StrokeMed: an integrated literature database for stroke and the differentiation of stroke syndrome

  • Complex diseases, such as stroke and cancer, have two or more genetic influences and are affected by environmental factors, which complicate them. Due to the complex characteristics of these diseases, we must search and study comprehensive literature-based article resources. Some disease-related literature databases have been developed through specialized journal issues or major websites. Most of them, however, are scattered throughout a website, and users encounter difficulties in finding accurate and comprehensive information easily and quickly. We developed StrokeMed, an integrated literature database for stroke and the differentiation of stroke syndrome. The system allows users to explore PubMed search results, categorized by MeSH (Medical Subject Headings), and the differentiation of stroke syndrome in Oriental medicine. StrokeMed collects data from important sites, such as PubMed, Scirus, and Scopus, automatically to maintain higher-quality and updated content. Currently, the system indexes more than 20,000 PubMed abstracts that are related to stroke, stroke etiology, and Oriental medicine. The system provides valuable literature information to the scientific and medical fields in stroke.

Association Study of Glutathione-S-Transferase M1/T1 Gene Polymorphism with Deficiency-Excess Differentiation-syndrome in Korean Bronchial Asthmatics (한국인 기관지 천식 환자에서 허설변증과 Glutathione-S-Transferase 유전자의 다형성 연구)

  • Backgrounds : Glutathione-s-transferase (GST) is a kind of phase II metabolism enzyme and plays an important role in the detoxification of various toxic chemicals. It was reported that the genetic polymorphism of GSTM1 and GSTT1 genes may be responsible for asthma development and susceptibility to allergy. Traditional oriental medicine uses a unique diagnostic technique. differentiation-syndrome. to analyze signs and symptoms of patients synthetically. Through differentiation-syndrome. asthma patients can be divided into two groups: the deficiency syndrome group (DSG) and the excess syndrome group (ESG). Objectives : The purpose of this study was to investigate the possible association of GST gene polymorphism with clinical phenotype by differentiation-syndrome of bronchial asthma patients. Materials and Methods : One hundred and ten participants were evaluated by pulmonary function test. Patients with 53 DSG and 31 ESG by differentiation-syndrome were assessed for genetic analysis. GSTM1 and GSTT1 deletion polymorphism was performed by polymerase chain reaction (PCR). Results : GSTM1 gene deletion was detected in 43.4% of individuals in the DSG and in 38.71 % in the ESG. The distribution of GSTM1 polymorphism between DSG and ESG was not significantly different [$x^2$=0.1767, p=0.6742; OR(95% CI)=1.2139(0.4915-2.9979)]. The proportion of GSTT1 null genotypes was 41.51% in the DGS and 45.16% in the ESG. The distribution of GSTT1 polymorphism between DSG and ESG was also not significantly different [$x^2$=0.1065, p=0.7442; OR(95% CI)=0.8618(0.3525-2.1065)]. In the combined analysis of GSTM1 and GSTT1 genes, the frequency of both null type of GSTM1/GSTT1 genes was not significantly different from both positive type of GSTM1/GSTT1 genes[$x^2$=0.0768, p=0.7817; OR(95% CI)=1.2000(0.3303-4.3602)] Conclusions : These results indicate that polymorphism of the GST gene might not be associated with the symptomatic classification of DSG and ESG by differentiation-syndrome in Korean asthmatics.

  • This study was aimed at investigating the syndrome differentiation and herbal medicine of Thromboangiitis Obliterans by reviewing Chinese traditional medicine's journals. The journal search was carried out using China National Knowledge Infrastructure(CNKI) and PubMed from September 2009 to August 2014. Searching key words were the various combination of "Thromboangiitis Obliterans", "traditional chinese medicine", "syndrome differentiation", "herbal medicine". The final selection of 33 studies were selected and summarized by researchers. The syndrome differentiation was classified as yang deficiency, cold syncope and blood stasis. The most frequently herbs were blood-activating and stasis-resolving medicinals.

