Prognostic Value of Artificial Intelligence-Driven, Computed Tomography-Based, Volumetric Assessment of the Volume and Density of Muscle in Patients With Colon Cancer

  • Objective: The prognostic value of the volume and density of skeletal muscles in the abdominal waist of patients with colon cancer remains unclear. This study aimed to investigate the association between the automated computed tomography (CT)-based volume and density of the muscle in the abdominal waist and survival outcomes in patients with colon cancer. Materials and Methods: We retrospectively evaluated 474 patients with colon cancer who underwent surgery with curative intent between January 2010 and October 2017. Volumetric skeletal muscle index and muscular density were measured at the abdominal waist using artificial intelligence (AI)-based volumetric segmentation of body composition on preoperative pre-contrast CT images. Patients were grouped based on their skeletal muscle index (sarcopenia vs. not) and muscular density (myosteatosis vs. not) values and combinations (normal, sarcopenia alone, myosteatosis alone, and combined sarcopenia and myosteatosis). Postsurgical disease-free survival (DFS) and overall survival (OS) were analyzed using univariable and multivariable analyses, including multivariable Cox proportional hazard regression. Results: Univariable analysis showed that DFS and OS were significantly worse for the sarcopenia group than for the non-sarcopenia group (P = 0.044 and P = 0.003, respectively, by log-rank test) and for the myosteatosis group than for the non-myosteatosis group (P < 0.001 by log-rank test for all). In the multivariable analysis, the myosteatotic muscle type was associated with worse DFS (adjusted hazard ratio [aHR], 1.89 [95% confidence interval, 1.25-2.86]; P = 0.003) and OS (aHR, 1.90 [95% confidence interval, 1.84-3.04]; P = 0.008) than the normal muscle type. The combined muscle type showed worse OS than the normal muscle type (aHR, 1.95 [95% confidence interval, 1.08-3.54]; P = 0.027). Conclusion: Preoperative volumetric sarcopenia and myosteatosis, automatically assessed from pre-contrast CT scans using AI-based software, adversely affect survival outcomes in patients with colon cancer.

슬관절 부분 치환술의 장기 생존 분석 (Long-Term Survival Analysis of Unicompartmental Knee Arthroplasty)

  • 목적: 본 연구의 목적은 단일 술자가 시행한 슬관절 부분 치환술의 장기 임상적, 방사선적 결과 및 생존율을 분석하는 것이다. 또한 치환물의 생존에 영향을 미치는 인자를 분석하고 그에 따른 생존 곡선을 비교하는 것이다. 대상 및 방법: 1982년 12월부터 1996년 1월까지 시행된 슬관절 부분 치환술 99예를 대상으로 하였다. Modular II 10예, Microloc 44예, Allegretto 45예였다. 평균 추시기간은 16.5년이었다. 임상적 평가로 술 전, 술 후 1년, 최종 추시 hospital for special surgery(HSS) 점수와 관절운동범위를 조사하였다. 방사선적 평가로 술 전, 술 후 2주, 최종 추시 대퇴 경골각을 조사하였다. Kaplan-Meier 생존 분석을 이용하여 생존율을 분석하였다. Cox 비례위험모형을 이용하여 나이, 성별, 체질량 지수, 술 전 진단, 치환물의 종류 중 생존에 영향을 미치는 요인을 조사하였다. 생존에 영향을 미치는 인자에 따라 군을 나누어 생존 곡선을 비교하였다. 결과: 전체 HSS 점수는 평균 술 전 57.7점, 술 후 1년 92.7점, 최종 추시 79.1점이었다(p<0.001). 관절운동범위는 평균 술 전 134.3도, 술 후 1년 138.4도, 최종 추시 129.3도였다(p<0.001). 전체 대퇴 경골각은 평균 술 전 내반 0.8도, 술 후 2주 외반 4.1도, 최종 추시 외반 3.0도였다(p=0.003). 전체 생존율은 5년 91.8%, 10년 82.9%, 15년 71.0%, 20년 67.0%였다. 부분 치환술의 생존에 영향을 미치는 요인은 환자의 나이와 치환물의 종류였다. 나이가 높을수록 실패 위험이 낮았으며(hazard ratio=0.933), Microloc 사용 시 Modular II와 Allegretto에 비해 위험률이 높았다(hazard ratio=0.202, 0.430, respectively) 60세 미만군의 생존 곡선이 60세 이상 군에 비해 의미 있게 낮았다(p=0.003). Microloc 사용군의 생존 곡선이 Modular II와 Allegretto 사용군에 비해 낮았다(p=0.025). 결론: 고정형 치환물을 사용한 슬관절 부분 치환술의 장기 임상적, 방사선적 결과 및 생존율은 만족스러웠다. 부분 치환물의 장기 생존을 위해 적절한 환자와 치환물의 선택이 중요할 것이다.

