A STUDY ON CORRELATIONSHIP BETWEEN CRANIOFACIAL GROWTH PATTERN AND SYMPHYSIS MORPHOLOGY (악안면 성장양상에 따른 하악이부 헝태에 관한 연구)

  • Craniofacial growth pattern is an important diagnostic data in the course of orthodontic diagnosis and treatment planning ; it also has great influence in the establishment of occlusion as well as shaping and development of face. There have been many studies to classify different craniofacial growth patterns and attempts to predict growth patterns. This study aimed to correlate craniofacial growth pattern and symphysis morphology. 120 adult patients with age from 19 to 39 (mean age : 23.1) were chosen as subjects , using lateral cephalometric films. their anterior to posterior facial height ratios were calculated. They were divided into 3 groups - clockwise growth pattern with $56\%-62\%$(36subjects), counter-clockwise growth pattern group with $65\%$-80\%$(43subjects) and normal growth pattern group with $62\%-65\%$(41subjects). Symphysis morphology and Prominence evaluation in each subject were studied and the following conclusions were drawn : 1. In comparison of symphysis morphology between the sex groups, men showed large symphysis height and prominence. 2. Concerning the symphysis morphology, the clockwise growth pattern group showed larger height, H/D ratio and actual length but smaller depth, angle, effective length and E/A ratio compared to the counter -clockwise growth pattern group. 3. Those with smaller prominance of symphysis showed clockwise growth tendency and those with larger prominance showed counter-clockwise growth tendency.

  • Transverse mandibular deficiency has a many problem in growing patients. Therefore, Mandible symphysis widening is recommended. A new surgical technique has been developed to widen the mandible. The purpose of this study is to report the effect of mandibluar symphyseal distraction osteogenesis by use of bone-borne type distractor. The surgical procedure was accomplished under general anaesthesia with step-line osteotomy. Bone-borne type distractor was used to generally widen the mandible. The expansion achieved in the mandible was 8.31mm at the device, 6.32mm at the canines, 4.06mm at the first molars. The symphyseal distraction gaps were bridged by new bony regeneration. Mandibular symphseal distraction osteogenesis increased mandibular arch width, stabilized occlusion, and corrected dental crowding.

  • Separation of the pubic symphysis in association with delivery is very rare. The Korean orthopedic and obstetrical literature pertaining to this syndrome is absent. The physiological widening is small and asymptomatic, but a separation of more than one centimeter may occur and is usually symptomatic. A twenty-six-year-old woman of separation of symphysis pubis associated with delivery was treated successfully with the treatment of reduction and pelvic band, with absolute bed rest in the lateral decubitus position.

  • The purpose of this study was to pursue the morphology and position of mandibular symphysis and the positioning of lower incisors in 36 male and female adults with severe skeletal Class III malocclusion indicated for surgical orthodontic treatment. The following results were obtained. 1. Skeletal Class III malocclusion samples had thinner labio-lingual depth and more lingual inclination of mandibular symphysis than that of normal occlusion in both sexes. 2. Male and female with the skeletal Class III malocclusion showed marked lingual tipping of lower incisors. 3. In skeletal Class III malocclusion samples, lingual basal bone was thinner than that of normal occlusion in both sexes.

  • By studying the relationship between the morphology of mandibular symphysis and craniofacial morphology in classIII malocclusion, this study aims at deciding whether the morphogy of mandibular symphysis can be used as a predictor on the growth of mandible. The materials used for this study were the cephalometric radiographs of male class III malocclusion. The subjected age groups were 10-12(G1 group) and 20 and above(G2 group): 50 were selected from each group. Each group was again divided, according to the ratio of symphysis, into Large(L), Average(A), and Small(S). The results of this study were summarized as follows: 1. In average the ratio of symphysis, G2 group showed significantly bigger than G1 group(p<0.05) 2. In both G1 and G2 groups, the ratio of symphysis had no relationship with the measurements on the cranial base and the maxilla(p>0.05). 3. In both G1 and G2 groups, there was not distinct difference in the antero-posterior positions among L, A, S subgroups. 4. L and A subgroups showed significantly larger than S subgroup in lower gonial angle and chin angle in G1 group (p<0.05). 5. In the measurements on the vortical relation of the face, anterior total face height(ATFH) and anterior lower face height(ALFH) of L subgroup were significantly larger than that of S subgroup in G1 group(p<0.05) and also mandible showed a tendency to grow downward vertically. 6. In the measurements on the tooth position and inclination, L subgroup showed as compared with S subgroup a tendency of extrusion of maxillary and mandibular teeth in G1 group, but G2 group showed such tendency only in mandibular teeth. 7. In the measurements on the abnormal growth prediction by Schulhof, in G1, there was no significant difference among L, A, S sugroups. 8. In the correlative analysis of the ratio of symphysis and other measurements, G1 group showed significant correlationships in chin angle, PP/MP angie, ANS-Me and other, while G2 group showed the same only in MP-LIT and MP-LMMC(p<0.05, p<0.01). In summarizing the above, in the G1 group, consisting of young males, no difference was noted in horizontal relation between L and S subgroups; in vertical relation, L subgroup showed a stronger tendency of downward growth of mandible than S subgroup. In adult male G2 group, however, no distinct morphological difference of craniofacial complex by the ratio of symphysis.

