A systematic review of the biological mechanisms linking physical activity and breast cancer

  • [Purpose] Epidemiological evidence has shown that leisure-time physical activity and structured exercise before and after breast cancer diagnosis contribute to reducing the risk of breast cancer recurrence and mortality. Thus, in this review, we aimed to summarize the physical activity-dependent regulation of systemic factors to understand the biological and molecular mechanisms involved in the initiation, progression, and survival of breast cancer. [Methods] We systematically reviewed the studies on 1) the relationship between physical activity and the risk of breast cancer, and 2) various systemic factors induced by physical activity and exercise that are potentially linked to breast cancer outcomes. To perform this literature review, PubMed database was searched using the terms "Physical activity OR exercise" and "breast cancer", until August 5th, 2020; then, we reviewed those articles related to biological mechanisms after examining the resulting search list. [Results] There is strong evidence that physical activity reduces the risk of breast cancer, and the protective effect of physical activity on breast cancer has been achieved by long-term regulation of various circulatory factors, such as sex hormones, metabolic hormones, inflammatory factors, adipokines, and myokines. In addition, physical activity substantially alters wholebody homeostasis by affecting numerous other factors, including plasma metabolites, reactive oxygen species, and microRNAs as well as exosomes and gut microbiota profile, and thereby every cell and organ in the whole body might be ultimately affected by the biological perturbation induced by physical activity and exercise. [Conclusion] The understanding of integrative mechanisms will enhance how physical activity can ultimately influence the risk and prognosis of various cancers, including breast cancer. Furthermore, physical activity could be considered an efficacious non-pharmacological therapy, and the promotion of physical activity is probably an effective strategy in primary cancer prevention.

갱년기 증후군으로 인한 불면의 침 치료 무작위 대조군 최신 연구의 체계적 문헌 고찰과 메타 분석 (A Systemic Review of Recent Randomized Controlled Trials of Insomnia in Climacteric Syndrome Treated with Acupuncture)

  • Objectives: This study was aimed to review the effect of acupuncture for insomnia in climacteric syndrome. Methods: Researchers searched data through 5 online databases up to December 2021. The data was limited to randomized controlled trial studies on patients diagnosed with insomnia in climacteric syndrome. Results: 8 Randomized controlled trials were included in this study. 6 of studies were published in Chinese, in China. 2 were published in English. Intervention of treatment group were composed of simple acupuncture. Intervention of control group were made up with Estazolam, Alprazolam, Lorazepam and sham-acupuncture. Outcome measurements were Total effective rate, Pittsburgh Sleep Quality Index (PSQI), Kupperman index, Menopause Quality of Life scales (Men-QoL), the level of hormones (E2, FSH, LH) etc. In all outcome measurements, treatment group were more effective than control group and it was statistically significant. Conclusions: Acupunture therapy is effective for treating insomnia in climacteric syndrome. Pittsburgh Sleep Quality Index (PSQI) is most frequently used for sleep assessment indicator. 神門 (HT7) is the most frequently used acupoint for treatment group. However because insomnia in climacteric syndrome is easy to recur, additional long-term research is needed.

속발성 무월경의 한의학적 치료에 대한 무작위 대조 임상시험의 체계적 문헌 고찰 (A Systemic Review of Randomized Controlled Trials on Interventions of Korean Medicine for Secondary Amenorrhea)

  • Objectives: The purpose of this study is to review the recent clinical study trends on Korean Medicine treatment for Secondary Amenorrhea by analyzing randomized controlled trials. Methods: We searched relevant studies published from electric databases including 4 domestic databases and 3 foreign databases. Data retrieval was conducted on May 30, 2023. and the papers published from January 1, 2018 to May 30, 2023 were included. The risk of bias was assessed by using Cochrane's risk of bias tool. Results: 119 studies and 193 studies were searched in each domestic databases and foreign databases, and 9 studies were finally selected. The control group was all treated with only western medicine. Among the treatment group, 2 studies were treated with only oral herbal medicine, 1 study was treated with herbal medicine and acupuncture, 5 studies were treated with combined treatment of herbal medicine and western medicine, 1 study was treated with combined treatment of herbal medicine, acupuncture and western medicine. In all 9 studies, the total effective rate and cure rate were higher in the treatment group than those of the control group (p<0.05). Conclusions: Korean medicine alone or combined with western medicine is more effective for treating Secondary Amenorrhea compared to using western medicine alone.