Association of miR-193b Down-regulation and miR-196a up-Regulation with Clinicopathological Features and Prognosis in Gastric Cancer

  • Dysregulated expression of microRNAs (miRNAs) has been shown to be closely associated with tumor development, progression, and carcinogenesis. However, their clinical implications for gastric cancer remain elusive. To investigate the hypothesis that genome-wide alternations of miRNAs differentiate gastric cancer tissues from those matched adjacent non-tumor tissues (ANTTs), miRNA arrays were employed to examine miRNA expression profiles for the 5-pair discovery stage, and the quantitative real-time polymerase chain reaction (qRTPCR) was applied to validate candidate miRNAs for 48-pair validation stage. Furthermore, the relationship between altered miRNA and clinicopathological features and prognosis of gastric cancer was explored. Among a total of 1,146 miRNAs analyzed, 16 miRNAs were found to be significantly different expressed in tissues from gastric cancer compared to ANTTs (p<0.05). qRT-PCR further confirmed the variation in expression of miR-193b and miR-196a in the validation stage. Down-expression of miR-193b was significantly correlated with Lauren type, differentiation, UICC stage, invasion, and metastasis of gastric cancer (p<0.05), while over-expression of miR-196a was significantly associated with poor differentiation (p=0.022). Moreover, binary logistic regression analysis demonstrated that the UICC stage was a significant risk factor for down-expression of miR-193b (adjusted OR=8.69; 95%CI=1.06-56.91; p=0.043). Additionally, Kaplan-Meier survival curves indicated that patients with a high fold-change of down-regulated miR-193b had a significantly shorter survival time (n=19; median survival=29 months) compared to patients with a low fold-change of down-regulated miR-193b (n=29; median survival=54 months) (p=0.001). Overall survival time of patients with a low fold-change of up-regulated miR-196a (n=27; median survival=52 months) was significantly longer than that of patients with a high fold-change of up-regulated miR-196a (n=21; median survival=46 months) (p=0.003). Hence, miR-193b and miR-196a may be applied as novel and promising prognostic markers in gastric cancer.

Carcinoma Microsatellite Instability Status as a Predictor of Benefit from Fluorouracil-Based Adjuvant Chemotherapy for Stage II Rectal Cancer

  • Purpose: Rectal cancers with high microsatellite-instable have clinical and pathological features that differentiate them from microsatellite-stable or low-frequency carcinomas, which was studied rarely in stage II rectal cancer, promoting the present investigation of the usefulness of microsatellite-instability status as a predictor of the benefit of adjuvant chemotherapy with fluorouracil in stage II rectal cancer. Patients and Methods: Data of 460 patients who underwent primary anterior resection with a double stapling technique for rectal carcinoma at a single institution from 2008 to 2012 were retrospectively collected. All patients experienced a total mesorectal excision (TME) operation. Survival analysis were analyzed using the Cox regression method. Results: Five-year rate of disease-free survival (DFS) was noted in 390 (84.8%) of 460 patients with stage II rectal cancer. Of 460 tissue specimens, 97 (21.1%) exhibited high-frequency microsatellite instability. Median age of the patients was 65 (50-71) and 185 (40.2%) were male. After univariate and multivariate analysis, microsatellite instability (p= 0.001), female sex (p<0.05) and fluorouracil-based adjuvant chemotherapy (p<0.001), the 3 factors were attributed to a favorable survival status independently. Among 201 patients who did not receive adjuvant chemotherapy, those cancers displaying high-frequency microsatellite instability had a better 5-year rate of DFS than tumors exhibiting microsatellite stability or low-frequency instability (HR, 13.61 [95% CI, 1.88 to 99.28]; p= 0.010), while in 259 patients who received adjuvant chemotherapy, there was no DFS difference between the two groups (p= 0.145). Furthermore, patients exhibiting microsatellite stability or low-frequency instability who received adjuvant chemotherapy had a better 5-year rate of DFS than patients did not (HR, 5.16 [95% CI, 2.90 to 9.18]; p<0.001), while patients exhibiting high-frequency microsatellite instability were not connected with increased DFS (p= 0.696). It was implied that female patients had better survival than male. Conclusion: Survival status after anterior resection of rectal carcinoma is related to the microsatellite instability status, adjuvant chemotherapy and gender. Fluorouracil-based adjuvant chemotherapy benefits patients of stage II rectal cancer with microsatellite-stable or low microsatellite-instable, but not those with high microsatellite-instable. Additionally, free of adjuvant chemotherapy, carcinomas with high microsatellite-instable have a better 5-year rate of DFS than those with microsatellite-stable or low microsatellite-instable, and female patients have a better survival as well.