Morphological difference of symphysis according to various skeletal types using cone-beam computed tomography (안면골격 유형에 따른 하악 전치 치조골의 형태 차이: Cone-beam CT를 이용한 정량적 평가)

  • Purpose: The aim of this study was to investigate differences between the morphology of the mandibular symphysis and four facial skeletal types. Materials and Methods: 40 cone-beam computed tomographies were selected and classified in to 4 groups according to their vertical and anterior-posterior skeletal patterns. The bone volume ($mm^3$) of the symphysis, the cross sectional area corresponding to the 4 mandibular incisors' axis: the cross sectional area of total bone ($mm^2$), the area of the cancellous bone ($mm^2$) and the thickness (mm) of labial and lingual alveolar bone at 2 mm, 3 mm under the cemento-enamel junction (CEJ) were measured. General linear model (GLM), Kruskal-Wallis test and Tukey honestly significant difference (HSD) test were subsequently used for statistical analysis. Results: The lingual cortical bone thickness of the lateral incisors at 2, 3 mm under CEJ was greater in the Class I low angle group than the other 3 groups (P < 0.05). There were no statistically significant differences in the volume of the mandibular incisor bony support, cross-sectional area of total bone and cancellous bone at the mandibular incisor' axis. Conclusion: Patients in Class I, low angle group have a thicker lingual mandibular symphysis than Class I, high angle patients.

Quality and Morphology on cortico-cancellous bone in Korean mandibular symphysis area (한국인 하악 유합부에서의 피질골-해면골의 밀도 및 형태)

  • In performing implant procedures in the anterior portion of the maxilla, many difficulties exist because of anatomical reasons, such as the proximity of the nasal floor, lateral extension of the incisive canal, and labial concavity. On the other hand, in the posterior region of the maxilla, there is often insufficient recipient bone between the maxillary sinus and alveolar ridge due to alveolar ridge resorption and pneumatization of the maxillary sinus. In order to perform implants in such regions, ridge augmentation procedures such as onlay bone graft, guided bone regeneration, and maxillary sinus grafting are performed. In studies of Caucasians, use of autograft from mandibular symphysis has been reported to be highly successful in maxillary sinus grafting. However, in a clinical study of Koreans, autograft of mandibular symphysis has been reported to have significantly low success rate. It has been hypothesized that this is because of insufficient cancellous bone due to thick cortical bone. In order to test this hypothesis, bone quality and morphology of Koreans can be compared with those of Caucasians. In this study, the bone density and morphology of the cortical bone and cancellous bone in the mandibular symphysis of 35 Korean cadavers were evaluated. The following results were obtained: 1. In terms of bone density, type I, type II, and type III consisted of 1.4%(3/213), 72.3%(154/213), and 26.3%(56/213) of the cross-sectioned specimens, respectively. In general, the bone density tended to change from type II to type III, as cross-sectioned specimens were evaluated from the midline to the canine. Type IV wasn't observed in this study. 2. The distance between the root apex and the lower border of the cancellous bone was 18.34mm-20.59mm. Considering that the bone has to be cut 5mm below the root apex during the procedure, autografts with about 15mm of vertical thickness can be obtained. 3. The thickness of cortical bone on the labial side increased from the root apex to the lower border of the mandible. The average values ranged from 1.43mm to 2.36mm. 4. The labio-lingual thickness of cancellous bone ranged from 3.43mm to 6.51mm. The thickness tended to increase from the apex to the lower border of the mandible and decrease around the lower border of cancellous bone. From the above results, the anatomic factors of the mandibular symphysis (bone density, thickness, quantity and length of the cortical bone and cancellous bone) didn't show any difference from Caucasians, and it cannot be viewed as the cause of failure in autografts in the maxillary sinus for implants.

