The Role of Bronchodilators in Preventing Exacerbations of Chronic Obstructive Pulmonary Disease

  • Bronchodilators are the cornerstone of symptomatic chronic obstructive pulmonary disease (COPD) treatment. They are routinely recommended for symptom reduction, with a preference of long-acting over short-acting drugs. Bronchodilators are classified into two classes based on distinct modes of action, i.e., long-acting antimuscarinics (LAMA, once-daily and twice-daily), and long-acting ${\beta}2$-agonists (LABA, once-daily and twice-daily). In contrast to asthma management, evidence supports the efficacy of both classes of long-acting bronchodilators as monotherapy in preventing COPD exacerbations, with greater efficacy of LAMA drugs versus LABAs. Several novel LAMA/LABA fixed dose combination inhalers are currently approved for COPD maintenance treatment. These agents show superior symptom control to monotherapies, and some of these combinations have also demonstrated superior efficacy in exacerbation prevention versus monotherapies, or combinations of inhaled corticosteroids plus LABA. This review summarizes the current data on clinical effectiveness of bronchodilators alone or in combination to prevent exacerbations of COPD.

Hydronephrosis during Conservative Treatment for a Renal Injury Patient

  • A 21-year-old male visited our emergency room. He could not remember the mechanism of injury. He was found beside a motorcycle. Initial vital sign was stable. Observation and conservative treatment were planned at the intensive care unit (ICU). On the third day at ICU, he complained sudden flank pain. It was colicky and hard to control. Without the pain, he had no specific symptom, sign, or laboratory findings. On computed tomography, renal pelvis was filled with hematoma which induced hydronephrosis. Double-J catheter and percutaneous nephrostomy was implemented by an intervention radiologist. Hematome in the renal pelvis was aspirated during the procedure. Symptom of the patient was subsided after the procedure. He was discharged without specific complications.

  • An efficient diagnostic approach for real-time operation aiding expert system in chemical process plants is discussed. The approach is based on the hybrid of the simplified symptom tree(SST) and the fault consequence digraph(FCD), representation of propagation patterns of fault states. The SST generates fault hypothesis efficiently and the FCD resolve the real fault accurately. Frame based knowledge representation and object-oriented programming make diagnostic system general and efficient. Truth maintenance system enables robust pattern matching and provides enhanced explain facilities. A prototype expert system for supports operation of naphtha furnaces process, called OASYS, has been built and tested to demonstrate this methodology. Utilization of diversified process symbolic data, produced using dynamic normal standards, overcomes the problem of qualitative Boolean reasoning and enhance the applicability.

Malondialdehyde Levels in Middle Ear Fluid from Patients of Otitis Media with Effusion

  • Otitis media with effusion (OME) is an inflammatory disease of the middle ear cleft. Oxygen free radicals have been implicated in a variety of inflammatory disorders. Oxygen free radicals may also be involved in the pathogenesis of OME. To evaluate the involvement of oxygen free radicals in the pathogenesis of OME, the level of malondialdehyde, which gives an index of lipid peroxidation by oxygen free radicals, was measured by the reaction with thiobarbituric acid. Malondialdehyde level in the middle ear fluid from the OME group was higher than that in the normal control group. Malondialdehyde level in the middle ear fluid from a mucoid subgroup was higher than that in the serous subgroup. Malondialdehyde levels in the middle ear fluid from the serous subgroup was significantly correlated with symptom duration. The Pearson correlation coefficient between malondialdehyde levels in the middle ear fluid from the serous subgroup and symptom duration was 0.842 (P<0.05). These results indicate that lipid peroxidation by oxygen free radicals may be involved in the pathogenesis of human OME.

  • Urinary disturbance is deficit in volitional control of voiding. Stroke can cause temporary or permanent urinary disturbance. The purpose of this study was to confirm the effect of Jasin-hwan on urinary disturbance as heat syndrome. This study was carried out on 3 patients with urinary disturbance caused by stroke. We measured components of IPSS (international prostate symptom score) at pre-treatment and post-treatment. After prescribing medication to the patients, symptoms improved rapidly within three weeks. These results support that Jasin-hwan is effective in reduction of urinary disturbance as heat syndrome.

  • The purpose of urodynamic investigation is to obtain the information on the function of the urinary system. The aim of this study is to acquire the useful information of lower urinary tract symptom (LUTS) diagnosis through void force signal as noninvasive method. The system which could evaluate the function of compensatory hypertrophy with noninvasive and comfortable method was implemented to measure uroflow and void force during urination. The implemented system composes of the sensor parts, signal conditioning parts and PC monitoring program. For the evaluation of the implemented system, the simulation of control part of the system was performed and the model system for the lower urinary system was designed. The superiority of a measuring characteristic of the implemented system was verified using the model system. From the evaluation of the model system, we have found out that the void force was dependent on the occlusion degree and compensatory hypertrophy significantly.