Change in Palliative Performance Scale (PPS) Predicts Survival in Patients with Terminal Cancer

  • 목적: Palliative Performance Scale (PPS)는 진행성 암환자에서 널리 사용되는 예후도구이다. PPS 측정이 생존에 대한 예측을 의미하지만, 연속적인 PPS 측정에 대한 유용성은 추가적인 연구가 필요하다. 본 연구에서는 완화 병동에 입원한 진행성 암환자를 대상으로 PPS score의 변화와 생존간의 연관성에 대해 진행한 연구이다. 방법: 2010년 1월부터 2012년 12월까지 서울성모병원 완화의학과에 입원한 환자 606에 대한 의무기록을 통하여 입원 당일의 PPS score와 입원 3일째의 PPS score 그리고, 두 score의 차이를 측정하여 점수의 변화와 생존간의 관련성을 분석하였다. 결과: PPS score의 변화와 생존과는 통계적으로 유의한 관련이 있었다. PPS score의 변화가 30% 이하인 군과 비교하였을 때, 30% 초과된 군에서 hazard ratio가 2.66(95% CI 2.19~3.22)로 확인되었다. 입원 3일째 PPS score가 30% 이하인 경우 독립적으로 생존에 대한 예측이 가능하였으며, PPS score 30% 초과된 군과 비교했을 때 hazard ratio는 1.67 (95% CI 1.38~2.02)로 확인되었다. 입원 당시의 PPS score는 생존과 독립적인 관련성은 없었다. 결론: 후향적으로 의무기록 분석을 통해 이루어진 본 연구에서 30% 이상의 PPS score 변화는 입원중인 말기 암환자의 생존과의 관련이 확인되었다. 입원 당시의 PPS score는 생존을 예측하지 못했다. PPS score의 변화는 말기암환자에서 단일 PPS score 측정보다 더 민감한 지표일 수 있다. 차후 더 많은 환자들에서 이에 대한 다기관, 전향적 연구가 필요하다.

동절기 온도가 꽃매미 월동 알의 생존율에 미치는 영향 (The Effect of Winter Temperature on the Survival of Lantern Fly, Lycorma delicatula (Hemiptera: Fulgoridae) Eggs)

  • 꽃매미는 약충과 성충이 무리지어 식물체 흡즙하거나 그 과정에서 감로를 배출하여 그을음병을 유발시킴으로서 포도 등에 피해를 주는 외래해충이다. 본 연구는 경기도내 주요 포도 재배지에서 꽃매미의 발생특성과 월동기 극저온이 꽃매미 월동난괴의 생존율에 미치는 영향을 분석하기 위해 2010년부터 2013년까지 수행하였다. 경기지역에서 꽃매미 월동난괴는 5월 상순부터 부화하기 시작하여 5월 중순에 가장 높은 약충 밀도를 보이고 있으며 7월 하순부터 출현한 성충은 11월 상순까지 산란하는 특성을 보였다. 동절기 온도가 꽃매미 월동난괴 생존율에 큰 영향을 미쳤으며, 1월 중 극저온 출현일수(x)와 생존율(y) 간 관계를 선형회귀모형으로 분석하였다. 그 결과 예측치와 실측치 간 잔차자승합(residual sum of square) 가장 낮은 모형은 $-11^{\circ}C$를 임계 극저온으로 취급한 y = -1.0486 x + 94.496 ($R^2=0.7067$) 이었다. 본 연구 결과는 월동 중 꽃매미 알의 생존율 예측과 봄철 발생시기 정보를 제공함으로써 꽃매미 관리전략 수립에 유용하게 활용될 수 있을 것으로 판단된다.