여성 만성 변비의 침 치료 효과에 대한 체계적 문헌 고찰 (Systemic Review of Acupuncture Treatment for Female Chronic Constipation)

  • Objectives: The purpose of this systemic review is to overview and evaluate the efficacy of acupuncture in female patients with chronic constipation. Methods: We conducted a comprehensive search of the literature to identify randomized controlled trials that investigated the effects of acupuncture on female patients with chronic constipation. Key words searched included "Female Constipation", "Women Constipation", "Acupuncture" in database KISS, DBPIA, RISS, Cochrane Central, Scopus, Pubmed, CNKI. Results: Nine randomized controlled trials met our inclusion criteria and were included in this systematic review. Our analysis found that acupuncture was associated with significant improvements in spontaneous bowel movements, complete spontaneous bowel movements, stool consistency, and quality of life in female patients with chronic constipation. Acupuncture was generally well-tolerated, with few adverse effects reported. Conclusions: Our findings suggest that acupuncture treatment would be effective way to treat chronic constipation of female patients.

유방암의 보완대체의학적 치료에 관한 연구 (Systemic Review on Complementary and Alternative Medicine for Breast Cancer)

  • Purpose: The aim of this study was to review systemically journals on the studies for Complementary and Alternative Medicine in the treatment of breast cancer. Methods: Through medical websites, foreign clinical literatures about complementary and alternative medicines of breast cancer were searched. The cite used was http://www.Pubmed.gov. And then they were divided into three groups. Medication, Non-medication therapies and questionnaire reports. Results: 1. We researched 23 papers about herb medicines. Most of papers were about single herb and there were rarely about mixed composition. And there were papers about Ocimum gratissimum, elliptilimba, seeds of Livistona chinensis, golden feverfew which were not commonly used in Korea. 2. We researched 16 papers about acupuncture. Acupuncture had a possitive effect on such symptoms like flushing, nausea and vomitting and pain on upper limb caused by anticancer therapy or tamoxifen or surgery. 3. We researched 36 papers about questionnaire study. Most were about research for women who diagnosed as breast cancer or women after breast cancer surgery. Subjects were about proportion of using CAM, purpose of using it, most popular CAM therapy, satisfaction degree, and relation with age, aducation and social position. And most conclusion were that patient-doctor communication was needed. Conclusion: Afterwards we have to focus on realisitic clinical studies about breast cancer patients, especially postsurgery and people who takes anticancer therapy. And we have to be interest in acupuncture therapy on breast cancer patients.

소아 일과성 고관절 활액막염에 대한 한약의 효과: 체계적 문헌 고찰 및 메타분석 (Effect of Herbal Medicine on Transient Synovitis of Hip in Children: A Systemic Review and Meta-analysis )

  • Objective This study aimed to evaluate the effect of herbal medicines on transient hip synovitis (TSH) in children. Methods We searched 10 Korean, English, Japanese, and Chinese databases for studies published up to September 19, 2023. Randomized controlled trials (RCTs) evaluating the effect of herbal medicines on TSH levels in children were included. The Cochrane risk of bias was used to evaluate the quality of the studies. A meta-analysis was conducted according to outcome measurements, such as total effective rate (TER), visual analog score (VAS), time to pain relief, and hip joint mobility disorder disappearance time, using the Review Manager website. Results Eleven RCTs were included in this study. TER in the treatment group was significantly higher than in the control group. The duration of pain relief was significantly shorter in the treatment group than in the control group. The VAS score of the treatment group was significantly lower than that of the control group, although the statistical heterogeneity was substantial. The hip joint mobility disorder disappeared in the treatment group treated with external therapy, which was significantly shorter than in the control group. Regarding safety, two of the 11 studies reported that there were no adverse events. Conclusions Herbal medicines are effective in the treatment of TSH in children. However, the safety of herbal medicines should be carefully considered due to the lack of data.