Efficacy of Red Ginseng by Oriental Medical Obesity Syndrome Differentiation on Obese women : Randomized, Double-blind, Placebo-controlled Trial (비만 여성에서 한방 비만 변증에 따른 홍삼의 비만 개선 효과 : 무작위 배정, 이중 맹검, 위약 대조 임상시험)

  • Objectives This trial was conducted to evaluate the efficacy of Red Ginseng by oriental medical obesity syndrome differentiation on obese women as compared to placebo. Methods 50 obese women were recruited and randomized to receive Korean Red Ginseng(n=24) or placebo(n=26) for 8 weeks. Anthropometric factors, serum lipid, glucose, blood pressure, pulse rate, resting metabolic rate, Korean version of obesity-related quality of life(KOQOL) scale and oriental medical obesity syndrome differentiation questionnaire were measured at baseline and 8 weeks. Adeverse events and safety outcomes variables were also checked during trials. Results and Conclusion There was significant efficacy of Red Ginseng on obese women in body weight, body fat mass, waist-hip ratio, food intake, KOQOL as compared with baseline. But there was no efficacy as compared with placebo group except KOQOL. There was no difference of efficacy compared with the oriental medical obesity syndrome differentiation.

A Review of Etiology, Syndrome Differentiation, and Herbal Medicine of Epilepsy (뇌전증의 병인, 변증, 한약 치료에 대한 고찰 -한국, 일본, 중국 데이터베이스를 중심으로-)

  • Objectives This study aimed to establish a foundation for research on Korean medicine for epilepsy by analyzing its etiology, syndrome differentiation, and the use of herbal medicines. Methods Articles were extracted from five databases in Korea, Japan, and China: the Oriental Medicine Advanced Searching Integrated System (OASIS), Citation Information by the National Institute of Informatics (CiNii), Chinese National Knowledge Infrastructure (CNKI), Wangfang Database, and Chinese Scientific Journal Database (VIP). The etiology, syndrome differentiation, and herbal medicines used for epilepsy were investigated. Results A total of 64 articles were selected, including seven clinical studies, 16 case reports, and 41 reviews. Epilepsy is a complex disease with various etiologies, and among the five viscera, syndrome differentiation mainly involved the liver, spleen, and kidney, with pathological factors that included phlegm, wind, and blood stasis. Frequently used herbal medicines included Modified Jeongganhwan, Sihogyejitang, Yukgunjatang, and Sihogayonggolmoryotang. Conclusions This study analyzed epilepsy's etiology, syndrome differentiation, and herbal treatments. Further evidence is needed to better understand the efficacy and safety of Korean herbal medicine for treating epilepsy.

  • Background: It is insufficient information that are syndrome differentiations and detail symptoms in Korean. Methods: 19 traditional korean medical agencies collected 190 cases with vital sign, body measurement, patient own symptoms report and doctor diagnosis. And then we analyzed general distributions and comparisons of deficiency and non-deficiency. Results: The most past histories, in order, were the arthritis(20%), gastroenteropathy, hypertension, anemia, diabete mellitus. In chief complains, musculoskeletal occupies 60 percent of the total, and digestive system, head in order. In the syndrome differentiation, it appeared deficiency, spleen, Qi deficiency in order. Age(p=0.000), systolic blood pressure(p=0.044) and the waist-hip ratio(p=0.000) was significantly higher in the deficiency group compared with non-deficiency group. Patients with condition which dislikes the wind and with anxiety and with amazing and fear(驚恐) were significantly more in deficiency group(p=0.029, p=0.017, p=0.044). Conclusions: These statistics are continually revised report and it will be reported comparison of several syndrome differentiations and therapeutic effects by treatment methods.

Re-evaluation of Obesity Syndrome Differentiation Questionnaire Based on Real-world Survey Data Using Data Mining (데이터 마이닝을 이용한 한의비만변증 설문지 재평가: 실제 임상에서 수집한 설문응답 기반으로)

