Medical Managements of Musculoskeletal Diseases in Shipbuilding Industry

  • Objective: This study is to understand medical management method in shipbuilding industry. Background: In shipbuilding industry, medical management for prevention of work-related musculoskeletal diseases due to limitations of engineering measures may be important measure. Results: Medical management of musculoskeletal diseases can be divided into primary, secondary, tertiary preventions. Primary preventions consist of symptom survey, appropriate work placement with work capacity evaluation, health promotion. Second preventions are early detection of symptomatic patient and appropriate treatment. Tertiary preventions are rehabilitation treatment and early return-to-work by return-to-work evaluation. In addition, patients with psychological counseling for emotional problems are needed. Conclusion: Medical management measures such as improving the work environment to be made are expected to exert greater effects.

  • Myelodysplastic syndromes (MDS) represent a heterogeneous group of clonal hematologic neoplasms characterized by morphologic dysplasia, aberrant hematopoiesis and peripheral blood refractory cytopenias. MDS is recognized to be associated with an increased risk of symptomatic anemia, infectious complications and bleeding diathesis, as well as a risk of progression to acute myeloid leukemia, particularly in patients with a high IPSS score. The advent of use of hematopoietic growth factors such as granulocyte colony-stimulating factor (G-CSF) and recombinant erythropoietin (EPO) has improved symptoms in MDS patients in addition to some data that suggest there might be an improvement in survival. G-CSF is an effective therapeutic option in MDS patients, and it should be considered for the management of refractory symptomatic cytopenias. G-CSF and EPO in combination can improve outcomes in appropriate MDS patients such as those with lower-risk MDS and refractory anemia with ring sideroblasts (RARS). This article reviews use of growth factors for lower-risk MDS patients, and examines the data for G-CSF, EPO and thrombopietic growth factors (TPO) that are available or being developed as therapeutic modalities for this challenging disease.

세 마리 개에서 발생한 심저부 종양 증례 (Heart-base Tumors in Three Yorkshire Terriers)

  • 본 증례는 호흡곤란, 기침과 식욕결핍으로 내원한 세 마리의 요크셔테리어 개 (12년령, 13년령, 15년령)를 대상으로 하였다. 실험실적 검사 상 특이소견은 없었다. 그러나 흉부 방사선 검사상 세 마리에서 모두 심저부위의 종괴를 확인할 수 있었고, 기관지의 상승과 대동맥 융기를 확인할 수 있었다. 복부 방사선 검사 시 특이 소견은 관찰할 수 없었다. 초음파 검사상 균질한 고 에코의 종괴가 대동맥 주위에서 확인되었고 이첨판 역류를 확인할 수 있었다. 복부초음파 검사 상 특이소견은 관찰할 수 없었다. 컴퓨터 단층 촬영 결과 경계가 명확한 종괴를 좌측 앞대정맥에서 확인할 수 있었다. 종괴의 크기는 대략 $3{\times}4cm$였고 조영 증강을 확인할 수 있었다. 이상의 검사를 바탕으로 심저부 종양으로 진단하였다. 치료를 위해 심부전과 기관 허탈에 대한 대증 처치를 실시하였다. 진단 후 증례 1 (12년령)은 3 개월, 증례 2 (13년령)는 5개월, 증례 3 (15년령)은 32개월동안 생존하였다. 본 증례는 심저부 종양으로 의심되는 개에서의 임상증상, 흉부 방사선 검사, 컴퓨터 단층 촬영과 대증 처치에 대한 보고이다.

Symptomatic Post-Discectomy Pseudocyst after Endoscopic Lumbar Discectomy

  • Objective: The objectives of this study were to determine the frequency of symptomatic postdiscectomy pseudocyst (PP) after endoscopic discectomy and to compare the results of surgical and conservative management of them. Methods: Initial study participants were 1,503 cases (1,406 patients) receiving endoscopic lumbar discectomy by 23-member board of neurosurgeons from March 2003 to October 2008. All patients' postoperative magnetic resonance imaging (MRI) scans were evaluated. On the postoperative MRI, cystic lesion of T2W high and T1W low at discectomy site was regarded as PP. Reviews of medical records and radiological findings were done. The PP patients were divided into two groups, surgical and conservative management by treatment modality after PP detection. We compared the results of the two groups using the visual analogue scale (VAS) for low back pain (LBP), VAS for leg pain (LP) and the Oswestry disability index (ODI). Results: Among 1,503 cases of all male soldiers, the MRls showed that pseudocysts formed in 15 patients, about 1.0% of the initial cases. The mean postoperative interval from surgery to PP detection was 53.7 days. Interlaminar approach was correlated with PP formation compared with transforaminal approach. (p=0.001).The mean VAS for LBP and LP in the surgical group improved from 6.5 and 4.8 to 2.0 and 2.3, respectively. The mean VAS for LBP and LP in the conservative group improved from 4.4 and 4.4 to 3.9 and 2.3, respectively. There was no difference in treatment outcome between surgical and conservative management of symptomatic PP. Conclusion: Although this study was done in limited environment, symptomatic PP was detected at two months' postoperative period in about 1% of cases. Interlaminar approach seems to be more related with PP compared with transforaminal approach.