악교정 수술시 보스민 거즈에 의해 나타난 폐부종 증례보고 (PULMONARY EDEMA INDUCED BY BOSMINE SOAKED GAUZE IN ORTHOGNATHIC SURGER)

  • 구강악안면영역은 그 해부학적 구조상 혈관 분포가 풍부하고 따라서 구강내로 접근하는 수술시 신체 다른 부위와 달리 출혈 소견을 많이 보이며 수술중 과다출혈의 방지와 지혈의 목적으로 epinephirne soaked gauze즉 bosrnine gauze의 사용이 널리 이루어지고 있고 또한 사용이 불가피하다고 할 수 있다. 그러나 이 부분은 혈관 분포가 풍부하므로 epinephrine의 systemic uptake의 가능성 또한 존재한다 할 수 있겠다. 고농도의 epinephrine은 이론상 혈관수축을 일으켜 순간적인 systemic volume overload의 상태를 야기시켜 폐부종 및 pulmonary effusion을 일으킬 수 있으나 아직까지 보고된 바 없었다. 악안면부위는 혈관공급이 많아 수술중 지혈이 용이하지 못해 bleeding control을 위해 또 습관적으로 bosmine 거즈의 packing이 행해지고 있지만 수술자에 의한 정확한 농도로 희석된 bosmine 거즈 사용이 필수적이다. 본원에서 양악 악교정 수술시 소독간호사에 의해 잘못 희석된 고농도의 bosmine 거즈를 상악후방에 packing하여 발생된 전신적 합병증으로써 폐부종이 발생한 바 bosmine 거즈의 사용시 사용부위에 따라 농도의 확인과 술자의 주의를 요한다고 생각된다.

  • 정상 폐로의 체순환 동맥 공급은 드문 것으로 알려져 있는데 저자들은 단순 흉부 X-선상 우연히 폐의 이상 음영을 발견하여 조영증강 전산화단층촬영상 좌하엽 저분절로 가는 체순환 동맥 분지를 진단한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

좌측 하폐엽에 공급하는 비정상적인 체동맥의 치료 (수술과 색전술의 비교) (Treatment of Systemic Arterial Supply to Lower Lobe of Left Lung (Operation vs. Embolotherapy): Comparison of Two Cases and Literature Review)

  • 폐에 체동맥이 분포하는 선천성 기형은 드문 질환으로 하폐엽의 기저분절에 폐동맥 대신 대동맥에서 1개 이상의 체동맥이 나오는데 정상적인 기관지와 연결 유무에 따라 폐 분획증과 좌측 폐동맥 이상 기시증(Anomalous Origin of Left Pulmonary Artery, AOLPA)으로 구분된다. 한일병원 흉부외과에서는 하행 흉부 대동맥에서 시작되는 비정상적인 체동맥을 가진 두 예의 폐동맥 기형을 경험하고, 1예는 기저분절절제술을, 다른 예는 동맥색전술을 시행하였고, 2년간 관찰한 결과 양호하였다. 이 결과를 바탕으로 외과적 수술이 이 기형의 근본적인 치료법이지만, 전신상태가 불량하거나 폐기능이 저하된 고위험군에서 동맥색전술도 선택적으로 시행할 수 있는 가능한 치료법으로 생각된다.

A Systemic Review of Body Awareness Therapy in Patients with Musculoskeletal Disorders

  • This study aimed to systematically review the available body awareness interventions in patients with musculoskeletal disorders. A total of 3,462 studies were searched in relevant databases using the search keywords, and six studies that met the selection criteria after review were included in the final analysis. The qualitative level of the studies was classified using the five levels of evidence-based classification, and systematic analysis was conducted according to PICO model (patient/population, intervention, comparison and outcomes). Of the six studies included in the final analysis, there were one, three, and two randomized controlled trials, non-randomized two-group studies, and case reports, respectively. Body awareness therapy was applied to patients with idiopathic scoliosis, non-specific musculoskeletal disorder, hip osteoarthritis, and amputation, who showed positive improvements after the interventions. Our results suggested that body awareness therapy is an important intervention with positive effects on the recovery of patients with musculoskeletal disorders.