  • A 72-year-old woman with metastatic lung cancer to bone and brain and with left external iliac vein thrombosis was under the care of a community palliative care provider. She experienced an acute pain crisis due to acute limb ischemia of the left lower limb. Goals-of-care discussions were held with the patient and her family; she prioritized symptom control and end-of-life care at home. The family and patient were aware of her short prognosis. Her complex pain was managed by the community palliative team, and her family was empowered to give subcutaneous injections. We illustrate a case showing the importance of community health services with palliative care support in providing symptom management and support to patient and family caregivers throughout the course of a life-limiting illness. It also highlights family caregivers' potential psychological distress in delivering subcutaneous injections in terminal care for a patient at home.

A Psychological Study on the MPDS Patients (인성검사를 이용한 MPDS환자의 심리적 분석)

  • A reserch scale of 48 items selected from MMPI by McCall et al. was used to compare psychologic aspects of 80 patients with MPDS to control I subjects and control II subjects. For this, study subjects were selected 80 patients of MPDS who visited the Dept. of Dentistry. K.N.U.H., from December in 1982 to September in 1983. Control I subjects were selected 80 outpatients without MPDS who required dental treatment from December in 1982 to September in 1983. And control II subjects were selected 80 persons without MPDS who required no dental treatment and lived in Taegu. The results were as follow : In study subjects,somatic gruop and behavioral group are markedly high than control (I,II)subjects. In somatic group and behavioral group, there was no significant differencd between control I subjects and control II subjects. In somatics group and begavioral group, there was no significant difference between male and female in each subjects: In study subject, general nature somatic category was higher than head, neck, throat and GIT somatic category.(P<0.05) In study subjects, somatic group and behavioral group gradually increased according to increase in number of MPDS symptom.

  • We get following conclusion in relationship of emotions and several diseases from the viewpoint of Gi(氣) movements. "Naegyeong(內經)" state mainly relationship of emotions and five organs, relationships of emotion and Gi(氣) movements in detail, but refer about symptoms of diseases that happen by anger, fear, and astonishment. "Yumunsachin(儒門事親)" supplement the relationships of emotions and five organs in "Naegyeong(內經)", and "Uihak-ipmun(醫學入門)" present medical treatments for each symptom of diseases that is led by emotion. According to "Naegyeong", as well as emotions can lead to various symptoms of diseases by affecting Gi(氣) movements, particular symptoms of diseases can affect in emotions by affecting Gi (氣) movements. Because emotions can affect to symptoms of diseases through Gi(氣) movements, we can treat with drugs that control Gi(氣) as well as with methods that control emotions in case of emotions leading to particular symptoms of diseases. Because particular symptoms of diseases can affect to emotions through Gi(氣) movements, we can treat with methods that control emotions as well as with drugs that control Gi(氣) movements in case of particular symptoms of diseases leading to emotions. We think the theory of Gi(氣) movements can be used as a principle of acupuncture as well as of drugs by examining the relationship of emotions and symptom of diseases from the viewpont of Gi(氣) movements.

A Study on the Relationship of Climacteric Women's Vasomotor Symptoms and Body Temperature, Kidney Deficiency Pattern (腎虛證) (폐경기 여성의 혈관운동성 증상과 체열분포, 신허증 변증유형의 상관성 연구)

  • Objectives: The purpose of this study is to investigate the relationship between vasomotor symptoms with upper-lower body temperature differences, and to identify Kidney Deficiency pattern (腎虛證) in menopausal women. Methods: 51 women who visited Kangnam Kyung-Hee Oriental Hospital from July to December, 2013 were analyzed. The experimental group consisted of 30 patients reported with vasomotor symptoms (Vasomotor symptom group, VG), while the control group consisted of 21 patients without vasomotor symptoms (Control group, CG). According to their chief complaints, VG was further divided into VHG (vasomotor-hot flash group) and VCG (vasomotor-cold hypersensitivity group). The temperature differences between upper and lower body part (CV17-CV4), proximal and distal extremity part (LU4-HT8, ST32-LR3) were checked. All patients reported other complaints such as headache, sweating, anxiety, dyspepsia, leukorrhea, which belong to Kidney Yang Deficiency pattern (腎陽虛證) or Kidney Yin Deficiency pattern (腎陰虛證). The relationship between vasomotor symptom and repetition rate of additional complaints were analyzed. Results: Statistical analysis showed that lower abdomen temperature of experimental group was lower than the control group. Temperature differences of upper and lower body (CV17-CV4) was significantly larger in vasomotor symptom group. VHG had more symptoms of sweating, chest discomfort, constipation, which belong to Kidney Yang Deficiency pattern group. VCG had more symptoms of leukorrhea, diarrhea, and dyspepsia, which belong to Kidney Yin Deficiency pattern group. Conclusions: Climacteric women who suffered from vasomotor symptoms showed lower temperature tendency in [CV4], larger temperature differences in [CV17-CV4] compared to the control group. Among them, VHG showed more symptoms of Kidney Yin Deficiency pattern, whereas VCG showed more symptoms of Kidney Yang Deficiency pattern.