  • Symptomatic neuromas after amputation can be troublesome to treat and make it difficult to properly fit a brace. Surgical management is required when conservative management such as prosthetic socket modification or local injections fail. However, small cutaneous nerves adhere to adjacent soft tissue and they are difficult to locate. The authors suggest that ultrasonography guided tattoo localization using a charcoal suspension is useful to find a visually indistinguishable neuroma.

  • A fistula from the right coronary artery draining into the left ventricle is a rare form of coronary artery fistula. Here, we describe the case of a symptomatic neonate with a large fistula of this type. The neonate was successfully treated with surgical closure of the fistula.

  • The incidence of acute pancreatitis (AP) has increased in the pediatric population over the past few decades and it stands to follow that the complications of severe AP, including symptomatic pancreatic fluid collections (PFCs) will increase as well. In adults, the therapeutic options for this situation have undergone a dramatic evolution from mainly surgical approaches to less invasive endoscopic approaches, mainly endoscopic ultrasound-guided transmural drainage (EUS-TD) followed be direct endoscopic necrosectomy if needed. This has proven safe and effective in adults; however, this approach has not been well studied or reported in pediatric populations. Here we demonstrate that EUS-TD seems to offer a safe, efficacious and minimally invasive approach to the management of large PFCs in pediatric patients by reviewing two representative cases at our institution.

Breast Lesions in Adolescents and Young Women in Pakistan - a 5 Year Study of Significance of Early Recognition

  • Background: Breast cancer is the most frequent malignant disease amongst young women. If we review local data in Pakistan then breast cancer represents approximately a third of all cancers in females. The age standardized incidence rate (ASR) world per 100,000 is 53.8 and crude incidence rate is 30.9. We have observed during our surgical pathology practice and it is also reported by other Asian studies that breast carcinoma is amongst the leading malignancies in the region and the patients are at least a decade younger than counterparts in developed nations. Age is an important issue in effective screening, diagnosis and management of breast cancer, especially in this geographical region where late presentation and poor prognosis are a hallmark of the disease. Objective: The aim of this study is to determine the frequency of malignant breast lesions in symptomatic young females presenting with breast lumps. Materials and Methods: This is a retrospective study conducted at the Pathology Department, Liaquat National Hospital and Medical College, Karachi. Descriptive and pathology data of malignant breast tumors 1st January 2004 to 31st December 2009 were reviewed, using the departmental archived data. It included both male and female patients up to the age of 25 years. Results: A total of 714 surgical specimens from/of symptomatic breast lesions were received at the pathology department of Liaquat National Medical, in the five years study period, in young females. There were 575 (80%) benign, 119 (16%) inflammatory and 20 (2.8%) malignant lumps. Conclusions: The obtained data for females only up to 25 years of age suggest a massive burden which requires urgent attention. Early assessment of lesions is essential in order to avoid mortality from malignancies.

박탈성피부염(剝脫性皮膚炎) 증례(證例) 보고(報告) 1례(例) (A Case Report on Exfoliative Dermatitis)

  • Exfoliative Dermatitis is a name of a group of symptoms which is characterized by red rash, itching and scale in almost whole body. The patients are suffered from hypoalbuminemia, general weakness and fever sign in addition to characteristic skin lesions. So in the treatment of this disease, adequate symptomatic treatment is important with adequate topical skin management. We experienced a 69 year old Korean male patient diagnosed as Exfoliative Dermatitis based on clinical symptoms and a medical certificate. We concluded that the symptoms of this patient are Sun-symptoms(癬症) and gave him Bangpungtongsung-san(剝脫性皮膚炎) and Geunsun-bang(乾癬方). After taking Oriental Medicine(the hebal medicine of internal use), clinical improvement was noticed. Based on this experience, internal use of Oriental Medicine have a good effect in Exfoliative Dermatitis.

Outcome of Surgical Management for Tethered Spinal Cord

  • Objective : The tethered cord syndrome results in progressive neurological deficits. Although it may remain controversial, many physicians recommend definitive surgery to untether the cord as soon as this condition is identified. We retrospectively evaluate the pre-operative and post-operative course of 38 tethered cord patients with spinal dysraphism in an attempt to learn the natural history of the disease and to determine the effectiveness of the surgical treatment. Methods : The medical records, operation notes and radiographs were evaluated. The follow up period ranged from 4 months to 12 years with a mean follow-up of 28.6 months. Twenty-seven patients were younger than 15 years of age. Results : At presentation, 26 of the patients were asymptomatic. In three of 11 adult symptomatic patients, their neurological deficits worsened after trauma or exercise. Improvement of motor strength was documented in two out of 5 patients. Five of nine patients with bladder symptoms improved, however, none had a complete return of their bladder function. Conclusion : Childhood patients were less symptomatic than the adult patients. Adult patients showed progression of their symptoms that were not improved even after the operation in most of the cases. Asymptomatic tethered spinal cord can be symptomatic as time passes by and even at an old age. Future research should be focused on the operative methods to prevent the delayed deterioration after surgery, rather than on the usefulness of preventive surgery in asymptomatic patients.