  • Objectives: The purpose of this study is to re-evaluate the importance of questions of obesity syndrome differentiation (OSD) questionnaire based on real-world survey and to explore the possibility of simplifying OSD types. Methods: The OSD frequency was identified, and variance threshold feature selection was performed to filter the questions. Filtered questions were clustered by K-means clustering and hierarchical clustering. After principal component analysis (PCA), the distribution patterns of the subjects were identified and the differences in the syndrome distribution were compared. Results: The frequency of OSD in spleen deficiency, phlegm (PH), and blood stasis (BS) was lower than in food retention (FR), liver qi stagnation (LS), and yang deficiency. We excluded 13 questions with low variance, 7 of which were related to BS. Filtered questions were clustered into 3 groups by K-means clustering; Cluster 1 (17 questions) mainly related to PH, BS syndromes; Cluster 2 (11 questions) related to swelling, and indigestion; Cluster 3 (11 questions) related to overeating or emotional symptoms. After PCA, significant different patterns of subjects were observed in the FR, LS, and other obesity syndromes. The questions that mainly affect the FR distribution were digestive symptoms. And emotional symptoms mainly affect the distribution of LS subjects. And other obesity syndrome was partially affected by both digestive and emotional symptoms, and also affected by symptoms related to poor circulation. Conclusions: In-depth data mining analysis identified relatively low importance questions and the potential to simplify OSD types.

  • Objectives : The purpose of this paper is to investigate the educational meaning of Shanghanjiushilun in Shanghanlun education. Methods : The formal characteristics in medical records were compared between Shanghanjiushilun and the other 5 kinds of medical record books and the educational meaning was drawn by analysing medical records of Shanghanjiushilun a little more deeply. Resultss & Conclusions : 1. In a formal aspect, although Shanghanjiushilun is inferior to Liuduzhou's Clinical Medical Records Selection, it has merits in that it explains through quoting medical literature. 2. Xushuwei explained treatment based on syndrome differentiation in the new point of view in Shanghanjiushilun. 3. Through Shanghanjiushilun medical records Xushuwei's work to explain treatment based on syndrome differentiation in Shanghanlun in the new perspective is an effort to read medical classics to keep up with the times and has sufficient educational significance.

Study on Syndrome Differentiation of Gastritis by Korean Standard Classification of Dsease and Cause of Death (한국표준질병 사인분류에 따른 위염(胃炎)의 한의학적 변증 연구)

  • This article is for understanding relations between the classifications of gastritis and syndrome differentiation types of Korean Medicine through research on syndrome differentiations of clinically applied gastritis and literature of Korean Medicine. Clinical papers were searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI) from 1995 to 2015. Conclusions are as follows. First, disease mechanism of chronic gastritis are qi stagnation, damp stagnation, heat obstruction, blood stasis obstruction, yin damage, damage to collaterals with healthy qi deficiency and pathogenic qi. And qi movement stagnation is shown through the status of chronic gastritis. Second, chronic superficial gastritis belongs to qi aspect syndrome and mainly pathogen excess syndrome. And the key mechanisms are congestion and disharmony of stomach qi sometimes combined with liver depression, food accumulation and dampness-heat. Third, chronic atrophic gastritis belongs to qi-blood syndrome and deficiency-excess complex syndrome with the root of spleen qi deficiency and stomach yin deficiency and the tip of blood stasis, qi stagnation. And key mechanism is damage to collaterals with healthy qi deficiency and toxin-blood stasis. Forth, pathogen excess syndromes are shown at the early stage of chronic gastritis and healthy qi deficiency syndromes after the middle stage. Qi deficiency is shown at the beginning of the disease and yin deficiency at the late stage. And qi deficiency is related with superficial gastritis and yin deficiency with atrophic gastritis.

Literature Review of Clinical Research Herbal Treatment on Retinal Vein Occlusion - Focusing on Traditional Chinese Medicine's Journals - (망막정맥폐쇄증의 한약치료 임상연구에 대한 문헌 고찰 - 중국논문을 중심으로)

  • The objective of this study was to investigate the diagnosis and treatment of Retinal vein occlusion such as syndrome differentiation and herbal medicine by reviewing Chinese traditional medicine`s journals. The journal search was carried out using China National Knowledge Infrastructure and Pubmed from January 2000 to December 2014. Searching key words were the various combination of "Retinal vein occlusion", "traditional chinese medicine", "syndrome differentiation". The final selection of 11 studies were selected and summarized by researchers. The effective rate was significantly higher than control but, methodological quality of the trials was generally low questioning their liability. The syndrome differentiation was classified as hyperactivity of liver yang and blood stasis. The most frequently prescribed herbal medication was Hyulbuchookeotang, Boyanghwanotang and Cheomagudeungum. This finding could be widely utilized in future clinical practice and form the basis for the development of clinical practice guidelines in advance.
