfied with their treatments. The effective treatment results were the major reason for the satisfaction. 3. 49.56% of those who had oriental medical treatments chose specific oriental medical facilities for the children's ailment. Most specific oriental medical facilities were local oriental medical clinics. Reputation fame were the main reason in choosing the clinics. 4. In 2009, 55.26% had received care in the specific oriental medical clinics for the diseases, such as respiratory symptoms, systemic symptoms, gastrointestinal symptoms, skin problems, neurological and psychiatric disease for infants, and respiratory symptoms, gastrointestinal symptoms, systemic symptoms, skin problems, neurological and psychiatric symptoms, urinary symptoms and musculoskeletal symptoms for toddlers. 5. The average duration of herbal treatment were 7-14 days, preferably 4-6 days, and preferred number of medical visits were once a week on Saturdays and preferred day for night cares were on Friday nights. Average treatment fee per treatment was less than 10,000 won which coincided with the preferred treatment fee per visit. 6. The very first medical institutions chosen by parents once their children are ill are the following; Western medical institutions, western medical clinics, pharmacies, oriental medical clinics, health centers, and lastly, oriental medical institutions. The major concerns in receiving oriental medical cares include pesticides in herbs and hazardous substances in heavy metals. Preferred form of oriental medicine was decoction, the negative feedback of oriental medicine was mostly due to the bitter taste of the oriental herbal decoctions. Preferred frequency of intake was twice a day with less than 10cc~20cc per single intake. Conclusions: Taken all together, we conclude that the growing recognition of Oriental Pediatric medicine is needed among parents.

Severe dapsone hypersensitivity syndrome in a child

  • Dapsone (4,4'-diaminodiphenylsulfone, DDS), a potent anti-inflammatory agent, is widely used in the treatment of leprosy and several chronic inflammatory skin diseases. Dapsone therapy rarely results in development of dapsone hypersensitivity syndrome, which is characterized by fever, hepatitis, generalized exfoliative dermatitis, and lymphadenopathy. Here, we describe the case of an 11-year-old Korean boy who initially presented with high fever, a morbilliform skin rash, generalized lymphadenopathy, hepatosplenomegaly, and leukopenia after 6 weeks of dapsone intake. Subsequently, he exhibited cholecystitis, gingivitis, colitis, sepsis, aseptic meningitis, disseminated intravascular coagulation, syndrome of inappropriate antidiuretic hormone secretion, pneumonia, pleural effusions, peritonitis, bronchiectatic changes, exfoliative dermatitis, and acute renal failure. After 2 months of supportive therapy, and prednisolone and antibiotic administration, most of the systemic symptoms resolved, with the exception of exfoliative dermatitis and erythema, which ameliorated over the following 4 months. Agranulocytosis, atypical lymphocytosis, aseptic meningitis, and bronchiectatic changes along with prolonged systemic symptoms with exfoliative dermatitis were the most peculiar features of the present case.

Additional Biological Characteristics of TMV Strains Isolated from Tobacco, Tomato and Pepper Plants (담배, 토마토 및 고추에서 분리한 TMV 계통의 추가적인 생물학적 특성)

  • Tobacco (Nicotiana tabacum) cultivars including NC 82 and KF 114, and Datura stramonium, Physalis floridana, Gomphrena globosa, and Chenopodium spp. were added to the previous host plants tested for the further examination on the biological characteristics of tobacco mosaic virus (TMV) strains isolated from tobacco (TMV-Common), tomato (TMV-Tomato), and pepper (TMV-Pepper), In TMV-Tomato and TMV-Pepper, different symptoms were noted in P. floridana (no symptom development), and NC 82 (local lesion production on the inoculated leaves) from TMV-Common with which systemic mosaic symptoms were developed. Local lesions were developed in KF 114, D. stramonium, G. globosa, and Chenopodium spp by TMV-Common and TVM-Tomato, while no symptom was observed in KF114 and G. globosa. Also the number and size of local lesions were smaller in KF 114 than Xanthi-nc tobacco (local lesion host) infected with TMV-Tomato. Systemic necrosis was induced in Xanthi-nc and KF 114 when infected with TMV-C at high temperature, but not with the other strains.

Incidentally found unicentric plasma cell variant Castleman's disease in mesentery: focus on ultrasonography and CT findings (우연히 발견된 장간막의 단중심성 형질세포형 Castleman병 1예: 초음파와 CT 소견 중심으로)

  • Castleman's disease is a benign lympho-proliferative disorder that commonly occurs in mediastinum. It is known that the disease rarely occurs in mesentery. Most localized abdominal Castleman's diseases are histologically hyaline vascular type. The contrast-enhanced CT in patient with hyaline vascular type Castleman's disease shows a well-defined mass with homogenously intense enhancement. On the other hand, the patient with plasma cell variant has systemic symptoms, but not specific imaging features. We report a unicentric plasma cell variant Castleman's disease in mesentery nearby superior mesenteric artery as presenting a single mass, not accompanied by systemic symptoms with similar characteristics to hyaline vascular type.

The influence of stress on oral mucosal disease, dry mouth and stress symptoms in adults (성인의 스트레스가 구강 점막 질환, 구강 건조감 및 스트레스 증상에 미치는 영향)

  • Objectives : The aim of the study is to investigate the influence of the stress of adults on their oral mucosal diseases, dry mouth and physical, mental stress symptoms. Structured equation model (SEM) was used to analyze the hypotheses of the study. Methods : The subjects were 500 adultsfrom July 1 to December 31, 2012. The data were analyzed using SPSS 18.0 (SPSS 18.0 K for window, SPSS Inc USA) and IBM SPSS Amos 18.0 (SPSS Inc, Chicago, IL, USA) set at the level of significance as 0.05. Results : The level of stress had a direct influence on oral mucosal diseases, and oral mucosal diseases affected stress symptoms directly. The level of stress had a significant impact on stress symptoms, and that exercised an indirect influence on stress symptoms through the medium of oral mucosal diseases and dry mouth. The level of stress affected dry mouth in a direct effects, and dry mouth had a direct impact on stress symptoms. Conclusions : The stress of adults had direct and indirect impacts on their oral health and systemic diseases. The oral health of adults should be promoted to let them stay healthy, and how to help them to get rid of their stress should be considered to improve their quality of life.