  • The configuration of mandibular symphysis is likely to be dependent upon the genetic predeterminants and/or compensatory adjustments onto functional demands. The interrelation of morphological distinctives of symphysis in conjunction to the craniofacial skeleton had been scarcely anecdotal, therefore, the objective was to determine the correlation of morphological modifications between the mandibular symphysis and craniofacial complex. Lateral cephalometric headfilms of 212 subjects were employed for the conventional measurements. The proportion of chin height against chin depth length was referred as chin ratio, then, Low Symphysis (IS) and High Symphysis (HS) groups were turned out by means of the chin ratio. These samples yielded 35 in LS and 35 in HS groups. The data per capita were statistically analyzed and the following results were drawn ; 1. Overall characteristics of the craniofacial skeleton in HS group manifested hyperdivergence and LS group showed hypodivergence. 2. Gonial angle increased as chin ratio increased and was highly correlated to the chin ratio. 3. The chin ratio presented high correlation to the vertical face height, especially in terms of the chin height to anterior face height and the chin depth to posterior face height. 4. The morphological configuration of chin was hardly correlated with hyoid bone position.

A CASE REPORT OF COMPLICATIONS DURING MANDIBULAR TRANSVERSE SYMPHYSIS WIDENING (하악 이부 확장술 시 나타나는 합병증의 치험례)

  • Orthodontists often treat cases which are difficult to treat with conventional orthodontics. In such cases, it could be treated with surgical procedures with the help of an oral surgeon. Especially, transverse deficiency of the mandible can be corrected by widening the transverse width of mandibular symphysis, using distraction osteogenesis. Transverse widening of mandibular sympysis is known as a safe treatment but still complications could occur during the treatment. We are reporting some complications of cases that mandibular symphysis transverse widening were applied. Some cases showed complications because of the inappropriate osteotomy line. Since straight vertical osteotomy line was inclined to the left, only the left bony segment was likely to expand. According to bio-mechanical considerations, it will be better to perform a step osteotomy, cutting the eccentric area of the alveolar crest and the centric area of the basal symphyseal area. Complications could also occur by the failure of the distraction device. The tooth borne distraction device was attached on the lingual side of the tooth with composite resin. During the distraction period, it was impossible to obtain appropriate distraction speed and rhythm because of frequent fall off of the distraction device. Therefore, distraction device should be attached firmly with orthodontic band or bone screw, etc. Tooth mobility increasement could also occur as a complication. 'Walking teeth phenomenon' was observed during the distraction period, showing severe teeth mobility and pain during mastication. These symptoms fade out during the consolidation period. Since the patient could feel insecure and uncomfortable, it should be notified to the patient before the procedure. Finally, alveolar crestal bone loss could occur. Alveolar crestal bone loss occurred because of lack of distraction device firmness and teeth trauma caused by lower lip biting habit. Therefore, adequate firmness of the distraction device and habit control will be needed.

Relation between Location of Pelvic bone Fractures and the Injury to the Urinary bladder, Urethra or Lower gastrointestinal tract (골반 골절 위치와 방광, 요도, 하부 위장관계 손상의 관련성)

  • Purpose: This research was conducted to study whether the specific location of pelvic-bone fractures could increase the risk for injury to the urinary bladder, urethra, or lower gastrointestinal tract. Methods: We retrospectively reviewed the data of 234 patients with pelvic-bone fractures who visited the emergency department of Kyungpook National University Hospital from January 2004 to December 2006. The location of the pelvic-bone fracture was divided into 8 parts. The association of fracture location with injury to the urinary bladder, urethra, or lower gastrointestinal tract was analyzed with Fisher' s-exact test and multiple logistic regression. Results: Nineteen(19) patients had urinary bladder injury, 8 had urethral injury, and 9 had lower gastrointestinal tract injury. The following fracture locations were found to be significant; urinary bladder: sacroiliac (SI) joint (p<0.001), symphysis pubis (p=0.011), and sacrum (p=0.005); urethra: SI joint (p=0.020); lower gastrointestinal tract: symphysis pubis (p=0.028). After the multiple logistic regression analysis, the primary and the independent predictors for each of the injuries were as follows; urinary bladder: sacroiliac joint (p=0.000, odds ratio [OR]=10.469); lower gastrointestinal tract: symphysis pubis (p=0.037, OR=7.009). Conclusion: Consideration of further workup for injuries to the lower gastrointestinal and urinary tract is needed for some locations of pelvic-bone fractures because certain pelvic-bone fracture locations, especially the sacroiliac joint and the symphysis pubis, are associated with increased risk for injury to the lower gastrointestinal and urinary tracts.