생존수영교육 참여가 초등학생의 수중불안, 신체적 자기효능감 및 수상안전 의식에 미치는 영향 (Effects of Participation in Survival Swimming Education on Elementary School Students' Underwater Anxiety, Physical Self-efficacy and Water Safety Awareness)

  • 본 연구는 생존수영교육 참여를 통한 초등학생의 수중불안, 신체적 자기효능감 및 수상안전 의식의 변화와 변인 간 영향관계를 살펴보고자 하였다. 이를 위해 편의표본추출법을 이용하여 부산광역시 소재 초등학생 중 생존수영교육에 참여한 초등학생 312명 중 300명을 유효표본으로 사용하였다. 수집된 자료는 SPSS 25.0 통계 프로그램을 이용하여 탐색적 요인분석, 신뢰도 분석, 대응표본 t-검정, 다중회귀분석을 통해 분석하였으며, 이에 따른 결과는 다음과 같다. 첫째, 생존수영교육 참여한 초등학생은 사전·사후효과를 비교하였을 때 사후 수중불안은 감소하고, 신체적 자기효능감과 수상안전 의식은 증가하였다. 둘째, 생존수영교육 참여 초등학생의 수중불안은 신체적 자기효능감과 수상안전 의식에 부분적으로 부정적인 영향을 미치며, 신체적 자기효능감은 수상안전 의식에 부분적으로 긍정적인 영향을 미치는 것으로 나타났다. 이러한 결과를 바탕으로 생존수영교육의 효과성을 확인할 수 있었으며, 프로그램의 질적 확대 방향성에 대해 재고해 볼 수 있을 것이다.

데이터 마이닝을 활용한 병원 재방문도 영향요인 분석 : 외래환자의 만족도를 중심으로 (On the Determination of Outpatient's Revisit using Data Mining)

  • Patient revisit to used hospital is a key factor in determining a health care organization's competitive advantage and survival. This article examines the relationship between customer's satisfaction and his/her revisit associated with three different methods which are the Chi Square Automatic Interaction Detection(CHAID) for segmenting the outpatient group, logistic regression and neural networks for addressing the outpatient's revisit. The main findings indicate that the important factors on outpatient's revisit are physician's kindness, nurse's skill, overall level of satisfaction, hospital reputation, recommendation, level of diagnoses and outpatient's age. Among these ones, physician's kindness is the most important factor as guidelines for decision of their revisit. The decision maker of hospital should select the strategy containing the variable amount of the level of revisit and size of outpatient's group under the constraint on the hospital's time, budget and manpower given. Finally, this study shows that neural networks, as non-parametric technique, appear to more correctly predict revisit than does logistic regression as a parametric estimation technique.

Regression Analysis of Doubly censored data using Gibbs Sampler for the Incubation period

  • In standard time-to-event or survival analysis, the occurrence times of the event of interest are observed exactly or are right-censored. However in certain situations such as the AIDS data, the incubation period which is the time between HIV infection time and the diagnosis of AIDS is usually doubly censored. That is the HIV infection time Is interval censored and also the time of the diagnosis of AIDS is right censored. In this paper, we Impute the Interval censored infection time using the conditional mean imputation and estimate the coefficient factor of the regression analysis for the incubation period using Gibbs sampler. We applied parametric and semi-parametric methods for the analysis of the Incubation period and compared the results.

Does total regression of primary rectal cancer after preoperative chemoradiotherapy represent "no tumor" status?

  • Purpose: Insistence that total regression of primary tumor would not represent long-term oncologic outcomes has been raised. Therefore, this study aimed to evaluate the outcomes of these patients after preoperative chemoradiotherapy (PCRT) and radical surgery and to evaluate the associated risk factors. Methods: We included 189 patients with rectal cancer who showed total regression of the primary tumor after PCRT, followed by radical resection, between 2001 and 2012. Recurrence-free survival (RFS) was calculated using the Kaplan-Meier method, and the results were compared with 77 patients with Tis rectal cancer who received only radical resection. Factors associated with RFS were evaluated using Cox regression analysis. Results: Sphincter-saving resection was performed for 146 patients (77.2%). Adjuvant chemotherapy was administered to 168 patients (88.9%). During the follow-up period, recurrence occurred in 17 patients (9%). The 5-year RFS was 91.3%, which was significantly lower than that of patients with Tis rectal cancer without PCRT (P = 0.005). In univariate analysis, preoperative CEA and histologic differentiation were associated with RFS. However, no factors were found to be associated with RFS. Conclusion: RFS was lower in patients with total regression of primary rectal cancer after PCRT than in those with Tis rectal cancer without PCRT, and it would not be considered as the same entity with early rectal cancer or "disappeared tumor" status.
