Endovascular treatment of Takayasu arteritis in a middle-aged woman with syncope and limb claudication: a case report

  • Takayasu arteritis (TA) is a disease that causes inflammation and stenosis of medium to large blood vessels. We report a case of a 50-year-old female patient with newly developed hypertension, syncope, and claudication of the extremities. Total occlusion of the left subclavian artery at the origin was found and significant stenosis of the right common iliac artery was revealed by hemodynamic analysis. She was successfully treated with percutaneous angioplasty for multiple peripheral arterial diseases and was finally diagnosed with TA. In consultation with a rheumatologist, medical treatment for TA was initiated, the patient's hypertension disappeared, and her claudication symptoms improved.

A case of syncope in a villager with hypertrophic cardiomyopathy after hydrogen sulfide exposure by an unauthorized discharge of wastewater

  • Background: Hydrogen sulfide is a toxic substance that humans can be exposed to occupationally, and cases of hydrogen sulfide poisoning of workers in industrial sites are commonly reported. However, there have been no cases of poisoning of the public due to an unauthorized discharge of wastewater, so it is important to describe this incident. Case presentation: In a small village in Jeollanam-do, Republic of Korea, accounts of a terrible stench had been reported. A 26-year-old man who lived and worked in a foul-smelling area was taken to the emergency room with a headache, dizziness, nausea, and repeated syncope. A subsequent police and Ministry of Environment investigation determined that the cause of the stench was the unauthorized discharge of 9 tons of wastewater containing hydrogen sulfide through a stormwater pipe while the villagers were sleeping. The patient had no previous medical history or experience of symptoms. Leukocytes and cardiac markers were elevated, an electrocardiogram indicated biatrial enlargement, left ventricular hypertrophy, and corrected QT interval prolongation. Myocardial hypertrophy was detected on a chest computed tomography scan, and hypertrophic cardiomyopathy was confirmed on echocardiography. After hospitalization, cardiac marker concentrations declined, symptoms improved, and the patient was discharged after 7 days of hospitalization. There was no recurrence of symptoms after discharge. Conclusions: We suspect that previously unrecognized heart disease manifested or was aggravated in this patient due to exposure to hydrogen sulfide. Attention should be paid to the possibility of unauthorized discharge of hydrogen sulfide, etc., in occasional local incidents and damage to public health. In the event of such an accident, it is necessary to have government guidelines in place to investigate health impact and follow-up clinical management of exposed residents.

  • Anomalous aortic origin of left coronary artery is a rare congenital coronary anomaly that can cause clinical manifestations such as ischemic chest pain, arrhythmic syncope or even sudden cardiac death. We describe a case of anomalous aortic origin of left main coronary artery presented as a cardiogenic shock which was successfully treated by coronary artery transfer.

  • The aim of this article is to provide major announcements within the last decade or so about possible medical emergencies in dentistry. This would be helpful to improve the knowledge on first aid suitable for dental environment. Syncope was the most common medical emergencies in dentistry. Medical emergency situations can be divided into urgency and true emergency. Urgency situations can be solved well if proper treatment is taken. However, even in an urgency, if the correct treatment is not performed, it may soon turn into an emergency. With the joint efforts of the Korean Dental Anesthesiology Society and the Korean Cardiopulmonary Resuscitation Association, dental advanced life support (DALS) has been prepared since 2015. Through 17 training experiences until January 2020, scenarios and textbooks were developed. Dentists and dental care teams need to be prepared to strengthen their competence as professionals and to increase their team-level response capabilities..

항부정맥 치료의 기본 원칙

  • 모든 부정맥이 치료를 필요로 하는 것은 아니지만, 다양한 부정맥들이 허약 (weakness), 실신 (syncope)을 유발하거나 울혈성 심부전을 악화시키며 일부의 부정맥들은 치명적인 심장 이상을 일으킨다. 일반적으로 임상증상을 유발하는 부정맥들과 급사의 지표가 될 수 있는 부정맥들은 치료한다. 불행하게도, 수의학 분야의 심장전문의들 사이에서 어떤 부정맥들을 치료해야만 하고 어떻게 관리하는 것이 최선인가에 관한 내용이 명확하게 일치하고 있지는 않다. 사실은 어떤 항부정맥 약물을 우선적으로 선택해야 하는가에 관해서 명확한 합의점이 없을 뿐만 아니라 어떤 범주의 약물이 효과적인가에 대해서도 논란의 여자기 있다. 예를 들어, 심각한 심실성 부정맥이 성공적으로 치료되지 않으면 현저한 이소성 박동(ectopy)이 지속적으로 남을 수도 있다. 또한 만일 임상증상들 (실신 등)이 해소되고 지속적인 심실성 부정맥이 잘 통제된 이러한 류의 질문들에 대한 대답은 대부분 명확하지 않고 신중하게 숙고한 의견에 불과할 수도 있다.

  • Stellate ganglion block is most commonly used in pain clinic because it is an easy procedure and it has broad indications reported that Angina pectoris, tachyarrhythmia and long QT syndrome (LQTS) are indicated. LQTS is a disorder of the abnormalities of cardiac sympathetic innervation and of myocardial repolarization. LQTS is characterized by marked prolongation of the QT interval, often manifestating as syncope, seizures, or sudden death due to polymorphic ventricular tachyarrhythmia known as torsades de pointes. Treatment of symptomatic patients usually begin with beta blocker. The elective treatment of LQTS patients unresponsive to beta blocker is the left cardiac sympathetic denervation. We report a case of LQTS patient who had received stellate ganglion block.

  • The objective of this study was to investigate the diagnosis and treatment of Raynaud disease such as syndrome differentiation and herbal medicine by reviewing Chinese traditional medicine's journals. The journal search was carried out using China National Knowledge Infrastructure(CNKI) and PubMed from January 2008 to August 2013. Searching key words were the various combination of "Raynaud disease", "traditional chinese medicine", "syndrome differentiation", "herbal medicine". The final selection of 38 studies were selected and summarized by researchers. The syndrome differentiation was classified as yang deficiency and cold syncope, qi stagnation and blood stasis. The most frequently prescribed herbal medication was Dangguisinitang.

Disappearance of pericardial effusion by suspected pericardial-pleural fistula in a Miniature Schnauzer dog

  • A 13-year-old spayed female Miniature Schnauzer was presented with complaints of intermittent syncope. Pericardial effusion was confirmed based on the physical examination, thoracic radiographs and echocardiography. Subsequently, prompt pericardiocentesis was performed. Clinical abnormalities were immediately improved after pericardiocentesis. However, the clinical signs associated with acute collapse recurred. After the second pericardiocentesis, thoracic radiographs revealed pleural effusion, and the clinical signs resolved rapidly. The dog underwent pleural aspiration. Analysis of pleural fluid revealed almost similar features as the previous pericardial fluid. It was possible that a pericardial-pleural fistula was created during the pericardiocentesis. The pericardial and pleural effusion disappeared after the procedures.

Analysis of Patients Injured in Demonstraions (장기간의 집회시위로 인한 환자 발생에 대한 분석)

  • Purpose: This study was conducted to determine the level of medical care required for mass-gatherings and to describe the types of illness and injury that may occur during demonstrations. Methods: We conducted a retrospective review of the medical records for patients injured during demonstrations. Results: From May to August at 2008, a total of 932,000 participants attended demonstrations. Most patients were occurred from June to July, a total of 126 patients were evaluated and treated at the emergency center of our hospital. The mean patient age was $31.8{\pm}9.8$ years, and men predominated over women. The vast majority of patients were experienced trauma (88.9%). The diagnostic categories were contusion (49.2%), laceration (20.6%), fracture (6.3%), syncope/dizziness (5.5%), ocular injuries (3.9%), dyspnea (3.9%), other trauma (3.1%), and abdominal complaints (1.5%). Two patients were admitted. Conclusion: The rate and the acuity of patients seen at these demonstrations was low. Nevertheless, a full on-site physician and transportation system is recommended during similar incidents.

A Report of Left Heart Failure by Aortic Valvular Stenosis in a Mongrel Dog (개에서 대동맥판막협착에 의한 좌심부전 증례)

  • A 4 year old mixed female dog was presented dyspnea, anorexia and syncope. In blood smear test, a lot of heartworm larvae were observed. In thoracic radiography, "reverse D" shape was observed. So, this case was considered with heartworm disease. In echocardiography, marked left heart hypertrophy was presented. In M-mode view, aortic valvular stenosis was found. After mongrel dog died, aortic valvular stenosis was reconfirmed in necropsy. After all, the dog died of pulmonary edema because of aortic valvular stenosis caused left heart failure.t failure.
